Aims and background: To study if 25-hydroxy cholecalciferol levels correlate with clinical outcomes in a cohort of critically ill children requiring pediatric intensive care unit (PICU) admission. Materials and methods: All children between the ages of 1 month and 14 years admitted to a PICU were included in this study. The vitamin-D level was measured within 24 hours of admission to the PICU for each patient. The patient's clinical details, vitamin-D levels, and biochemical parameters were collected. Results: There were 119 critically ill children (47 females and 72 males) admitted to our PICU. A total of 56 children were in the vitamin-D-deficient group, giving a prevalence of 47.05%. Sixty-three children had either insufficient or normal levels of 25(OH)D. Mean serum 25-OH cholecalciferol was 22.82 ± 16.48 nmol/L. There were no significant differences in O2 utilization, ventilation requirement, length of PICU stay, or the frequencies of use of antibiotics and steroids between the groups. The overall mortality rate in this study was 5.8% (three children died in the deficient group as compared with four in the insufficient/normal group). Conclusion: Even though vitamin-D deficiency was highly prevalent in the PICU, there were no statistically significant differences in O2 utilization, length of PICU stay, duration of mechanical ventilation, the use of antibiotics/steroids, and mortality outcome for both deficient and insufficient/normal groups. Introduction: More than 28.7 million individuals throughout the globe suffer from diabetes mellitus, with an estimated 11 percent of the population living with the condition in India. Changes in lifestyle and a variety of treatment plans are used in management. Metformin is a key drug for glycemic control, both when used alone and in combination. Our research compares the effectiveness of glycemic control achieved by empagliflozin plus sitagliptin. Methods: This study took place from November 2022 to April 2023 at the tertiary care hospital. The study did not begin until the ethical review was completed. There were two groups of patients, A and B. Everyone received a daily dose of Metformin 1,000 milligrams. Sitagliptin (50 mg twice daily) was administered to individuals in Group A, whereas Empagliflozin (10 mg once daily) was given to those in Group B. After three months of therapy, HbA1c was used to compare the two groups' levels of glycemic control to those at the start of treatment. To do this, we employed a proforma. Version 25 of the Statistical Package for the Social Sciences (SPSS Inc., Chicago, USA) was used for the analysis. Results: The average age of the 300 patients that participated in the trial was 42.33. There were 57.67% men and 42.33% females. "The mean reduction in HbA1c from baseline in Group A was -0.65 ± 0.11% and in Group B was -1.34 ± 0.13% with statistically significant P-value (P-value = 0.000)." Conclusion: The combination of Empagliflozin and Metformin is superior to that of Sitagliptin and Metformin for the maintenance of glycemic control.

Aim: To understand the psychodynamics of tobacco use among 11- to 14-year-olds in Ahmedabad, India. Materials and Methods: A self-administered questionnaire was used to gather information on tobacco use from 5,266 students, aged 11 to 14, enrolled in government-aided English medium schools in Ahmedabad. IBM® SPSS® 26.0 software along with Chi-square tests were used to analyze the data. Results: A total of 7.2% (376) had ever used tobacco. Of the users, 96.6% (363) knew it was harmful, 76.3% (287) were boys, and 85.2% (320) were initiated by family and friends. Conclusion: Despite being aware of the dangers of tobacco and being unable to purchase from vendors, children continued to obtain tobacco products, being influenced by their close social networks. An effective cessation measure should be directed toward this psychodynamic etiology.

Introduction: The coronavirus disease 2019 (COVID-19) pandemic has seen multiple surges globally since its emergence in 2019. The second wave of the pandemic was generally more aggressive than the first, with more cases and deaths. This study compares the epidemiological features of the first and second COVID-19 waves in Kozhikode district of Kerala and identifies the factors associated with this change. Methods: A comparative cross-sectional study was conducted in Kozhikode district. A total of 132,089 cases from each wave were selected for the study using a consecutive sampling method. Data were collected from the District COVID-19 line list using a semistructured proforma and analyzed using Statistical Package for Social Sciences (SPSS) ver. 18. Results: The second wave had a higher proportion of symptomatic cases (17.3%; 20.1%), cases with severe symptoms (0.3%; 0.6%), intensive care unit (ICU) admissions (11.2%; 17.9%), and case fatality rate (0.69%; 0.72%). Significant difference was noted in the age, gender, locality, source of infection, comorbidity profile, symptom, and the pattern of admission in various healthcare settings between the first and second wave. Among the deceased, gender, duration between onset of symptoms and death, comorbidity status, and cause of death were significantly different in both waves. Conclusion: The presence of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Delta variant, as well as changes in human behavior and threat perception as the pandemic progressed, resulted in significant differences in various epidemiological features of the pandemic in both waves, indicating the need for continued vigilance during each COVID-19 wave.

RESUMO

A second subterranean species of Pangio is described from an old dug-out well in Kerala, Southern India. The new species, Pangio pathala is unique within the genus in possessing the highest number (27) of caudal vertebrae. Pangio pathala is distinguished from P. bhujia, the only subterranean Pangio species known so far, in having four pectoral-fin rays (vs. three), five anal-fin rays (vs. six), 67 vertebrae (40 abdominal and 27 caudal vertebrae) (vs. 6263), and a raw genetic distance of 8.18.7% in the mitochondrial cytochrome oxidase subunit 1 gene. This paper also provides an additional record of Pangio bhujia from a location 40 km south of the type locality.

Atrial flutter (AFL) is a macro-reentrant tachycardia that can be provoked by numerous factors, including acute pericarditis. We present a case of new-onset AFL masking acute pericarditis in a man with multiple comorbid conditions, including hypertension, chronic kidney disease, and obstructive sleep apnea. After a failed attempt of rate control, the patient underwent successful cardioversion, which revealed electrocardiographic findings consistent with acute pericarditis. Colchicine was avoided in the setting of chronic kidney disease and the patient was treated with a steroid taper. Pericarditis is a rare cause of AFL, and this case demonstrates the diagnostic and management considerations for AFL and acute pericarditis.

There is an abundance of literature that highlights the importance of patient-centered communication with cancer patients requiring surgical intervention. While the need for communication for patients requiring surgery is well understood, less attention is brought to patients with severe mental illnesses. More literature is needed to highlight the importance and application of patient-centered care for patients suffering from both severe mental illness and cancer requiring surgical intervention. It is unclear if poor communication between patients and cancer-care specialists is part of the reason for the underlying discrepancy. Efforts to reduce this discrepancy may be worth considering as a priority for health care systems. We present a case of a 63-year-old man with schizophrenia who received a late cancer diagnosis after a missed screening, resulting in an extensive surgical resection for colon cancer. We explore the possibility of careful communication between the treating physician, patient, and patient's caretakers potentially preventing the delay in his cancer diagnosis. Effective communication is especially important with mental health patients because of its effect on long-term physical and mental outcomes. We hope to further the discussion on how to better cater to this specific population of patients undergoing cancer surgery.

Fungal infective endocarditis is a rare and serious form of endocarditis associated with severe morbidity and mortality. The greatest propensity for infection can be found in patients with implanted prosthetic valves, implanted cardiac devices, and intravenous drug use. We present a case of a 45-year-old male with a prior bioprosthetic mitral valve who was diagnosed with Candida parapsilosis endocarditis. Computed tomography imaging of the abdomen was significant for splenic infarcts, and transesophageal echocardiography demonstrated a 1.23 cm x 0.55 cm lesion and 1.02 cm x 0.545 cm lesion on the bioprosthetic valve. The patient was subsequently treated with Amphotericin B and life-long Fluconazole. This case highlights the imaging findings and treatment of a rare disease process.

Primary cutaneous gamma delta T-cell lymphoma (PCGD-TCL) is a rare lymphoma that makes up less than 1% of all cutaneous T-cell lymphomas. Patients with PCGD-TCL typically present with rapidly progressing plaques and ulceronecrotic nodules most frequently located on extremities without lymph node or bone marrow involvement. The overall prognosis is poor with a median overall survival of approximately 15 months. This case highlights a patient with concomitant PCGD-TCL, hemophagocytic lymphohistiocytosis, and human immunodeficiency virus-1-acquired immunodeficiency syndrome. There is a paucity of case reports describing PCGD-TCL and there are no evidence-based treatment recommendations. Further studies are needed to optimize strategies to treat patients with these diseases.

RESUMO

A unique, new species of eel loach, Pangio bhujia, is described from Kerala, India. It is the first species of Pangio to be described from subterranean waters. It possesses several unusual characters including absence of both dorsal and pelvic fins, the presence of only 3 pectoral-fin rays, 6 anal-fin rays and a unique count of 38 precaudal + 24 caudal vertebrae.

Men with 47,XYY syndrome present with varying physical attributes and degrees of infertility. A retrospective chart review was performed on a male infertility and genetic anomaly database. Three patients with 47,XYY were found. Each presented with > 2 years of infertility. All were tall with elevated body mass indices. Scrotal findings ranged from normal to atrophic testicles. Semen analyses demonstrated oligospermia and varying endocrine profiles. Because of the diverse phenotype and potential lack of symptoms, identification and diagnosis of men with 47,XYY syndrome may be difficult. We recommend careful screening of 47,XYY patients and referral to primary physicians for long-term follow-up for increased incidence of health-related comorbidities.
