RESUMEN

The authors report a case of a 58 year old male presenting with bilateral ureteral compression by peri-aneurysmal retroperitoneal fibrosis. A ureterolysis and prosthetic replacement of the abdominal aorta gave good results. Eighteen months postoperatively, proteinuria and an edema of the lower limbs revealed an extramembranous glomerulitis, for which no cause could be found. Was this rare association fortuitous, or does it result from the action of some unknown antigen?

RESUMEN

Surgical cure of recurrent prolapsus of the genital organs or after hysterectomy is not an easy to attain. Most authors use prosthetic to obtain a satisfactory anatomic and functional results which lasts. Since 1985, the tissue-reinforcement prosthesis GORE-TEX* was used in this type of situation for 50 patients. The operation remains simple, but certain technical details are of great importance. The material is solid and well tolerated. Results are good with long follow-up. One of the important points for a successful treatment is probably to treat directly and simultaneously all the lesions involved, recognizing that prolapsus of the genital organs is a continuous disease process.

RESUMEN

In 3 cases, total parathyroidectomy, with graft of fragments of one of the glands into the forearm gave rapid correction of the clinical and biological disorders associated with secondary hyperparathyroidism. The secretion of parathyroid hormone by the autograft was confirmed by radioimmunoassay on brachial vein samples. This technique appears to be more satisfactory than 7/8 parathyroidectomy, the postoperative course of which is not always free of complications, which involves a risk of necrosis of the gland fragment left in place and in which reoperation is difficult.

RESUMEN

The authors performed 35 uretero-colonic anastomoses in 19 children using the technique of Mathisen. Indications were varied (ectopia vesicae 13, neurological bladder 5, female hypospadias 1). Four types of operation were done: direct uretero-sigmoidostomy 6, Bricker reimplanted in the sigmoid 8, permanent Bricker 4, enlargement sigmoido cystoplasty 1. There were 7 failures: 4 stenoses and 1 reflux. These failures required treatment by trans-uretero-ureterostomy (3 successes out of 4). The insertion of a trans-anastomotic catheter during the postoperative period is strongly advised

RESUMEN

In 22 cases we observed the association of post traumatic intra and retro peritoneal bleeding the latter due to kidney rupture. In such cases the usual policy consists in treating the abdominal visceral lesion first and post pone the management of the kidney rupture. In 13 cases we decided to undertake simultaneous surgical treatment of the abdominal and kidney lesion. The preoperative diagnosis was established by combination of exploratory peritoneal dialysis and I V P. The surgical exploration was achieved by means of medial incision. In 6/13 cases it disclosed kidney lesions which necessitated immediate nephrectomy. Our experience shows that one stage surgical procedure is beneficial to the patients.

Forty infants with a uropathy diagnosed during the first two months of life were studied. Presenting signs were urinary tract infection in one-half of the cases, disorders of micturition, pelvic or abdominal wall malformations, abdomino-pelvic mass, and macroscopic hematuria. Obstructive uropathy was observed in 17 children and vesicoureteric reflux in 29. We noted a high incidence of extrarenal malformations (14 of the 40 cases) in this series of uropathies diagnosed during the neonatal period. Despite early diagnosis, the course was not favorable in 11 cases with congenital anomalies of the renal parenchyma.

RESUMEN

An assessment has been made of renal growth and of the incidence of recurrent urinary tract infection in 116 children who had bilateral re-implantation of the ureters. Although reflux was prevented, 28% of the children continued to have infections long after the operation. The infections were mostly of the lower urinary tract whereas before the operation there was a significant incidence of acute pyelonephritis. After operation 82% of the kidneys grew normally and in 4,5% growth rate increased which is a smaller proportion than that reported in other series. In contrast 13,5% of the kidneys grew more slowly but they were usually abnormal radiologically before the operation.

RESUMEN

On the basis of their experience of upper digestive fiberscopy in paediatrics in hiatal hernia and gastrointestinal haemorrhage, the authors wish to point out the value of this technique as a complement to radiological investigations. They indicate the possibility, in a specialised centre of the everyday use of currently available fiberscopic equipment, from the neonatal period onwards, and emphasize the definite contribution of endoscopic findings in the definition of indications for surgery in the pathological states which they have chosen to illustrate here.
