BACKGROUND Obturation is a crucial step in dentistry, involving the filling and sealing of a prepared root canal with sealer and core material to prevent bacterial invasion and ensure successful treatment. This study employed scanning electron microscopy to compare the effectiveness of 3 obturation techniques - single-cone technique (SCT), cold lateral compaction technique (CLCT), and continuous-wave technique (CWT) - in sealing dentin to recently introduced root canal bioceramic sealer in 30 extracted mandibular second premolars. The goal was to determine the optimal technique for minimizing gaps at the sealer-dentin interface. MATERIAL AND METHODS Thirty premolars were divided into 3 groups (n=10 each) based on obturation technique: SCT, CLCT, and CWT. All groups used CeraSeal bioceramic root canal sealer. Root samples were sectioned at apical, middle, and coronal thirds, and high-resolution scanning electron microscopy was performed to measure marginal/internal gaps. Data were analyzed using one-way ANOVA and Tukey range test, with P≤0.05 considered statistically significant. RESULTS CWT showed fewer voids at all levels, with non-significant differences within a technique. Among the techniques, SCT had the largest mean gaps at all levels [apical (5.43±0.16), middle (5.28±0.20), coronal (5.73±0.24)], while CWT had the smallest mean gaps [apical (3.02±0.19), middle (2.95±0.14), coronal (2.76±0.15)]. The differences in means between techniques were statistically significant (P≤0.05). CONCLUSIONS CWT obturation with CeraSeal root canal sealer produces fewer marginal gaps at the sealer-dentin interface.

This narrative review highlights the role of endodontists and the significance of various dental tools in forensic dentistry. An online search was conducted in peer-reviewed journals, including MEDLINE (Ovid), PubMed, Web of Science, Scopus, and Google Scholar databases, to retrieve studies regarding "the role of an endodontist in victim identification using different tools". The searches used controlled vocabulary and free-text terms. Articles written in English and published from 1923 to 2023 were selected. An essential stage in forensic dentistry is dental identification of the dead person and is regarded as an initial step for both judicial and humanitarian purposes if fingerprint records are missing or the remains have undergone significant changes. Endodontists should be aware of all available dental tools that aid in identification. The four fundamental tools for identification are dental radiographs, hard and soft dental structures, and dental materials. Dental radiographs provide a substantial nondestructive record for estimating age and sex. Moreover, maxillofacial hard and soft structures provide important tools for individual identification as they are considered the strongest structures in the human body and can withstand severe chemical and temperature changes. In addition, endodontic and restorative materials can be identified under different conditions and serve as excellent forensic identification measures.

The aim of the study is to assess the knowledge, attitude, practice, and self-reported record for a list of items that are useful to forensic dentistry (FD) among dental professionals from the western region of Saudi Arabia. This cross-sectional survey was conducted among 406 dental health professionals. A pre-validated questionnaire written in Google Forms was used as a study tool. The questionnaire contained 4 parts: 1 - demographics; 2 - knowledge about FD; 3 - the attitudes; 4 - assessing dental record maintenance. Statistical data were analyzed using Pearson chi-square (χ2). The degree distribution amongst 406 respondents was: 64 bachelors; 162 masters; 180 Doctor of Philosophy (PhDs). 52% of respondents agreed that dentists should have enough knowledge about FD. Yet, 90.4% did not receive any training. All qualifications were aware of FD with no significant difference between groups (Pâ =â .06). According to degree, a significant difference between the 3 groups (Pâ =â .004), where a bachelor degree showed the least desire in testimony in court for FD evidence (50%) compared to master and PhD degrees (80%, 75%, respectively). Almost 56% of consultants reported that it is not primarily for the dentist to have a good knowledge of FD, while all other qualifications agreed to the opposite (66%). 55.4% of general dentists, 73% of residents, and 94% of consultants believed that FD is an accurate, sensitive, and reliable method in the identification of unknown victims or criminals. Our study revealed that there is a lack of knowledge, attitude, and practice among undergraduates and general dental practitioners. However, this scenario can be upgraded by the inclusion of FD in the dental curriculum in Saudi Arabia.

The study aimed to radiographically assess the characteristics of pulp stones (PS) and pulp canal obliteration (PCO) in teeth and examined their associations with various dental parameters such as caries, restorations, periodontal status, and age. This cross-sectional observational study was conducted at the Faculty of Dentistry, King Abdulaziz University between September 2022, and May 2023, involved 101 patients exhibiting 402 teeth with PS or PCO. Data were collected from periapical and bitewing radiographs, and analyzed by two calibrated dentists. Multiple dental parameters were assessed, including caries level, presence and level of restorations, periodontal condition, and age of the patient. The study population consisted of 62 females and 39 males, with an age range of 18-65 years. Inter- and intra-examiner reliability were high (Kappa = 0.88 and 0.98 respectively). PS were more commonly found in molars (81.2%), while PCO were presented in only 115 teeth (23%). Age significantly affected the type of calcification (p < 0.001), with PS more common in the 20-30 age group and PCO more common in individuals over 40. Presence of caries was significantly associated with the type of calcification (p = 0.013), but restoration was not. The majority of teeth with PS (76%) or PCO (93%) had healthy periodontium. Around 40% of teeth with PCO showed signs of periapical changes, a finding significantly different from those with PS (p < 0.001). Pulp calcifications were significantly associated with various dental parameters, including caries presence, age, and periodontal status. The findings provide crucial insights into the epidemiology and aetiology of pulp calcifications.

This study aimed to assess the adaptability and penetration depth capacity of recent bioceramic systems, including regular EndoSequence (BC) versus HiFlow (BCH) sealers in the presence of BC points. A total of 54 single-rooted teeth were instrumented and obturated with either the cold or warm compaction technique (n = 9), using either BC, BCH, or AH Plus (AHP) combined with BC points. The adaptation, film thickness, and gaps/voids were evaluated by scanning electron microscopy. The sealer/dentin interface was evaluated by Raman spectroscopy, and depth penetration was evaluated by a confocal laser scanning microscope. According to the normality test, the data were statistically analyzed by ANOVA or Kruskal-Wallis and Mann-Whitney U tests at p < 0.05. BCH sealer showed the significantly thinnest film with the greatest flow (p > 0.001), with further improvement when subjected to the warm compaction technique. Moreover, it exhibited close adaptation with deep penetration into radicular dentin, forming a tag-like structure. The Raman spectra also indicated close contact with the dentin surface. The use of BC sealer with BC points exhibited homogenous, single-unit obturation, either with a cold or warm technique. Furthermore, the use of the warm compaction technique with BCH sealer achieved a gap-free interface associated with tag-like structures, which exhibit the monoblock phenomenon.

OBJECTIVE: Pulp canal calcification is a dental condition that is characterized by the deposition of mineralized tissue within the dental pulp space. While it is primarily a local phenomenon, recent studies have suggested a potential link between pulp calcification and systemic diseases. This study aimed to determine the correlation between certain systemic diseases, medications, and the presence of pulp canal calcification. Second, it aimed to estimate the prevalence of pulp calcification in the smoker population. METHODS: A pair-matched case-control observational study was conducted from June 2022 to June 2023 at the Faculty of Dentistry, King Abdulaziz University. Digital periapical and bitewing radiographs were used for case-based sampling. Patients were categorized into two study groups: the cases group (n=100), consisting of patients with pulp canal calcification including either pulp stone, pulp canal obliteration, or both. Whereas the control group (n=100), consisted of patients without pulp canal calcification. Detailed medical histories were obtained to identify the presence of systemic diseases. Additionally, systemic medications and smoking status were documented. RESULT: The participants were divided into two groups; cases (n=100), diagnosed with teeth calcifications and confirmed by radiographic examination, and controls (n=100), showing no evidence of teeth calcifications. Among the patients in the calcification group, 26% had a history of systemic disease compared to 17% in the control non-calcification group with no significant difference between both groups. No correlation between certain medications and pulp canal calcification. Smoking did not demonstrate a statistically significant association with teeth calcifications (p > 0.05). CONCLUSION: Pulp calcification on routine radiographic examination could indicate an underlying unnoticed systemic disorder that demands accurate referral and subsequent therapy.

This work is designed to assess the antimicrobial efficacy, chelating efficacy, and dissolving capability of the bioactive agent of the Nigella sativa plant (thymoquinone). Four freeze-dried microorganisms were studied. Each species was treated with either 6% sodium Hypochlorite, thymoquinone, or sterile water. The zone of inhibition was measured. Thirty extracted human premolar teeth were utilized to evaluate the smear layer removal. Root canals were mechanically instrumented and then irrigated with either 6% sodium Hypochlorite, 17% ethylenediaminetetraacetic acid, or thymoquinone for 1 min and scanned under the scanning electron microscopic to evaluate the cleanliness of the root canal and the remainder of the smear layer. To evaluate the tissue dissolving effect, Bovine Pulp Tissue was utilized. Randomly treated samples included: 6% sodium Hypochlorite, thymoquinone, or isotonic saline for 30 min. The remaining pulp tissue was weighed. Thymoquinone reported the highest inhibition of microbial multiplication compared to other irrigants (p < 0.001). Thymoquinone solution had an excellent antibacterial effect on endodontic pathogen and did not affect the inorganic and organic tissue inside the root canal. Meanwhile, it reported weak chelating and dissolving effects. Tissue dissolution was statistically significant with sodium Hypochlorite solution compared to other groups (p < 0.001).

Introduction: Expired gutta-percha (GP) can lose its mechanical properties. This study aimed to compare the tensile strength values between expired and nonexpired GP-cones and to assess the outcome of different heat temperatures on this strength. Materials and Methods: Two-hundred-forty ProTaper points were obtained from nonexpired and 2 years expired pack with two different subgroups (sealed or exposed to air and light). All groups were subjected to the following treatments: control, 90°C water bath and cool either at room temperature or 0°C and 70°C water bath and cool either at room temperature or at 0°C. The strength under the tensile load was calculated using the universal testing machine. Results: The collected data were evaluated by one-way analysis of variance. Tensile strength for all control groups did not show any difference. Significant increase in the tensile strength in groups heated for 1 min in a water bath at 70°C, the tensile strengths were 8.19 ± 2.72 Mpa and 5.94 ± 1.14 Mpa cooled at 25°C and 0°C, respectively, P ≤ 0.001. Two years expiration did not lower the tensile strength of the GP and exposure to air and light did not show any change. Appropriate storage of GP is a key factor to extend its lifespan. Conclusion: A clinician may consider using expired GP as long as there are no noticeable changes in their structure and properties. A simple chairside technique using hot water may be helpful in restoring the mechanical properties of expired GP.

BACKGROUND: Autofluorescence (AF) spectroscopy is a potent yet non-invasive technique with diagnostic and therapeutic applications. It identifies and characterizes changes occurring in the human body based on the changes in the fluorescence signatures. AIM: The primary purpose of this systematic review is to address the question "Is it feasible to detect dental plaque using light-induced auto-fluorescence in adult patients with invisible grade 1 plaque?" MATERIALS AND METHODS: The literature search was conducted independently MEDLINE, Embase, Cochrane Library, Web of Science, Google Scholar, and Scopus databases for relevant studies from January 2000 and June 2021, using the following terms in various combinations: detection, dental biofilm, plaque, light, auto-fluorescence, caries, gingivitis, sensitivity and specificity. Studies describing various fluorescence techniques for the detection of plaque, including sensitivity and specificity, were included. RESULTS: The majority of the studies indicated that AF spectroscopy allows fluorescence-based detection of various diseases, including early-stage dental plaque. While results of conventional plaque detection techniques are inconsistent, the data from the AF technique is reliable and reproducible, which can be used for patient documentation. However, a wide range of non-uniformity existed in these studies. AF spectroscopy, as a non-invasive technique, represents a viable and patient-friendly clinical tool for the early detection of dental biofilm plaque, and its meticulous removal has been directly responsible for the prevention of this disease. CONCLUSION: Despite the heterogeneity and limitations of studies included in this review, the future for light-induced autofluorescence spectroscopy technologies in diagnostic dentistry certainly presents an accurate and potentially applicable option.

Oral bacterial infections are fairly common in patients with diabetes mellitus; however, due to limited treatment options, herbal medicines are considered an alternate solution. This study aimed to formulate a stable essential-oil-loaded nanoemulsion for the treatment of oral bacterial infections. Essential oils from edible sources including coriander, clove, cinnamon and cardamom were extracted by hydrodistillation. The response surface methodology was used to optimize the nanoemulsion formulation by applying the Box-Behnken design. The oil concentration, surfactant concentration and stirring speed were three independent factors, and particle size and polydispersity index were two responses. The particle size, polydispersity index and zeta potential of the optimized formulation were 130 mm, 0.222 and -22.9, respectively. The ATR-FTIR analysis revealed that there was no incompatibility between the active ingredients and the excipients. A significant release profile in active ingredients of nanoemulsion, i.e., 88.75% of the cinnamaldehyde and 89.33% of eugenol, was recorded after 24 h. In the ex vivo goat mucosal permeation study, 71.67% of the cinnamaldehyde permeated and that of the eugenol 70.75% from the nanoemulsion. The optimized formulation of the essential-oil-loaded nanoemulsion showed a 9 mm zone of inhibition against Staphylococcus aureus and Staphylococcus epidermidis, whereas in anti-quorum sensing analysis, the optimized nanoemulsion formulation showed an 18 mm zone of inhibition. It was concluded that formulated essential-oil-loaded nanoemulsion can be used against S. epidermidis and S. aureus infections in oral cavity.

PURPOSE: This systematic review addressed flap designs in endodontic surgery which can have an impact on the Oral Health Related Quality of Life (OHRQoL). MATERIALS AND METHODS: Four electronic databases were searched (PubMed, Cochrane Library, Web of Science, and Scopus) to identify all studies up to November 2019 that investigated the effect of flap designs on gingival recession and quality of life among healthy adults. RESULTS: The initial search identified 2701 references. Ten studies were included in this systematic review; two were randomised clinical trials and eight were non-randomised clinical trials. Studies showed that sulcular incision increases the risk of gingival recession and decreases OHRQoL. Two studies were included in the meta-analysis in relation to gingival recession. The pooled results demonstrated that submarginal incision showed a decreased weighted mean difference in gingival recession by 0.31 mm (95% CI: 0.12 - 0.51) (p = 0.002) compared to sulcular incision. CONCLUSION: Sulcular incision flap unfavourably affect the level of gingiva and OHRQoL. All nonrandomised studies had a statistically significant bias and the sample sizes in all studies were relatively small. More gingival recession and lower OHRQoL were associated with sulcular incision. Additional investigations are warranted to provide more evidence.

For the management of necrotic immature teeth, regenerative endodontics offers the advantage of further root lengthening, thickening of dentin wall, and apical closure. This prospective study aimed to evaluate the long-term outcome of regenerative endodontics in immature necrotic permanent teeth. A total of 23 immature roots were medicated by triple antibiotic paste. After 21 days, bleeding was induced by over-instrumentation, and then mineral trioxide aggregate and coronal restoration were applied. Patients were scheduled for clinical and radiographic follow-up for 8 years. The radiographic changes of root dimensions were assessed using the ImageJ Plugin and statistically analyzed by Kruskal-Wallis test at a 95% confidence level. For qualitative evaluation, images were overlapped and analyzed using Photoshop software. All teeth were asymptomatic one month after the treatment. All teeth (n = 18) with preoperative periapical radiolucency showed complete resolution within 6-9 months. Recall rate at two, three, and eight years was 69.6%, 56.5%, and 34.8%, respectively. Continuous root development with a significant increase in root length and thickening of dentin wall accompanied by a significant decrease in apical canal diameter was seen at the end of the observation period (p < 0.001). In conclusion, the long-term outcome of regenerative endodontics revealed successful clinical and radiographic results with appropriate case selection.

This study aimed to evaluate the characterization of chemical interaction of root canal irrigants on the surface of EndoSequence root repair materials using spectroscopy analysis. Round discs of putty and paste were obtained and immersed in saline, sodium hypochlorite (NaOCl), or chlorhexidine. On the surface of chlorhexidine treated putty, diffuse red pigmentation was detected by Raman analysis and diffuse black pigmentation having unusual needle-like shaped crystals was detected by scanning electron microscopy. This pigmentation formed from nitrogen-rich compounds detected by Raman, infrared spectroscopy, and X-ray diffraction analysis. Mineral/amide ratios were increased by NaOCl and significantly decreased by chlorhexidine. Carbonate/phosphate ratios showed no significant changes by NaOCl, while a significant decrease by chlorhexidine. A full half of maximum widthRaman of the phosphate band was significantly increased by NaOCl, while decreased by chlorhexidine. In conclusion; nitrogen-rich compounds produced by chlorhexidine altered the surface morphology of the putty without crystallinity reduction. However, NaOCl reduced the organic fillers that affected its binding to phosphate ions. This reaction affects the chemical properties and perfection of root repair material. The clinicians should be aware of different irrigants to be used after repairing the perforation.

The American Association of Endodontists (AAE) released a case difï¬culty assessment form to help general dentists and students treat cases within their expertise or refer advanced cases to reduce the risk of iatrogenic errors. The purpose of this study was to determine the incidence of iatrogenic complications after the use of the case difï¬culty assessment form. Arandom sample of 1000 cases that received root canal treatment in undergraduate clinics during the academicyear (2016-2017) was selected. Case difficulty assessment was made for each case before treatment onset. Once the case was approved for treatment in the undergraduate clinics, the endodontic treatments were performed following the standard procedure the endodontic department of the Dentistry School of King Abdulaziz University mandates. Digital radiographs were obtained during routine root canal treatment and were evaluated by four observers to detect any iatrogenic errors, after which the data were analyzed statistically. Iatrogenic errors were correlated signiï¬cantly with case difï¬culty (p= 0.003), and were detected in 22.1% of all teeth treated in the undergraduate clinics. Underfilling accounted for the highestpercent of errors detected (8.4%), followed by ledge formation (4.2%). Molar teeth had the highest frequency of errors, and mesio-buccal roots of maxillary molars showed the highest percentage of errors. The AAE developed asignificantly useful tool to determine the difficulty of each case treated in undergraduate clinics, and following their recommendations will minimize the risk of iatrogenic errors.

The purpose of this study was to compare different irrigants' effect on two EndoSequence root repair materials' push-out bond strength. Sixty root slices were filled either with EndoSequence premixed fast-set putty or regular-set paste, and then immersed either in sodium hypochlorite, chlorhexidine gluconate, or saline (as control) for 30 min, after which the slices were subjected to the push-out test. The surface structures were evaluated with Scanning Electron Microscopy and Fourier Transform Infrared. Fast-set putty exhibited greater displacement resistance when immersed in saline and subjected to adhesive failure mode, while regular-set paste showed greater resistance when immersed in chlorhexidine and subjected to cohesive failure mode. Infrared analysis showed changes in organic filler, and carbonate and phosphate bands after using irrigants. The lowest carbonate/phosphate ratio was found for chlorhexidine in both materials. Therefore, sodium hypochlorite reduced EndoSequence root repair materials' displacement resistance markedly.

This study aimed to evaluate the morphology and chemistry of an apatite layer induced by fast-set versus regular-set EndoSequence root repair materials using spectroscopic analysis. Holes of a 4 mm diameter were created in the root canal dentin, which were filled with the test material. Fetal calf serum was used as the incubation medium, and the samples incubated in deionized water were used as controls. The material-surface and material-dentin interfaces were analyzed after 28 days using Raman and infrared spectroscopy, scanning electron microscopy/energy dispersive X-ray, and X-ray diffraction. After incubation in fetal calf serum, both materials formed a uniform layer of calcium phosphate precipitate on their surfaces, with the dentinal interface. This precipitated layer was a combination of hydroxyapatite and calcite or aragonite, and had a high mineral maturity with the regular-set paste. However, its crystallinity index was high with the fast-set putty. Typically, both consistencies (putty and paste) of root repair material have an apatite formation ability when they are incubated in fetal calf serum. This property could be beneficial in improving their sealing ability for root canal dentin.
