RESUMEN

Incidence of gastrointestinal tuberculosis increases in the western world. Exact epidemiological data about this rare extrapulmonological disease are not available in Hungary. Three advanced cases with intestinal tuberculosis have been reported, all of them recognised during surgery due to complications. The clinicopathological correlations are briefly reviewed, and the importance of early diagnosis is emphasized.

RESUMEN

In a retrospective study from 210 foetal autopsies carried out in a period between 1992 and 1999 fifteen hydropic foetuses were found. The cause of the hydrops was shown to be Rh incompatibility in one case only. The cause of hydrops was not discernible in one case. In the others pathological examination clarified the cause and pathomechanism of non-immune hydrops. One isolated cystic hygroma, one monochorionic twin pregnancy with twin to twin transfusion, one case of sacrococcygeal teratoma and 4 cases of congenital heart diseases were reported. Postmortem interphase cytogenetic examination showed X0 monosomy in 2 cases. In further 4 foetuses pathognomic viral inclusions in the proerythroblasts raised the probability of parvovirus B19 infection what was confirmed by immunohistochemistry and electronmicroscopic examination. The occurrence of the parvovirus B19 associated cases of foetal hydrops was shown to be higher (4/15) in this series than in the literature. The accumulation of cases in 1998 is suggestive of an outbreak. The prenatal diagnostic implications and the attempts on further management are also discussed.

Although it can involve any segment of the gastrointestinal tract, Crohn's disease confined to the gastroduodenum is extremely rare. We report the story of a 20-years old male patient admitted for pyloric obstruction that developed despite medical treatment. Clinical manifestations necessitated operative treatment; Polya-gastrectomy was performed. Histology identified Crohn's disease and inflammatory changes as the cause of pyloric obstruction. Postoperative recovery was uneventful. The lesson of this case is, that if duodenal Crohn's disease would have been recognized preoperatively, gastrectomy could have been avoided. We review the literature and discuss treatment options for gastroduodenal Crohn's disease.

AIMS: Low-grade mucosa-associated lymphoid tissue (MALT) lymphoma and high-grade B-cell non-Hodgkin's lymphoma (NHL) of the stomach may occur simultaneously. To determine the clonal relationship between these tumours, we compared the immunoglobulin heavy chain gene (IgH) rearrangements of low and high-grade components of a multifocal gastric NHL. METHODS AND RESULTS: The complementary determining region 3 (CDR3) of the IgH gene rearrangements were polymerase chain reaction (PCR) amplified, cloned and sequenced. The analysis of the CDR3 sequences rearranged by tumour cells of low-grade MALT and the high-grade NHL revealed different nucleic acid sequences. CONCLUSION: These findings suggest that low-grade MALT and high-grade B-cell components of multifocal gastric NHL may represent unrelated clones.
