RESUMEN

As the result of high flood occurring during the period of June 21-22, 2002, in the Stavropol Territory, 65 settlements were affected. About 200,000 inhabitants found themselves lacking centralized drinking water supply. From the very first days of the emergency situation the state sanitary and epidemiological service took a complex of measures aimed at the liquidation of the medico-sanitary consequences of the high flood, the prevention of the appearance of mass infectious diseases among the affected population. As the result of these measures, the appearance of the outbreaks of acute enteric infections, viral hepatitis A aggravations in malaria and other infectious diseases could be prevented.

RESUMEN

Materials on the organization and execution of disinfection, disinfestations and deratization works at the period of the liquidation of the medico-sanitary consequences of the high flood in the Stavropol Territory in 2002 are presented. Timely and full-scope treatment, carried out in the course of these works, played a positive role in preventing epidemiological aggravations among the affected population.

RESUMEN

The system of organizational and prophylactic measures, carried out with a view to prevent epidemiological aggravations in connection with the natural calamity in the Southern Federal District of the Russian Federation in 2003, is described. Proposals on the improvement of the system of preventive measures, including the monitoring of the epidemiological, sanitary and hygienic situation, personnel training, technical provision, the cooperation of different services and departments, capable of ensuring the liquidation of the consequences of emergency situations of the natural character, are made.

RESUMEN

Materials on the organization and realization of prophylactic measures with respect to acute enteric infections (AEI) and viral hepatitis A (VHA) at the period of the liquidation of medico-sanitary consequences of the high flood are presented. As shown in these materials, the epidemiological surveillance on AEI and VHA in the areas affected by the emergency situation included the effective system of monitoring on these diseases. On the basis of monitoring optimum decisions were taken and concrete prophylactic measures were realized. This made it possible to detect the foci of infectious diseases in due time and efficiently liquidate them, as well as to prevent the development of the epidemiological consequences of the high flood.

RESUMEN

As the result of the high flood, an increase in the area of anopheligenic water reservoirs occurred on the inundated territories, which created favorable conditions for the proliferation of Anopheles mosquitoes, the vectors of malarial plasmodia, thus creating the threat of the formation of the local sources of malaria in case of its penetration from other regions unfavorable with respect to this disease. In the affected towns and regions hydrotechnical measures aimed at decreasing the surface of anopheligenic water reservoirs, as well as measures aimed at the decrease in the number of the larval and imaginal stages in the development of mosquitoes, were taken, which made it possible to prevent the probable epidemiological aggravations in malaria.

RESUMEN

Information on emergency situations (ES) on the territory of the North Caucasus from the 90s of the last century till the present moment is presented. The importance of the specific immunoprophylaxis of infections for the liquidation of epidemiological aggravations in ES and for their prevention is shown. Information on the scope of specific immunoprophylaxis in the zones of ES and on different approaches to its carrying out. The conclusion was made on the expediency of making corrections in the normative documents of the Ministry of Health of the Russian Federation, regulating planned vaccinations and vaccinations made on epidemic indications in connection with different kinds of ES.

The actual tasks faced by the family physician in the field of oncology can be considered in eight lines of activities according to successive stages of management of patients, that include identification and monitoring of risk factors; screening of early forms of cancer; diagnosis that must be based on results of the screening during the regular health checkups of patients and their applying for medical advice; primary finding out the extent of the process (staging); rendering the patient and his/her family phychological support; the family physician's work together with that of the oncologist; keeping the patient and his/her family under medical surveillance; terminal stages management.

An ever-growing cost of health care in Ukraine attest to the need for an optimum employment of resources in diagnosing and treating disorders of major public health significance, testicular carcinoma (TC) among their number. The authors consider it expedient to ascertain TC costs and design an analytical framework for the diagnostic and therapeutic options to be assessed properly. The following combination chemotherapy regimens were found to be associated with high response rates and acceptable longterm survival: BEP, PE programmes for primary treatment and PEI programme for recurrent or refractory TC. The minimum cost of the above regimens in one course of treatment (with 2 to 4 usually prescribed) for one patient, his height 175 cm, weight 75 kg--S = 1.9 m2, is US$ 613.0, 385.9, 813.5 for BE100P, PE100, and PEI respectively. In the Lviv region with the population about 2,750,000 TC cytostatic therapy is estimated to be about US$ 75,168.6 this being 62.6% of the annual budget appropriation for drug therapy of all malignant diseases taken together. The importance is emphasized of pharmaco-economic principles in the clinical management of patients allowing for decisions to be made with taking account of the cost and expected outcome of the therapy instituted.

RESUMEN

From the point of view of patients nausea and vomiting is a very important side effect of chemotherapy. The above events can be successfully controlled with modern antiemetic therapy. Use of high doses of metoclopramide in combination with dexamethasone and lorazepame is one of the routine means of administration of chemotherapeutic treatments with the benefit being derived in 30 to 70% of patients. Introduction into the clinical practice of a new group of medicines such as antagonists of receptors to serotonin (5-HT3-blockers) opens up good prospects for the nausea and vomiting prophylaxis and treatment in chemotherapy of cancer.

RESUMEN

We studied 400 patients with ischemic stroke, including 200 patients (the main group), who received 3 courses of phenotropil in the complex rehabilitation treatment in dosage 400 mg/day during 1 year after stroke. The extent of restoration of neurologic functions and daily living activities was assessed with Barthel, Lindmark, Scandinavian scales and Merton and Sutton scale. The restoration of neurologic and daily living activities was significantly (p < 0.0001) better in the main group compared to the control one.
