RESUMEN

The use of cytology in evaluating the state of the uterine cervix is well recognized and accepted in the United States. The role of colposcopy has not been completely defined although its acceptance is slowly gaining ground in many clinics. It is the purpose of this article to compare the two methods and to discuss the relationship of these methods to each other. Most attention will be directed to cytology and colposcopy of the cervix. However, the applications of both methods to evaluation of the vulva and vagina will also be considered.

RESUMEN

The work presents the results of studies on the behaviour of SCC antigen in 52 patients aged 21-70 years, with pathologic changes involving the uterine cervix and classified to the groups of erosion, dysplasia and carcinoma in different stage of advancement. The SCC antigen was determined in patients blood sera by radioimmunologic method, on the basis of Abbott Firm testes. An increase in concentration was observed exceed the norm (2 ng/ml) in cases of CIN and CIS. The rise of marker depended on the stage of the neoplastic process advancement in the uterine cervix.

One hundred and forty patients, admitted to the Center of Oncology Research of the USSR Academy of Medical Sciences with a suspicion to cervical cancer, were examined by the complex method comprising in addition to routine gynecological examination also extended colposcopy, cytological investigation of smears from pathological foci surface as well as histological studies, "aimed biopsy and cervical canal curetting. It was found that the cytological picture of moderate dysplasia of cervical epithelium is more frequently recognized in patients with pseuderosions developed with unchanged in size and form uterine cervix in the background, in the presence of benign colposcopic picture. The cytological picture of marked epithelial dysplasia and dysplasia with clear-cut cell atypism is observed in patients with bleeding pseuderosions against the background of hypertrophic and deformed cervix. Histological assay in these patients revealed early cervical cancer in 67% of observations. Marked extensive leucoplakia of the cervix found in clinical examination is manifested by different forms of epithelial dysplasia, while histological assay revealed early cervical cancer in 29.4% of observations.

RESUMEN

814 patients with various pathological changes in the uterine neck were subjected to the complex investigation, taking into consideration the fact that all specialists should be equally competent in their fields. Diagnostic errors were analysed in each method of investigation in relation to a histological conclusion in every series of patients. The diagnosis of preinvasive cancer proved to be most difficult. The number of errors was less in microcarcinoma. The organization of large colpocytological centres in big cities is advocated.

Cervical ectopy is common in adolescents, pregnant women, and those taking high doses of estrogen-containing contraceptives. The majority of cases have spontaneous reversion, but some cases can be persistent. Studies suggested that the adequacy of a Pap smear could be affected and there is an increased risk cervical infections. This study is a cross-sectional study conducted from December 2009 to February 2011 with 457 women with cervical ectopy and 736 without ectopy. Cervical samples were collected in vials for analysis by ThinPrep cytology (Hologic, Marlborough, MA). The Mann-Whitney test and Fisher's exact test (95% CI) were applied. The study was approved by the ethics committee of the Federal University of Ceará. The mean ages of the study group and control group were 28.7 (±14.8) and 33.6 (±7.5) years old, respectively (P < 0.0001). Negative diagnosis for malignancy and intraepithelial lesion was present in 399 (87%) cases and 705 (96%) in the study and control groups, respectively (P < 0.0001). Shift in the flora suggestive of bacterial vaginosis (BV) was observed more frequently in the study group: 74 (16.2%) than in the control group: 86(11.7%) (P = 0.017). The differences among the other morphotypes showed no significance. The smears were atypical in 12.7% (58/457) of the patients from the study group and in 4.2% (31/736) in the control group (P < 0.001; RR = 3 [2.033-4.712]). The association between ectopy and inflammatory cytology, the presence of the shift in the flora suggestive of BV and cytological atypia is evident.
