Traumatic dental injuries (TDIs) are common in growing children. Among all the dental injuries, complicated crown root fractures (Andreasen Class VI) of maxillary permanent anterior teeth are relatively widespread. Such fractured teeth are often considered as hopeless and are extracted. However, if the tooth is to be retained, various treatment strategies have been proposed. The aim of the present case report is to suggest a new technique to treat a complicated crown root fracture. The management of this case included endodontic procedure and orthodontic extrusion to move the fracture line above the supragingival level. A customized removable Hawley's appliance with a modified single cantilever spring was fabricated and an anchoring Begg bracket was bonded on the residual crown of the tooth. This method is useful in the mixed dentition when there is insufficient anchorage of adjacent teeth because of preshedding mobility and trauma. RESUMEN
Oral habits in the form of thumb sucking and tongue thrusting are commonly learned patterns of behavior seen in preschool children and they are associated with anxiety, fear, hunger, oral pressure, and sleep. Chronic practice can cause dentoalveolar, perioral problems, and atypical root resorption (ARR) of anterior primary teeth. The ARR is provoked by the thumb sucking habit, and leads to early loss of anterior primary teeth. The early loss of anterior tooth may result in speech and masticatory problems, and psychological disturbance to the child. Hence, pediatric dentists play a crucial role in giving necessary information to parents and guardians. Starting from counseling to appliance therapy, various treatment modalities have been reported in the literature. One of them is bluegrass appliance; it is a nonpunitive habit reminder therapy. The present case report describes a customized hybrid bluegrass appliance designed to eliminate thumb sucking and tongue thrusting habit, and to perform as an esthetic functional space maintainer. RESUMEN
Construction of complete denture with adequate retention is a complex procedure. Use of suction cup in a maxillary denture is one of the techniques to improve retention. Palatal Suction cup, which provides high retention by inducing negative pressure on the mucosal surface, is not being recommended because of its pathological effect on the palatal tissues leading to palatal perforation. Surgical closure of palatal perforation is challenging, technique sensitive, and it can be achieved by different surgical methods depending on the size and location of the defect. In this case report, we present a patient with palatal perforation due to prolonged use of a denture with a suction cup for over 10 years, which was surgically closed by a palatal rotational flap. The result was good as the defect was successfully closed and the donor site healed uneventfully.

The dental arch perimeter and transpalatal width are the most significant arch dimensions in the developing individuals. Its management during the primary, mixed and early permanent dentition are greatly important for the normal development of the dental arches and significantly improve the occlusion in adulthood. Malocclusion is a constant source of threat to the facial appearance. Severe anterior crowding and narrow maxillary arches adversely affect the smile, facial profile and consequently patient becomes handicapped socially as well as psychologically. Maxillary expansion has been used in resolving the problems of crowding, constricted arches and allow for forward growth of the mandible in growing children. This case report describes the use of a customized modified Haas appliance in intercepting the maxillary anterior crowding and gain the transpalatal width and arch perimeter of the maxilla, enhance the smile and facial profile and to allow the forward mandibular growth without any anterior occlusal interferences.

Early loss of permanent anterior teeth in growing children has a psychological impact on the child. Anterior teeth are important both aesthetically and functionally. When a permanent tooth is lost, the teeth adjacent to the created space tend to migrate into the space resulting in the space loss. Management of regaining space with the removable appliance always depends on child cooperation for using the appliance as well as for the recall visits. The advantages of fixed appliances over the removable appliances are minimal discomfort, reduced need for patient cooperation and increased control of tooth movements in all three directions of space. Thus, a short course of fixed appliance like the modified 2 by 3 fixed appliance followed by fixed functional space maintainer could be an ideal treatment option for such cases.
