RESUMEN

In children with relapsing DU in 50.8% cases there is a complicated disease course with Candida infection, as evidenced by the positive results of mycological examination of biopsy specimens of duodenal mucosa and detection of circulating Candida antigen in serum. The use in the complex therapy in children with DU associated with Candida species of instant New Zealand goat milk "Amalteya" leads to an overall therapeutic effect in 81.3% cases, which is manifested in the reduction of the duration of relapse 1.4 times prolongation of remission in 3 times and milder disease. Carried out in the dynamics fibrogastroduodenoscopy after treatment showed that 100% of the observed total endoscopic remission which is manifested in the disappearance of opalescent gray overlay on the bottom of the ulcer and ulcer epithelization.

RESUMEN

Research of influence of differentiated dietary therapy in a complex with antiulcer actions on dynamics of return development of clinical displays of disease at 117 patiens with gastric ulcer and duodenal ulcer, working on the chemical enterprise is studied. There are objective data testifying that at purpose of a diet with the increased quantity of fiber, results in essentially improve treatment of disease, especially in groups of the patients who treated during work and were exposed to combined action of connections of nitrogen and organic solvents.

RESUMEN

Peptic ulcer disease is a multifactorial and complex disease involving gastric and duodenal ulcers. Despite medical advances, the management of peptic ulcer and its complications remains a challenge, with high morbidity and death rates for the disease. An accumulating body of evidence suggests that, among a broad reach of natural molecules, dietary polyphenols with multiple biological mechanisms of action play a pivotal part in the management of gastric and duodenal ulcers. The current review confirmed that dietary polyphenols possess protective and therapeutic potential in peptic ulcer mediated by: improving cytoprotection, re-epithelialization, neovascularization, and angiogenesis; up-regulating tissue growth factors and prostaglandins; down-regulating anti-angiogenic factors; enhancing endothelial nitric oxide synthase-derived NO; suppressing oxidative mucosal damage; amplifying antioxidant performance, antacid, and anti-secretory activity; increasing endogenous mucosal defensive agents; and blocking Helicobacter pylori colonization associated gastric morphological changes and gastroduodenal inflammation and ulceration. In addition, anti-inflammatory activity due to down-regulation of proinflammatory cytokines and cellular and intercellular adhesion agents, suppressing leukocyte-endothelium interaction, inhibiting nuclear signaling pathways of inflammatory process, and modulating intracellular transduction and transcription pathways have key roles in the anti-ulcer action of dietary polyphenols. In conclusion, administration of a significant amount of dietary polyphenols in the human diet or as part of dietary supplementation along with conventional treatment can result in perfect security and treatment of peptic ulcer. Further well-designed preclinical and clinical tests are recommended in order to recognize higher levels of evidence for the confirmation of bioefficacy and safety of dietary polyphenols in the management of peptic ulcer.

In a randomized, double-blind, controlled clinical study, the effects of 5 g of guar gum, a dietary fiber composed of galactose and mannose, or placebo added to the diet of 20 patients with duodenal ulcer for 1 wk each were examined. Ten patients derived evident benefit and five some help from guar gum, on comparing symptoms during administration of guar gum with those experienced earlier or during the placebo week, whereas four patients found that neither guar gum nor placebo had any effect (p less than 0.001). The beneficial effect was associated with increased feelings of repletion after meals. Patients with fewest symptoms benefited only slightly, or not at all, from guar gum. In one patient, guar gum abolished pain felt earlier and on placebo, but also caused severe gastric retention after meals. This patient had pyloric stenosis. In patients who were intolerant to berries, fruits, sugar, sweet rolls, and pizza these foodstuffs were better tolerated during guar gum administration. The diarrhea which occurs in some patients ingesting guar gum was avoided by giving low initial doses. In three patients unpalatability of guar gum was a minor complaint. It is concluded that guar gum is helpful to many patients with uncomplicated duodenal ulcer, but that it is harmful to those having increased gastric emptying, eg, pyloric stenosis patients, and that guar gum may exert its effects by increasing gastric emptying time.

RESUMEN

The effects of increasing the viscosity of a meal on postprandial acidity, acid load, and rate of emptying of gastric contents was studied in 15 duodenal ulcer patients in a double-blind manner by adding five gm of guar gum or placebo evenly to a test meal. The acidity of gastric juices was measured postprandially during a 120-min period using samples drawn via a nasogastric tube. Gastric emptying was measured via assessment of the degrees of dilution of polyethylene glycol added to the test meals. Guar gum decreased the total acid load, gastric juice acidity, and rates of emptying of gastric contents between 60 and 120 min postprandially, probably because of its effects on viscosity and neutralization of gastric acidity and may therefore be beneficial for duodenal ulcer patients.

RESUMEN

The hypothesis that peptic ulcer is a manifestation of a generalized metabolic disorder caused by excess dietary fat intake is supported by the results in a series of 134 patients with proven gastric or duodenal ulcer treated with a simple 'fat-free' diet, together with pyloroplasty in 82 cases and simple suture of a perforated ulcer in 16. Relief of symptoms and healing of the ulcer resulted in the great majority of cases so long as the diet was strictly adhered to, and there have been no side effects or morbidity.

Two hundred and twenty endoscopically proved cases of duodenal ulcer were studied in a double blind manner by dividing them into control and study groups and following them up for one year. All patients were given ranitidine 150 mg bd for 3 months. Additionally, the study group patients were asked to take dry meals (they were allowed their normal diet but asked not to take water from 1 hour before meals to 1 hour after meals) for one year. The two groups of patients were compared. The recurrence rate at one year was lower in the study group. Nausea/vomiting at the time of recurrence was also significantly lower in the study group (p < 0.02). Relief in symptoms, especially nausea/vomiting and dyspepsia was also seen to be better in the study although the difference was not statistically significant. The fall in recurrence rate observed as a result of this simple regime is very encouraging.

RESUMEN

The effect of acupuncture therapy for duodenal ulcer was investigated in 21 male and female patients. The diagnosis and healing were verified by gastroscopy. It was found that the needle therapy for 3 weeks led to complete recovery in 76 percent of the patients. Diet, alcohol and cigarette abstinency were necessary for healing. Cuti-visceral reflex activation eliciting the improvement of the secretory and motor function of the gastrointestinal tract and effects in the CNS leading to analgesia and tranquilization may play a role in the mechanism of action. Our conclusion is that acupuncture can be satisfactory method to cure duodenal ulcer.

RESUMEN

In dogs with a postpyloric fistula, the food with relatively large content of fat passed out of the stomach by equal portions although not exponentially. The size of these portions seem to be determined by the ability of small intestine to digest and absorb the portion of fat which has passed the pyloric sphincter. Besides, the fat in the food increases pH of chyme in the duodenum. Similar findings are reported for healthy subjects and patients with the duodenum ulcer. The inhibitory effect of fat on the stomach and duodenum motility depends on the initial background of contractile activity of the digestive tract. The positive role of fat in self-convalescence from the ulcer disease is suggested.

Dissimilar immunochemical, protein and vitamin parameters were found in blood of patients with gastroenterological diseases at the initial step of impairments under conditions of routine and special diets. Specific alterations in content of transferrin, haptoglobin, tocopherol as well as in glutathione peroxidase and glucose-6-phosphate dehydrogenase activities showed that compensatory mechanisms were maintained better under conditions of the special diet therapy.

Three groups of patients received chemotherapy for duodenal ulcer and associated chronic gastric erosion. Group I was treated by the drugs combination (methacin, almagel, gastrofarm, solcoseryl, tazepam, rudotel), group II received gastrocepin, group III venter. The results of duodenal ulcer healing were similar in all the groups, yet epigastric and/or pyloroduodenal pain was relieved more rapidly in groups II and III. The latter groups were also characterized by accelerated disappearance of fibrinous patches on the surface of the erosion.

A total of 148 patients aged 18-66 years were examined. Of these, 86 subjects received an experimental diet, while 62 were placed on the common dietetics. Both single and 10-day nutrition with products manufactured in ultra-high ovens (UHF products) did not produce any adverse effect on the mean estimates of acid-forming function of the stomach and pH in the basal phase of gastric secretion or on the pattern of gastric mucus formation in patients. Histological study of the biopsy specimens of the gastric mucosa in patients who received UHF products did not reveal any specific time course of changes in its morphology and function. The blood flow in the gastric mucosa varied within an insignificant range (10-15 cal/min). Fifty per cent of patients who received UHF products without any additional treatment methods showed moderate positive shifts in the clinical and physiological parameters under study. Based on the data obtained the authors draw the conclusion about the lack of an unfavourable effect of UHF products, thinking it advisable to use such products, particularly in a complex of therapeutic measures.

RESUMEN

It has been shown that the development of destructive lesions of the duodenal wall induced by stress action on the body of the experimental animal is attended by a decrease of the content of creatine phosphate and noradrenaline in homogenate of the duodenal tissue. The use of tyrosine, a precursor of catecholamine biosynthesis, both as a pharmacologic agent and as a component of rich in this amino-acid product-dried milk, resulted in normalization of the biochemical parameters studied and in significant diminution of destructive lesions of the duodenal wall.

RESUMEN

A protein-rich diet for children with peptic ulcer has been developed. The diet includes a new dietetic product--the proteinic enpit, prepared by the Institute of Nutrition of the AMS of the USSR. The effect of the diet in children with peptic ulcer at the time of exacerbation of the process was subjected to a complex study. This ration is shown to have a beneficial influence on basic symptoms of the affection, on the condition of the protein metabolism and reduces the period of the ulcerous defect cicatrization.

RESUMEN

Experimental rats were subjected to an extreme action: 3-hour immobilization with simultaneous electrostimulation. A complete healing of erosion-ulcerous defects of the gastroduodenal mucosa was recorded in rats given diets with an increased (by 4 g) protein content (group I), on day 5. The mucosal affections in the rats receiving a standard ration still persisted. The content of creatine-phosphate in the gastric wall tissue, decreased under the extreme action, was rapidly recovered in group I rats, no changes in the proper fluorescence level of reduced NADH and flavoproteins were noted. In group II rats both parameters returned to normal only on day 5. Blood serum ceruloplasmin (CP) activity increased during first 3 days with a similar rate in both animal groups. However, in the following 2 days CP activity in group I rats became completely normal, while that in group II animals was still rising. The results of the study have shown that the diet with increased protein content stimulates epithelization of the gastroduodenal mucosa due to the improvement of the bioenergy processes in the gastroduodenal area, and to intensified adaptation potentialities of the body.

RESUMEN

Fish oil preparation "Polyen" was used for treatment 21 patients with ulcerative diseases of the stomach or duodenum. The cicatrization of ulcer was diagnosed in 85% of patients treated by "Polyen" and in 60% of those who did not take fish oil. "Polyen" influenced fatty acid composition of erythrocyte membranes and lipid peroxidation. Authors draw a conclusion that omega-3 PUFA's can stimulate the reparative processes.

In 136 patients of young age features distinguishing the clinical course of peptic ulcer were studied. The diagnosis was established on the ground of roentgenological and endoscopic investigations. The nervous state was studied and the part played by psycho-emotional and stress factors in the development of peptic ulcer was ascertained on the basis of an analysis into the results of some vegetovascular and electrophysiological investigations. For the category of patients in question a high-protein and fat-rich diet (120-130 g) was composed. It was found to have a beneficial effect on the course of peptic ulcer, a high percentage of scarred ulcerations (78%) having been obtained by comparison with controls, who were kept on a diet with normal amounts of protein and fat. It is suggested that the effect of the treatment with the described diet is achieved thanks to increased buffer properties of the food ration, to the inhibitory action of the fat on the gastric secretion, and on account of intensified reperative processes going on in the gastroduodenal mucosa.

RESUMEN

The author summarizes the experience of UDT in 140 patients with chronic gastritis (53) and gastroduodenitis (87) and 46 with duodenal ulcer. It was found that chronic diseases of the gastroduodenal zone were characterized by changes in the hematological and biochemical indices of the blood against the background of altered immune reactivity and that UDT favoured their normalization.

The effect of a rational diet and of antiulcerous diets containing a proteinic-mineral enrichment (in an amount of 20--30 g) on the clinical symptomatology, the peripheral blood picture and some biochemical blood serum characteristics in patients with atrophic gastritis and duodenal ulcer was studied. In cases of atrophic gastritis with hypoferremia the proteinic enrichment was incorporated in the diet (rational) in an amount of 80--90 g per day. Control observations were conducted over analogous groups of patients receiving the same diets, but without proteinic enrichment. These observations continued over a period of 30--40 days. Thanks to a high content of easily assimilated iron and a well-balanced amino acids composition the proteinic-mineral enrichment has been found to be a valuable food product that is capable of favourably influencing the hemopoietic function of the organism and improving the blood proteins composition. Most patients with atrophic gastritis and peptic ulcer tolerate well the products and meals carrying additions of the proteinic enrichment.

The activity of amylase, lipase and trypsin was increased and that of the trypsin inhibitor decreased in the blood serum of young peptic ulcer patients as compared with healthy persons. The antiulcerous dietetics normalized significantly the parameters studied. Similar highly pronounced shifts in the activity of the blood pancreatic enzymes were revealed in atrophic gastritis patients. Hyperamilasuria and clinical symptoms of pancreatitis were not observed both in the peptic patients, and in the atrophic gastritis patients. The shifts in the blood content of pancreatic enzymes and their dynamics correlated mainly with the intensity and dynamics of the disturbances in the locomotive function of the gastroduodenal system.
