Objective: To examine the homicide trends among young people (10-24 years), adolescents (10-19 years), and young adults (20-24 years) in 33 countries in the Americas between 2000 and 2019, with a focus on inequalities between countries in the burden of homicides. Methods: An ecological study was performed using estimated deaths from 33 countries. Age-adjusted rates, percentage change (PC), average annual percentage change (AAPC), and relative risk (RR) were estimated; besides, analysis on social inequalities was performed. Results: In the Americas between 2000 and 2019, homicide has been the leading cause of death with 54 515 deaths on average each year and an age-adjusted rate of 23.6 per 100 000 among young people. The highest rate was found in the Andean subregion (41.1 per 100 000 young people), which also produced the highest decrease (PC = -37.1% and AAPC = -2.4%) in the study period. The risk of homicide in young men is 8.1 times the risk in young women, and the risk in young adults is 2.5 times the risk in adolescents. The three countries with highest risk of homicide for young people are Venezuela (relative risk [RR] = 35.1), El Salvador (RR = 28.1), and Colombia (RR = 26.7). The estimated excess mortality was 26.8 homicides per 100 000 in the poorest 20% of countries compared to the richest 20% of countries in the period 2000-2009, and it decreased to 13.9 in the period 2010-2019. Conclusions: The results of this study add to the knowledge of homicide among young people and can be used to inform policy and programming in countries. Given the great burden of homicide on young people in the region, it is critical that prevention opportunities are maximized, beginning early in life.

Objective: To determine lung cancer mortality trends in Colombia during the period 1985-2018 in the population aged 35 years and over and identify changes in the trend. Methods: Analysis of mortality time series. The specific standardized rates by sex and age group were calculated. Using joinpoint regression, the annual percentage change in the rates was estimated and points of change were identified. Results: During the period 1985-2018, 105 553 deaths from lung cancer were reported in the population aged 35 and over. The standardized rates exhibit a downward trend during the period 1985-2005, except in people over the age of 64. Conclusions: Lung cancer death rates in Colombia are trending downward. Primary and secondary prevention measures with respect to tobacco use need to be enhanced and other risk factors, such as residential radon or occupation, monitored.


Objetivo: Determinar a evolução da mortalidade por câncer de pulmão na Colômbia no período de 1985 a 2018, na população com 35 anos de idade ou mais, e identificar mudanças na tendência. Métodos: Análise de séries temporais de mortalidade. Foram calculadas taxas específicas e padronizadas por sexo e faixa etária. Por meio da regressão joinpoint, estimou-se o percentual de variação anual das taxas e foram identificados os pontos de variação. Resultados: No período de 1985 a 2018, foram registradas 105.553 mortes por câncer de pulmão na população com 35 anos de idade ou mais. As taxas padronizadas demonstram tendência decrescente no período de 1985 a 2005, exceto para maiores de 64 anos. Conclusões: A tendência das taxas de mortalidade por câncer de pulmão na Colômbia é descendente. É necessário promover medidas de prevenção primária e secundária acerca do consumo de tabaco e monitorar outros fatores de risco, como a exposição ao radônio residencial ou a ocupação.

This study characterises maternal mortality in southern Brazil and verifies its trends between 2000 and 2018. It is an ecological time-series study, analysing secondary data from the Department of Informatics of the Unified Health System. The trend of the maternal mortality ratio (MMR) was calculated using generalised linear regression, and the ratios of the rates according to women's characteristics, with a confidence interval (CI) of 95%. The MMR in the South region went from 53.4 to 36.8 deaths per 100,000 live births from 2000 to 2018, a reduction trend of 1.2 percentage points per year. Mortality was directly related to increasing age (p < .001) and inversely related to schooling (p < .001) and predominated in non-white women (p < .001). The main cause of death was direct causes, including hypertensive disorders. Despite the reduction trend in maternal mortality in southern Brazil, the MMR observed is constantly above the recommended by the World Health Organisation.Impact StatementWhat is already known on this subject? The Maternal Mortality Ratio (MMR) is an indicator that allows an analysis of women's health in relation to the socio-economic and care characteristics of the region where they live. Between 2000 and 2015, Brazil had presented a high MMR, with around 50 deaths per 100,000 live births, while WHO considers a reasonably adequate MMR of fewer than 20 deaths per 100,000 live births.What do the results of this study add? This study updates data about MMR in the Southern Region of Brazil, the one which has the lowest rates in the country, but with variable values between the states. There was a reduction in MMR in southern Brazil between 2000 and 2018 but higher rates for women over 30 years old and in a situation of social vulnerability, as low-income and non-white women. Santa Catarina State presented stable values in the period and remained below the RMM averages of the other states during all years.What are the implications of these findings for clinical practice and/or further research? Comparing previous and current Maternal Mortality Rates in the regional context is important to adapt public health policies for the most affected population. Maternal death is still a reality for single and low-income women, who have greater difficulty in access to health care. Strategies in the Unified Health System are needed to tackle this problem.

OBJECTIVES: The objective of the study is to analyze the mortality trend in indigenous peoples in Brazil. STUDY DESIGN: The study design used is an ecological study. METHODS: A study carried out with data from the indigenous deaths recorded on the Mortality Information System in Brazil between 2000 and 2016. The Prais Winsten regression was used to analyze the trend of overall mortality in the country and in its geopolitical regions, in addition to the trend by sex, age groups, and groups of basic causes of death. RESULTS: Of all deaths, 47,806 indigenous deaths were recorded between 2000 and 2016. The majority was male (56.5%) and 27.2% in children aged younger than 5 years. The major groups of causes of death were the circulatory diseases (15.8%) and external causes (14.9%). The affections of the perinatal period (23.7%) and infectious and parasitic diseases (18.5%) were the main causes in children aged younger than 5 years. The indigenous mortality rates showed an increasing trend in the country (annual percent change [APC] = 2.37%). There was growth in both sexes and in most age ranges. Mortality from external causes (APC = 4.71%), neoplasms (APC = 4.67%), and respiratory causes (APC = 4.14%) also presented growth. The infectious and parasitic causes and genitourinary causes showed a stable trend in the studied period. CONCLUSIONS: The increasing trend of mortality in the analyzed variables showed disparities between indigenous and non-indigenous peoples in Brazil, pointing to the need for greater investment in basic social sectors, such as health, education, housing, and sanitation.

OBJECTIVE: To compare cancer mortality rates in Mexico from two national death registries that independently code and attribute cause of death. MATERIALS AND METHODS: We compared 5-year age-standardized total cancer and sitespecific cancer mortality rates (2010-2014) from Mexico's official death registry with a death registry from a disease surveillance system. We obtained age-adjusted mortality rates and 95% confidence intervals using the direct method and World Population Prospects 2010 as a standard. RESULTS: Cancer mortality estimates for Mexico were minimally affected by the use of two distinct death certificate-coding procedures. Cancer mortality was 73.3 for Instituto Nacional de Estadística y Geografía and 72.7 for System for Epidemiologic Death Statistics per 100 000 women. The corresponding estimates for men were 68.3 and 67.8. CONCLUSIONS: Mexico's low cancer mortality is unlikely to be explained by death certificate processing. Further investigations into the process of death certification and cancer registration should be conducted in Mexico.


OBJETIVO: Comparar la mortalidad por cáncer en México a partir de dos registros de mortalidad nacionales. MATERIAL Y MÉTODOS: Se comparó la tasa de mortalidad estandarizada por edad para cáncer total y por sitio específico (2010-2014) utilizando dos fuentes con diferentes métodos de procesamiento de información. Se obtuvieron tasas estandarizadas e intervalos de confianza al 95% utilizando el método directo y como población estándar el World Population Prospects 2010. RESULTADOS: Las tasas de mortalidad no se vieron afectadas por métodos distintos para procesar información. La mortalidad por cáncer en mujeres fue de 73.3 por cada 100 000 en el Instituto Nacional de Estadística y Geografía y 72.7 en el Subsistema Epidemiológico y Estadístico de Defunciones. Las estimaciones para hombres fueron 68.3 and 67.8, respectivamente. CONCLUSIONES: Es poco probable que la baja mortalidad por cáncer en México se explique por el procesamiento de la información. Es necesario realizar estudios enfocados en el proceso de certificación y registro de muerte por cáncer.

OBJECTIVE: To describe the impact of deficient cause-of-death records on trends in death by suicide and homicide, through imputation of missing information. METHODS: Observational and descriptive study of temporal trends in recorded deaths from external causes in Argentina in the period 1997-2018. For imputation of intent, logistic models were adjusted on the basis of predictive variables from the Statistical Report on Deaths. Vital statistics and population projections were used as secondary sources. RESULTS: As measured by the original data, mortality from external causes declined, specifically for homicides and accidents. These declines were more pronounced with the corrected data, since imputation of intent had a greater impact early in the period. Death by suicide increased by 8.0%; and after the correction, by 12.9%. CONCLUSIONS: The correction does not substantively alter the trend in mortality from external causes, but it does significantly increase mortality from homicide and suicide.


OBJETIVO: Este estudo procurou descrever o impacto do registro inadequado da causa de morte sobre as tendências de mortalidade por suicídio e homicídio, por meio da imputação de informações ausentes. MÉTODOS: Estudo observacional e descritivo da tendência temporal de mortalidade por causas externas registradas na Argentina no período de 1997 a 2018. Para imputar a intencionalidade, ajustamos os modelos logísticos utilizando variáveis preditivas provenientes do Relatório Estatístico de Óbitos. Utilizamos estatísticas vitais e projeções populacionais como fontes secundárias. RESULTADOS: Medida com os dados originais, a mortalidade por causas externas diminuiu em termos de homicídios e acidentes. Essa diminuição foi mais acentuada com as informações corrigidas, uma vez que a imputação teve um impacto mais forte no início do período de estudo. A mortalidade por suicídio registrou um aumento de 8,0%, que subiu para 12,9% após a correção. CONCLUSÕES: A correção não modifica substancialmente a tendência de mortalidade por causas externas, mas tem um maior impacto sobre a mortalidade por homicídios e suicídios, elevando-a.

BACKGROUND: Mortality registries are an essential data source for public health surveillance and for planning and evaluating public policy. Nevertheless, there are still large inequalities in the completeness and quality of mortality registries between and within countries. In Ecuador, there have been few nationwide evaluations of the mortality registry and no evaluations of inequalities between provinces. This kind of analysis is fundamental for strengthening the vital statistics system. METHODS: Ecological study assessing the completeness, quality and internal consistency of mortality data in the provinces of Ecuador, using 13 years of mortality data (2001-2013). Completeness was assessed using three types of death distribution methods (DDMs), quality by estimating the percentages of garbage codes and deaths with unspecified age or sex in the registered deaths, and internal consistency by estimating the percentage of deaths with reported causes of deaths considered impossible in some age-sex combinations. Finally, we propose a classification of the mortality registry in the studied areas based on completeness and quality. RESULTS: Completeness estimates (mean of the three methods used) in the provinces ranged from 21 to 87% in women and from 35 to 89% in men. The percentage of garbage codes in the provinces ranged from 21 to 56% in women and from 25 to 52% in men. Garbage coding was higher in women and in older age groups. The percentage of deaths with unspecified age or sex, and the percentage of deaths with reported causes of deaths considered impossible in some age-sex combinations was low in all the studied areas. The mortality registry could only be classified as acceptable in one area for men and one area for women. CONCLUSIONS: We found substantial inequalities by sex, geographical areas and age in the completeness and quality of the mortality registry of Ecuador. The findings of this study will be helpful to direct measures to improve Ecuador's vital statistics system and to generate strategies to reduce bias when using mortality data to analyse health inequalities in the country.

Bladder cancer is one of the most economically costly types of cancer, but few studies have evaluated its mortality considering the factors that impact this outcome. This study aimed to investigate the impact of sociodemographic factors, period, cohort, and health services on bladder cancer mortality. This ecological study analyzed bladder cancer mortality data in Brazil from 2000 to 2022 and evaluated sociodemographic variables (race, region of residence), socioeconomic variables (gross domestic product per capita, Gini index of household income per capita, number of health professionals per inhabitant, expenditure on public health services, and consultations per inhabitant), and bladder cancer diagnosis and treatment procedures. These data were subjected to statistical analysis, which revealed that after the age of 50, there was a progressive increase in the risk of bladder cancer. Indigenous people had the lowest mortality rate, while white people had a significantly greater mortality rate than black and brown people. The North Region and Northeast Region presented the lowest mortality rates, whereas the South Region presented the highest mortality rates. In the South and Southeast Regions, a higher GDP was related to lower mortality. In the South, higher mortality was associated with a lower number of consultations per inhabitant per region. Fewer bladder punctures/aspirations and bladder biopsies were associated with higher mortality rates. In oncology, more procedures, such as total cystectomy, cystoenteroplasty, and total cystectomy with a single shunt, do not reduce the mortality rate. These results can serve as guidelines for adjusting public health policies.

BACKGROUND: Stroke is a public health concern and the availability of a stroke registry would provide valuable information. Administrative hospital data and mortality registries have been previously suggested as suitable sources of information. The aim of this study was to evaluate the utility of merging data from a hospital discharge database and the mortality registry (MR) to estimate the incidence of stroke in La Rioja, Spain. METHODS: A cross-sectional study about events of stroke occurred in our region in 2009 was carried out. Patients were selected among those registered in the discharge hospital database and in the MR, using the International Classification of Diseases. The databases were merged to detect duplicated occurrences and to estimate the number of cases identified by each source. Characteristics of the study population were analyzed and crude- and age-adjusted incidence rates were assessed. RESULTS: Using both databases, 1133 stroke events were detected, 153 (13.5%) representing coincident or recurrent cases. A total of 980 incident stroke cases were obtained, 150 provided by the MR (15.3%) and 830 by the hospital discharge database (84.7%). Incidence of stroke was similar in men than in women, the ischemic stroke being the major type. Most of the study population (66%) were older than 74 years. Crude incidence rate was 304.6 per 100,000, and age-adjusted rates were 171.1 per 100,000 (adjusted to the European population) and 107.9 per 100,000 (adjusted to the World population). CONCLUSIONS: Considering both the Minimum Basic Data Set and the MR as data sources appears advisable to build a stroke registry to evaluate the incidence and to perform epidemiological surveillance of stroke.

Testicular cancer is common in young men, and early detection and multimodality treatment can lead to successful outcomes. This study aims to identify sociodemographic and risk factors associated with higher testicular cancer mortality and poorer survival rates, while examining the impact of diagnostic and treatment procedures on reducing mortality. The retrospective ecological study analyzed mortality data from testicular cancer in Brazil from 2001 to 2020. Sociodemographic variables such as marital status, age, birth period, year of death (cohort), race, and geographic region were assessed. Risk factors included cryptorchidism and pesticide exposure. Data were subjected to statistical analysis, which revealed an increasing trend in mortality after 2011 among persons born after 1976 in the 15-40 age group. Individuals in the South Region, whites, and singles had higher age-standardized mortality rates (ASMRs), while singles had lower survival rates. The Northeast region had a higher survival rate. Fungicides and insecticides increase ASMR in Brazil. Herbicides increase ASMR in the Northeast and Midwest regions and insecticides increase ASMR in the Northeast, Southeast, and Midwest regions. High rates of implementation of diagnostic procedures in the Midwest were not sufficient to reduce ASMR. No treatment procedure was associated with mortality at the national or regional level.

Introduction: Rare diseases are characterized by their low prevalence, chronically debilitating and life-threatening nature. Objective: To determine the characteristics and factors associated with mortality due to rare diseases in Chile from 2002 to 2017. Materials and methods: We conducted an analytical cross-sectional study based on secondary mortality database from the Departamento de Estadística e Información en Salud (DEIS), Ministerio de Salud de Chile (Department of Statistics and Health Information, Chile Ministry of Health) from 2002 to 2017. The specific mortality rates adjusted by age and sex were calculated. A normality analysis was conducted using the Kolmogorov-Smirnov test. In addition, a chi-square test of independence for associations and multivariate logistic regression was applied to determine the probability of death. Results: Between 2008 and 2012 there were 10,718 deaths due to rare diseases, 53.2% of them occurred among women. The average annual mortality rate was 3.9 per 100,000 inhabitants: 4.1 in women and 3.8 in men. The main causes of mortality among women were Creutzfeldt-Jakob disease, anencephaly and autoinmune hepatitis, and among men, Creutzfeldt-Jakob disease, muscular dystrophy and anencephaly. Women are 1.75 times more likely to die than men (adjusted Odds Ratio (aOR) = 1.75; 95% CI: 1.69 - 1.82). The highest probability of dying occurred among children aged 0-4 years (aOR = 15.30; 95% CI: 14.10 - 19.20). Conclusion: Overall, the burden of mortality due to rare disease was higher among women of all ages in Chile between 2002 and 2017.


Introducción. Las enfermedades huérfanas se caracterizan por su baja prevalencia, comúnmente son de evolución crónica, debilitantes y potencialmente mortales. Objetivo. Determinar las características y los factores asociados a la mortalidad por enfermedades huérfanas en Chile, entre 2002 y 2017. Materiales y métodos. Es un estudio transversal y analítico a partir de datos secundarios oficiales del Departamento de Estadística e Información en Salud (DEIS) del Ministerio de Salud de Chile. Se calcularon las tasas de mortalidad específica, y las ajustadas por sexo y edad. Se efectuó un análisis de normalidad mediante la prueba de Kolmogórov-Smirnov. Se aplicaron la prueba de ji al cuadrado de independencia para las asociaciones y el análisis de regresión logística multivariada para determinar la probabilidad de muerte. Resultados. Durante el periodo de estudio, 10.718 defunciones se atribuyeron a enfermedades huérfanas; 53,2 % ocurrieron en mujeres. La tasa media anual de mortalidad fue de 3,9 por 100.000 habitantes: 4,1 en mujeres y 3,8 en hombres. Las principales causas de muerte, en mujeres, fueron enfermedad de Creutzfeldt-Jakob, anencefalia, hepatitis autoinmunitaria y, en hombres, enfermedad de Creutzfeldt-Jakob, distrofia muscular y anencefalia. Las mujeres tienen 1,75 más veces la posibilidad de fallecer por este grupo de enfermedades en comparación con los hombres (OR ajustado=1,75; IC95% 1,69-1,82). La mayor probabilidad de morir se presentó en los menores de 0 a 4 años (OR ajustado=15,30; IC95% 14,10-19,20). Conclusión. En Chile, las mujeres constituyeron el grupo de población de mayor riesgo de morir por enfermedades huérfanas durante los años 2002 y 2017.

Resumo Objetivo Analisar a capacidade intrínseca e sua capacidade preditiva sobre a mortalidade por todas as causas em pessoas idosas residentes em uma microrregião de saúde de Minas Gerais, Brasil. Método Estudo tipo inquérito domiciliar, de abordagem quantitativa. Os dados foram coletados no domicílio com instrumentos validados no país. Resultados A média do escore de capacidade intrínseca foi de 5,8 (±1,63) pontos. No período de acompanhamento, com média de tempo de seguimento de 58,0 (±13,1) meses, ocorreram 142 óbitos (16,4%). Observou-se que para cada acréscimo no escore de capacidade intrínseca, ocorreu diminuição do risco de mortalidade em 21% (p<0,001). Conclusão A capacidade intrínseca exerceu papel preditivo sobre a mortalidade das pessoas idosas.


Abstract Objective To analyze intrinsic capacity and its ability to predict all-cause mortality in older adults living in a health microregion of Minas Gerais state, Brazil. Method A household survey study with a quantitative approach was conducted. Data were collected at homes using instruments validated for use in Brazil. Results Mean intrinsic capacity score was 5.8 (±1.63) points. During the follow-up period, with a mean follow-up time of 58.0 (±13.1) months, 142 deaths (16.4%) occurred. For each 1 point increase in intrinsic capacity score, there was a 21% reduction in the risk of mortality (p<0.001). Conclusion Intrinsic capacity served as a predictor of mortality in older adults.

ABSTRACT INTRODUCTION Lung cancer (LC) is a relevant public health problem in Brazil and worldwide, given its high incidence and mortality. Thus, the objective of this study is to analyze the distribution of smoking and smoking status according to sociodemographic characteristics and disparities in access, treatment, and mortality due to LC in Brazil in 2013 and 2019. METHOD Retrospective study of triangulation of national data sources: a) analysis of the distribution of smoking, based on the National Survey of Health (PNS); b) investigation of LC records via Hospital-based Cancer Registry (HCR); and c) distribution of mortality due to LC in the Mortality Information System (SIM). RESULTS There was a decrease in the percentage of people who had never smoked from 2013 (68.5%) to 2019 (60.2%) and in smoking history (pack-years). This was observed to be greater in men, people of older age groups, and those with less education. Concerning patients registered in the HCR, entry into the healthcare service occurs at the age of 50, and only 19% have never smoked. While smokers in the population are mainly Mixed-race, patients in the HCR are primarily White. As for the initial stage (I and II), it is more common in White people and people who have never smoked. The mortality rate varied from 1.00 for people with higher education to 3.36 for people without education. Furthermore, White people have a mortality rate three times higher than that of Black and mixed-race people. CONCLUSION This article highlighted relevant sociodemographic disparities in access to LC diagnosis, treatment, and mortality. Therefore, the recommendation is to strengthen the Population-Based Cancer Registry and develop and implement a nationwide LC screening strategy in Brazil since combined prevention and early diagnosis strategies work better in controlling mortality from the disease and continued investment in tobacco prevention and control policies.


RESUMO INTRODUÇÃO O câncer de pulmão (CP) é um relevante problema de saúde pública no Brasil e no mundo, dada sua alta incidência e mortalidade. Assim, objetiva-se analisar a distribuição do tabagismo e da carga tabágica segundo características sociodemográficas e disparidades no acesso, no tratamento e na mortalidade por CP no Brasil, em 2013 e 2019. MÉTODO Estudo retrospectivo de triangulação de fontes de dados de abrangência nacional: a) análise da distribuição do tabagismo, baseada na Pesquisa Nacional de Saúde (PNS); b) investigação dos registros de CP, via Registros Hospitalares de Câncer (RHC); e c) distribuição da mortalidade por CP, no Sistema de Informação sobre Mortalidade (SIM). RESULTADOS Verificou-se redução do percentual de pessoas que nunca fumaram de 2013 (68,5%) para 2019 (60,2%), assim como da carga tabágica (anos-maço). Esta foi observada maior em homens em pessoas de faixas etárias mais avançadas e de menor escolaridade. Em relação aos pacientes registrados no RHC, a entrada no serviço de saúde se dá a partir de 50 anos, e apenas 19% nunca fumaram. Ao passo que os fumantes na população são majoritariamente pardos, os pacientes no RHC são em maioria brancos. Quanto ao estadiamento inicial (I e II), é mais frequente em pessoas brancas e que nunca fumaram. A taxa de mortalidade apresentou variação de 1,00, para pessoas com ensino superior, a 3,36, entre pessoas sem instrução, assim como pessoas brancas têm uma taxa de mortalidade três vezes maior que a de pessoas negras e pardas. CONCLUSÃO Este artigo apontou relevantes disparidades sociodemográficas no acesso ao diagnóstico, tratamento e mortalidade do CP. Assim, recomenda-se: fortalecer o Registro de Câncer de Base Populacional; desenvolver e implementar estratégia de screening de CP no Brasil, uma vez que a realização de estratégias de prevenção e diagnóstico precoce combinadas funcionam melhor no controle da mortalidade pela doença; e investimento contínuo nas políticas de prevenção e controle do tabagismo.

Resumo: Avaliou-se a influência da variação da implantação do Sistema de Informações sobre Mortalidade (SIM) nos resultados antes e depois da intervenção para aprimoramento do sistema em Pernambuco, Brasil. Foram explicitados o modelo lógico e a matriz de indicadores e julgamento do SIM. Coletaram-se dados primários nos 184 municípios e dados secundários na base do sistema. Obteve-se o grau de implantação (GI) a partir de indicadores de estrutura e processo, posteriormente relacionado aos de resultado, com base no modelo. A intervenção foi direcionada às falhas identificadas e se desenvolveu mediante etapas estratégicas. Foi calculado o percentual de variação anual do GI e dos resultados antes e depois da intervenção. Classificou-se o SIM como parcialmente implantado nas avaliações pré (70,6%) e pós-intervenção (73,1%), com incrementos em todos os componentes. As Regiões de Saúde seguiram a mesma classificação do âmbito estadual, excetuando-se a XII (80,3%), com escore implantado, após a intervenção. Cobertura do sistema, óbitos com causa básica definida, transferência mensal e envio oportuno de dados situaram-se acima de 90% nas duas avaliações. Houve melhora na completude das Declarações de Óbito infantil e no registro oportuno de eventos notificáveis. O fortalecimento da gestão e operacionalização do SIM por meio de intervenções aplicadas no contexto de produção dos dados pode aprimorar os resultados do sistema.


Abstract: This study evaluated the influence of the variation in the implementation of the Brazilian Mortality Information System (SIM) on the results, before and after the intervention to improve the system in Pernambuco, Brazil. The SIM logical model and matrix of indicators and assessment were described, primary data were collected from the 184 municipalities and secondary data were collected from the system database. The degree of implementation (DI) was obtained from the indicators of structure and process, and then related to result indicators, based on the model. The intervention was directed at the shortcomings identified, and developed using strategic stages. The percentage of annual variation of the DI and the results before and after the intervention were calculated. The SIM was classified as partially implemented in the pre- (70.6%) and post-intervention (73.1%) evaluations, with increments in all components. The Health Regions followed the same classification of the state level, except for XII (80.3%), regarding implemented score after the intervention. The coverage of the system; deaths with a defined underlying cause; monthly transfer; and timely submission of data were above 90% in both evaluations. There was an improvement in the completeness of infant Death Certificates and in the timely recording of notifiable events. Strengthening the management and operationalization of the SIM with interventions applied to data registration can improve the system's results.


Resumen: Se evaluó la influencia de la variación de la implantación del Sistema de Informaciones sobre la Mortalidad (SIM) en los resultados, antes y después de la intervención para mejorar el sistema en Pernambuco, Brasil. Se explicitaron el modelo lógico y la matriz de indicadores y juicio del SIM, recolectando datos primarios en los 184 municipios y datos secundarios en la base del sistema. Se obtuvo el grado de implantación (GI) a partir de indicadores de estructura y proceso, posteriormente, relacionado con los indicadores de resultado, basado en el modelo. La intervención se dirigió a los fallos identificados y se desarrolló a través de etapas estratégicas. Se calcularon el porcentaje de variación anual del GI y de los resultados antes y después de la intervención. El SIM se clasificó como parcialmente implantado en las evaluaciones previas (70,6%) y posteriores a la intervención (73,1%) con aumento en todos los componentes. Las Regiones de Salud han seguido la misma clasificación del ámbito estatal, salvo la XII (80,3%), con puntaje implantado, después de la intervención. La cobertura del sistema; óbitos debido a causa básica definida; transferencia mensual y envío oportuno de datos, estuvieron por encima del 90% en ambas evaluaciones. Hubo una mejora en la exhaustividad de los Certificados de Muerte infantiles y en el registro oportuno de eventos notificables. Fortalecer la gestión y la implementación del SIM a través de intervenciones aplicadas en el contexto de producción de datos puede mejorar los resultados del sistema.

Introduction: Rare diseases are characterized by their low prevalence, often of genetic origin, degenerative and life threatening. Objective: To describe mortality by orphan diseases and to analyze its trends in Colombia from 2008 to 2013. Materials and methods: We conducted a descriptive study to analyze mortality rate trends from the death certificates between 2008 and 2013. We calculated specific mortality rates and adjusted by age and sex. Results: Seven thousand one hundred and thirty five deaths were attributed to orphan diseases, and 51.4 % of them occurred among men of all ages. The mean mortality rate during the study period was 2.53 deaths per 100,000 people. Overall, the trend showed an increasing pattern of mortality although very heterogeneous across the country. Mortality rates were higher in Bogotá (20), and the Andes and the Caribbean regions (5.3 and 3.7 deaths per 100,000 population). The five most important causes of mortality among men were: acute lymphoblastic leukemia, muscular dystrophy, bronchopulmonary dysplasia originating in the perinatal period, multiple sclerosis, Guillain-Barré syndrome and gastroschisis, and among women: multiple sclerosis, acute lymphoblastic leukemia, gastroschisis, bronchopulmonary dysplasia originating in the perinatal period, Guillain-Barré syndrome and acute myeloid leukemia. The mean mortality rate by acute lymphoblastic leukemia was 0.17 deaths per 100,000 men younger than 15 years and that of multiple sclerosis was 0.16 in women over 40 years of age. Conclusion: The causes of death showed a similar pattern in both sexes. However, the burden of mortality was higher among men of all ages in Bogota.

Introduction: College students represent an important subpopulation of the United States, with over 19 million college students in the U.S. enrolled yearly. Methods: Descriptive analysis of the causes of death for all deceased students reported by the UW Dean of Students Office (DSO) between 2004 and 2018. We analyzed frequencies and yearly rates. Results: Our analysis shows that contrary to published data and national statistics for the relevant age groups, intentional by self-harm deaths lead causes of death in enrolled students from 2004 to 2018. Intentional by self-harm is the main cause of death in male students, younger students, and white students. "Other" causes of death is the main cause in female students, older students, and students of color. Conclusions: These results must be shared with different stakeholders across campus as well as with other universities in order to support and evaluate campus-wide prevention strategies for means restriction and environmental safety.


Introducción: Los estudiantes universitarios representan una subpoblación importante de los Estados Unidos, con más de 19 millones de matriculados anualmente. Sin embargo, hay pocos datos publicados sobre la mortalidad y causas de muerte en la población universitaria. El propósito de este estudio fue analizar las causas de muerte, basadas en datos de certificados de defunción, de estudiantes matriculados en University of Winconsin- Madison desde 2004 hasta 2018. Métodos: Análisis descriptivo de las causas oficiales de muerte de todos los estudiantes fallecidos reportados por la Oficina del Decano de Estudiantes entre 2004 y 2018. Se analizaron frecuencias y tasas anuales. Resultados: El análisis muestra que, contrariamente a los datos publicados y las estadísticas nacionales para los grupos de edad relevantes, las muertes intencionales por autolesión lideran las causas de muerte en los estudiantes matriculados entre esos años. Las autolesiones intencionales son la principal causa de muerte en los estudiantes varones, en los estudiantes más jóvenes y en los estudiantes blancos. Las causas incluidas en la categoría indicada como Otras son las principales en las estudiantes mujeres, en estudiantes mayores y en estudiantes de color. Conclusiones: Los resultados de este estudio deben compartirse con las diferentes áreas interesadas en todo el campus universitario y con otras instituciones universitarias, para apoyar y evaluar las estrategias de prevención, la aplicación de los medios de restricción y la seguridad ambiental.

Introducción:La inmunoterapia con pembrolizumab ha mejorado el pronóstico del cáncer de pulmón metastásico. En el presente caso se presenta la supervivencia extendidad y evolución de un paciente específico.Caso clínico:Hombre de 66 años, fumador. Diagnosticado de masa pulmonar en lóbulo infe-rior izquierdo de dimensiones 9 x 8 cm, con metástasis supra e infratentoriales intraaxiliares. Taller diagnóstico: Establecida como neoplasia de pulmón en estadio IVc, se comprobó el estado de PDL1 que positivo en un 80 % de la muestra de masa pulmonar. Debuta con me-tástasis cerebrales.Evolución: Se inció inmunoterapia con Pembrolizumab, el cual se mantubo hasta la presencia de un efecto secundario atribuido al pembrolizumab, cumpliendo 30meses de supervivencia hasta el cierre de esta observación no se reportó la muerte del paciente.Conclusiones:En el presente reporte, la determinación del biomarcador histológico PDL1 po-sitivo en cáncer de pulmón ayudo a prescribir un tratamiento con inmunoterpia dirigida, lo que demostró aumentar la supervivencia más allá que el tratamiento convencional con quimiote-rapia


Introduction: Immunotherapy with pembrolizumab has improved the prognosis of metastatic lung cancer. A specific patient's extended survival and evolution is presented in the present case.Clinical case: 66-year-old man, smoker. Diagnosed with a lung mass in the left lower lobe measuring 9 x 8 cm, with supra and infratentorial intra-axial metastases.Diagnostic workshop: To establisha stage IVc lung neoplasm, 80% of the lung mass sample was confirmed to be positive for PDL1.Evolution: Immunotherapy was started with Pembrolizumab, which was maintained until the presence of a side effect attributed to pembrolizumab, completing 30 months of survival until the closure of this observation, the patient's death was not reported.Conclusions: In the present report, the determination of the positive histological biomarker PDL1 in lung cancer helped prescribe treatment with targeted immunotherapy, which was shown to increase survival beyond conventional treatment with chemotherapy

Introducción:El cáncer representa un extraordinario problema para la Salud Pública mundial, suponiendo la primera causa de mortalidad global. El cáncer de pulmón es el más frecuente. El número de defunciones se ha ordenado de acuerdo a la Clasificación de Bertillon. El objetivo del estudio fue analizar las causas de mortalidad por cáncer; determinar que grupos de edad presentan más muertes y que estaciones; calcular las tasas de mortalidad.Materiales y métodos:mediante la lectura de los Libros de Defunción del Archivo Parroquial de la ciudad (APJC), se han obtenido 26.203 difuntos, 18.538 de los cuales tienen anotado el motivo de su muerte, los cuales son utilizados para su análisis posterior.Resultados:Se registraron 182 defuncionespor cáncer(1% del total), la gran mayoría adultos, 165 (90.7%), siendo las mujeres casi el doble, 121 (66.5%). La mayor mortalidad se da entre los 45 y los 74 años, 122 difuntos (67.0%). El cáncer más frecuente es el del apartado de otros órganos, con 50 defunciones (40.6%). El mes con más muertos es marzo.las papeletas de defunción solían presentar errores ortográficos o de transcripción. Estudios similares en otras poblaciones arrojan cifras dispares según el periodo analizado y la cifra de muertos. Existe una estrecha relación entre cáncer y edad.Conclusión:El cáncer de otros órganos es el más numeroso. La mortalidad difiere entre las distintas poblaciones comparadas. El número de difuntos es muy inferior al actual, debido a la falta de medios diagnósticos y conocimientos de la época y la posible existencia de un subregistro


Introduction: Cancer represents an extraordinary problem for Global Public Health and is the leading cause of global mortality. Lung cancer is the most common cancer. The number of deaths was ordered according to the Bertillon Classification. The objective of the study was to analyze the causes of mortality due to cancer, determine which age groups have the most deaths and which seasons, and calculate mortality rates.Materials and methods: By reading the Death Books of the city's Parish Archive (APJC), 26,203 deceased persons were obtained, 18,538 of whom were diagnosed with COVID-19, and these individuals were used for subsequent analysis.Results: A total of 182deaths due to cancer were recorded (1% of the total);the vast majority wereadults (165,90.7%), with nearlytwice as manywomen (121, 66.5%). The highest mortal-ity occurredbetween 45 and 74 years of age, with 122 deaths (67.0%). The most common cancer wasthat of other organs, accounting for50 deaths (40.6%). The month with the most deaths wasMarch. Death certificates often had spelling or transcription errors. The number of deaths is much lower than the current numberdue to the lack of diagnostic methods, knowledge of the time,and the possibility of underrecording

Introducción: Las estadísticas vitales son fundamentales para conocer las condiciones de salud de la población y diseñar intervenciones. De su calidad depende la precisión y validez de las métricas derivadas y la relevancia de las decisiones de política pública. Objetivo: Evaluar la calidad de las estadísticas vitales de niños menores de cinco años en Colombia, entre 2000 y 2018. Métodos: Estudio descriptivo y exploratorio. Se analizó la calidad de los registros de nacidos vivos y defunciones publicados por el DANE, utilizando los criterios recomendados por las Naciones Unidas: exactitud (precisión), cobertura (compleción) y oportunidad. Resultados: Se encontraron registros de nacimientos y defunciones con datos perdidos en categorías que indican gradiente social. El 8,2 % de los registros de defunción tenían como causa básica de muerte códigos poco útiles para la toma de decisiones en salud pública. El 97,8 % de los nacidos vivos fueron registrados durante el mismo año de ocurrencia. El subregistro de la mortalidad infantil se estimó en un 28,9 %, con diferencias entre territorios. Conclusiones: La calidad de las estadísticas vitales en niños menores de cinco años de Colombia mejoró en los 19 años evaluados, en particular en exactitud y oportunidad. Sin embargo, persisten problemas en la asignación de la causa básica de muerte y en el subregistro, con diferencias importantes entre territorios. Mejorar la calidad del sistema de estadísticas vitales debe ser un imperativo ético, en especial para comprender las poblaciones de áreas geográficas históricamente invisibilizadas en el país.


Introduction: Vital statistics are essential to identify the health conditions of a population and design interventions. Vital statistics are essential to understand population health and design public health interventions. The accuracy and validity of the derived metrics and the relevance of public policy decisions depend on its quality. Objetive: To evaluate the quality of the vital statistics of children under five years of age in Colombia, between 2000 and 2018. Methods: A descriptive and exploratory study was carried out and the quality of records of live births and deaths published by DANE was analyzed using the criteria suggested by the United Nations: accuracy (precision), opportunity, and coverage (completeness). Results: Of the death records, 8.2% show codes useless for public health purposes as underlying cause of death. Of the live births, 97.8% were registered during the same year of occurrence. We estimated the underregistration of infant mortality at 28.9%, with differences between territories. Conclusions: The quality of vital statistics of children under five years of age has improved in Colombia in the 19 years analyzed, particularly in accuracy and opportunity. However, problems persist in assigning the underlying cause of death and in underregistration, with significant differences between territories. Improving the quality of the vital statistics system must be an ethical imperative, especially to understand the populations of geographic areas historically invisible in the country.

Resumo Objetivo: descrever a incompletude da informação sobre profissão/ocupação nas bases de dados de síndrome respiratória aguda grave (SRAG), síndrome gripal (SG) e no Sistema de Informações sobre Mortalidade (SIM) no Brasil. Métodos: estudo descritivo utilizando os bancos de dados de SRAG, SG e SIM. Calcularam-se percentuais de incompletude na variável profissão/ocupação segundo sexo, macrorregiões e unidades da federação, em 2020-2021. Resultados: o percentual de incompletude foi de 94,7% no banco de SG; 97,7% no de SRAG; e 17,0% no SIM. Em todas as macrorregiões a incompletude foi superior a 91,0% nos bancos de SG e SRAG; e superior a 13,0% no SIM. Todas as unidades da federação apresentaram percentuais de incompletude acima de 90,0% para SG; de 74,0% para SRAG; e de 6,8% para óbitos. Amapá apresentou maior percentual de incompletude na base de dados de SG (98,1%); Rio Grande do Sul (99,4%) na de SRAG; e Alagoas (45,0%) no SIM. Conclusões: observaram-se elevados percentuais de incompletude da variável profissão/ocupação nos sistemas de informação estudados. Recomenda-se uma articulação intersetorial, envolvendo representantes dos governos e dos trabalhadores, para formulação de estratégias que contornem a falta de informação sobre ocupação/profissão nas bases de dados relevantes para a vigilância em saúde.


Abstract Objective: to describe the incomplete filling out of the profession/occupation variable in the flu-like syndrome, severe acute respiratory syndrome and mortality databases in Brazil. Methods: descriptive study with secondary data from flu-like syndrome (FLS), severe acute respiratory syndrome (SARS) and mortality databases (SIM). We calculeted the absolute and relative filling frequencies of the profession/occupation variable according to State, gender, regions, and federative units, for 2020 and 2021. Results: we found a 94.7% incompleteness on the FLS database, 97.7% of missing profession/occupation data on the SARS, and 17.0% on the SIM database. Incompleteness frequency was above 91.0% in all Brazilian regions for FLS and SARS. Incompleteness on the mortality database was over 13.0%. All federative units presented incompleteness above 90.0% for FLS, 74.0% for SARS, and 6.8% for mortality in all genders. Higher levels of missing data were found in the states of Amapá for FLS (98.1%), Rio Grande do Sul for SARS and Alagoas (45.0%) for mortality (99.4%). Conclusions: all databases showed a high incompleteness of the profession/occupation variable. We recommend an articulation between the Ministry of Health, Ministry of Labor and workers' representations to solve this lack of data on occupation/profession in public databases.
