RESUMO

Objective: Paramedicine in Canada has experienced significant growth in recent years, which has resulted in a misalignment between existing guiding conceptualizations and how the profession is structured and enacted in practice. As a result, well-established boundaries, directions, and priorities may be poorly aligned with existing frameworks. The objective of this study was to explore emerging and future states of paramedicine in Canada such that guiding principles could be derived. We asked: How should paramedicine be conceptualized and enacted in Canada going forward, and, what might be the necessary enablers? Methods: This study involved in-depth one-on-one semi-structured interviews with Canadian paramedicine thought leaders. We used purposive and snowball sampling strategies to identify potential participants. Interview guide questions were used to stimulate discussion about the future of paramedicine in Canada and suggestions for implementation. We used inductive qualitative content analysis as our analytical approach, informed by a constructivist and interpretivist orientation. Results: Thirty-five key informants from across Canada participated in interviews. Ten themes were identified: (1) prioritizing patients and their communities; (2) providing health care along a health and social continuum; (3) practicing within an integrated health care framework, and partnering across sectors; (4) being socially responsive; (5) enacting professional autonomy; (6) integrating the health of professionals; (7) using quality-based frameworks; (8) enacting intelligent access to and distribution of services; (9) enacting a continuous learning environment; and, (10) being evidence-informed in practice and systems. Six enablers were also identified: shift professional culture and identity, enhance knowledge, promote shared understanding of paramedicine, integrate data environments, leverage advancing technology, advance policy, regulation and legislation. Conclusions: Our results provide a conceptual framework made up of guiding principles and enablers that provide a consolidated lens to advance the paramedicine profession in Canada (and elsewhere as appropriate) while ensuring contextual and regional needs and differences can be accounted for.

RESUMO

Objective: The objective of this study was to assess the attitude and hesitancy toward vaccine against COVID-19 in a Pakistani Population. Materials and methods: A mix-method, prospective study was conducted and adults (aged ≥18 years) residing in Pakistan were invited to participate. The questionnaire was prepared, hosted in Google Forms and circulated through electronic platforms and was also available to be done in in-person. Data was compiled from 15th September to 30th November 2020. Results: The response rate was 80%. A total of 1003 participants were included in the final analysis. Of them, 75% completed survey questionnaire online, while remaining 25% responded in-person. The mean age of the participants was 29.62 ± 10.47 years. The majority of participants were females; 60.9% (n = 611). 57.02% (n = 572) of the participants were employed at the time of survey. Overall, 70.68% (n = 709) of the participants had previous experience of vaccines such as the flu vaccine Only 4.9% (n = 49) participants thought that they will be seriously ill from COVID-19 within six months and 39% (n = 392) participants were confident that they will get COVID-19. A total of 71.29% of the participants reported they would consider getting vaccinated once available. There was statistical association between gender and getting vaccinated (P < 0.001). Conclusion: This study demonstrated that majority of the participants showed positive attitude toward considering COVID-19 vaccine. However awareness with informed knowledge of efficacy, possible adverse effects and cost would be of added great value to increase the real response of Pakistani population toward COVID-19 vaccination.

@#
PURPOSE: The study purported to explore the lived experience of a select group of Filipino mothers who had undergone water birth delivery.

DESIGN: A qualitative, descriptive phenomenology research design was used to investigate the phenomenon. The sample (N=10) was comprised of women who had undergone water birth, 25-45 years old, primiparous and multiparous. A self-report method using face-to-face, in depth, semi structured interviews guided by an aide memoire abetted the data collection procedure. The principles of beneficence, respect for human dignity, and justice were observed throughout the research process.

MODE OF ANALYSIS: Significant statements were culled from the field text and analyzed using Colaizzi method. The validation techniques of members checking and critical friends were utilized.

FINDINGS: The findings revealed that mothers found the experience extraordinary and the warmth and buoyancy of the water used in such mode foster comfort and ease of delivery while enabling them to manage their pains positively. Four themes emerged namely: (1) In her own private world; (2) At her fullest power; (3) To her ease and comfort; and (4) At her own course.

CONCLUSIONS: Research in recent times has provided limited evidence in making water birth a prevalent alternative for women to welcome their babies into this world. It is vital to focus on the experience of water birth from the mother's lens. Water birth has potential benefits to both mother and neonate and promotes maternal satisfaction; however, it remains the responsibility of health practitioners to provide credible evidence of its safety to prove that water birth is a valuable option to conventional delivery.

RESUMO

Effort thrombosis of the upper extremity is secondary to thrombosis of the axillary and/or subclavian veins that develop from heavy arm exertion. This case illustrates venous thrombosis of the right brachiocephalic vein in a 32-year-old man who presented with a cyst-like swelling in the right neck with no associated pain or trauma. Our patient, a trained athlete, was preparing for a triathlon at the time of presentation. He was treated by first-line therapy of subcutaneous and oral anti-coagulation medication. In this study, we highlight the importance of early investigation and treatment of symptomatic athletes so that long-term disability can be prevented. This study also shows the timely use of computed tomography imaging, which can help to identify the syndrome in previously undiagnosed patients.
