RESUMEN

BACKGROUND AND METHODS: The authors describe the rising of four postoperative pancreatic pseudocysts in a series of 20 patients treated for periampullary tumours by pancreaticoduodenectomy (PDT), with the closure of pancreatic stump, between 1989-1996. The operation was always performed by the pylorus-preserving technique according to Traverso-Longmire. RESULTS: In four patients (20%) 3, 4, 4, and 8 months after PDT with closure of the pancreatic stump, the authors observed the development of pseudocysts (PPC) in the pancreatic remnant. All these patients previously had a pancreatic fistula. Three symptomatic patients underwent surgical drainage of the pseudocyst in a jejunal loop (2) or in the stomach (1); in the asymptomatic patient, the clinical feature and the size of the cyst allowed for conservative treatment. In 2 cases PPC was the consequence of a pancreatic fistula evolved in acute pancreatitis of the stump. The PPC drained by cystogastrostomy appeared 4 months after the closure of the pancreatic fistula: the pancreatic stump was completely substituted by a cyst presenting a communication with the pancreatic duct. CONCLUSIONS: The authors conclude that PPC is a rare complication of the pancreatic resection that results from acute or chronic pancreatitis; sometimes pathogenesis is due to a cyst resulting from the pancreatic duct occlusion by phlogistic tissue as a consequence of the surgical suture.

The aim of this study was to evaluate the efficacy and tolerability of thiamphenicol glycinate hydrochloride (TGH) i.m. versus clarithromycin in acute lower respiratory infections due to Chlamydia pneumonia. 113 patients with suspected pneumonia were screened. 40 patients with IgM and/or IgA titers > or = 1:16 and/or IgG titers > or = 1:512 were assigned to 10 days of treatment with TGH 1500 mg daily or clarithromycin 1000 mg daily. 34 patients were considered a clinical success. 33 patients were a radiological success. 22 patients showed a decrease in IgG values. 3 patients had an increase in IgG values. Blood/urine values presented no clinically significant variations. Clinical efficacy was similar in both treatment groups. These are the first results confirming in vivo the recent in vitro evidence that TGH is effective against acute lower respiratory tract infections due to C. pneumoniae, thus representing an alternative therapy to clarithromycin.

RESUMEN

This paper describes a case of hemorrhage from an aortic leiomyosarcoma in which the patient exceptionally survived for 4.2 years after surgery. The operation consisted of resection of the aortic tract involved by the tumour and graft of a prosthesis. The diagnosis of aortic leiomyosarcoma had been based on the histological analysis and confirmed by histochemical and immunohistochemical findings.

Takayasu's arteritis is a chronic inflammatory disease of unknown etiology, which affects the aorta and its branches. The resulting lesions can be of steno-occlusive or aneurysmatic type; the latter form is relatively rare. The authors report 2 cases which came under their observation for treatment. The first was a young man of 22 years with aneurysm of the common right carotid artery and of the subclavian left intrathoracic artery. The carotid aneurysm was operated first; arterial reconstruction was ensured by means of a dacron bypass as the saphenous vein was not usable. Three months later, exeresis of the subclavian aneurysm was performed with a left thoracotomy in the IV space. The postoperative course was uneventful and no untoward event has been observed during follow-up (15 months). The second case was a young girl of 16 years with poststenotic aneurysmatic dilatation of the anonyma artery, tight stenosis of the left renal artery and occlusion of the right renal artery. The patient underwent median sternotomy and removal of the aneurysmatic wall together with the stenosis, and a dacron patch was applied. During preoperative angiography, the stenosis of the left renal artery had been treated with PTA; one month after the first operation, a right aorto-renal bypass was fashioned with the saphenous vein. Postoperative stay was free from complications on both occasions and complete normalization of the pressure values was obtained, which persists after 12 months.

BACKGROUND: Thrombosis is the most frequent late complication of surgical procedures in the aortofemoral area. In the presence of aortobifemoral bypass, graft limb occlusion generally occurs within the first two years. Various techniques have been proposed to revascularize the ischemic limb. Personal experience in the treatment of occlusions of aorto-femoral bypasses branch by femoro-femoral bypasses is reported and the immediate and long-term results are analyzed. METHODS: The study was retrospective and the medium follow-up was 51 months (1-14 years). The patients were all operated from 1976 to 1995 in the Division of Vascular Surgery of the University of Bari. The group consisted of 40 patients affected by unilateral occlusion that occurred after aorto-femoral bypass. The femoro-femoral bypass was performed using non-ringed 6 or 8 mm Dacron prostheses; the proximal anastomosis was made a few cm above the femoral anastomosis of the permeable branch of the existing aorto-bifemoral bypass and the distal anastomosis on the cross-leg profunda femoris. Postoperative follow-up consisted of clinical examination, continuous wave Doppler examination or US-color-Doppler scan in order to assess the permeability of the graft. RESULTS: The immediate results showed 2 and long-term results 9 occlusions of the bypass. No perioperative mortality was observed. CONCLUSIONS: In conclusion the authors consider the use of femoro-femoral bypass indicated in a high number of patients affected by unilateral occlusion of the aorto-bifemoral bypass.

BACKGROUND AND AIMS: The authors examine the treatment of steno-occlusive diseases of the subclavian artery using transluminal percutaneous angioplasty in order to evaluate the correct indications and analyse the results. METHODS: The classic method was used according to Gruentzig's technique, following the surgical isolation of the brachial artery. All 15 patients (11 males, 4 females, mean age 64 years old) were suffering from stenosis of the subclavian ischemia during physical exercise (13), resting (2) or vertebrobasilar insufficiency. Diagnostic evaluation took the form of bilateral sphynghomanometric measurement, echo-Doppler of the cervico-brachial vessels when resting and during exercise, as well as selective arteriography in 13 cases. Intra- and postoperative arteriographic control was always performed and the method was declared successful if residual stenosis was less than 30% with a delta AP between the two lower limbs of less than 10 mmHg. Mean follow-up was 12 months. RESULTS: The immediate results included the onset of complications linked to hematoma of the arm and two small dissections of the subclavian artery which were treated conservatively. Residual stenosis, albeit not hemodynamically significant, appeared in 26.7% of patients (4 cases). One case of total occlusion without clinical symptoms and two non-hemodynamically significant re-stenoses were observed in the long term. CONCLUSIONS: The authors discuss the indications and results of this method and compare them with the data reported in the literature.

RESUMEN

Popliteal pseudoaneurysm is an infrequent outcome of closed or penetrating injuries to the popliteal artery. An unusual case of iatrogenic right popliteal pseudoaneurysm in a female patient referred to our attention 20 days after arthroscopic meniscectomy is described.

The Authors report their own experience in treating 16 patients suffering from aneurysms of the supra-aortic trunks, 6 of the innominate artery, 4 of the common carotid artery, 3 of the intrathoracic subclavian artery and 3 of the extrathoracic subclavian artery. Eleven cases were of atherosclerotic nature, 3 from Takayasu's, 1 dysplasic and 1 mycotic. To aneurysms of the innominate artery with diameter less than 2 cm and one of the subclavian artery, in a patient with on carcinoma of the lung, was treated non surgically; all other patients were operated. The mortality was 0, and morbidity rate very low. With a mean follow-up of three years, only a patient, with Takayasu's disease, developed a recurrence and one, with mycotic aneurysm, died from persistence of the sepsis.

RESUMEN

Some problems of carotid body tumors regarding diagnosis and treatment are still controversial. From 1988 to 1998 we operated on 10 cervical paragangliomas in our Division of Vascular Surgery of Bari University. Three were bilateral and 2 were II Shamblin class. Seven tumors were considered familiar type (5 patients in the same family). Subadventitial dissection was performed in all cases after 4 vessel angiographies and Echo-dopplers. No complications occurred during the operation or post-operatively. Operative bleeding was reduced significantly by the use of subadventitial dissection which was accomplished in short times, thus avoiding more complicated and even hazardous techniques like clamping, sectioning or ligatures of the external carotid artery.

The authors have carried out a perspective randomized trial between two homogeneous groups, each one of 30 pts., consisting of 50 males and 10 females with an average age of 42 (18-65) affected by an uncomplicated inguinal hernia. Group A underwent a Bassini repair technique and group B underwent a Trabucco's operation. The early results in terms of clinical conditions and days of post-operative course and period of return to work were analysed with Shapiro-Wilk and t tests. The group B patients had a significant reduction of the post-operative period (2.3 days vs 3.46), a significant reduction of post-operative pain (present in all group A patients vs 6.6%) with rapid return to normal functions and to work. All these conditions reduce the costs of the operation for society and the health service. There were no statistically significant differences in terms of morbidity and long-term follow-up (about 9 months).

Myasthenic vocal cord dysfunction (VCD), presenting with severe inspiratory stridor, was successfully treated with nasal continuous positive airway pressure (nCPAP), thus giving the medical staff time to make the diagnosis and avoiding intubation or tracheostomy. An important sign leading to diagnosis was the very high MEF(50)/MIF(50) ratio calculated from the flow-volume loop. nCPAP treatment induced prompt remission of stridor and a sharp reduction in the MEF(50)/MIF(50) ratio from 9.90 to 1.36. A review of the literature has shown that VCD with inspiratory stridor is an unusual onset symptom of myasthenia gravis and that nCPAP treatment may avoid emergency oral/tracheal intubation and tracheostomy. After diagnosis, the patient underwent thymectomy, and today, 3 years later, he is well without any further therapy.
