RESUMEN

In 2019, the nursing sciences section of the Conseil national des universités des disciplines de santé was created. One of its missions is to evaluate the applications of candidates wishing to qualify as university professors and associate professors. Qualification authorizes the applicant to apply a competitive examination for the position of university professor or assistant professor at a French university offering a position. The members of the section must assess whether the requirements in terms of research, teaching, and expertise have been met by the candidate so that they can sit the competitive examination. As a result, there are a number of requirements, which it is useful to reiterate in order to enhance the quality of future applications. Only those requirements relating to qualification as a associate professor will be covered here.


En 2019, la section « Sciences infirmières ¼ du Conseil national des universités pour les disciplines de santé a été créée. Parmi ses missions, elle évalue chaque année les dossiers de demande de qualification de maîtres de conférences et de professeurs des universités. La qualification autorise la personne à candidater à un concours de recrutement au poste de maître de conférences ou de professeur des universités dans une université française proposant un poste. Les membres de la section doivent apprécier si les requis en termes de recherches, d'enseignements et d'expertises sont suffisants pour pouvoir présenter le concours. De ce fait, certaines exigences sont attendues, qu'il paraît nécessaire de rappeler pour favoriser un gain en qualité des futurs dossiers de candidature. Seules les exigences relatives à la qualification aux fonctions de maître de conférences seront traitées ici.

RESUMEN

Nursing is recognized worldwide as an academic discipline. However, if we look at nursing training in Belgium, France, Luxembourg and Switzerland, this does not appear to be the case. Disparities persist not only in terms of initial training, but also in graduate and post-graduate training. This underlines the difficulties of establishing and recognizing nursing as an academic discipline in the French-speaking European area.

In France, successive reforms in nursing education have resulted in a 90 % reduction in hours of theoretical instruction in clinical psychopathology since 1972. As a result, novice nurses in psychiatry feel that they have not received sufficient theoretical training at the beginning of their careers. This particularly affects their ability to search for reliable data in order to guide and orient their clinical judgment. The objective of this exploratory study is to determine if a digital tool could serve as a lever to help these novice nurses in psychiatry to improve the various phases within clinical judgment. Similar digital tools have shown promising results in other medical fields but, to our knowledge, there are no reports of such a system for nursing psychiatry in the literature. A cross-sectional quotation qualitative study was carried out by interviewing seventeen novice nurses in psychiatry with different profiles. The interview guide, based on Benner's theory and Tanner and Rabardel's models, addresses several topics: professional background and motivation, evaluation of initial training, problems identified in practice, sources of information in the field of psychiatry, and finally acceptance of digital tools. The analysis of these interviews confirms the feeling of lack of knowledge among these professionals and indicates that a digital tool would be well accepted. Several examples were identified for the content of this tool, including ways to use it during the clinical judgment process.

RESUMEN

There are three stages in the professionalization process for nurses, within which academic training and nursing science are developing. The evolution of initial training and the 2025 referential will require an integrative alternation that will reinforce skills. The professionalization of students should be based on the analysis of professional practices, in a caring and ecological environment for individuals.

RESUMEN

Science is subject to inevitable evolutionary change. Philosopher of science Thomas Kuhn defined a scientific revolution as a period in time during which a particular field of scientific knowledge is set to transition to a new paradigm that will propel it toward a new milestone. Because person-centered care embraces the universal values of trans-disciplinary healthcare teams, evidence-based healthcare represents a paradigm shift toward evidence-based nursing. The rapid development of evidence-based practice is an evolutionary process of revolutionary importance to science. The resulting era of evidence-based nursing will help fully realize the development potential of the nursing sciences. This article introduces the process of knowledge construction in evidence-based nursing, reviews the literature to identify the general model used to promote evidence-based nursing internationally, and proposes practical strategies to promote evidence-based nursing in Taiwan. We hope this article will help further promote the clinical application of evidence-based nursing and the development of nursing science.

TESTIMONIALS OF ADVANCED PRACTICE STUDENT NURSES: The first advanced nursing practice training programmes became available in France in 2018. In this article, several students describe their experience of the course, their motivations and their expectations. Some have decided to undertake the training straight after their initial training, others already have a few years experience. All are looking to develop their skills and their knowledge.

RESUMEN

Nursing science's body of knowledge is mainly composed of the results of qualitative studies which, since the 1970s, have contributed notably to improving understanding of patients' experiences. This abundant scientific literature enables reviews to be carried out with the aim of producing solid theories. Meta-synthesis is a methodology which was created for this purpose. The theories which it produces constitute reference frameworks for researchers wishing to develop innovative nursing procedures and to test them within the context of clinical studies.

Esta tese em cotutela teve como objetivo analisar as repercussões da institucionalização do New Public Management (NPM) e da Nova Gestão Pública (NGP) nas práticas profissionais dos enfermeiros da atenção básica no Brasil e nos cuidados primários na França. A NGP tem sido considerada um novo modelo de gestão na administração dos serviços de saúde nos dois países, substituindo a administração burocrática. Material e Método: Esta é uma pesquisa sob o referencial teórico e metodológico da Análise Institucional, linha Socio-clínica Institucional. A pesquisa foi aprovada pelo Comitê de Ética da Escola de Enfermagem de Ribeirão Preto da Universidade de São Paulo, parecer nº 3.134.647, Certificado de Apreciação de Apresentação Ética (CAAE) 03164018.8.0000.539 e pelo Comitê da Secretaria de Saúde de Porto Alegre/RS, parecer nº 3.267.686, CAAE 03164018.8.3001.5338. O campo de intervenção da pesquisa integra o Brasil e a França. No Brasil, participaram do estudo 15 enfermeiros e 1 médico de Equipes de Saúde da Família (ESF), dos Núcleos de Saúde da Família (NSF), em Ribeirão Preto/SP, de Unidade Básica de Saúde (UBS), de ESFs e de consultório na rua, em Porto Alegre/RS. Na França participaram 15 enfermeiros de cinco departamentos, Yvelines, Val d'Oise, Maine e Loire, Essone e Bouches-du-Rhône. Dentre os serviços de saúde investigados estavam o Centro Municipal de Saúde (Centre Municipal de Santé), a Casa de Saúde (Maison de Santé), o Polo de Saúde (Pôle de Santé), o Centro de Planejamento e Educação Familiar (CPEF), o Centro de Proteção Materno-Infantil (PMI), o Centro Médico-Psicológico (CMP), o Serviço de Enfermagem Domiciliar (SSIAD), o Consultório de Enfermagem privado, a Creche, o Colégio e o Liceu. As fontes e instrumentos de pesquisa utilizados foram a análise documental, o diário de pesquisa, as entrevistas individuais e a restituição. A organização e a análise dos dados foram realizadas por meio da interrogação cruzada das práticas. Os resultados foram organizados em quatro eixos temáticos interdependentes: 1) Eixo 1 - Singularidade dos cuidados de enfermagem extramuros do hospital: a tríade cuidado-gestão-clínica na atenção básica no Brasil e nos cuidados primários na França; 2) Eixo 2 - Contradições institucionais da NGP mais fortes no público que no privado; 3) Eixo 3 - Contradição entre a valorização atual da autonomia e da reflexividade dos profissionais e um formato burocrático da NGP; 4) Eixo 4 - Resistências das enfermeiras à institucionalização da NGP. Os resultados apontaram que as interferências entre a NGP e o modelo biomédico amplificam a fragmentação dos cuidados e da gestão, com o centramento no cuidado individual e a ênfase no corpo biológico. Dentre os efeitos, a NGP transformou as práticas de enfermagem em atos técnicos, quantificáveis, com perda de autonomia, ao mesmo tempo em que pareceu dar visibilidade ao trabalho executado. Conclui-se que as transformações nas práticas profissionais dos enfermeiros, produzidas pela NGP não demonstraram desburocratizar a gestão, ao contrário, resultaram na redução do acesso e da qualidade dos serviços de saúde prestados.


This cotutelle thesis aims to analyze the repercussions of the institutionalization of the New Public Management (NPM) and the "Nouvelle Gestion Publique" (NGP) in the professional practices of primary care nurses in Brazil and France. The NPM was considered a new management model in the administration of health services in the two countries, replacing the bureaucratic administration. Material and Method: This is a research with the theoreticalmethodological framework of the Institutional Analysis, Institutional Socioclinic line. The research was approved by the Ethics Committee of the Ribeirão Preto School of Nursing of the University of São Paulo, report number 3.134.647, Certificate of Presentation for Ethical Consideration 03164018.8.0000.539 and by the Porto Alegre/RS Secretary of Health Committee, report number 3.267.686, Certificate of Presentation for Ethical Consideration 03164018.8.3001.5338. The scope of the research includes Brazil and France. In Brazil, 15 nurses and one doctor participated in the study of the Family Health Teams (ESF), Family Health Nucleos (NSF) of Ribeirão Preto/SP, Basic Health Unit (UBS), ESF and "consultório na rua" of Porto Alegre/RS. In France, 15 nurses from five departments, Yvelines, Val d'Oise, Maine and Loire, Essone and Bouches-du-Rhône participated. The health services surveyed included a the Municipal Health Center (CMS), a Maison de Santé, a Pôle de Santé, a Centre de Planification et d'Éducation Familiale (CPEF), a Centre de Protection Maternelle et Infantile (PMI), a Centre Médico-Psychologique (CMP), a Service de Soins Infirmiers à Domicile (SSIAD), a Cabinet de Soins Infirmiers (PS), a Crèche, a Collège and a Lycée. The research sources and instruments used in both countries were, document analysis, research diary, individual interviews and feedback. The organization and analysis of the data were performed bys cross-questioning the practices. The results were organised into four interdependent thematic areas: 1) Axis 1 - Singularity of nursing care "outside the hospital walls": the triad of care-management-clinic in basic care in Brazil and primary care in France; 2) Axis 2 - Institutional contradictions of NGP stronger in the public than in the private sector; 3) Axis 3 - Contradiction between the current valorisation of autonomy and reflexivity of professionals and the bureaucratic framework of NGP; 4) Axis 4 - Nurses' resistance to the institutionalization of NGP. The results showed that the interference between NGP and the biomedical model reinforces the latter by amplifying the fragmentation of care and management, with a focus on individual care and an emphasis on the biological body. Among the effects, NGP has transformed nursing practices into technical, quantifiable acts with a loss of autonomy, whereas it seemed to be able to give visibility to the work done. We conclude that the transformations in professional nursing practices produced by NPM have not shown to debureaucratise management, on the contrary, they have led to a reduction in access to care and in the quality of services provided.
