RESUMO

We report a case of pulmonary cystic echinococcosis in a child from eastern Finland with no history of travelling abroad. The cyst was surgically removed and the organism molecularly identified as Echinococcus canadensis genotype G10. This parasite is maintained in eastern Finland in a sylvatic life cycle involving wolves and moose; in the present case, the infection was presumably transmitted by hunting dogs.

BACKGROUND: Atrial fibrillation (AF) is a common complication after coronary artery bypass grafting (CABG) operations. The aim of this prospective trial was to test the hypothesis that intraoperative high-rate atrial pacing may induce AF by mimicking rapid atrial tachycardia and can identify the patients at risk for postoperative AF. METHODS; Eighty patients having on-pump CABG without additional procedures were included in the study. After cannulation but before initiation of cardiopulmonary bypass two pacing wires were placed on the lateral surface of the right atrium. The right atrium was paced with the rate of 200 beats per minute for 10 seconds. If the patient was in sinus rhythm after the high-rate pacing, the pacing test was repeated with the rate of 250 and finally 300 beats per minute. RESULTS: Postoperatively AF developed in 28 patients (35%). The high-rate atrial pacing test induced AF in 27 patients (33.7%). Of the 28 patients who experienced AF during the postoperative period, 17 patients were inducible in the atrial-pacing test (sensitivity 0.61). Of the 52 patients who did not develop AF postoperatively, 42 patients were not inducible in the atrial-pacing test (specificity of the test was 0.81). Positive and negative predictive values of the test were 0.63 and 0.79, respectively. CONCLUSIONS: The intraoperative high-rate atrial pacing test turned out to be a simple, safe, and fast way to identify the patients at risk for AF after CABG. The diagnostic accuracy of this test is sufficient to identify a group of patients to whom prophylactic treatment could be directed.

RESUMO

Single-lung ventilation using a double-lumen endobronchial tube is often performed for surgical procedures involving the thorax. Tracheobronchial rupture during use of an endobronchial tube is an uncommon and serious complication. We present the case of a patient undergoing a right pneumonectomy who experienced a bronchial rupture due to a double-lumen endobronchial tube. Institution of veno-venous extracorporeal membrane oxygenation allowed removal of the endobronchial tube, and the repair was successfully performed. We hope that this information can provide further insight into the management of such a complication.

BACKGROUND/PURPOSE: The aim of this study is to compare the experience with video-assisted thoracoscopic surgery (VATS) for patent ductus arteriosus (PDA) since 1995 with the results of conventional open surgery from the preceding 10 years. METHODS: The records of 60 children who underwent standard posterolateral muscle splitting thoracotomy and ligation of PDA in 1986-1995 were reviewed for the study. The data on 50 children who underwent VATS PDA ligation since 1995 were collected prospectively. RESULTS: All patients survived. Ductal bleeding requiring sutures with patches occurred once in the open surgery group. Two patients in the VATS group underwent immediate rethoracoscopy and clipping because of residual ductal flow in the postoperative echocardiography. Complications in the VATS group included 6 (12%) recurrent laryngeal nerve injuries (3 transient) and 2 chylothoraces. One patient in each group underwent open reoperation because of residual ductal flow 1 year after the initial operation. The operative time, duration of recovery room/neonatal intensive care unit care, duration of pleural drainage, and length of hospital stay were significantly shorter in the VATS group. CONCLUSIONS: VATS PDA ligation gave results equal to traditional open surgery with a shorter operative time, faster recovery, and shorter hospital stay. More complications, especially recurrent laryngeal nerve injuries, occurred in the VATS group.

RESUMO

Rupture of the right ventricle may occur because of sternal dehiscence or mediastinitis after cardiac surgery. Direct suture, polytetrafluoroethylene patch, fasciae, and muscle flaps have been used to close a right ventricular rupture. A unique occurrence of repair of a full-thickness right ventricle defect with a de-epithelized myocutaneous flap is presented. Our patient experienced a rupture of the right ventricle complicating sternal wound infection. The rupture was reconstructed with a polytetrafluoroethylene patch, but the patch needed to be removed because of infection. The defect was reconstructed with a de-epithelized myocutaneous latissimus dorsi flap. The patient indicated no signs of complication during follow-up.
