New software-hardware system for diagnostic and treatment (RF patents: number 75294, 2008 and number 75314, 2008) allowing to use the technique of microcirculatory bed regional pulse vessels control (RF patent number 2383369, 2008) for optimization of patients' rehabilitation process after dental implantation operation. It was shown the normalizing action of alternating magnetic field with the frequency of spontaneous local vessels oscillation by comparison of regional blood flow indices.

New way of using alternating magnetic field is described. Basic functional description of new treatment and diagnostic offered (patents of RF No75294 and 75314), which included digital plethysmograph and regulation of angiotonic microcirculation channel device. Comparative estimation of clinical and functional indiced of regional blood flow of patients in the main group and in the control group showed a positive correlation. Expressed influence of alternating magnetic field on frequency of oscillations pulse vessels microcircution channel and acceleration of leveling disorder of blood circulation in tissue after surgical treatment was proved.

Positive properties and perspectivity of demineralized osseous allogenic implants were pointed to. Experimental model of primary osseous plasty of mandible defects was described. The comparison of character and pace of reparative osteogenesis in experimental bone defects with Perfoost use and without transplantation was done. Perfoost had expressed osteoinductive and osteoconductive properties providing quicker reparative osteogenesis. In clinic osseous plasty operations of upper, middle and lower parts of face were done with the use of Perfoost. All in all 14 operations in 13 patients were conducted: in 3 patients - contour plasty of frontal region of head, in 5 patients- deformation correction of zygomatic region, lower edge and bottom of eye-socket; in 5 patients - defect substitution of mandible body and branch of the 8-10 cm length (in combination with titanium plates). In 2-3 months after operation the first signs of transplant rebuilding were detected during X-ray control. The expressed rebuilding of Perfoost with its substitution by newly formed bone was seen in 1-1.5 year. No complications followed, terms of observation - up to 8 years.

A brief review of the adoption and the use of reconstructive titanium plates (RTP) to make up for the defects of the mandible is presented in the article. 33 patients aged from 19 to 63 were observed. The defects of various areas of the mandible from 3 to 15 cm were made up with RTP. In 10 patients there were combinations with other materials. The periods of observation were up to 8 years. Absence of complications during the whole period of observations is stated. Secondary osteogenesis induction has been noted as a result of RTP interaction with the bone and the periosteum. Attention is paid to the necessity to observe technical requirements of RTP application. It allows to use the plates in making up for the defects of mandible (including vast defects) in the course of long (not less than 8 years) periods. Respectivity of the study of conditions and factors influencing optimum reparative regeneration of bone tissue surrounding RTP is conformed.

94 patients with odontogenous phlegmons of the face and neck were followed up in the clinic to study the mechanisms of antimicrobe nonspecific defense and morphologic peculiarities of acute purulent odontogenous inflammation. Bactericidal neutrophil activity in the focus of inflammation and phagocytosis were studied using lysosomal test. Their ability to adhesion, absorptive activity and the intensity of enzymic processes in cytoplasm were defined in the peripheral blood. Systemic immunity (cellular and humoral) and tissue morphology obtained in the course of surgical phlegmon treatment were studied. Elements of productive chronic inflammation accompanied by the tissue changes characteristic of acute purulent inflammation were found to be present. Characteristic dynamics of the indices in the neutrophil functional activity was noted. Correlation between the dynamics in the course of acute inflammatory disease and indices of bactericidal neutrophil activity has been established. A number of conclusions has been formulated. Immune complex nature of odontogenous phlegmons is suggested.

The authors studied the species-specific and quantitative composition of the large intestinal microbiocenosis on exposures to various factors of endo- and exogenous etiologies: the presence of a pathological process and its specific features, dietary trace element composition, unfavorable environmental (chemical) factors, as well as the impact of coexistence in the same family and the factors of family variability (the individual genotype of the macro-organism). The microenvironment of non spore-forming anaerobes that colonize the large bowel was found to be influenced by a number of various factors, both exo- and endogenous. The magnitude of these changes is associated with the intensity and specificity of an influencing factor.

Presented are data on morphological characteristics of maxillofacial odontogenic phlegmons observed in 98 patients with chronic periodontitis. The phlegmons have arisen in the presence of chronic inflammation of the soft tissue. Microbial flora in the inflammation focus proved opportunistic, abundant in nonclostridial anaerobic agents. Its composition contributed to morphological varieties of the phlegmons. There is a relationship between apparent destructive fasciitides and myositides and aerobic-anaerobic associations. Mechanisms of resistance to infection involve inhibited bactericidal activity of nonenzymatic cationic proteins of polymorphonuclear leukocytes reflecting nonspecific resistance in inflammation. Immunity status of the organism seems normal.

The microflora of the large intestine in patients with odontogenic phlegmons of different localization were studied. 80.8% of such patients were found to have microecological disturbances, characterized by a decrease in the number of bacteria belonging to the genera Bifidobacterium, Lactobacillus, Enterococcus and Bacteroides and by an increase in the number of opportunistic microorganisms.

Rapid techniques were applied to study functional activity of peripheral blood phagocytes in acute sick patients and upon discharge. E-rosette formation is recommended as such rapid technique, for it revealed the cells' superficial E-receptors. The nitro blue tetrazolium reduction test proved useful to probe the oxygen related bactericide mechanisms.

Patients with uni- and bilateral traumatic fractures of the mandible at risk for pyoinflammatory complications were subjected to vibration massage at frequencies identical to those of cardiovascular system. Rheographic evaluation of the blood flow showed vibration-related normalization of the regional circulation at the lesion which went faster and reached the values on the healthy side on day 14. In controls similar improvement took place only in 58% of cases. The results suggest that vibration massage at cardiovascular frequencies can effectively prevent pyoinflammatory complications in the mandible fractures.

Surgical interventions on soft tissues of the face and neck induce a nonspecific reaction of the body, activating the nonenzymic antioxidative system for regulation of lipid peroxidation processes. The activity of this system is suppressed in pyoinflammatory complications of postoperative wounds, whereas the level of free-radical processes persists high and impaired regulation of lipid peroxidation results in prolongation of all phases of the wound process and in longer wound healing. At the same time a reduction of ceruloplasmin and transferrin levels in the first 24 hrs after surgery may be used as diagnostic test in postoperative pyoinflammatory complications.

A method for making transplants for repair of abutment tissue defects and deformations based on computer technology is proposed. The technology unites surgical treatment, clinical and x-ray computer-aided diagnosis, computer processing of information and models, experiments, and production. Correspondence of the implant to repaired structures, site of implantation, and individual defect was confirmed experimentally [5], clinically [10], and by control computer-aided tomography. For the first time an implant was made by computer technology, identical to abutment tissue defect, and a method for plastic repair of defects and deformations in the skull by individually prepared implants developed and realized under clinical conditions.

The authors studied immunological status of patients with paradontosis. If included estimation of extended structure of lymphocytes and functional status of thymus. There was marked correlation between clinical of patients and immunological indices especially in severe form of paradontitis. As immunological preparation T-activin was used in injections and applications. Using of T-activin gave positive results in patient developed of paradontitis.

Bacteriological and cytological analysis of the saliva, dental plaques, and gingival fluid was carried out in 48 patients with periodontitis and 25 donors. The concentrations of proinflammatory cytokines IL-1 and TNF-alpha were increased, the levels of anti-inflammatory cytokines IL-4 and TFR beta-1 were decreased, and MIF production was suppressed in the patients in comparison with donors. Important theoretical and practical recommendations are offered.
