RESUMEN

Cyclic peptides are attracting attention as therapeutic agents due to their potential for oral absorption and easy access to tough intracellular targets. LUNA18, a clinical KRAS inhibitor, was transformed-without scaffold hopping-from the initial hit by using an mRNA display library that met our criteria for drug-likeness. In drug discovery using mRNA display libraries, hit compounds always possess a site linked to an mRNA tag. Here, we describe our examination of the Structure-Activity Relationship (SAR) using X-ray structures for chemical optimization near the site linked to the mRNA tag, equivalent to the C-terminus. Structural modifications near the C-terminus demonstrated a relatively wide range of tolerance for side chains. Furthermore, we show that a single atom modification is enough to change the pharmacokinetic (PK) profile. Since there are four positions where side chain modification is permissible in terms of activity, it is possible to flexibly adjust the pharmacokinetic profile by structurally optimizing the side chain. The side chain transformation findings demonstrated here may be generally applicable to hits obtained from mRNA display libraries.

BACKGROUND: Accurate segmentation of lung nodules can help doctors get more accurate results and protocols in early lung cancer diagnosis and treatment planning, so that patients can be better detected and treated at an early stage, and the mortality rate of lung cancer can be reduced. PURPOSE: Currently, the improvement of lung nodule segmentation accuracy has been limited by his heterogeneous performance in the lungs, the imbalance between segmentation targets and background pixels, and other factors. We propose a new 2.5D lung nodule segmentation network model for lung nodule segmentation. This network model can well improve the extraction of edge information of lung nodules, and fuses intra-slice and inter-slice features, which makes good use of the three-dimensional structural information of lung nodules and can more effectively improve the accuracy of lung nodule segmentation. METHODS: Our approach is based on a typical encoding-decoding network structure for improvement. The improved model captures the features of multiple nodules in both 3-D and 2-D CT images, complements the information of the segmentation target's features and enhances the texture features at the edges of the pulmonary nodules through the dual-branch feature fusion module (DFFM) and the reverse attention context module (RACM), and employs central pooling instead of the maximal pooling operation, which is used to preserve the features around the target and to eliminate the edge-irrelevant features, to further improve the performance of the segmentation of the pulmonary nodules. RESULTS: We evaluated this method on a wide range of 1186 nodules from the LUNA16 dataset, and averaging the results of ten cross-validated, the proposed method achieved the mean dice similarity coefficient (mDSC) of 84.57%, the mean overlapping error (mOE) of 18.73% and average processing of a case is about 2.07 s. Moreover, our results were compared with inter-radiologist agreement on the LUNA16 dataset, and the average difference was 0.74%. CONCLUSION: The experimental results show that our method improves the accuracy of pulmonary nodules segmentation and also takes less time than more 3-D segmentation methods in terms of time.

The convolutional neural network (CNN) has become a powerful tool in machine learning (ML) that is used to solve complex problems such as image recognition, natural language processing, and video analysis. Notably, the idea of exploring convolutional neural network architecture has gained substantial attention as well as popularity. This study focuses on the intrinsic various CNN architectures: LeNet, AlexNet, VGG16, ResNet-50, and Inception-V1, which have been scrutinized and compared with each other for the detection of lung cancer using publicly available LUNA16 datasets. Furthermore, multiple performance optimizers: root mean square propagation (RMSProp), adaptive moment estimation (Adam), and stochastic gradient descent (SGD), were applied for this comparative study. The performances of the three CNN architectures were measured for accuracy, specificity, sensitivity, positive predictive value, false omission rate, negative predictive value, and F1 score. The experimental results showed that the CNN AlexNet architecture with the SGD optimizer achieved the highest validation accuracy for CT lung cancer with an accuracy of 97.42%, misclassification rate of 2.58%, 97.58% sensitivity, 97.25% specificity, 97.58% positive predictive value, 97.25% negative predictive value, false omission rate of 2.75%, and F1 score of 97.58%. AlexNet with the SGD optimizer was the best and outperformed compared to the other state-of-the-art CNN architectures.

Grasslands are essential natural and agricultural ecosystems that encompass over one-third of global lands. However, land conversion and poor management have caused losses of these systems which contributed to a 10% reduction of net primary production, a 4% increase in carbon emissions, and a potential loss of US $42 billion a year. It is, therefore, important to restore, enhance and conserve these grasslands to sustain natural plant communities and the livelihoods of those that rely on them. We installed low cost rock structures (media lunas) to assess their ability to restore grasslands by slowing water flow, reducing erosion and improving plant establishment. Our treatments included sites with small and large rock structures that were seeded with a native seed mix as well as sites with no seed or rock and sites with only seed addition. We collected summer percent cover for plants, litter, and rock and spring seedling count data. We also collected soil for nutrient, moisture, and microbial analysis. Within the first year, we found no change in plant cover between rock structures of two rock sizes. We did find, however, an increase in soil moisture, litter, fungal richness, and spring seedling germination within the rock structures, despite a historic drought. This work demonstrates that rock structures can positively impact plants and soils of grasslands even within the first year. Our results suggest that managers should seriously consider employing these low-cost structures to increase short-term plant establishment and possibly, soil health, in grasslands.

PURPOSE: Therapeutic strategies for prostate cancer (PCa) have been evolving dramatically worldwide. The current article reports on the evolution of surgical management strategies for PCa in Italy. METHODS: The data from two independent Italian multicenter projects, the MIRROR-SIU/LUNA (started in 2007, holding data of 890 patients) and the Pros-IT-CNR project (started in 2014, with data of 692 patients), were compared. Differences in patients' characteristics were evaluated. Multivariable logistic regression models were used to identify characteristics associated with robot-assisted (RA) procedure, nerve sparing (NS) approach, and lymph node dissection (LND). RESULTS: The two cohorts did not differ in terms of age and prostate-specific antigen (PSA) levels at biopsy. Patients enrolled in the Pros-IT-CNR project more frequently were submitted to RA (58.8% vs 27.6%, p < 0.001) and NS prostatectomy (58.4% vs. 52.9%, p = 0.04), but received LND less frequently (47.7% vs. 76.7%, p < 0.001), as compared to the MIRROR-SIU/LUNA patients. At multivariate logistic models, Lower Gleason Scores (GS) and PSA levels were significantly associated with RA prostatectomy in both cohorts. As for the MIRROR-SIU/LUNA data, clinical T-stage was a predictor for NS (OR = 0.07 for T3, T4) and LND (OR = 2.41 for T2) procedures. As for Pros-IT CNR data, GS ≥ (4 + 3) and positive cancer cores ≥ 50% were decisive factors both for NS (OR 0.29 and 0.30) and LND (OR 7.53 and 2.31) strategies. CONCLUSIONS: PCa management has changed over the last decade in Italian centers: RA and NS procedures without LND have become the methods of choice to treat newly medium-high risk diagnosed PCa.

Background: Fungicides describe all chemicals used to control fungi that infect plants. Luna Experience SC-400 is a new line of fungicide that consist of Fluopyram and Tebuconazole. Objective: In this study, We investigated the genotoxicty and cytotoxicty of Luna Experience-SC 400 using comet assay, micronucleus test and polychromatic erythrocytes number in rat bone marrow. The present study is the first report indicating the effects of genotoxic and cytotoxic of Luna experience SC-400 on rat bone marrow cells. Material and Methods: We used three different doses (5mg/kg, 10mg/kg, 20mg/kg) of Luna Experience SC 400 at 48 h intervals during 30 days by gavage in rats.Genotoxicity was evaluated using comet assay and micronucleus test and cytotoxicity was measured the PCE/NCE rate in rat bone marrow. Results: Based on these experimental results, we report that Luna Experience-SC 400 fungicide presents genotoxic and cytotoxic potential on rat bone marrow. There is a significant difference between negative control group and all the doses of Luna Experience-SC 400 (p < 0.05) for comet assay and micronucleus. Even moderate and high doses of fungicides seem to have reached the values of almost positive control group for Genetic Damage Index (GDI) and Damaged Cell Percentage (DCP). In this study, we also investigated the PCE/NCE rate. Fungicide caused a decrease in the level of significant in the PCE/NCE ratio (p < 0.05). Conclusion: Our in vivo study suggests that the gavage exposure to Luna experience SC 400 used in the present investigation may be genotoxic and cytotoxic in rat bone marrow in view of these findings. Because this findings is first report represented in the pesticide biology, it is important to carry out more investigations using various cytogenetic tests under different experimental conditions to definitively resolve the the possible genotoxic and cytotoxic risk associated with new generation pesticides-fungicides.

RESUMEN

Flow diversion has become a well-accepted option for the treatment of cerebral aneurysms. Given the significant treatment effect of flow diverters, numerous options have emerged since the initial Pipeline embolization device studies. In this review, the authors describe the available flow diverters, both endoluminal and intrasaccular, addressing nuances of device design and function and presenting data on complications and outcomes, where available. They also discuss possible future directions of flow diversion.

Giant silk moths (Lepidoptera: Saturniidae) typically are not well represented as larvae or adults in community level inventories of Lepidoptera, and as a result, little is known about their population dynamics. Furthermore, in recent years, many species of silk moths appear to have experienced population declines. Volatile sex pheromones are powerful sampling tools that can be used in operational conservation and monitoring programs for insects. Here, we describe the identification of the sex attractant pheromone of a giant silk moth, the luna moth Actias luna. Coupled gas chromatography-electroantennographic detection and gas chromatography-mass spectrometric analyses of extracts from pheromone glands of female luna moths supported the identification of (6E,11Z)-6,11-octadecadienal (E6,Z11-18:Ald), (6E)-6-octadecenal (E6-18:Ald), and (11Z)-11-octadecenal (Z11-18:Ald) as the compounds in extracts that elicited responses from antennae of male moths. These identifications were confirmed by synthesis, followed by testing of blends of the synthetic compounds in field trials in Ontario, Canada, and Kentucky, USA. Male moths were attracted to synthetic E6,Z11-18:Ald as a single component. Attraction appeared to be enhanced by addition of E6-18:Ald but not Z11-18:Ald, suggesting that the luna moth pheromone consists of a blend of E6,Z11-18:Ald and E6-18:Ald.

BACKGROUND: Prehabilitation is a novel strategy in preoperative management. The aim of this study was to investigate the effect of prehabilitation programs on peri- and postoperative outcomes and to verify if LUNA EMG has the capacity to monitor compliance with prehabilitation programs. METHODS: A total of seventy patients with suspected ovarian cancer were recruited between April 2021 and September 2022 and were divided into a prehabilitation group (36 patients) or a control group (34 patients). A LUNA EMG device was utilized to monitor muscle strength and tension. RESULTS: Within the prehabilitation group, we observed a significant increase in the 6-Minute Walk Test distance by 17 m (median, IQR: 0-42.5, p < 0.001) and a significant increase in muscle strength measured with LUNA EMG. In comparison to the control group, the prehabilitation group showed fewer complications according to the Clavien-Dindo classification (47.2% vs. 20.6%, p = 0.02) and shorter postoperative hospital stays (median 5.0 days [IQR: 4.0-6.2] vs. 7.0 days [IQR: 6.0-10.0], p < 0.001). CONCLUSION: Prehabilitation has a positive effect on physical capacity and muscle strength and is associated with a reduction in the number of complications after surgery. LUNA EMG can be a useful tool for monitoring patients' adherence to prehabilitation programs.

Background: The aim of the study was to assess the inter-rater and intra-rater agreement of measurements performed with the Luna EMG (electromyography) multifunctional robot, a tool for evaluation of upper limb proprioception in individuals with stroke. Methods: The study was conducted in a group of patients with chronic stroke. A total of 126 patients participated in the study, including 78 women and 48 men, on average aged nearly 60 years (mean = 59.9). Proprioception measurements were performed using the Luna EMG diagnostic and rehabilitation robot to assess the left and right upper limbs. The examinations were conducted by two raters, twice, two weeks apart. The results were compared between the raters and the examinations. Results: High consistency of the measurements performed for the right and the left hand was reflected by the interclass correlation coefficients (0.996-0.998 and 0.994-0.999, respectively) and by Pearson's linear correlation which was very high (r = 1.00) in all the cases for the right and the left hand in both the inter-rater and intra-rater agreement analyses. Conclusions: Measurements performed by the Luna EMG diagnostic and rehabilitation robot demonstrate high inter-rater and intra-rater agreement in the assessment of upper limb proprioception in patients with chronic stroke. The findings show that Luna EMG is a reliable tool enabling effective evaluation of upper limb proprioception post-stroke.

The Woven EndoBridge (WEB) and other intrasaccular flow disruptors show promise in treating intracranial aneurysms. We performed a bibliometric analysis to provide novel insights into the trends and trajectory of these devices. We systematically assessed bibliometric data such as citations, journals, study designs, open access status, and multi-institutional involvement for the top-cited articles on WEB and other disruptors. The top 100 cited studies on the WEB had citations from 7 to 144 (mean ± standard deviation 35.6 ± 29.5), while only 33 studies were published for other intrasaccular flow disruptors (4.24 ± 8.45). Of the other devices, the Contour has the most publications (n = 21). Retrospective reviews were the most common study design for both WEB and other intrasaccular devices. France published the most studies in the top 100 WEB papers (n = 35), while Germany led for other flow disruptors (n = 10). In all studies analyzed, no senior authors from Africa are present. The top 100 WEB publications had a higher mean citation count (35.6 vs. 4.24, P<0.001), higher mean citations per year (5.24 vs. 1.03, P<0.01), and a higher proportion of multi-institutional collaborations (44.0% vs. 12.1%, P<0.01) than other intrasaccular flow disruptors. In conclusion, countries with the most publications on WEB are not necessarily the ones leading the way with newer intrasaccular devices, while study designs remain similar. There is a limited contribution to the literature outside of Europe and North America. Our findings identify notable collaborators and trends, providing a snapshot of the field and a roadmap for future research.

BACKGROUND: Low-dose computed tomography (LDCT) has been shown useful in early lung cancer detection. This study aimed to develop a novel deep learning model for detecting pulmonary nodules on chest LDCT images. METHODS: In this secondary analysis, three lung nodule datasets, including Lung Nodule Analysis 2016 (LUNA16), Lung Nodule Received Operation (LNOP), and Lung Nodule in Health Examination (LNHE), were used to train and test deep learning models. The 3D region proposal network (RPN) was modified via a series of pruning experiments for better predictive performance. The performance of each modified deep leaning model was evaluated based on sensitivity and competition performance metric (CPM). Furthermore, the performance of the modified 3D RPN trained on three datasets was evaluated by 10-fold cross validation. Temporal validation was conducted to assess the reliability of the modified 3D RPN for detecting lung nodules. RESULTS: The results of pruning experiments indicated that the modified 3D RPN composed of the Cross Stage Partial Network (CSPNet) approach to Residual Network (ResNet) Xt (CSP-ResNeXt) module, feature pyramid network (FPN), nearest anchor method, and post-processing masking, had the optimal predictive performance with a CPM of 92.2%. The modified 3D RPN trained on the LUNA16 dataset had the highest CPM (90.1%), followed by the LNOP dataset (CPM: 74.1%) and the LNHE dataset (CPM: 70.2%). When the modified 3D RPN trained and tested on the same datasets, the sensitivities were 94.6%, 84.8%, and 79.7% for LUNA16, LNOP, and LNHE, respectively. The temporal validation analysis revealed that the modified 3D RPN tested on LNOP test set achieved a CPM of 71.6% and a sensitivity of 85.7%, and the modified 3D RPN tested on LNHE test set had a CPM of 71.7% and a sensitivity of 83.5%. CONCLUSION: A modified 3D RPN for detecting lung nodules on LDCT scans was designed and validated, which may serve as a computer-aided diagnosis system to facilitate lung nodule detection and lung cancer diagnosis.


A modified 3D RPN for detecting lung nodules on CT images that exhibited greater sensitivity and CPM than did several previously reported CAD detection models was established.

Transmission dynamics and the maintenance of mammarenaviruses in nature are poorly understood. Using metagenomic next-generation sequencing (mNGS) and RT-PCR, we investigated the presence of mammarenaviruses and co-infecting helminths in various tissues of 182 Mastomys natalensis rodents and 68 other small mammals in riverine and non-riverine habitats in Zambia. The Luna virus (LUAV) genome was the only mammarenavirus detected (7.7%; 14/182) from M. natalensis. Only one rodent from the non-riverine habitat was positive, while all six foetuses from one pregnant rodent carried LUAV. LUAV-specific mNGS reads were 24-fold higher in semen than in other tissues from males. Phylogenetically, the viruses were closely related to each other within the LUAV clade. Helminth infections were found in 11.5% (21/182) of M. natalensis. LUAV-helminth co-infections were observed in 50% (7/14) of virus-positive rodents. Juvenility (OR = 9.4; p = 0.018; 95% CI: 1.47-59.84), nematodes (OR = 15.5; p = 0.001; 95% CI: 3.11-76.70), cestodes (OR = 10.8; p = 0.025; 95% CI: 1.35-86.77), and being male (OR = 4.6; p = 0.036; 95% CI: 1.10-18.90) were associated with increased odds of LUAV RNA detection. The role of possible sexual and/or congenital transmission in the epidemiology of LUAV infections in rodents requires further study, along with the implications of possible helminth co-infection.

High mountain lakes and their catchments are remote ecosystems in areas with low anthropogenic disturbance. High mountain lakes integrate changes in the atmosphere and catchment areas (e.g., acid rain, airborne pollutants, climate change). The present research analyses long-term datasets of meteorological and limnological variables representing two tropical high mountain lakes, El Sol and La Luna, in Central Mexico to identify the impacts of anthropogenic disturbance (i.e., sentinels of global/climate change). The 54-year meteorological analysis showed marked interannual variability with no statistically significant air temperature or rainfall trends. However, from 2000 to 2018, the air temperature increased by 0.5 °C. Accordingly, the lake water temperature increased (Lake El Sol: 0.8 °C, Lake La Luna: 0.6 °C). Although the rainfall displayed no change, the water level decreased in both lakes (1.5 m), most likely associated with increased evapotranspiration. Unexpectedly, the dissolved inorganic nitrogen (DIN) concentration in the lakes decreased. The initial acid pH rose to close neutrality in Lake La Luna and to alkaline values in Lake El Sol. The latter may be a consequence of the lowered SOx and NOx emissions from governmental regulations promulgated to control atmospheric pollution beginning in 2000 and probably resulting in less acidic deposition. An additional explanation for the lakes' deacidification is the increased deposition of alkaline ions derived from activities at the volcano slopes. Since the atmospheric supply is the primary nitrogen source to high mountain lakes, the DIN concentration decline could reflect the reduction in atmospheric HNO3. Thus, Lakes El Sol and La Luna evidenced global change. Both lakes are inside the same crater and are subjected to similar influences; thus, they showed similar responses to global change (increasing lake water temperatures, declining water levels, higher pH value, and lower DIN concentrations). Nevertheless, their differences (e.g., catchment size, surface area, water volume, water depth, trophic status) influenced the magnitude of the impacts, with higher pH increases recorded in Lake El Sol and higher DIN concentrations in Lake La Luna.

We report a rare case of Wells syndrome in which a 61-year-old Caucasian male presented with three distinct skin lesions including a cutaneous bulla, an erythematous plaque, and a linear streak located on the patient's left anterior thigh, left dorsal wrist, and left anterior forearm, respectively. Histologic examination revealed diffuse and interstitial eosinophilic infiltrate admixed with lymphocytes and macrophages that predominantly involve the dermis. Nodular aggregates of eosinophils surrounding dermal collagen fibers suggestive of 'flame figures' were identified. Luna histochemical stain was used and highlighted the deposition of eosinophilic granules over the collagen bundles confirming the presence of flame figures. Laboratory workup revealed peripheral eosinophilia, but a comprehensive clinical evaluation failed to reveal a systemic disease and ultimately the diagnosis of eosinophilic cellulitis 'Wells Syndrome' was rendered. After a short course of immunosuppressive therapy, the patient experienced a complete resolution of the skin lesions on his last follow-up visit several weeks from the initial diagnosis. This case highlights the various clinical forms that Wells syndrome may present with and may serve as a good example for the use of Luna stain as a simple and cost-effective diagnostic tool that can help to arrive at the accurate diagnosis and inform therapy.

RESUMEN

PURPOSE: Multiview two-dimensional (2D) convolutional neural networks (CNNs) and three-dimensional (3D) CNNs have been successfully used for analyzing volumetric data in many state-of-the-art medical imaging applications. We propose an alternative modular framework that analyzes volumetric data with an approach that is analogous to radiologists' interpretation, and apply the framework to reduce false positives that are generated in computer-aided detection (CADe) systems for pulmonary nodules in thoracic computed tomography (CT) scans. METHODS: In our approach, a deep network consisting of 2D CNNs first processes slices individually. The features extracted in this stage are then passed to a recurrent neural network (RNN), thereby modeling consecutive slices as a sequence of temporal data and capturing the contextual information across all three dimensions in the volume of interest. Outputs of the RNN layer are weighed before the final fully connected layer, enabling the network to scale the importance of different slices within a volume of interest in an end-to-end training framework. RESULTS: We validated the proposed architecture on the false positive reduction track of the lung nodule analysis (LUNA) challenge for pulmonary nodule detection in chest CT scans, and obtained competitive results compared to 3D CNNs. Our results show that the proposed approach can encode the 3D information in volumetric data effectively by achieving a sensitivity >0.8 with just 1/8 false positives per scan. CONCLUSIONS: Our experimental results demonstrate the effectiveness of temporal analysis of volumetric images for the application of false positive reduction in chest CT scans and show that state-of-the-art 2D architectures from the literature can be directly applied to analyzing volumetric medical data. As newer and better 2D architectures are being developed at a much faster rate compared to 3D architectures, our approach makes it easy to obtain state-of-the-art performance on volumetric data using new 2D architectures.

Introduction Minimally invasive spine surgery has become more prevalent in recent years, but the delivery of interbody devices with small footprints may insufficiently restore the disc space, which may lead to instability and non-union. Vertically expandable interbody implants have partially addressed this limitation, but lateral fusion support remains a concern. The purpose of this study was to evaluate two-year safety and effectiveness outcomes with a multidimensional, expandable interbody fusion device (Luna 3D Interbody Fusion System, Benvenue Medical, Inc., Santa Clara, CA) that is delivered through a minimally invasive approach (6-8 mm) that expands in situ to approximate an anterior lumbar interbody fusion footprint of 25 mm diameter. Material and methods This was a retrospective, single-center study that evaluated the clinical utility of a multi-expandable interbody cage in patients undergoing posterior or transforaminal lumbar interbody fusion. Key patient-reported outcomes included back pain severity, leg pain severity, and the Oswestry Disability Index (ODI). Radiographic assessments included disc height (anterior, posterior, and average), foraminal height, segmental lordosis, subsidence, implant migration, and pseudarthrosis. Patients were followed at regular intervals over two years postprocedure. Results A total of 50 consecutive patients were treated with transforaminal lumbar interbody fusion (TLIF) using the multidimensional expandable implant. Procedural blood loss was minimal (median 200 ml) and the mean hospital stay was 2.1 days. Perioperative complications were reported in three patients and included a dural tear, postoperative ileus, and end-plate violation. All complications were successfully managed conservatively. There were no nerve root injuries or perioperative infections. Over the two-year follow-up period, one case of subsidence and one case of implant migration were noted on radiographic imaging but required no treatment. Comparing the values reported at baseline and two years, the mean ODI score decreased by 61%, back pain severity decreased by 67%, and leg pain severity decreased by 80% (all p<0.001). Comparing radiographic measures from baseline to two years, anterior disc height increased from 7.6 mm to 15.5 mm, posterior disc height increased from 2.9 mm to 10.1 mm, average disc height increased from 5.6 mm to 13.3 mm, foraminal height increased from 12.2 mm to 20.2 mm, and segmental lordosis increased from 6.2 degrees to 14.0 degrees (all changes p<0.001). One case of non-union was observed and the corresponding two-year fusion rate was 98%. Conclusions The utilization of a minimally invasive, multidimensional, expandable interbody implant was safe and effective over two years of clinical follow-up. The implant allows the surgeon to re-establish sagittal balance and to provide a larger surface area for fusion as compared to traditional minimally invasive interbody devices.

Mastomys natalensis is widespread in sub-Saharan Africa and hosts several arenavirus species, including the pathogenic zoonotic Lassa virus in West Africa. Mitochondrial lineages sub-divide the range of M. natalensis and have been associated with cryptic structure within the species. To test specificity of arenaviruses to hosts carrying these lineages, we screened 1772 M. natalensis in a large area of Tanzania where three mitochondrial lineages meet. We detected fifty-two individuals that were positive for one of three arenaviruses: Gairo, Morogoro, and Luna virus. This is the first record of Luna virus in Tanzania. We confirmed the specificity of each arenavirus to a distinct host mitochondrial lineage except for three cases in one locality at the centre of a host hybrid zone. No arenaviruses were detected in a large part of the study area. Morogoro and Gairo virus showed differences in prevalence (Morogoro virus lower than Gairo virus) and in genetic structure (Morogoro virus more structured than Gairo virus). However, both viruses have genetic neighbourhood size estimates of the same order of magnitude as Lassa virus. While differences in arenavirus and/or host evolutionary and ecological dynamics may exist, Tanzanian arenaviruses could be suited to model Lassa virus dynamics in M. natalensis.

RESUMEN

Endosaccular flow disruption is an innovative method of treating wide-necked complex aneurysms. Currently four types of devices have obtained the CE mark for use within Europe. These are the Woven EndoBridge device (WEB), the Luna Aneurysm Embolization System, the Medina Embolic Device (Medtronic), and the Contour Neurovascular System. The aim of this article is to provide an overview of these devices and to summarize the evidence in the literature pertaining to the treatment of intracranial aneurysms with them.

OBJECTIVES: The aim of this study was to compare the relative compliance and the dermatological and pulmonary outcomes when the Provox Luna system (Atos Medical, Malmö, Sweden) is added during the night to the usual tracheastoma care of laryngectomized subjects. METHODS: This was a multicenter randomized crossover trial conducted in the Netherlands Cancer Institute, Erasmus Medical Center, and Maastricht University Medical Center in The Netherlands. The study included 46 laryngectomized subjects with prior heat and moisture exchanger (HME) and adhesive experience. RESULTS: A significant improvement in the number of compliant individuals was found: Luna: n = 43 of 45 (96%); usual care: n = 35 of 46 (76%), P = 0.02. The Luna period was associated with longer intervals of daily HME use (Luna 23.2 hours [range: 15.6-24.0 hours], usual care [UC]: 21.5 hours [range: 6.0-24.0 hours], P = 0.003) and an increased frequency of skin improvement overnight (Luna 3.9 days [standard deviation (SD)]: 7.0 days), Usual Care: 8.1 days ([SD: 10.8 days], P = 0.008). Fifty-six percent (n = 26) of participants wanted to continue using the Provox Luna system at the conclusion of the study. CONCLUSION: An improvement in compliance and skin recovery overnight was observed when the Provox Luna was added to the usual adhesive and HME use. Therefore, there is utility in supplementing the usual post-total laryngectomy care with the Provox Luna system at night, particularly in the setting of compliance concerns and in subjects who desire dermatological relief overnight.
