RESUMEN

The aim of this study is designed an improved ResNet 50 network to achieve automatic classification model for pain expressions by elderly patients with hip fractures. This study built a dataset by combining the advantages of deep learning in image recognition, using a hybrid of the Multi-Task Cascaded Convolutional Neural Networks (MTCNN). Based on ResNet50 network framework utilized transfer learning to implement model function. This study performed the hyperparameters by Bayesian optimization in the learning process. This study calculated intraclass correlation between visual analog scale scores provided by clinicians independently and those provided by pain expression evaluation assistant(PEEA). The automatic pain expression recognition model in elderly patients with hip fractures, which constructed using the algorithm. The accuracy achieved 99.6% on the training set, 98.7% on the validation set, and 98.2% on the test set. The substantial kappa coefficient of 0.683 confirmed the efficacy of PEEA in clinic. This study demonstrates that the improved ResNet50 network can be used to construct an automatic pain expression recognition model for elderly patients with hip fractures, which has higher accuracy.

RESUMEN

Objective To explore the relationship between the polymorphisms of the primary gout predisposing gene (BCAS3) rs11653176 locus and the incidence of gout in Han Chinese men in coastal areas of Shandong Province. Methods One hundred and fifty-two cases of patients with gout remission,68 cases of acute stage,252 patients with hyperuricemia, and 280 healthy subjects, total males, were enrolled. Genotyping the rs11653176 locus of BCAS3 gene by TaqMan probe technique. The expression level of BCAS3 gene mRNA in each PBMC was measured by RT-qPCR. The levels of tumor necrosis factor-α(TNF-α), interleukin-6(IL-6), and interleukin-18(IL-18)in serum were measured by ELISA. Results The change of allele frequency of rs11653176 locus in BCAS3 gene was associated with gout(P<0.01). BCAS3 mRNA in patients with gout was significantly higher than that of healthy people and patients with hyperuricemia(P<0.01). In gout patients, the expression level of BCAS3 gene containing C allele was higher than that of T allele(mRNA,P<0.05). The inflammatory factors in the acute phase of gout were significantly higher than those in phases of remission and hyperuricemia(P<0. 01). Conclusion Changes in the allele frequency of BCAS3 alleles rs11653176(high C, low T)may contribute to the expression of this gene,and lead to gout. And the onset of gout is closely related to the production of inflammatory factors.

RESUMEN

Objective To compare the efficiency of intra-articular injection of tranexamic acid (TXA) and autolo-gous transfusion drain on the blood loss after total knee arthroplasty (TKA). Methods A total of 124 patients (124 knees) with varus knee osteoarthritis, who were performed TKA,were retrospectively analyzed. Patients included 24 males and 100 females. The mean age was(65.03±6.84)years. Due to the blood loss control method, patients were divided into two groups including TXA application group (test group, n=49) and autologous transfusion drain group (control group, n=75). The data of blood routine examination, blood loss and blood transfusion after TKA were analyzed. Results The blood transfusion rates were 10.20%(5/49) in test group and 17.33%(13/75) in control group 7 days after TKA surgery. There was no signifi-cant difference between two groups (P>0.05). There were significant differences in red blood cell (RBC), hemoglobin (Hb) and haematocrit (HCT) between groups and effects of interaction in the two groups (P<0.01). There were significantly higher values of RBC, Hb and HCT at 1, 3 and 7 days after surgery in test group than those of control group (P<0.05).The total blood loss 1 and 3 days after TKA was significantly lower in test group than that of control group (P<0.05). There was no sig-nificant difference in the blood loss 7 days after surgery between two groups (P>0.05). Conclusion The single dose intra-articular injection of tranexamic acid is a safe and effective procedure for hemostasis after TKA.
