OBJECTIVES: This study aimed to evaluate the effect of systemically administered methylphenidate hydrochloride (MPH) on new bone formation in premaxillary suture after rapid maxillary expansion (RME). SETTING AND SAMPLE POPULATION: Thirty-three Wistar rats were divided into four groups: Group 1 (high dose, 30/60 mg/kg MPH), Group 2 (low dose, 4/10 mg/kg MPH), Group 3 (positive control) and Group 4 (negative control). METHODS: RME was applied on the 70th day of the study. A 5-day RME period was followed by a 12-day retention period. The experiment was terminated on the 87th day. Micro-CT for radiological evaluation, haematoxylin-eosin and Masson's trichrome staining methods were used for histomorphometric evaluation. RESULTS: Among experimental groups with RME, the lowest number of osteoblasts and capillaries in Group 1 (P < .05). New bone formation, fibrous callus formation, distal osteotomy line, proximal osteotomy union and cortex remodelling were observed to be lower in Group 1 and Group 2 than Group 3 (P < .05). There was a statistically significant difference between Group 4 and each of the other groups (P = .000) in the evaluation of the results for bone mineral density, bone volume, bone volume percentage, trabecular thickness and trabecular number. CONCLUSIONS: MPH reduces cellular activity for new bone formation in suture in RME groups. Before performing rapid maxillary expansion in patients using MPH, the use of the drug should be postponed after a multidisciplinary decision process or clinical doses should be lowered.

PURPOSE: The main disadvantage of distraction osteogenesis is the prolonged treatment protocol. Recently, oxytocin (OT) has been found to have anabolic effects on bone metabolism. In this experimental study, the effects of OT on the mandibular distraction gap in rabbits at 2 different distraction rates were evaluated. MATERIALS AND METHODS: This experimental study was conducted on 28 male New Zealand white rabbits. The animals were divided into 3 experimental groups and 1 control group. Group A (control group, n = 7) consisted of animals with distraction at a rate of 1 mm/day, and group B (n = 7) consisted of animals with a distraction rate of 2 mm/day; groups A and B received postoperative saline solution injection. Group C (n = 7) consisted of animals with distraction at a rate of 1 mm/day, and group D (n = 7) consisted of animals with a distraction rate of 2 mm/day; postoperative OT injection was performed in groups C and D. RESULTS: Both histomorphologic and micro-computed tomography evaluations showed increased bone healing in the OT-treated groups. CONCLUSIONS: On the basis of the evaluation of both the histomorphometric and micro-computed tomographic data, systemic OT administration was found to increase new bone formation and bone healing with distraction osteogenesis.

BACKGROUND: Rheumatoid arthritis (RA) is the most common chronic inflammatory disorder and is associated with progressive destruction of synovial joints and physical disability. Therapies with known benefits include disease-modifying antirheumatic drugs (DMARDs), such as methotrexate, as well as more recent biologic agents, such as tumor necrosis factor inhibitors (anti-TNF therapy). METHOD: This was a retrospective study, which included 205 RA and 112 early RA (ERA) patients from the Rheumatology Clinic at Gaziantep University School of Medicine Research Center as well as 104 healthy controls. RESULTS: The mean neutrophil to lymphocyte ratio (NLR) was found to be 3.15 ± 2.64 in the patient group and 2.03 ± 0.94 in the control group. The mean platelet to lymphocyte ratio (PLR) was 162.39 ± 107.76 in the patient group and 131.23 ± 48.09 in the control group. There was a significant difference in both the NLR and PLR between the patient and control groups (both p < 0.01). There was a significant difference in both the NLR and PLR between patients with active disease and remission (both p < 0.001) in RA, including anti-TNF therapy and DMARDs groups. There was a significant difference in NLR (p = 0.001) but not in PLR (p = 0.051) between active disease and remission in ERA. CONCLUSION: The results of the present study suggest that the NLR may be considered a useful marker of disease activity in RA and one that can aid the diagnosis of ERA. The PLR can be used in the assessment of disease activity in RA patients undergoing anti-TNF therapy but is not suitable for diagnosing ERA.

RESUMEN

Rhupus is a rare syndrome characterized by overlap of rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE). Previous reports mentioned that rhupus patients have prominent RA associated clinical manifestations and only mild organic damage related to SLE. Progressive or life-threatening manifestations are rare in rhupus patients. Our patient diagnosed as rhupus was a young women, presented with multi-organ involvement of systemic vasculitis. Rheumatologists should be aware of possibility that rhupus may be accompanied by progressive or life-threatening conditions such as vasculitis.

RESUMEN

Instead of a textured surface with irregular pore size and distribution as in conventional dental implants, the use of lattice structures with regular geometric structure and controlled pore size produced by selective laser powder bed fusion melting (LPDF) technique will provide more predictable and successful results regarding osseointegration and mechanics. In this study, biomimetic dental implants with 2 different pore designs were fabricated by LPDF technique and compared with conventional dental implants in terms of surface characterization and resistance to biomechanical forces. Finite element analysis, scanning electron microscopy, computed micro tomography scanning, ISO 14801 tests and detork tests were used for the comparison. The tested biomimetic implants were found to be as durable as conventional implants in terms of mechanical strength and detork values. They were also found to be 40-60% more advantageous than conventional dental implants with respect to surface area and volume. As a result, it was concluded that biomimetic dental implants with sufficient mechanical strength and complex surface geometries can be made as designed without changing the reliable base material and can be produced using a different manufacturing method.

AIM: The purpose of this study was to investigate proliferation, morphology, mineralization and mRNA expressions of mineralized tissue associated proteins of PDL cells on smooth (S), sandblasted small-grit (SSG), sandblasted large-grit (SLG) and sodium titanate (NaTi) coated titanium alloys, in vitro. METHODS AND MATERIALS: PDL cells were cultured with DMEM media containing 10% FBS on the S, SSG, SLG and NaTi titanium surfaces. PDL cell proliferation, mineralization and immunohistochemistry experiments for Bone Sialoprotein (BSP) were performed. The morphology of the PDL cells was examined using confocal and scanning electron microscopy (SEM). Gene expression profiles of cells were evaluated using a quantitative-polymerase chain reaction (Q-PCR) for type I collagen (COL I), Osteocalcin (OCN), osteopontin (OPN) and Runt-related transcription factor-2 (Runx2) on days 7 and 14. RESULTS: Proliferation results on days 6 and 10 were similar in groups, while those of day 13 revealed a decrease in the NaTi group when compared to the S group. NaTi surface induced BSP mRNA expression which was correlated with mineralization tests and BSP immunostaining results. Increased Runx2 mRNA expression was also noted in the NaTi surface when compared to other surfaces. CONCLUSIONS: This study considers the NaTi surface as a potential alternative to SSG and SLG surfaces. This surface might provide a promising environment for PDL ligament-anchored implants.

RESUMEN

Tumor necrosis factor α (TNFα) is a pro-inflammatory cytokine and a key mediator of inflammation. Several TNFα antagonists have been used therapeutically. One of them, infliximab, is a chimeric monoclonal antibody with anti-TNFα activity. Numerous studies have shown TNFα antagonists to be effective in treating psoriasis--particularly severe resistant forms--and arthritis. Additionally, several case reports and studies showing the effects of TNFα on new dermatologic indications have been published. The intention of this review is to summarize the new off-label uses of infliximab according to published data.

Oral alitretinoin (9-cis retinoic acid) is an endogenous retinoid related to vitamin A. Studies have shown that oral alitretinoin is effective and well-tolerated in the treatment of severe chronic hand eczema, so that it is approved for this indication. This review summarizes new studies and clinical experience on the off-label use of alitretinoin.

RESUMEN

We describe a case of a previously healthy 35-year-old man who presents with meniscal symptoms, and present the arthroscopic findings of a complicated tear of black lateral meniscus. Investigations revealed that he had underlying alkaptonuria, which was previously undiagnosed without any other findings. After the surgical treatment, the patient's complaints were alleviated and almost no complaints were registered, during the next follow-up.

BACKGROUND: Interferons are molecules with antiviral effects, which have been used for the treatment of verruca for many years. AIMS: To determine if sublesional interferon (IFN)-alpha injection offers an effective alternative treatment for common warts. METHODS: We compared the results of single-dose sublesional IFN application in different types of verruca and with placebo for the treatment of single verruca plantaris lesions. In total, 53 patients (mean age 22.6 years) were enrolled in the study. Of these, 45 patients received a single sublesional injection of 4.5 MU IFN-alpha2a (three study groups), and eight patients with single verruca plantaris lesions were injected with physiological saline as placebo (control group). As local anaesthesia, liquid nitrogen was sprayed only on to the injection site for 3-4 s. The injection was made directly under the lesion through the border of the lesion, at with approximately a 45 degrees angle from healthy skin. RESULTS: At the 12-month follow-up in the group of patients with single verruca plantaris, there were 19 complete cures (7.2%) and 2 partial responses (8.3%), and 3 patients (12.5%) had no response. In the control group, only 2 patients (25%) had a partial response to treatment. CONCLUSIONS: These results suggest that a single sublesional dose of 4.5 MU IFN-alpha may be of value in the treatment of patients with verruca, especially in those with single verruca plantaris lesions.

All major opencast mining activities produce dust. The major operations that produce dust are drilling, blasting, loading, unloading, and transporting. Dust not only deteriorates the environmental air quality in and around the mining site but also creates serious health hazards. Therefore, assessment of dust levels that arise from various opencast mining operations is required to prevent and minimize the health risks. To achieve this objective, an opencast coal mining area was selected to generate site-specific emission data and collect respirable dust measurement samples. The study covered various mining activities in different locations including overburden loading, stock yard, coal loading, drilling, and coal handling plant. The dust levels were examined to assess miners' exposure to respirable dust in each of the opencast mining areas from 1994 to 2005. The data obtained from the dust measurement studies were evaluated by using analysis of variance (ANOVA) and the Tukey-Kramer procedure. The analyses were performed by using Minitab 14 statistical software. It was concluded that, drilling operations produce higher dust concentration levels and thus, drill operators may have higher incidence of respiratory disorders related to exposure to dust in their work environment.

The mechanism of osseointegration is related to many factors, including the quality of the bone, the biocompatibility and surface characteristics of the implant material, the surgical technique, and functional loading. The purpose of this study was to investigate the effects of hyaluronic acid gel on the osseointegration of implants placed in defined areas of the mandible in rabbits. Hyaluronic acid is known to have an osteoinductive effect during regeneration of bony defects, and we thought that it might also have a favourable effect on osseointegration, a specialised mechanism to heal bone. Ten New Zealand rabbits aged 10 weeks and weighing 2.5-3.0kg were used, and sites for implants that were far enough from the apices of the teeth in the mandibular molar area were chosen. Two cavities were prepared in each rabbit, one (anterior) for the control implant, and one (posterior) for the implant with hyaluronic acid gel (Medical Instinct GmbH, Bovenden). New bone and the osteoid matrix content around the dental implants were evaluated histologically and histomorphometrically two months after the operation, and no significant difference was found between the two groups.

BACKGROUND: In addition to their cholesterol-lowering effects, hydroxymethylglutaryl coenzyme A reductase inhibitors ("statins") might have pleiotropic, nonlipid effects. Insulin resistance syndrome is known to increase the risk for cardiovascular disease. However, the effects of statins on insulin resistance are a subject of controversy. OBJECTIVE: We aimed to investigate the effects of atorvastatin on insulin resistance in hyperlipidemic patients. METHODS: This 12-week, prospective, nonrandomized, open-label study was conducted at the outpatient cardiology clinic, Ege University Medical School, Bornova-Izmir, Turkey. Hyperlipidemic patients aged ≥18 years with insulin resistance and no other components of the metabolic syndrome were included in the study. Atorvastatin 10 mg QD (after the evening meal) was administered by mouth (tablet) over a 12-week period. At baseline and after 12 weeks of treatment, insulin sensitivity was assessed using homeostasis model assessment (HOMA) index methodology. Serum lipid parameters and fasting levels of plasma glucose and insulin (FPG and FPI, respectively) were measured at the same 2 time points. The tolerability of atorvastatin was assessed using laboratory analysis and physical examination, including vital sign measurements. RESULTS: Fifteen white patients (9 women, 6 men; mean [SD] age, 52 [8] years) participated in the study. No significant changes in HOMA index were found (mean [SD], 3.1 [1.5] vs 3.2 [1.9]). The lipid profile was improved significantly at 12 weeks compared with baseline (mean [SD] low-density lipoprotein cholesterol, 173.2 [21.3] vs 110.8 [43.6] mg/dL; total cholesterol, 270.9 [21.5] vs 201.2 [46.7] mg/dL; and triglycerides, 269.5 [46.3] vs 205.5 [49.3] mg/dL; all, P < 0.001). No significant change in mean (SD) plasma high-density lipoprotein cholesterol level (45.5 [6.6] vs 43.7 [8.1] mg/dL) was found. In addition, no significant changes in FPG (85.3 [12.71 vs 84.8 [10.4] mg/dL), or FPI (13.5 [9.7] vs 13.9 [10.1] µU/mL) were found. None of the patients required withdrawal of medication due to an adverse event. CONCLUSION: In this pilot study in hyperlipidemic patients with insulin resistance, 12 weeks of treatment with atorvastatin 10 mg QD was effective in controlling hyperlipidemia but did not reduce the severity of insulin resistance.

BACKGROUND: To evaluate the incidence and importance of lumbar vertebra transverse process fractures in polytraumatized patients who had undergone emergent laparatomy, and to find out the relevant risk factors. METHODS: The medical records of 312 laparotomized trauma patients who referred to our emergency department between January 2001 and January 2002 were retrospectively assessed. Hundred and six cases who met inclusion criteria were investigated. Data relevant to the trauma, demographics, hemoglobine levels, additional system traumas, complications were recorded. Correlations between collected data and the fractures were investigated. Results were evaluated statistically using SPSS 11,0 software package program. RESULTS: Lumbar vertebra transverse process fractures were detected in 58,5 % of the cases. They were more frequently encountered in women, elderly and after falls or traffic accidents.. Abdominal organ injuries were more common in these cases. These fractures are associated with hepatic and splenic injuries. Their presence also increases the risk of vertebra corpus fractures seven fold. CONCLUSIONS: Lumbar vertebra transverse process fractures usually occur after higher energy traumas They must be evaluated as serious findings which might lead to potential organ injuries. Once these fractures were detected in trauma patients, every attempt should be made to exclude probably fatal and more serious additional organ injuries.

Controversy exists with regard to the role of balloon angioplasty in the treatment of native aortic coarctation. Recent data and studies have showed that percutaneous balloon angioplasty is a safe and effective treatment for aortic coarctation. We report a young adult with aortic coarctation who has been treated with successful balloon angioplasty.

We had previously investigated skin problems of musicians (N = 117) working in a professional orchestra. The results of this survey suggest a significant incidence of occupational and stress-related skin problems in musicians such as hyperhydrosis, lichen planus, psoriasis, seborrheic dermatitis and urticaria. It was thought that emotional factors exacerbate their problems [6]. This article discusses the importance of psychiatric consultations in dermatologic problems.

Pulmonary involvement in Behçet's disease (BD) is reported to indicate poor prognosis and high mortality. Our aim was to evaluate the pulmonary vascular status in BD. As 123I-meta-iodobenzylguanidine (123I-MIBG) shares the same uptake, storage and release mechanisms as norepinephrine, associated with the functional state of pulmonary endothelium, it may reflect endothelial injury. Twenty-five patients (13 males, 12 females; mean age, 36.44 years) and 12 age-matched controls were included. After intravenous injection of 123I-MIBG, thorax images were taken at 15 min and 4 h. Appropriate regions of interest were drawn, and heart to mediastinum (H/M) ratios and lung retention indices (LRI) were calculated. The difference between the LRI of controls (75.6+/-4%) and patients (87.7+/-10%) was found to be extremely significant (P<0.005). The LRIs of active (92.85+/-12%) and inactive (83.65+/-5%) states of BD were significantly different (P<0.05) from each other. There was no significant difference between the H/M ratios of controls (2.4+/-0.27) and patients (2.39+/-0.31) or between patients in active and inactive disease states. Our study revealed prolonged lung retention of 123I-MIBG in BD, probably reflecting the severity of the disease. In conclusion, 123I-MIBG lung retention is a valuable finding in the evaluation of pulmonary vascular status and may be a potential marker of prognosis in BD.

RESUMEN

A 23 year old woman who presented with severe acne had been treated with 40 mg/day isotretinoin. On her monthly control, severe cheilitis and bilateral toe nail onycholysis were observed. It is well known that systemic retinoids have several side effects. Although dystropic nail, paronichia-like changes, median nail dystrophy have previously been reported with isotretinoin therapy, onycholysis is rare. In this report, we describe a case with isotretinoin induced nail fragility and onycholysis.
