RESUMEN
Magic syndrome is a very uncommon disease, and vascular involvement is exceptional; only one case has been reported in the literature associated to a true aortic aneurysm. The treatment of aneurysms recommended in these patients is based on isolated cases and includes corticosteroids, other immunosuppressant drugs, and surgery. We report a case of a patient with Magic syndrome who developed aneurysm at the end of the aorta during treatment with infliximab, corticosteroids, and cyclosporine and who needed endovascular prosthesis implantation. After 12 months, she suffered an aneurysm of the ascending aorta, dilatation of the sinotubular junction, and severe aortic insufficiency, which forced surgery. During this time, the patient finally died.

Results of a study about the chronic ischaemia of the lower limbs and its correlation with age, sex and previous diagnosis, are presented. Also, a comparison between previous diagnosis and results following the methodology exposed, was made. Study group included 433 older patients, randomizadely selected and permanent residents of some geriatric centers of the Oriental Andalucy.

Because the high incidence of cardiovascular disease between aged people and because the problematic features of Andalusian community, like its vast extension, high percentage of people older than 65 years, low sociocultural level and lack of physicians specialized on Angiology; an important study was made in order to analyze such type of disorders as well as their factors of risk. Study was made transversally with 443 hospitalized aged patients.

Sanguineous flow is studied in a group of patients with unilateral obliteration of iliac artery. This flow was particularly studied during exercise with the purpose of determinate the modifications on the flow produced by exercise. For this purpose, 11 patients with no associated pathologies were studied during more than a year follow-up.

The appearance of minimally invasive surgery, and within it, the endovascular techniques, in relation to the sociocultural evolution of the doctor-patient relationship from the era of the so-called medical paternalism that, once the human rights and the rights of the patients came on the scene, patients gained freedom as regards their health, diagnosis and therapeutic techniques they receive. Finally, the history of endovascular techniques is briefly described, and the name Endovascular Surgery is formally proposed to include these. The yearly average of patients suffering from severe cranio-encephalic injuries who are admitted to our department is 492. From 1972 to 1980 we have been able to operate on 11 delayed traumatic intracerebral hematomas. Bollinger, in 1981, was the first to describe delayed traumatic apoplexy. Since the treatment is usually surgical evacuation of the hematoma. Results of treatment and conclusions are dealt with.

The study is concerned with cases of tympanicum jugulare paragangliomas of special interest due to their localisation in unusual positions. These atypical positions have been established by the analysis of 5 cases operated on in our department. The surgical and analytical procedures are described in detail and a study of the nature of these T.J.P.s is provided.

134 patients suffering from spontaneous pneumothorax were seen at the Surgical Department of Granada-University. Most of the patients were of young age (average age 23 years), with men prevailing. Until today its aetiology is unknown. Conservative treatment will do for cases with only partial pnth. In all other cases a suction drainage (Bülau-type) is installed. Surgery is only performed in those cases where no complete re-expansion of the lung can be achieved by suction drainage.

The treatment of undifferentiated thyroid cancers differs in no way from that of other malignant tumours. An oncological resection has to be performed. As far as well-differentiated thyroid carcinomas are concerned, we have divided our cases into two groups ('low risk' and 'high risk') according to factors of age, sex and pathological type. In low-risk patients we recommend lobectomy; together with a limited homolateral neck dissection in cases of papillary carcinoma. In the high-risk group we performed a subtotal thyreoidectomy which is complemented by treatment with radioactive ioidine until a total thyreoidectomy is achieved. By following this method, nearly all our patients (77 out of 82) were still at life after 7-4 years; the exceptions were five cases of patients with anaplastic carcinoma who died within the year. In our view, such positive results justify a conservative surgical procedure.

Clinical findings, diagnosis, and pathology of heart myxoma are discussed on the basis of personal experience with nine patients. There was a striking variety of signs and symptoms caused by tumor embolization, hemodynamic obstruction, and autoimmunologic reactions. Echocardiography is the method of choice, although angiography may still be necessary in atypical or negative echocardiographic findings. The tumor should be removed as soon as possible after diagnosis. There is danger of tumor embolization in the course of the operation. Long-term results are good, if resection of the tumors is performed before catastrophic complications occur. Morphologically, myxomas are genuine tumors characterized by myxomatous stromata and cells.
