RESUMEN

OBJECTIVE: To investigate the effect of dexamethasone on the expression levels of P-selectin protein in multiple organs of rats with severe acute pancreatitis (SAP). METHODS: Rats were randomly divided into sham-operated, model control, and dexamethasone-treated groups. At 3, 6, and 12 h after operation, the expression levels of P-selectin protein in one-third of the rats in each group were observed. RESULTS: In the treated group, the expression levels of P-selectin protein in the pancreas head (at 6 h), lung (at 12 h), liver (at 3 h), and spleen (at 6 and 12 h) were significantly lower than those in the model control group (P < 0.05). Additionally, the products of the staining intensity and positive rate of P-selectin protein in liver (at 3 h), lung (at 6 and 12 h), and spleen (at 12 h) in the treated group were significantly lower than those in the model control group (P < 0.05). CONCLUSIONS: Dexamethasone can inhibit P-selectin protein expression in multiple organs of SAP rats.

BACKGROUND AND AIMS: To explore the protecting effects and mechanisms of dexamethasone on spleen injury in rats with severe acute pancreatitis (SAP). METHODS: The rats were randomly divided into a model control group, treated group and sham-operated group. The contents of plasma endotoxin, serum NO, phospholipase A(2) enzyme (PLA(2)) and endothelin-1 (ET-1) were determined. The mortality rate, pathological changes and changes of Bax and Bcl-2 protein expression levels and apoptotic indexes in the spleen of rats were observed in all groups, respectively, at 3, 6 and 12 h after operation. RESULTS: Although the survival rate was significantly higher in the treated group than in the model control group, there was no significantly different between them (P > 0.05). The expression levels of Bax and Bcl-2 proteins and apoptotic indexes were significantly higher in the treated group than in the model control group at different time points (P < 0.05 or P < 0.01) while other blood indexes contents and pathological severity scores of spleen were significantly lower in the treated group than in the model control group (P < 0.05, P < 0.01 or P < 0.001). CONCLUSION: Dexamethasone can protect spleen from injury during SAP mainly by reducing the content of inflammatory mediators in blood.

Objective To investigate the clinical value of the duodenum double lumen cannula suction in duodenal rupture repair.Methods According to the operation methods,60 patients with duodenal rupture undergoing duodenal repair were divided into the surgical drainage alone group(26 cases)and double sets attract tube group (34 cases).The surgery effect and complications were observed.Results The difference of traffic flow 7 days after the operation between the two groups was statistically significant(t =7.564,P <0.05).The length of stay and hospital costs of the surgical drainage alone group were more than those of the double sets attract tube group,the differences were statistically significant(t =3.567,4.123,all P <0.05).The fistula,intra -abdominal infections,wound infec-tions,pulmonary infection rates of the surgical drainage alone group were 30.77%,42.31%,46.15%,26.92%, which were higher than 2.94%,11.76%,8.82%,2.94%,5.88% of the double sets attract tube group,the differ-ences were statistically significant(χ2 =8.949,7.330,10.950,2.986,5.116,all P <0.05).Conclusion Applica-tion of the double lumen cannula duodenal suction in duodenal rupture repair can significantly reduce the incidence of postoperative complications,has certain security,it is recommended in clinical practice.

OBJECTIVE: The objective of the study was to observe the effects of Salvia miltiorrhiza on intercellular adhesion molecule 1 (ICAM-1) protein expression in the lungs of rats with severe acute pancreatitis (SAP) or obstructive jaundice (OJ). METHODS: A total of 288 rats were used for SAP- and OJ-associated experiments. The rats were randomly divided into sham-operated, model control, and treated group. According to the difference of time points after operation, the SAP rats of each group were subdivided into 3-, 6-, and 12-hour groups, whereas the OJ rats were divided into 7-, 14-, 21-, and 28-day groups. The contents of interleukin (IL) 6, IL-18, nitric oxide, malondialdehyde, and superoxide dismutase in serum were determined, and pathological changes and ICAM-1 protein expression in the lungs were observed. RESULTS: Compared with the respective model control groups, in treated groups of SAP and OJ rats, the numbers of dead rats declined; serum superoxide dismutase content significantly increased, and serum IL-18, IL-6, and malondialdehyde contents were significantly decreased; the positive staining intensity of ICAM-1 protein in the lungs decreased significantly (P < 0.05, P < 0.01, or P < 0.001); and pathological changes in the lungs were relieved. CONCLUSIONS: Salvia miltiorrhiza plays a positive role in the protection of the lungs of SAP and OJ rats.
