OBJECTIVE: To describe magnetic resonance imaging (MRI) findings in musculoskeletal paracoccidioidomycosis (PCM). METHODS: Retrospective case series study after IRB approval. Two musculoskeletal radiologists reviewed in consensus the MRI findings of 11 patients with microbiologically and/or pathologically proven osteoarticular PCM. The MRI evaluation included discrimination of abnormalities in joints, bones and soft tissues. RESULTS: Mean age of patients was 29 years (10-55 years), eight men and three women. Musculoskeletal involvement was the only or the primary presentation of the disease in seven patients (63%). Osteomyelitis was the most common presentation, with seven cases (63%). Primary arthritis was found in one patient (9%). Isolated extra-articular soft tissue PCM was found in three patients: myositis (2) and subcutaneous infection (1). All cases showed regions with signal intensity higher than or similar to the signal of muscle on T1-weighted images. Penumbra sign was present in five cases (45%). T2-weighted images showed reactive soft tissue oedema in eight cases (72%). Post-gadolinium images showed peripheral (8/9) or heterogeneous (1/9) enhancement. Synovial enhancement was present in all cases of joint involvement (6/6). Lipomatosis arborescens was documented in one case of chronic knee involvement. CONCLUSION: To our knowledge, this is the first case series describing MRI findings of musculoskeletal PCM. Musculoskeletal involvement was the primary presentation of the disease in most cases, and therefore, neoplasms were initially in the differential diagnosis. Osteomyelitis was the most common presentation, often with secondary involvement of joint and or soft tissue.

OBJECTIVES: Quantify the muscle mass and body weight variation in critically ill patients and to identify associated factors. METHOD: A descriptive follow-up study. Data for demographic variables, body weight, fluid balance, daily kilocalories, the amount of sedation and muscle relaxants received and motor physiotherapy applied were collected. Three consecutive measurements were performed in the brachial biceps and quadriceps rectus by using ultrasound, upon admission and every 5 days until discharge. RESULTS: 68 patients were included. Average age was of 73.5 [57-78,5] years. The median length of stay was 9.5 [5.5 -15] days. The median 16 (SD=5.7) daily kilocalories per kg/weight, 91.2% received sedation, 44.1% received muscle relaxants and 20% received physiotherapy. The patients presented a muscle wasting of 4.9 (SD=3.9)mm, p <.001 in the brachial biceps and 5.6 (SD=4.8)mm, p <.001 in the quadriceps rectus. Regression analysis selected the length of stay and the muscle relaxants are the most influential variables in the brachial biceps muscle wasting (R2=0.4), and length of stay as the most influential in the quadriceps rectus muscle wasting (R2=0.3). Patient's mean body weight on admission was of 81.1 (SD=15)kg and 81.2 (SD=14.2)kg on discharge, p=.95. CONCLUSIONS: The critically ill patient presents a significant muscle waste related with the length of stay and the treatment received with muscle relaxants. Patients are being discharged with a similar body weight to which they were admitted but with a significant reduction of muscle mass.

RESUMEN

Eponyms reflect the history of medicine, of disease, and of physicians, but eponyms also reflect the history of the societies in which these physicians lived. Both loved and hated, eponyms are at the center of an interminable debate about whether they should continue to be used or whether they should disappear, and this debate has become more intense since some scientific societies have purged their terminologies of eponyms related to Nazism. Eponyms abound in conventional musculoskeletal radiology. In this article, rather than attempt an exhaustive review of all these eponyms, we take a few representative examples to illustrate the contributions of eponyms to medicine and the history of medicine.

Introduction: Introduction: sarcopenia is characterized by loss of muscle mass and function. Spinal disorders are a risk factor for muscle deterioration. In turn, sarcopenia is associated with adverse outcomes in the postoperative period of spinal surgery. Objectives: to evaluate sarcopenia and the relationship of muscle strength with bioimpedance parameters in the preoperative consultation. Material and methods: cross-sectional observational study. Muscle strength (dynamometry) and body composition (bioimpedance spectroscopy) were measured as part of the functional assessment of nutritional status. Sarcopenia was defined according to the European Working Group on Sarcopenia in Older People 2019 (EGWSOP2) and dynapenia/myopenia in the case of low strength or isolated muscularity. The relationship between the dynamometry values and the bioimpedance variables was summarized with the Spearman's coefficient. Results: twenty-two patients were included. The median (IQR) age was 60 years (43-65) and 72 % were women. The most frequent diagnosis and surgical procedure was spondylolisthesis (45 %) and posterior fixation (50 %). The mean body mass index (BMI) was 28.3 (± 4.59) kg/m2. The presence of sarcopenia, dynapenia and myopenia was 18.2 %, 13.6 % and 22.7 %, respectively. Dynamometry was correlated with lean tissue index: 0.61 (p 0.002); body cell mass: 0.68 (p 0.000); appendicular skeletal muscle: 0.49 (p 0.021); phase angle: 0.46 (p 0.031); and resistance index 200/5 kHz: -0.47 (p 0.028). Conclusion: muscle strength is correlated with bioimpedance parameters. Sarcopenia, dynapenia and myopenia are frequent and objectifiable in the preoperative spinal surgery consultation.


Introducción: Introducción: la sarcopenia se caracteriza por la pérdida de masa y función muscular. Los trastornos de la columna vertebral son un factor de riesgo del deterioro muscular. A su vez, la sarcopenia se asocia a resultados adversos en el postoperatorio de cirugía espinal. Objetivos: evaluar sarcopenia y la relación de la fuerza muscular con parámetros de la bioimpedancia en la consulta preoperatoria. Material y métodos: estudio observacional transversal. Se midieron fuerza muscular (dinamometría) y composición corporal (bioimpedancia por espectroscopia) en el marco de la evaluación funcional del estado nutricional. Sarcopenia se definió según el Grupo de Trabajo Europeo sobre Sarcopenia en Personas Mayores (EGWSOP2) y dinapenia/miopenia, en caso de baja fuerza o muscularidad aislada. La relación entre los valores de la dinamometría y las variables de la bioimpedancia se resumió con el coeficiente de Spearman. Resultados: se incluyeron 22 pacientes. La mediana (RIQ) de edad fue de 60 (43-65) años y el 72 %, de sexo femenino. El diagnóstico y el procedimiento quirúrgico más frecuentes fueron espondilolistesis (45 %) y fijación posterior (50 %). El índice de masa corporal (IMC) promedio fue de 28,3 (± 4,59) kg/m2. La presencia de sarcopenia, dinapenia y miopenia fue del 18,2 %, 13,6 % y 22,7 %, respectivamente. La dinamometría se correlacionó con el índice de tejido magro: 0,61 (p 0,002); masa celular corporal: 0,68 (p 0,000); músculo esquelético apendicular: 0,49 (p 0,021); ángulo de fase: 0,46 (p 0,031); e índice de resistencia 200/5 kHz: -0,47 (p 0,028). Conclusión: la fuerza muscular se correlaciona con los parámetros de la bioimpedancia. La sarcopenia, dinapenia y miopenia es frecuente y objetivable en la consulta preoperatoria de cirugía espinal.

OBJECTIVE: To investigate the overall effect of whey protein supplementation on skeletal muscle mass in adults with type 2 diabetes mellitus (T2DM). METHODS: Systematic review of reports on corporal muscle mass from clinical trials that assessed the use of whey protein supplementation by means of validated techniques in patients with T2DM. PubMed, SCOPUS, Web of Science, LILACS, and SciELO databases were searched up to April 2022. Risk of bias was assessed by the Cochrane Collaboration Risk of Bias tool. We conducted a qualitative synthesis of information. RESULTS: Four studies (424 participants) that met the selection criteria were identified out of 1,787 records. Of these, 3 studies assessed the total muscle mass using dual-energy X-ray absorptiometry (DXA), and 1 reported changes to the transverse diameter of the vastus lateralis muscle with ultrasound imaging. In the intervention groups, DXA assessments demonstrated an increase in total muscle mass in 3 studies and in the appendicular muscle mass in 2. Changes to the proportion of muscle mass were not seen in the DXA studies and only a discrete difference was seen in the comparative groups studied by ultrasound imaging. CONCLUSION: Following the administration of whey protein supplementation in patients with T2DM, a partially positive effect was seen in skeletal muscle mass gain with a moderate certainty of evidence.

INTRODUCTION: The skeletal muscle area (SMA) and prognostic nutritional index (PNI) are both considered as predictive parameters for mortality and morbidity for various type of cancer. In this study, we aimed to identify the effects of pre-operative SMA and PNI values on post-operative mortality and morbidity in patients with periampullary region tumors (PRT). METHODS: Patients between 2010 and 2020 were retrospectively analyzed and divided into two groups according to SMA and PNI cutoff values. Univariate and multivariate analysis was performed to find potential risk factors. RESULTS: The mean age was 65.94 ± 11.242 and 54 (60.6%) of the patients were male. Hypertension was found a reducing factor for morbidity in both univariate and multivariate analysis (p = 0.039; p = 0.045). Chronic obstructive pulmonary disease and low PNI were found as factors affecting mortality in univariate analysis (p = 0.046; p = 0.014). However, only low PNI was found as an enhancing factor for mortality in multivariate analysis. CONCLUSION: Although SMA is not a risk factor for post-operative morbidity and mortality, PNI can be considered as a risk factor for mortality in patients with PRT.


INTRODUCCIÓN: El área del músculo esquelético (SMA) y el índice nutricional pronóstico (PNI) se consideran parámetros predictivos de mortalidad y morbilidad para varios tipos de cáncer. En este estudio, nuestro objetivo fue identificar los efectos de los valores preoperatorios de SMA y PNI sobre la mortalidad postoperatoria. y morbilidad en pacientes con tumores de la región periampular (PRT). MÉTODOS: Los pacientes entre 2010-2020 fueron analizados retrospectivamente y divididos en dos grupos según los valores de corte de SMA y PNI. Se realizaron análisis univariados y multivariados para encontrar posibles factores de riesgo. RESULTADOS: La edad media fue de 65.94 ± 11.242 y 54 (60.6%) de los pacientes eran varones. Se encontró que la hipertensión es un factor reductor de la morbilidad tanto en el análisis univariado como en el multivariado (p = 0.039; p = 0.045). La EPOC y el PNI bajo se encontraron como factores que influyen en la mortalidad en el análisis univariante (p = 0.046; p = 0.014). Sin embargo, solo el PNI bajo se encontró como un factor potenciador de la mortalidad en el análisis multivariado. CONCLUSIÓN: Aunque la SMA no se consideró un factor de riesgo de morbilidad y mortalidad posoperatorias; La PNI puede considerarse un factor de riesgo de mortalidad en pacientes con PRT.

INTRODUCTION: The aim of this study was to determine the behaviour of ultrasound biomarkers of fascicle density and muscle strength in patients with amyotrophic lateral sclerosis (ALS). METHODS: We conducted an observational, cross-sectional pilot study of 14 patients with ALS (28.6% women) and 14 controls. Bilateral cross-sectional ultrasound scans were performed in the abductor pollicis brevis (APB) and tibialis anterior (TA) muscles, with recording of muscle thickness (MT) at rest and in contraction, and the difference in thickness. In the median, sciatic, and common peroneal nerves, we analysed the cross-sectional area (CSA), number of fascicles (NF) and fascicle density (FD). Analyses were nested by laterality. RESULTS: Intra- and interrater agreement regarding NF was very good, with a minimum detectable error of < 0.7%. In patients with ALS, MT was lower in the APB both at rest (P = .003; g-Hedges = 1.03) and in contraction (P = .017; g-Hedges = 0.78) and in TA at rest (P = .002; g-Hedges = 0.15) and in contraction (P = .001; g-Hedges = 0.46), with lower thickening capacity. In the nerves, patients displayed lower CSA, with lower NF and higher FD. Significant correlations were found between MT of the ABP and Medical Research Council (MRC) scores for muscle strength (r = 0.34; r2 = 12%; P = .011) and with revised ALS Functional Rating Scale scores (r = 0.44; r2 = 19%; P < .001). The difference in TA thickening correlated with MRC scores (r = 0.30; r2 = 15%; P = .003) and with revised ALS Functional Rating Scale scores (r = 0.26; r2 = 7%; P = .049). NF in the sciatic nerve showed a significant correlation with MRC scores (r = 0.35; r2 = 12%; P = .008). CONCLUSION: MT measurements derived from dynamic testing together with NF and FD may be useful biomarkers for monitoring patients with ALS and establishing a prognosis.

Maternal obesity has been described as a clinical entity associated with an increased incidence of metabolic diseases in the offspring, indicating a fetal programming phenomenon during this critical development period. Fetal exposure to an obesogenic environment affects multiple organs and tissues, including skeletal muscle, which is particularly susceptible to stressors from the external environment. Several studies have described alterations in the morphology and composition of skeletal muscle tissue secondary to obesogenic exposure in utero. In addition, modifications in signaling pathways related to the metabolism of energy substrates have been found in children born to mothers with obesity during pregnancy. This review addresses the current evidence describing the consequences of fetal exposure to an obesogenic maternal diet on skeletal muscle tissue, focusing on changes in tissue composition, alterations in signaling pathways related to glucose and fatty acid metabolism, mitochondrial biogenesis, and oxidative phosphorylation.


La obesidad materna se ha descrito como una entidad clínica asociada con el aumento en la incidencia de enfermedades metabólicas en el producto de la gestación, lo que indica la existencia de un fenómeno de programación fetal que se lleva a cabo durante este periodo crítico del desarrollo. La exposición del feto a un ambiente obesogénico afecta múltiples órganos y tejidos, incluyendo el músculo esquelético, el cual es particularmente susceptible a estresores del ambiente externo. Diversos estudios han descrito alteraciones en la morfología y composición del tejido muscular esquelético secundarias a una exposición obesogénica in utero. Además, se han encontrado modificaciones en vías de señalización relacionadas al metabolismo de sustratos energéticos en los productos de madres con obesidad durante la gestación. En la presente revisión se aborda la evidencia actual que describe las consecuencias de la exposición fetal a una dieta materna obesogénica sobre el tejido muscular esquelético, con especial enfoque en los cambios en la composición del tejido, las alteraciones en las vías de señalización relacionadas con el metabolismo de la glucosa y los ácidos grasos, así como la biogénesis mitocondrial y la fosforilación oxidativa.

BACKGROUND & OBJECTIVE: Diabetic peripheral neuropathy (DPN) is considered to be a risk factor for development of sarcopenia. Therefore, our study aimed to detect the association between peripheral neuropathy with skeletal muscle mass and function in type two diabetes mellitus (T2DM) patients. METHODS: A total of 176 participants, ≥45 years were included in the study. Out of 176, 60 were healthy volunteers, 60 had T2DM without neuropathy, 56 had T2DM with neuropathy. In all the participants peripheral nerve function was assessed by nerve conduction studies (Common peroneal and Sural nerve) and sarcopenia parameters were evaluated according to the Asian Working Group for Sarcopenia (AWGS) criteria. RESULTS: The present study suggested that diabetic peripheral neuropathy (DPN) was associated with decline in muscle mass, which was found only in men. Our study showed a positive correlation between appendicular skeletal muscle index (ASMI) and common peroneal nerve amplitude and sural nerve amplitude with r=0.527, p<0.05; r=0.847, p<0.001 respectively. Furthermore, in multiple linear regression analyses, we found a positive relationship between ASMI and sural nerve amplitude after adjustment for confounders like age, duration of diabetes, and HbA1C (B=0.739; p<0.001). CONCLUSION: As DPN patients are more prone to developing sarcopenia, and periodic assessment of skeletal muscle mass and function is warranted to initiate early lifestyle interventions in these patients, which will improve their quality of life.

INTRODUCTION: Introduction: protein is an essential nutrient that can be obtained through different food sources. In recent years, a growing development in the food industry of dairy products with higher protein content has emerged in the national market. Métodos: the present work analyzed the nutritional information, protein content, and amino acid profile of high protein yogurts. Five main brands of high-protein yogurt (PY) were selected, as was a sample of regular yogurt (RY), and one of whey protein isolate (WP), which underwent a proximal analysis and amino acid profile by high-performance liquid chromatography (HPLC). Results: it was observed that the protein content of the analyzed YP ranged between 7.2 and 15.1 g/portion, representing 6.8 % to 11 % of total content, twice the content of YBAT (3,4 %). Regarding carbohydrates and fats, these were found to range between 6.25 and 13.5 g/serving and 0.9 and 5.3 g/serving, respectively. Leucine content of the PY varied between 0.6 and 1.5 g/portion, which was higher than RY (0.3 g/portion) and lower than WP (2.2 g/portion). The sum of essential amino acids was found to be between 3 and 6.9 g/portion in PY, 1.7 g/serving y 8.5 g/serving in WP. Conclusion: these results show that PYs are a good alternative to promote a correct protein intake, similar to proteins for sports use, so their recommendation could be useful to promote consumption in different populations according to their needs.


INTRODUCCIÓN: Introducción: la proteína es un nutriente esencial que se puede obtener a través de diferentes fuentes alimentarias . En los últimos años, un creciente desarrollo de la industria alimentaria de productos lácteos con mayor aporte en proteínas ha surgido en el mercado nacional. Métodos: el presente trabajo analizó la información nutricional, el contenido proteico y el perfil de aminoácidos de una serie de yogures altos en proteína. Se seleccionaron 5 principales marcas de yogures altos en proteína (YP), una muestra de yogur batido (YBAT) y una de proteina de suero de leche aislado (WP) a las que se realizó un análisis proximal y un perfil de aminoácidos por cromatografía líquida de alto rendimiento (HPLC). Resultados: se pudo observar que el contenido de proteína de los YP analizados varió entre 7,2-15,1 g/porción, lo que representa del 6,8 al 11 % del contenido total, el doble del contenido del YBAT (3,4 %). Respecto a los hidratos de carbono y grasas, estos se encontraron entre 6,25 y 13,5 g/porción y 0,9 y 5,3 g/porción, respectivamente. El contenido de leucina de los YP varió entre 0,6 y 1,5 g/porción, con valores superiores a los de los YBAT (0,3 g/porción) e inferiores a los de las WP (2,2 g/porción). La suma de aminoácidos esenciales se encontró entre 3 y 6,9 g/porción en los YP, en 1,7 g/porción en los YBAT y 8,5 g/porción en las WP. Conclusión: estos resultados demuestran que los YP son una buena alternativa para promover una correcta ingesta proteica, semejante a las proteínas de uso deportivo, por lo que su recomendación podría ser útil para promover su consumo en diferentes poblaciones según sus necesidades.

Contexto e objetivo: O envelhecimento está associado a diversas alterações fisiológicas que podem afetar a aptidão física e a capacidade funcional, como a sarcopenia e a dinapenia. O presente estudo teve como objetivo descrever e comparar aspectos físicos e funcionais de adultos em relação aos critérios de sarcopenia e dinapenia. Desenho e local: Os participantes faziam parte do Projeto Misto Longitudinal de Aptidão Física e Capacidade Funcional de São Caetano do Sul, que busca avaliar e acompanhar a saúde e o desempenho físico de idosos na região. A amostra foi composta por 1480 participantes com idade acima de 50 anos e média de 66,5 anos. Métodos: Para a classificação da sarcopenia e dinapenia, foram adotados os critérios do Asian Working Group for Sarcopenia e do European Working Group on Sarcopenia in Older People. Uma análise estatística foi realizada utilizando a análise de covariância ANCOVA não paramétrica, ajustada pelo sexo, idade e índice de massa corporal (IMC), buscando comparar a capacidade funcional entre os grupos. Foi adotado um nível de significância de P < 0,05 e o software utilizado para as análises foi o SPSS versão 25. Resultados: A prevalência de sarcopenia na amostra foi de 3% e de dinapenia, 62,3%. Foram identificadas diferenças estatisticamente significativas nas variáveis de aptidão física e capacidade funcional. Conclusão: A massa muscular isoladamente pode não ser um critério adequado para definir sarcopenia em adultos, pela não-linearidade da relação entre massa muscular e capacidade físico-funcional.

ABSTRACT Introduction: Physical exercise can be an alternative for preventing and treating the harmful effects of obesity, mainly inflammatory effects on skeletal muscle and liver tissues. However, no consensus exists regarding this purpose's best physical training model. Objective: Evaluate morphological, metabolic, and inflammatory alterations in rats' skeletal and hepatic muscle tissues caused by aerobic and resistance training. Methods: 24 Wistar rats were divided into sedentary (S), aerobic (AE), and resistance training (R) groups. Blood glucose, total cholesterol, and serum triglycerides were measured periodically. After euthanasia, body mass was measured to calculate the total mass gain during the experiment. High-density lipoprotein (HDL) was measured. Adipose tissue was extracted to calculate its percentage relative to body mass and the liver, soleus, and gastrocnemius muscles for morphological analyses and concentrations of glycogen, lipids, and Tumor Necrosis Factor α (TNF-α). The Kruskall-Wallis test and Dunn's post-test were performed for statistical analysis, adopting p<0.05. Results: Both training models reduced the percentage of adipose tissue, body mass gain, and hepatic TNF-α concentration (p<0.05). AE increased serum HDL, gastrocnemius fiber diameter and reduced the fractal dimension in the soleus (p<0.05). R reduced blood glucose and serum and liver lipids, increased liver and soleus glycogen concentrations, increased gastrocnemius fiber diameter, and decreased TNF-α (p<0.05). Conclusion: Both training models reduced body mass, relative visceral adipose tissue, serum total cholesterol concentration, and liver inflammation. However, resistance training was more effective in promoting metabolic effects in the liver and skeletal muscle and reducing muscle inflammation in rats. Level of Evidence V; Expert Opinion.


RESUMEN Introducción: El ejercicio físico puede ser una alternativa para prevenir y tratar los efectos nocivos de la obesidad, principalmente los efectos inflamatorios sobre los tejidos del músculo esquelético y del hígado. Sin embargo, no existe consenso sobre cuál es el mejor modelo de entrenamiento físico para este fin. Objetivo: Evaluar las alteraciones morfológicas, metabólicas e inflamatorias del entrenamiento aeróbico y de resistencia en sobre los tejidos músculo esqueléticos y hepáticos de ratas. Métodos: 24 ratas Wistar se dividieron en grupos sedentarios (S), aeróbicos (AE) y de entrenamiento de resistencia (R). Se midieron periódicamente glucosa en sangre, colesterol total y triglicéridos. Después de la eutanasia, se midió la masa corporal para calcular la ganancia de masa total durante el experimento. Se midió la lipoproteína de alta densidad (HDL). Se extrajo tejido adiposo para calcular su porcentaje relativo a la masa corporal, así como hígado, músculos sóleo y gastrocnemio para análisis morfológicos y concentraciones de glucógeno, lípidos y Factor de Necrosis Tumoral α (TNF-α). Para el análisis estadístico fueron utilizados Kruskall-Wallis y el post-test de Dunn, adoptando p<0,05. Resultados: Ambos entrenamientos redujeron el porcentaje de tejido adiposo, masa corporal y la concentración de TNF-α hepático (p<0,05). AE aumentó el HDL sérico, el diámetro de la fibra del gastrocnemio y redujo la dimensión fractal en el sóleo (p<0,05). R redujo la glucosa en sangre y los lípidos séricos y hepáticos, aumentó las concentraciones de glucógeno hepático y sóleo, aumentó el diámetro de la fibra del gastrocnemio y disminuyó el TNF-α (p<0,05). Conclusión: Ambos modelos de entrenamiento redujeron la masa corporal, el tejido adiposo visceral relativo, la concentración sérica de colesterol total y la inflamación hepática. El entrenamiento de resistencia demostró ser más eficaz para promover los efectos metabólicos en el hígado y el músculo esquelético, además de reducir la inflamación muscular en ratas. No entanto, não há consenso quanto ao melhor modelo de treinamento físico para tal finalidade. Objetivos: Avaliar alterações morfológicas, metabólicas e inflamatórias dos treinamentos aeróbico e resistido sobre os tecidos muscular esquelético e hepático de ratos. Métodos: 24 ratos Wistar foram divididos nos grupos sedentário (S), treinamento aeróbico (AE) e resistido (R). Glicemia, colesterol total e triglicerídeos séricos foram mensurados periodicamente. Após a eutanásia, a massa corporal foi mensurada para calcular o ganho total de massa durante o experimento. A lipoproteína de alta densidade (HDL) foi dosada. O tecido adiposo foi extraído para cálculo de sua porcentagem relativa à massa corporal assim como o fígado e os músculos sóleo e gastrocnêmio para as análises morfológicas e das concentrações de glicogênio, lipídios e Fator de Necrose Tumoral α (TNF-α). Para análise estatística, foram utilizados o teste de Kruskall-Wallis e o pós-teste de Dunn, adotando-se p<0,05. Resultados: Ambos os modelos de treinamento reduziram o percentual de tecido adiposo, ganho de massa corporal e concentração hepática de TNF-α (p<0,05). AE aumentou o HDL sérico, o diâmetro das fibras do gastrocnêmio e reduziu a dimensão fractal no sóleo (p<0,05). R reduziu a glicemia e os lipídios séricos e hepáticos, aumentou a concentração de glicogênio hepático e sóleo, aumentou o diâmetro das fibras gastrocnêmicas e diminuiu o TNF-α (p<0,05). Conclusão: Ambos os modelos de treinamento reduziram a massa corporal, o tecido adiposo visceral relativo, a concentração sérica de colesterol total e a inflamação hepática. No entanto, o treinamento resistido mostrou-se mais eficaz em promover efeitos metabólicos no fígado e no músculo esquelético, além de reduzir a inflamação muscular em ratos. Introducción: la sarcopenia se caracteriza por la pérdida de masa y función muscular. Los trastornos de la columna vertebral son un factor deriesgo del deterioro muscular. A su vez, la sarcopenia se asocia a resultados adversos en el postoperatorio de cirugía espinal.Objetivos: evaluar sarcopenia y la relación de la fuerza muscular con parámetros de la bioimpedancia en la consulta preoperatoria.Material y métodos: estudio observacional transversal. Se midieron fuerza muscular (dinamometría) y composición corporal (bioimpedanciapor espectroscopia) en el marco de la evaluación funcional del estado nutricional. Sarcopenia se definió según el Grupo de Trabajo Europeosobre Sarcopenia en Personas Mayores (EGWSOP2) y dinapenia/miopenia, en caso de baja fuerza o muscularidad aislada. La relación entre losvalores de la dinamometría y las variables de la bioimpedancia se resumió con el coeficiente de Spearman.Resultados: se incluyeron 22 pacientes. La mediana (RIQ) de edad fue de 60 (43-65) años y el 72 %, de sexo femenino. El diagnóstico y elprocedimiento quirúrgico más frecuentes fueron espondilolistesis (45 %) y fijación posterior (50 %). El índice de masa corporal (IMC) promedio fuede 28,3 (± 4,59) kg/m2. La presencia de sarcopenia, dinapenia y miopenia fue del 18,2 %, 13,6 % y 22,7 %, respectivamente. La dinamometríase correlacionó con el índice de tejido magro: 0,61 (p 0,002); masa celular corporal: 0,68 (p 0,000); músculo esquelético apendicular: 0,49 (p0,021); ángulo de fase: 0,46 (p 0,031); e índice de resistencia 200/5 kHz: -0,47 (p 0,028).Conclusión: la fuerza muscular se correlaciona con los parámetros de la bioimpedancia. La sarcopenia, dinapenia y miopenia es frecuente yobjetivable en la consulta preoperatoria de cirugía espinal.(AU)


Introduction: sarcopenia is characterized by loss of muscle mass and function. Spinal disorders are a risk factor for muscle deterioration. Inturn, sarcopenia is associated with adverse outcomes in the postoperative period of spinal surgery.Objectives: to evaluate sarcopenia and the relationship of muscle strength with bioimpedance parameters in the preoperative consultation.Material and methods: cross-sectional observational study. Muscle strength (dynamometry) and body composition (bioimpedance spectroscopy)were measured as part of the functional assessment of nutritional status. Sarcopenia was defined according to the European Working Groupon Sarcopenia in Older People 2019 (EGWSOP2) and dynapenia/myopenia in the case of low strength or isolated muscularity. The relationshipbetween the dynamometry values and the bioimpedance variables was summarized with the Spearman’s coefficient.Results: twenty-two patients were included. The median (IQR) age was 60 years (43-65) and 72 % were women. The most frequent diagnosisand surgical procedure was spondylolisthesis (45 %) and posterior fixation (50 %). The mean body mass index (BMI) was 28.3 (± 4.59) kg/m2.The presence of sarcopenia, dynapenia and myopenia was 18.2 %, 13.6 % and 22.7 %, respectively. Dynamometry was correlated with leantissue index: 0.61 (p 0.002); body cell mass: 0.68 (p 0.000); appendicular skeletal muscle: 0.49 (p 0.021); phase angle: 0.46 (p 0.031); andresistance index 200/5 kHz: -0.47 (p 0.028).Conclusion: muscle strength is correlated with bioimpedance parameters. SUMMARY: The identification of children and adolescents who are at risk of sarcopenic obesity development often requires specialized equipment and expensive test procedures. Therefore, the establishment of cheaper and faster methods would be greatly useful, especially if they could be applied in the field. The study's objective was to establish if identification of female adolescents who suffer the risk of developing sarcopenic obesity can be obtained through the standing-long-jump test application. To achieve the research objectives, various anthropometric and body composition measurements were performed and lower limb explosive strength was assessed using the standing long jump fitness test. The research was conducted on a sample of 535 female respondents randomly selected from 9 elementary schools in the Skopje region of the Republic of North Macedonia. The respondents were divided into quintiles according to BMI z-scores, and the arithmetic means and SD about muscle-to-fat ratio were calculated for each quintile. The cutoff was determined based on the mean and standard deviation of the muscle-to-fat ratio for the 3rd quintile of BMI and the percentage of respondents with sarcopenic obesity was examined. The optimal cut-off value of the long jump fitness test results for predicting sarcopenic obesity in an adolescent girl showed that the area under the ROC curve was 0.781 (95 % CI 0.743-0.815). The standing-long-jump test values, on grounds of odds ratio (OR 95 % CI) about the girls at risk of sarcopenic obesity development, which was identified on muscle-to-fat ratio base, were 8.76 (4.39 - 17.54, p 0.001). It can be used to predict sarcopenic obesity presence in female adolescents, which can be vital in case of health intervention.


La identificación de niños y adolescentes que corren riesgo de desarrollar obesidad sarcopénica a menudo requiere equipos especializados y procedimientos de pruebas costosos. Por lo tanto, el establecimiento de métodos más baratos y rápidos sería de gran utilidad, especialmente si pudieran aplicarse en el campo. El objetivo del estudio fue establecer si la identificación de mujeres adolescentes que sufren riesgo de desarrollar obesidad sarcopénica se puede obtener mediante la aplicación de la prueba de salto de longitud de pie. Para lograr los objetivos de la investigación, se realizaron diversas mediciones antropométricas y de composición corporal y se evaluó la fuerza explosiva de los miembros inferiores mediante la prueba de aptitud de salto de longitud de pie. La investigación se realizó con una muestra de 535 mujeres encuestadas seleccionadas al azar de 9 escuelas primarias de la región de Skopje, en la República de Macedonia del Norte. Los encuestados se dividieron en quintiles según las puntuaciones z del IMC, y se calcularon las medias aritméticas y la DE sobre la relación músculo-grasa para cada quintil. El límite se determinó en función de la media y la desviación estándar de la relación músculo-grasa para el tercer quintil del IMC y se examinó el porcentaje de encuestados con obesidad sarcopénica. El valor de corte óptimo de los resultados de la prueba de condición física de salto de longitud para predecir la obesidad sarcopénica en una adolescente mostró que el área bajo la curva ROC fue 0,781 (IC del 95 %: 0,743-0,815). Los valores de la prueba de salto de longitud de pie, sobre la base del odds ratio (OR IC del 95 %) sobre las niñas en riesgo de desarrollar obesidad sarcopénica, que se identificó sobre la base del ratio músculo-grasa, fueron 8,76 (4,39 - 17,54, p. 0,001). Puede utilizarse para predecir la presencia de obesidad sarcopénica en adolescentes, lo que puede ser vital en caso de intervención sanitaria.

ABSTRACT Objective: To study the temporal changes of autophagy related factors in skeletal muscle of rats after exhaustive exercise and blunt trauma. Methods: Forty-two male SD rats were divided into 7 groups with 6 rats in each group: Quiet control group (C), immediately after exhaustive exercise (E0), 24 hours after exhaustive exercise (E24), 48 hours after exhaustive exercise (E48), immediately after blunt trauma (D0), 24 hours after blunt trauma (D24), 48 hours after blunt trauma (D48). All groups of rats were killed and samped respectively at different time points specified above, and the right gastrocnemius muscle was taken, which was divided into two parts, one for mRNAs of, Lamp-2, BNIP3 and NIX by real-time fluorescent quantitative PCR, and the other for p62 protein by Western blotting. Results: (1) Compared with group C, mRNA levels of p62, Lamp-2 and NIX in group E48 were significantly increased after exhaustive exercise(P<0.05), suggesting that autophagy increased in 48h after exhaustive exercise. (2) Compared with group C, p62mRNA and Lamp-2 mRNA levels were significantly increased immediately after blunt trauma(P<0.05) and decreased significantly in 48h after blunt trauma(P<0.05), suggesting that autophagy activity was enhanced immediately after blunt trauma and decreased in 48h after injury. Conclusions: Generally, there were differences at each recovery phase between blunt trauma and exhausted exercise models, and the basal autophagy factors and mitochondrial autophagy factors were also inconsistent. Basal autophagy factors p62 and Lamp-2 increased significantly 48 hours after eccentric exhaustive exercise and immediately after blunt trauma. Mitochondrial autophagy factor BNIP3 did not increase after exhaustive exercise and blunt trauma, but NIX only increased after exhaustive exercise. RESUMEN Objetivo: Estudiar los cambios temporales de los factores relacionados con la autofagia en el músculo esquelético de ratas tras el ejercicio exhaustivo y el traumatismo contuso. Métodos: Se dividieron 42 ratas SD macho en 7 grupos con 6 ratas en cada grupo: grupo de control silencioso (C), inmediatamente después del ejercicio exhaustivo (E0), 24 horas después del ejercicio exhaustivo (E24), 48 horas después del ejercicio exhaustivo (E48), inmediatamente después de un traumatismo contuso (D0), 24 horas después de un traumatismo contuso (D24), 48 horas después de un traumatismo contuso (D48). Todos los grupos de ratas fueron sacrificados y rotulados, respectivamente, en diferentes momentos especificados anteriormente, y se extrajo el músculo gastrocnemio derecho, dividido en dos partes, una para los ARNm Lamp-2, BNIP3 y NIX mediante PCR cuantitativa fluorescente en tiempo real, y la otra para la proteína p62 mediante Western blotting. Resultados: (1) En comparación con el grupo C, los niveles de ARNm de p62, Lamp-2 y NIX en el grupo E48 aumentaron significativamente tras el ejercicio exhaustivo (P<0,05), lo que sugiere que la autofagia aumentó en las 48 horas posteriores al ejercicio exhaustivo. Métodos: Quarenta e dois ratos machos SD foram divididos em 7 grupos com 6 ratos em cada grupo: Grupo de controle silencioso (C), imediatamente após o exercício exaustivo (E0), 24 horas após o exercício exaustivo (E24), 48 horas após o exercício exaustivo (E48), imediatamente após o trauma contuso (D0), 24 horas após o trauma contuso (D24), 48 horas após o trauma contuso (D48). Todos os grupos de ratos foram mortos e rotulados, respectivamente, em diferentes momentos especificados acima, e o músculo gastrocnêmio direito foi retirado, dividido em duas partes, uma para mRNAs de Lamp-2, BNIP3 e NIX por PCR quantitativo fluorescente em tempo real, e a outra para a proteína p62 por imunotransferência. Resultados: (1) Em comparação com o grupo C, os níveis de mRNA de p62, Lamp-2 e NIX no grupo E48 aumentaram significativamente após o exercício exaustivo (P<0,05), sugerindo que a autofagia aumentou em 48 horas após o exercício exaustivo. (2) Em comparação com o grupo C, os níveis de mRNA de p62mRNA e Lamp-2 foram significativamente aumentados imediatamente após o trauma contuso (P<0,05) e diminuíram significativamente em 48 horas após o trauma contuso (P<0,05), sugerindo que a atividade de autofagia foi aumentada imediatamente após o trauma contuso e diminuiu em 48 horas após a lesão. Conclusão: Houve, via de regra, diferenças em cada fase de recuperação entre os modelos de trauma contuso e de exercício exaustivo, sendo que os fatores de autofagia basal e os fatores de autofagia mitocondrial também foram inconsistentes. Objetivo: Objetiva-se avaliar a relação entre consumo de proteínas (CP), atividade física (AF) e massa muscular (MM) em indivíduos com 60 anos ou mais de idade. Metodologia: Trata-se de um estudo prospectivo a partir da linha de base e segunda onda do estudo ELSA-Brasil. O CP foi avaliado por meio de um questionário de frequência alimentar semiquantitativo (QFA). A AF foi mensurada pelo International Physical Activity Questionnaire (IPAQ). A MM foi estimada por meio de equação de predição, e calculada a diferença de MM entre a 2a e a 1a onda. Análises bivariadas foram realizadas adotando o valor de p < 0,05. Para as análises multivariadas, utilizou-se a regressão de Poisson, com quatro modelos distintos, que incluíram as covariáveis com valor de p < 0,20. Utilizou-se o pacote estatístico SPSS versão 21. Resultados: A amostra foi constituída de 2216 idosos, sendo 55,10% de mulheres, com média de idade de 65,20 ± 4,15. Indivíduos com redução de MM entre as duas ondas estão situados no primeiro quartil de consumo de proteína. Além disso, a média de AF mostrou diferença significativa entre os grupos e a AF no lazer apenas para as mulheres (p < 0,05). Após ajuste por variáveis sociodemográficas, de saúde e hábitos de vida, indivíduos com menor consumo de proteínas apresentaram risco de 1,45 (1,29 ­ 1,63) de apresentar MM diminuída. Conclusões: O menor CP e AF forte estão associados à MM diminuída, e aqueles com menor CP no primeiro e segundo quartis apresentam maior risco de possuir MM diminuída. (AU)


Objective: The objective was to evaluate the relationship between protein consumption, physical activity, and muscle mass in individuals aged ≥ 60 years. Methods: This prospective study was based on the baseline and second wave of the ELSA Brazil study. Protein consumption was assessed using a semiquantitative food frequency questionnaire. Physical activity was measured using the International Physical Activity Questionnaire. Muscle mass was estimated using a prediction equation, and the difference in MM between the first and second waves was calculated. Bivariate analyses were performed, with p < 0.05 considered significant. Multivariate analysis consisted of 4 Poisson regression models including covariates with p < 0.20. The statistical analysis was performed in IBM SPSS Statistics 21. Results: The sample included 2216 older adults, 55.10% of whom were women, with a mean age of 65.20 (SD, 4.15). Participants whose muscle mass decreased between the waves were in the first quartile of protein consumption. Mean physical activity significantly differed between the groups, while leisure-time physical activity differed only for women (p < 0.05). After adjusting for sociodemographic, health, and lifestyle variables, participants with lower protein intake had a 1.45 (1.29­1.63) relative risk of muscle mass loss. Conclusions: Lower protein consumption and higher physical activity were associated with decreased muscle mass, and those with protein consumption in the first and second quartiles are at higher risk of muscle mass loss. Introduction: Sarcopenia is defined as a decrease in muscle mass associated with the reduction of performance and physical function. It is an indicator of adverse outcomes in elderly people. Its diagnosis requires body composition data of healthy young population to establish reference values of low muscle mass. Objective: To establish a provisional cut-off point for sarcopenia in the elderly population from Caldas based on the skeletal muscle composition data of young people using electric bioimpedance. Materials and methods: Body composition data from healthy young people (18 to 35 years old) were used to calculate the skeletal muscle mass index. The Kolmogorov-Smirnov test was applied to evaluate the normality of the data and then the average and standard deviation of muscle mass index were established for both men and women. Results: Based on these data, we established as low muscle mass a skeletal muscle mass index of two standard deviations below the mean of the muscular mass index in young people. The cut-off points were 6.42 kg/m2 and 8.39 kg/m2 for women and men, respectively. Conclusion: This is the first study that offers a description of skeletal muscle body composition in young Colombians and provides cut-off points for the diagnosis of sarcopenia in the elderly population from the Central-Andean region. The data gathered are similar to those proposed by some Asian authors, which shows that the population of this Colombian region has body composition characteristics similar to those from Asia.


Introducción. La sarcopenia es la disminución de masa muscular y la consecuente reducción del rendimiento y de la función física. Su aparición puede ser indicativa de riesgo de otras condiciones en los ancianos, y su diagnóstico requiere la comparación con los datos sobre la composición corporal de la población joven sana para establecer valores de referencia de la disminución de la masa muscular.Objetivo. Establecer puntos provisionales de corte recolectados mediante bioimpedancia eléctrica para el diagnóstico de sarcopenia en ancianos caldenses a partir de los datos de la composición muscular esquelética de jóvenes.Materiales y métodos. Se utilizaron las mediciones de la composición corporal de jóvenes sanos entre los 18 y los 35 años de edad tomadas mediante bioimpedancia eléctrica con base en las cuales se estableció el índice de masa muscular esquelética. Se aplicó la prueba de Kolmogorov-Smirnov para evaluar la normalidad de los datos y se estableció el promedio de dicho índice con sus desviaciones estándar tanto para hombres como para mujeres.Resultados. A partir de estos datos, se estableció como bajo un índice de masa muscular esquelética con un valor de dos desviaciones estándar por debajo de la media del índice en jóvenes. En las mujeres el valor fue de 6,42 kg/m2 y, en hombres, de 8,39 kg/m2.Conclusión. Estos valores son similares a los determinados por algunos autores en población asiática, lo que evidencia que esta tiene características de composición corporal similares a la población de estudio. Hasta donde se sabe, este es el primer estudio que ofrece una descripción de la composición corporal muscular esquelética en jóvenes colombianos y establece puntos de corte para el diagnóstico de sarcopenia en ancianos de esta región centro-andina del país.

Introdução: a insuficiência cardíaca (IC) e a sarcopenia são condições prevalentes e inter-relacionadas, figurando como importantes causas de limitações funcionais. Objetivo: avaliar critérios de sarcopenia, e suas relações com parâmetros cardiometabólicos, em pacientes de 40 a 64 anos hospitalizados por IC. Metodologia: estudo de corte transversal com caráter analítico envolvendo indivíduos com IC confirmada. A massa muscular apendicular (MMA) foi avaliada através da absorciometria por raios-X de dupla energia (DXA), considerando-se baixa MMA mulheres com MMA/altura² <5,5 kg/m² ou MMA/índice de massa corporal (IMC) <0,512 e homens com MMA/altura² <7,0 kg/m² ou MMA/IMC <0,789. Baixa força de preensão manual (FPM) foi considerada quando <16 kg em mulheres e <27 kg em homens. Resultados: avaliou-se 109 pacientes (50,5% mulheres), com mediana de idade de 58 anos. Constatou-se baixa MMA em 41,3% e baixa FPM em 64,2%, não havendo correlação significativa entre FPM e MMA em nenhum dos gêneros. Baixa MMA se associou ao gênero masculino (68,9% versus 35,9%; p=0,001), a maiores idades (60,0 [53,0-63,0] versus 57,0 [51,3-60,0] anos; p=0,039) e maiores níveis séricos de paratormônio (48,0 [30,5-94,4] versus 29,9 [23,0-54,1] pg/mL; p=0,009). Baixa FPM se associou a maior sintomatologia cardíaca (75,7% com baixa FPM tinham classificação funcional da New York Heart Association III-IV, versus 51,3% daqueles com FPM normal; p=0,009). Conclusões: há uma relevante prevalência de sarcopenia em pacientes de 40 a 64 anos hospitalizados por IC, observando-se maior frequência de baixa MMA nos homens, associação entre baixa FPM e sintomatologia cardíaca, e maiores níveis de paratormônio naqueles com perda muscular.


Introduction: heart failure (HF) and sarcopenia are prevalent and interrelated conditions, being important causes of functional limitations. Objective: to evaluate sarcopenia criteria, and their relationship with cardiometabolic parameters, in patients aged 40­64 years hospitalized for HF. Methodology: Cross-sectional study including patients with established HF. Appendicular skeletal muscle mass (ASMM) was assessed using dual-energy X-ray absorptiometry (DXA), considering low ASMM women with ASMM/height² <5.5 kg/m² or ASMM/body mass index (BMI) <0.512 and men with ASMM/height² <7.0 kg/m² or ASMM/BMI <0.789. Low handgrip strength (HGS) was considered when <16 kg in women and <27 kg in men. Results: we evaluated 109 patients (50.5% women), with a median age of 58 years. Low ASMM was found in 41.3% and low HGS in 64.2%, with no significant correlation between HGS and ASMM in either gender. Low ASMM was associated with male gender (68.9% versus 35.9%; p=0.001), older age (60.0 [53.0-63.0] versus 57.0 [51.3-60, 0] years; p=0.039) and higher serum parathyroid hormone (48.0 [30.5-94.4] versus 29.9 [23.0-54.1] pg/mL; p=0.009). Low HGS was associated with greater cardiac symptoms (75.7% with low HGS had a New York Heart Association III-IV functional classification, versus 51.3% of those with normal HGS; p=0.009). Conclusions: there is a relevant prevalence of sarcopenia in patients aged 40­64 years hospitalized for HF, observing a higher frequency of low ASMM in men, an association between low HGS and cardiac symptoms, and higher levels of parathyroid hormone in those with muscle wasting.

RESUMEN

Pyomyositis is a deep muscle infection of bacterial origin and subacute, which can evolve with multiple intramuscular abscesses. Despite being characterized as an affection of tropical zones, its incidence in temperate zones has been increasing due to immunosuppression factors, such as infection by the human immunodeficiency virus (HIV) and some types of immunosuppressive treatments. Even though it has been known for over a century, it is a rare and potentially serious condition that can lead to septic shock and death. This is a case report of a descriptive nature, which found the relevance of knowledge about this condition for an early diagnosis, enabling positive prognostic repercussions.


A piomiosite é uma infecção muscular profunda, de origem bacteriana e caráter subagudo, que pode evoluir com abcessos intramusculares múltiplos. Apesar de ser caracterizada como uma afecção de zonas tropicais, sua incidência em zonas temperadas vem aumentando por fatores de imunossupressão, como a infecção pelo vírus da imunodeficiência humana (HIV) e alguns tipos de tratamentos imunossupressivos. Mesmo sendo conhecida há mais de um século, é uma condição rara e potencialmente grave, podendo levar ao choque séptico e óbito. Trata-se de um relato de caso de caráter descritivo, que constatou a relevância do conhecimento desta afecção para um diagnóstico precoce, possibilitando repercussões prognósticas positivas.

RESUMEN

Abstract Acute distal biceps injuries clinically present with sudden pain and acute loss of flexion and supination strength. The main injury mechanism occurs during the eccentric load of the biceps. The hook test is the most significant examination test, presenting the highest sensibility and specificity for this lesion. Magnetic resonance imaging, the gold standard imaging test, can provide information regarding integrity and identify partial and/or complete tears. The surgical treatment uses an anterior or double approach and several reattachment techniques. Although there is no clinical evidence to recommend one fixation method over the other, biomechanical studies show that the cortical button resists better to failure. Although surgical treatment led to an 89% rate of return to work in 14 weeks, the recovery of high sports performance occurred in 1 year, with unsustainable outcomes.


Resumo As lesões agudas do tendão distal do bíceps se apresentam, clinicamente, com uma dor súbita associada a perda aguda de força de flexão e supinação. Seu principal mecanismo de lesão ocorre durante contração excêntrica do bíceps. O "Hook Test" é o principal teste semiológico, sendo o mais sensível e específico. A ressonância magnética, exame padrão ouro para o diagnóstico, pode fornecer informações sobre a integridade, identificando as lesões parciais e/ou completas. O tratamento cirúrgico pode ser realizado por duas vias principais: anterior e por dupla via porém as técnicas de reinserção tendínea são diversas não havendo evidência clínica que recomende um método de fixação em detrimento ao outro; embora o botão cortical apresente maior resistência a falha nos estudos biomecânicos. Com o tratamento cirúrgico o retorno as atividades laborais foi de 89% em 14 semanas (média) porém ao esporte de alto rendimento o prazo foi longo, média de 1 ano, e não duradouro.
