RESUMEN

The relevance of the study is associated with the widespread use of osteomodifying agents in patients with bone metastases and osteoporosis. Bisphosphonates and other osteo-modifying agents are widely used in oncology and prevention of age-related changes in the human bone system. The use, therapeutic effects and complications of therapy with osteo modifying agents are being investigated all over the world. However, the etiology and pathogenesis of drug-induced osteonecrosis of the jaws (MONCH) have not been fully studied, in this regard, the study of risk factors and mechanisms of its development remains relevant. New data on the etiology and pathogenesis of drug-induced osteonecrosis are presented. The literature review is carried out on the electronic resource PubMed.

THE AIM OF THE STUDY: Was to investigate the dynamics of mandibular density in cancer patients during therapy with zolendronic acid. MATERIALS AND METHODS: The study comprised 14 patients who received zolendronic acid at a dosage of 4 mg once every 28 days for bone metastases. In all 14 patients, measurements of mandibular density values on CT scans were performed over time. RESULTS: Using multiple linear regression analysis, a model was developed to predict the effect of the number of zolendronic acid injections «X1¼ on the dynamics of mandibular density «Y¼. The resulting formula for predicting mandibular density is Y = 5.9 X1 + 49 HU. CONCLUSION: The model has limitations due to the study design but still can be used by oncologists and dentists to assess mandibular density in patients taking zolendronic acid.

RESUMEN

THE AIM THE STUDY: To analyze the density of the mandible in cancer patients during treatment with zoledronic acid. MATERIALS AND METHODS: A retrospective cohort study included 45 patients with cancer aged 26-81 years (average age 55±12.88 years), of whom 14 patients had bone metastases (n=14) and took 4 mg of zolendronic acid once every 28 days. The patients underwent standard PET-CT examinations in the «whole body¼ mode, and the density of the mandible was examined on CT. Radiation therapy was performed by intracavitary administration of strontium 89 chloride; remote radiation therapy with cisplatin radiomodification. In the presence of bone metastases, patients received complex supportive therapy with zolendronic acid. The effect of zolendronic acid on the density of the mandible in the frontal and lateral sections was studied by multidimensional dispersion analysis. RESULTS: Statistically significant differences (p=0.002) were revealed for density indicators according to CT scans of the mandible in the frontal region against the background of zolendronic acid therapy. We attribute the absence of statistically significant differences for the density of the mandible in the lateral sections (p=0.101 and p=0.082) against the background of zolendronic acid therapy to a measurement bias. We attribute the absence of statistically significant differences in density indices against the background of hormonal, radiation, targeted and chemotherapy to the design of the study. CONCLUSION: Density measurement based on CT examination data can be recommended for use as an additional tool in assessing the effect of zolendronic acid on the density of the mandible. However, the method of measuring the density of the mandible in the lateral sections requires improvement to prevent measurement bias.

THE AIM OF THE STUDY: Comparative evaluation of determining the hygienic state of the mouth in patients using classical methods (index evaluation) and using the digital platform. MATERIAL AND METHODS: The study included 25 patients aged 16 to 60 years (women - 18, men - 7). A parallel determination of the level of oral hygiene using classical and digital approaches was carried out, the results obtained were compared and analyzed. RESULTS: When comparing the results obtained using both methods, it was revealed that in 24 out of 25 cases they turned out to be identical. The use of a digital protocol requires about two times less time than using the classical approach and allows for dynamic monitoring of the oral hygiene. CONCLUSION: The conducted research has shown that assessed digital platform is not inferior to the standard method of determining the level of hygiene. The result obtained using the digital platform gives a real-time visual representation of the level of oral hygiene of the patient, as it contains a transcript of the data and a detailed description of the type of dental deposits (the intensity of staining reflects the time interval of plaque formation).

RESUMEN

THE AIM OF THE STUDY: Was to assess the adaptation of patients to obturator at different periods of adaptation using objective digital methods of analysis: axiography, cone-beam computer tomography, digital facial scans, and intraoral scanning. MATERIAL AND METHODS: The study comprised 17 patients with postoperative defects of the upper jaw at different periods of removable obturators usage. RESULTS: In 88.2% of the subjects limited mouth opening and displacement of the mandible to the healthy side was detected. Contracture as a complication can develop in the later stages of rehabilitation. CONCLUSION: A long-term rehabilitation of patients is recommended using simulators and special gymnastic exercises in order to prevent extra-articular contracture.

BACKGROUND: Treatment of patients with temporomandibular joint pathology in the presence of systemic diseases should include a number of additional methods. OBJECTIVE: The aim of the study to consider an algorithm for providing care to a growing patient with TMJ arthritis on the background of juvenile rheumatoid arthritis complicated by distalization of the mandible. MATERIAL AND METHODS: A growing patient with temporomandibular joint pathology and juvenile rheumatoid arthritis is undergoing complex treatment by a rheumatologist and dentist in order to prevent pronounced degenerative changes in the TMJ bone structures, significant asymmetries of the facial skeleton and the postural component of the patient's body. RESULTS: The patient underwent rehabilitation in order to activate the growth zones, reduce the skeletal asymmetry of the maxillofacial region due to directed growth using conservative techniques. The growth of the branches of the lower jaw was obtained, the optical density of the cortical bone of both condyles improved to the age norm. CONCLUSION: A significant improvement in the prognosis of TMJ rheumatoid arthritis disease in growing patients is possible while creating conditions for adequate interaction of all structures of the maxillofacial region.

RESUMEN

Complex treatment of malignant tumors of an oral cavity and a throat includes basically a combination of surgical, beam and chemotherapeutic methods of treatment. All listed components of treatment applied in the field of a head and a neck, lead to development of stomatologic complications. Arising complications, accumulating against each other, cause symptoms of mutual burdening. Correction of the given conditions entirely to lay down on the doctor-stomatologist. Article is devoted the analysis of arising complications, and their influence against each other.
