BACKGROUND: Our nearly 500-member department implemented the shadowing program "Walk in My Shoes" to improve intradepartmental relationships and build a stronger sense of community. The program provides both clinical and non-clinical employees an opportunity to shadow colleagues in their various roles and learn more about one another's contribution to the overarching mission of caring for children and their families. The goal of this study was to understand the impact of the shadowing program on employee perceptions of various roles. OBJECTIVE: To bridge the gap of understanding among colleagues in order to strengthen workplace interrelatedness, increase understanding of various roles, and decrease preconceived notions about roles, through shadowing. MATERIALS AND METHODS: A preliminary survey distributed to our department in August 2018 assessed the level of interest in new wellness initiatives, including the shadowing program. The survey gauged which roles participants were interested in shadowing. The survey results revealed that 67 employees were interested in the shadowing program. We selected 39 participants and matched them to a coworker in their area of interest. The roles for shadowing included administrator, Child Life specialist, information technologist, medical assistant, nurse, radiologist, researcher and technologist. Participants were required to complete pre- and post-shadowing surveys to assess their experience. Individuals who hosted the shadow experience also completed a survey. RESULTS: A total of 39 clinical and non-clinical staff members participated in the program. We summarized the pre- and post-survey data using median and interquartile range (IQR) and compared the results using the Wilcoxon signed-rank test. The distribution of preconceived notions about each role was not significantly different between the pre- and post-surveys (P=0.094). However, participants' value, understanding of the role they shadowed, and understanding of how the roles relate to each other were significantly greater (P<0.001). In addition, participants showed great interest in shadowing the specific role again (82%) and shadowing another role (92%). Furthermore, almost all hosts would repeat the experience (96%). CONCLUSION: Our study showed that intradepartmental shadowing can improve clinical and non-clinical staff employees' perceptions and understanding of each other's roles in overall patient care, which in turn contributes to the broader initiative of workplace wellness. The enthusiasm and willingness of the hosts were essential for sustainability of the program and demonstrated that this type of program is feasible in a large, busy department.

Previous research has demonstrated that stretching can enhance athletic performance and induce cardiovascular adaptations. This study aims to assess whether a 4-week preventative stretching routine can enhance heart rate variability and heart rate recovery, faster blood lactate clearance, and improve performance following submaximal strength exercises. Twenty-four healthy adults were recruited and randomly allocated to either the experimental group or the control group. Both groups engaged in submaximal strength exercises (5 sets to voluntary failure at 60% of 1RM) comprising bench press and back squat exercises under baseline conditions and after stretching protocol. The experimental group followed the Stretching Protocol, while the control group adhered to their regular training routine. ANOVA analysis revealed a significant pre-post interaction effect between groups in the variable of squat repetitions, although no notable pre- or post-differences were observed in heart rate variability, heart rate recovery, blood lactate concentration, or bench repetitions in either group. A 4-week preventative stretching program does not appear adequate to enhance lactate clearance and cardiovascular adaptation after submaximal strength exercises in resistance-trained individuals compared to the control group. However, it is plausible that such a stretching routine may mitigate muscle fatigue, though further investigation is warranted to substantiate this hypothesis.

This study was performed with the aim of evaluating the relative tribological behaviour at high temperature of (Ti(1-x)Al(x))N coatings commercially deposited on WC inserts. The (Ti(1-x)Al(x))N multilayered, nanostructured and single-layer coatings, which contained different Ti/Al atomic ratios varying from 7/3 to 2/3 respectively, were deposited by employing a commercial PVD cathodic arc process. The absolute hardness value for each coating is also reported and has been calculated from the Vickers microhardness measurements by using one of the models published in the literature. Standard ball-on-disc testing was conducted in order to determine friction coefficients and wear rates for these systems against a 6 mm alumina ball. These tests have been carried out in conditions that are not common in industrial use, e.g. metal cutting tools inasmuch as alumina is not a representative workpiece material. The sliding tests were performed out at 25, 500 and 700 °C with 5 N normal loads. At 25 °C, a wear volume, V, of approximately 10(-2) mm(3) was obtained for all the tested coatings. When the test temperature increased to 500 °C, the single-layered coatings showed a wear volume of the same order of magnitude as those tested at room temperature. The multilayered coated samples decreased their wear volume by one order of magnitude, whereas the nanostructured samples showed almost no wear. At 700 °C, the wear volume values reported for all samples were similar and of the same order of magnitude as those tested at room temperature. The wear mechanism is discussed together with the morphological and compositional characteristics, determined by SEM coupled with EDX analysis.

The aim of the study was to determine the properties of a standardized extract of ginseng root in inducing a higher immune response in vaccination against influenza. Attention was also paid to the common cold in this multicentre, two-arm, randomized, placebo-controlled, double-blind investigation. A total of 227 volunteers who visited 3 private practices in Milan received daily oral capsule doses of either placebo (113) or 100 mg of standardized ginseng extract Ginsana G 115 (114) for a period of 12 weeks within which they received an anti-influenza polyvalent vaccination at week 4. As a result, while the frequency of influenza or common cold between weeks 4 and 12 was 42 cases in the placebo group, it was only 15 cases in the G115 group, the difference being statistically highly significant (p < 0.001). Whereas antibody titres by week 8 rose to an average of 171 units in the placebo group, they rose to an average of 272 units in the G115 group (p < 0.0001). Natural killer (NK) activity levels at weeks 8 and 12 were nearly twice as high in the G115 group as compared to the placebo group (p < 0.0001). In all the volunteers, laboratory values of 24 safety parameters showed no significant differences between the end and the beginning of the 12-week study in either of the groups. There were only 9 adverse events in the study, the principal one being insomnia.

Central pattern generators (CPGs) are genetically determined neuronal aggregates in the mesencephalon, pons and spinal cord subserving innate motor behaviours essential for survival (feeding, locomotion, reproduction etc.). In higher primates CPGs are largely under neocortical control. We describe how certain motor events observed in parasomnias and epileptic seizures could have similar features and resemble motor behaviours, which can be the expression of the same CPG. Both epilepsy and sleep can lead to a temporary loss of control of neomammalian cortex that facilitates through a common platform (arousal) the emergences of stereotyped inborn fixed action patterns. Therefore we suggest that, independently from the nature of the trigger, be it a seizure or a parasomnia, the same CPGs can be involved, "caught up", leading to a common motor semiology (the "Carillon theory").

The clinical efficacy and safety of aztreonam were studied in 30 patients suffering from urinary tract infections caused by gram-negative bacteria. Aztreonam was given in a dose of 1 g i.m. at intervals of 12 or 24 hours. Duration of therapy was 2-8 days. Bacteria isolated in urine cultures were: Escherichia coli (60%); Proteus mirabilis (10%); Pseudomonas aeruginosa and Citrobacter sp. (7%); Proteus vulgaris, Serratia marcescens; Klebsiella pneumoniae and Pseudomonas species (3%). At completion of therapy and at follow-up all patients were bacteriologically and clinically cured. No significant side effects or toxicity were observed in all patients treated. It is concluded from these data that aztreonam is highly effective and well tolerated in patients affected either by upper or lower urinary tract infections.
