RESUMEN

The aim of this study was to evaluate the effect of cytoflavin inclusion in therapy regimens of patients with diabetic foot syndrome and medicalcinosis on the microcirculation of the lower limb blood vessels. For this purpose, 64 patients with type II diabetes mellitus (subcompensated form), diabetic foot syndrome and signs of medicalcinosis foot blood vessels of various severity were examined. All patients were divided into 2 groups depending on the therapy scheme: patients of group I (33 people) received insulin, antibiotics, disaggregants, statins, anti-inflammatory drugs, painkillers, detoxification therapy. Patients of group II (31 patients) in addition to the main therapy received cytoflavin according to the scheme: 10 ml per 0.9 ml NaCl 200 ml intravenously, drip at a rate of 60 cap/min, 10 days course, then 2 tablets 2 times a day for 1 month. The severity of the microangiopathy of the lower extremities was assessed considering transcutaneous oximetry data in dynamics (before treatment, on the 10th and 40th day of therapy) gained using TCM-2 device (RADIOMETER, Denmark) with has heating oxygen electrode type Clarc. Before the treatment subcompensated level of metabolic disorders corresponding to the II degree of microcirculatory disorders was registered in all patients. Against the backdrop of cytoflavin inclusion in the complex therapy, there was an improvement in the metabolic rate (up to compensated) with a decrease in the degree of microcirculation disorders up to I degree. Positive dynamics of metabolic disorders significantly reduced frequency and extent of surgical interventions. The results obtained suggest that cytoflavin should be included in the treatment regimens of patients with this pathology.

The objective of this research was to study changes in parameters, characterizing endogenous intoxication in patients with acute small bowel obstruction with Reamberin included into therapy scheme. Full physical examination and surgical treatment of 202 patients with acute small bowel obstruction were conducted. The control group included 30 healthy individuals. Dynamics of such clinical biochemical parameters as medium mass molecules (MMM), malondialdehyde (MDA), diene conjugates (DC) in blood serum were analyzed in preoperative period and on the 1st, 5th and 14th day of postoperative period. Significant free radical production occurred both in preoperative period and after surgical intervention. This was the reason to include antioxidant therapy using Reamberin (STPF «POLYSAN¼) in addition to basic treatment. The drug is approved for use by Central Formulary Committee of the Ministry of Health of Ukraine. All patients were divided into 2 groups depending on the treatment scheme. Group I consisted of 100 patients with acute small bowel obstruction who underwent the comprehensive treatment according to recommendations of the Ministry of Health of Ukraine № 297 dated 02.04.2010 (Standards of medical care for patients with urgent surgical diseases of the abdominal cavity). 102 patients of group II received the comprehensive treatment of antihypoxant and antioxidant therapy with Reamberin added to basic scheme. The main active ingredient of Reamberin is succinic acid. The drug was administered intravenously by drop infusion in a dose of 400 ml a day during 7-day period. Administration rate did not exceed 90 drops per minute. The medicine administration was started during complex preoperative preparation and then was done immediately after the completion of surgical treatment under resuscitation conditions. It was shown that the use of Reamberin promotes effective correction of free radical imbalance, reduction of endogenous intoxication and postoperative complications.

The aim of the study was to evaluate the effect of combined use of Cytoflavin and cognitive therapy on the dynamics of psychosomatic status and effectiveness of the treatment of generalized periodontitis in elderly patients. The results of treatment of 103 patients aged 60 and older were analyzed. All patients received standard therapy for 10-14 days depending on the severity of the process. In addition, the patients of the main group (51 patients) received Cytoflavin: 2 tabs. 2 times a day with an interval between doses of 8-10 hours for 25 days, and cognitive therapy aimed at training alternative realistic way of life perception. Treatment results were monitored by PMA, Russell periodontal index (PI), Green-Vermillion hygienic index. Psychosomatic dynamics was assessed by means of Spielberger anxiety scale and SUN" methods of before and after a month of treatment. The inclusion in the treatment regimen of Cytoflavin and cognitive therapy had a positive impact: reduction of PMA by 61.8%, PI by 45.3% and Green-Vermillion index by 39.8%. The psychosomatic tests proved anxiety level decrease by 28.7% in terms of "reactive" anxiety and 31.5% in terms of "personal" anxiety, emotional status (21.2%), activity (21.7%) and mood (21.8%) improvement.

RESUMEN

The published data on the modern concept of the biological role of succinate, an intermediate of the citric acid cycle are analysed in the review. Special interest to succinate is determined by investigations on the mitochondrial functions at different pathologies, discovery of the hypoxia-inducible factor HIF-1 and studies on the human genome, that resulted in detection of the G-protein coupled receptors, which selectively are bound with succinate. According to the published experimental data, besides participation in oxidative reactions, succinate is considered as a key contributor to physiological, metabolic and genetic processes.
