RESUMEN

Screening, radiological and oscillometric tests showed, in 758 textile workers, overloaded muscles and osseo-articular system as well as degenerative and productive changes in upper limb and cervical spine. Pains and radiological as well as degenerative and productive changes in acromial girdle joints and cervical spine occurred predominantly in weaving women aged 41-50, employed for over 20 years. The disease has been found to result from occupational exposure. The authors suggest that a greater attention be paid to the prevention of overloading, degenerative and productive pathologies in textile workers.

RESUMEN

The Lódz Branch of the Polish Orthopedics and Traumatology Society (PTO i Tr.) was founded an march 10, 1951. The first Board was selected at the plenary PTO i Tr. meeting on May 7, 1951. The aim of the first Board was to make up for the inadequacies of orthopedic and trauma care in our region and assure orthopedic it's due place in the medical sciences. It also encouraged it's members to take-up scientific and social work. In the past years the number of Branch members grew from 18 to 169, proportionately to the number of new orthopedic and trauma care facilities in Lódz and it's surroundings. The members of the Branch participated in local scientific meetings a domestic PTO i Tr. congresses as well in congresses abroad. They delivered on the local meetings near 1401 research reports, which level were high. 81 members received Ph. D. and 9 of them achieve the degree of D. M. Sc. The average frequency at the scientific local meetings was 33%. From 1951 to 2000 members of Lódz Branch presented 192 papers in 23 domestic congresses of PTO i Tr. and published 325 papers as well 15 communications in organ of PTO i Tr. Lódz Branch organised three domestic meetings so called Orthopaedic Days in 1951 and 1960 as well three congresses in 1966, 1984 and 1998. Its members were also included in General Board of PTO i Tr.

RESUMEN

University Orthopedic Department in Lódz was founded on January 1, 1964. The first head of the Department was prof. E. Bartkowiak M.D., Ph.D. and prof. H. Zwierzchowski M.D., Ph.D. took over in 1979. During past years out of 38 surgeons employed 8 became head of other orthopedic-traumatologic departments. An orthopedic training completed 22 graduates, Ph.D. was received by 20. The Department took part in pre- and post-graduate teaching. 14,570 operations have been performed on 31,091 admitted patients (12,969 children, 18,122 adults). Out Patient Department helped 139,493 persons. The employees published 232 papers, 35 of them abroad. They participated in 21 congresses, meetings and symposia abroad as well as in 15 domestic PTO I TR congresses with 72 presentations and organized two of them in 1966 and 1984. They were also included in General Board of PTO I TR as well as its Lódz branch.

RESUMEN

Thirteen patients aged 36-63 years underwent surgery for full thickness rotator cuff rupture. The average follow-up was 21.1 months (range 12-36 months). Supraspinatus tendon was torn in 7 cases, supraspinatus and infraspinatus in 4, subscapularis in 1 and all three tendons were ruptured in 1 patient. Subacromial decompression and cuff reconstruction was done in 11 cases, decompression combined with cuff debridement and partial subscapularis tendon reconstruction in 1 case and subscapularis reconstruction in 1 patient. Constant Score (version corrected for age and sex) was used for shoulder function evaluation. There were 6 very good, 4 good, 1 fair and 2 bad results. Recurrence of cuff tear was diagnosed sonographically in 2 cases of both supra- and infraspinatus rupture. Statistical analysis disclosed significant correlation between extent of the tear and final functional result. Constant Score can be recommended as an objective and useful method for shoulder function assessment.

RESUMEN

Clinical, radiographic and sonographic presentation of a newborn with slipped proximal femoral epiphysis has been delineated. Characteristic sonographic changes within osteochondral junction of the femoral bone were pointed out. Great advantage of ultrasound in differential diagnosis of hip diseases in newborn has been proved.

RESUMEN

The 3 year study included 469 children with flat feet in preschool and school age from a section of the town of Lodz. In 2 separate age groups the influence of exercises and/or hindfoot supinating inserts on the deformity regression has been assessed. The results were being compared at every stage of the study with the deformity evaluations in control group of not treated children. The best results have been recorded in children doing exercises and wearing inserts--in 50 percent the deformity retreated. Little potential for self correction of this deformity indicates mandatory treatment of all children with flat feet.

Frequency of chosen clinical symptoms has been analyzed in 236 patients aged 10 to 80 years with shoulder pathology. It was found that "impingement syndrome" is characteristic for periarthritis humeroscapularis simplex. Active and passive movement restriction of the shoulder was frequently observed in periarthritis humeroscapularis simplex acuta; passive one greater than 50% was typical for capsulitis adhesiva. Crepitus at the movements, muscle wasting, impingement syndrome pain at the abduction and external rotation against resistance is indicative of periarthritis humeroscapularis destructive. Sonographic assessment of the shoulder is recommended in every case with the biceps long head rupture suspicion.

RESUMEN

The data collected at taking medical history were analyzed on the basis of clinical, radiographic and sonographic differential diagnosis in shoulder pathology. Material included 236 patients. Hedtmann and Fett classification has used. Age of the patient, duration of the condition and its relation to the trauma in a broad meaning are of paramount importance. The authors recommend sonographic examination in all cases of shoulder contusion with no bony abnormalities on radiographs and in all cases of anterior shoulder dislocation in order to assess rotator cuff condition.

RESUMEN

Results of surgical treatment of the congenital dislocation of the hip in 40 children (2-5 years) are presented. The mean observation period was 8 years. 80% of very good and good results were obtained in the included material. One-stage hip reposition -reconstruction with Dega's transiliacal osteotomy, applied to children of 2-3 years, gives positive results, adding to further development of the hip joint.

RESUMEN

The value of Sowinski coracoplasty for recurrent, trauma related dislocation of the shoulder was evaluated on the basis of multidirectional clinical, Xray, CT and ultrasound examinations in 15 patients operated on between 18 and 40 years of age. Good results were achieved in 14 cases after 2-14 years follow up. In one case of epileptic patient 4.5 years after surgery traumatic dislocation of the shoulder occurred with a fracture of incorporated bony graft. The authors have proved the coracoplasty performed to be efficient in preventing recurrent dislocation of the shoulder by means of creating new biomedical conditions around the joint. The bony graft by elongation of the coracoid process of ca 35 mm and change in its alignment imposes strong mechanical support for the head of the humerus and indirectly makes the acetabulum deeper. The graft located above anterior rim of the acetabulum increases the tension of subscapular muscle and stabilizes the capsule thus aiding to the stability of the joint. In 4 cases some adhesions between the graft and subscapularis muscle have slightly restricted external sotation of the shoulder.

RESUMEN

Three cases of rare functional psychogenic contractures in girls aged 6, 12, and 14 years have been presented. The contractures occurred suddenly, without an underlying reason and without pain. The diagnosis was given after elimination of other diseases. Recovery without recurrence was obtained after simulate rehabilitation, psychotherapy, and psychogagia for 4-8 week.

A series of 68 patients aged 3 months to 18 years treated for the sequele of septic arthritis of the hip in neonatal period at the Orthopedic Department in Lodz between 1964 and 1993 is presented. Modes of treatment depending on the age at the presentation, localization of the septic process and the extent of damage to articular structures are discussed. The results are reported in 42 cases with mean follow-up of 4 years (range 2 to 8 years). According to Hunck classification there were 36 good and 6 poor results. Closed reduction of the pathological dislocation of the hip in infants older then 6 months is not likely to succeed. Surgical intervention has never aggravated septic process. Poor results should be mainly attributed to great defects within the femoral head and neck. All the patients should be followed-up periodically for timely correction of deformities resulting from growth disturbances that increase with age.

We reviewed a series of 47 patients-43 female and 4 male-of whom 41 were available for follow-up, the average age of patients was 61 years (range, 50-72 years)-with osteoarthritic knee that were treated by high tibial osteotomy and external fixation. Patients complained of knee varus deformity-10/20 degree, pain, effusion, ligaments laxity and flexion contracture about 10-15 degrees. At a mean follow up 3 years (range, 1-5 years) the results in 15 were rated very good, the results in 17 as good, results in seven as fair and in two as poor. Complications included 5 cases of transient peroneal nerve neuropraxia and superficial infection in two patients. High tibial osteotomy is most effective in the treatment of osteoarthritis of the knee with varus deformity, when correction is made to 10 degrees valgus position. The stable external fixation enables immediate mobilization, dispenses with the use of the cast and have not required second operation for removing fixation frame.

The sequelae of arthritis of the hip in neonatal period have been analyzed in a series of 75 hips in 68 patients aged from 2 months to 14 years. The diagnosis was set on the basis of history, medical documentation, clinical and radiological examination with standard radiographs, arthrography and sonography included; sometimes as late as at surgery. In 15 joints with synovial type of septic arthritis pathological dislocation has been found. In 60 hips the inflammatory process involved proximal end of the femur. The sequelae strictly depended on localization of the pathology. On the basis of ossification disturbances, deformities and defects found within the head and neck on radiographs in 35 per cent of cases damaged epiphysis, physis and metaphysis were detected. In 23 per cent the neck was involved; in 42 per cent extensive damage to the head and neck was noted.

Radiographic assessment of the hip joint and clinical results after implantation of cementless Mittelmeier ceramic cup and metal Parchofer-Munch cup are presented. Material included 151 hips (56 ceramic cups and 95 metal cups). A-P radiograph and Engh criteria served for radiological assessment of the cup. Vertical, horizontal and angular migration as well as radiolucency in three De Lee zones were evaluated on postoperative and follow-up radiographs. Vertical migration and increased radiolucency within III De Lee zone were the most frequent findings. All types of migration and increased radiolucency in De Lee zones were observed more often after ceramic cup implantation.

RESUMEN

Radiological assessment and results after cementless stems implantation at Mittelmeier (Autophor 900 and 900S) and Parhofer-Mönch total hip replacement are presented. Engh criteria were used to assess implantation of the stem on a-p radiograph. Vertical migration, presence of ossifications around the tip of the stem within medullar cavity and radiolucency in 7 Gruen zones were evaluated. The investigation revealed that vertical migration, presence of ossifications around the tip of the stem within medullar cavity and radiolucency around the stem were more frequent after implantation of stems with no microporous surface and Autophor 900 type stems. Radiolucency appeared mainly within IV and VII Gruen zone.

Sixteen patients, aged between 6 and 29 years, underwent en bloc resection for bone tumour or tumour like conditions, supplemented by a fresh fibular autograft. The patients were observed for between 3 and 9 years showing considerable regeneration ability in the grafted fibula.
