RESUMO

AIM: To evaluate the efficacy and safety of collagen biomaterial application during the 4-week follow-up of patients with diabetic foot syndrome. MATERIAL AND METHODS: 75 patients with diabetic foot (Wagner II (69.3%) and III (30.7%)) aged 30-80 years were included in the multicenter study, among them were 50.7% with the wound unhealed for 1.5-6 months and 49.3% over 6-48 months. Patients were randomized into 2 groups: 1) standard therapy (n=37), 2) the additional use of the collagen material Collost (n=38). Observation period was at least 4 weeks for each patient. The size of ulcers, results of general and biochemical blood tests, oximetry, microbiological testing, ultrasound of lower extremities vessels as well as a detailed medical history, social and functional status, level of cardiovascular comorbidity and ongoing therapy were estimated. RESULTS: Additional use of a collagen biomaterial has led to a significant reduction ulcers of all sizes from 13.5 to 2.1 cm2 (in the comparison group - from 12.5 to 7 cm2). The best dynamics have been registered in Wagner II (4.4-fold average wound area regress in Collost group, from 8.8 to 2.0 cm2; average wound area regress by 1.8 times, from 10 to 5.6 cm2 in the comparison group) than in Wagner III group (in the main group from 55 to 21.3 cm2; in the control group from 36 to 32.4 cm2) and in ulcers existing less than 6 months. Treatment with biological material Collost within standard therapy after 4 weeks led to increase of complete epithelialization by 2.6% (21.1% as compared to 14.7%), while decreasing the frequency of ineffective treatment by 4.1 (7.9% in primary and 32.4% in the comparison group). CONCLUSION: We have proved the efficacy and safety of collagen biomaterial topical application in a diabetic foot syndrome treatment.

UNLABELLED: : Commonly used multiplicity adjustments fail to control the error rate for reported findings in many expression quantitative trait loci (eQTL) studies. TreeQTL implements a hierarchical multiple testing procedure which allows control of appropriate error rates defined relative to a grouping of the eQTL hypotheses. RESUMO

AIM: To assess the effectiveness of the collagen biomaterial in treatment process in patients with DFS. MATERIAL AND METHODS: 71 patients 30-80 y.o. with diabetic foot syndrome of varying severity were included in prospective multicenter study. Patients were randomized into two homogeneous groups: control group (n=35) - standard therapy, other 36 patients (main group) were additionally treated with medical device (MD) Collost in accordance with the instructions for use. RESULTS: Biomaterial Collost using in complex treatment of diabetic foot syndrome resulted in more rapid and effective healing of the ulcer. The treatment success increased from 43% to 72%. Complete epithelialization was achieved by 2.6 times more rapidly in conjunction with reduction the incidence of unsuccessful treatment results by 4.1 times.

RESUMO

High-priority research directions for the genetics and breeding of the sugar beet in the 21st century were developed with consideration of the available scientific achievements of domestic and foreign scholars. These directions unite the classical and molecular approaches to solving the problems of increasing the effectiveness of sugar beet breeding carried out on a genetic basis, and they correspond to the contemporary level of scientific research. Seven such directions are proposed.

The research included 40 patients with chronic trophic ulcers of lower extremities of different etiology (arterial insufficiency - 14 patients, venous insufficiency - 20 patients, diabetic foot syndrome - 6 people). According to the data of prime bacteriological inoculation, the main pathogens were: gram-positive coccus (Staphylococcus spp., Staphylococcus aureus - 75%, Staphylococcus epidermidis - 7,5%) and yeast-like fungi (Candida albicans - 7,5%). Microbial semination in plentiful quantity (more than 106 KOE) was detected at first inoculation in 85% of the patients. The ointment was applied for the patients. After 20 days, the lack of growth and the decrease of contamination level (lower than critical (less than 102 KOE) were noted. A visual analog scale estimated an intensity of pain in patients and it consisted of 39,8% before the treatment, 27,1% - after 10 days, 14,6% - after 20 days. The "Oflomelid" application allowed the reduction of the terms of wound cleansing from nonviable tissues in majority of patients and gained the fast transition from the granulation to epithelization phase. The principle of wound management with the application of ointment "Oflomelid" should be followed in a moist environment. A modern wound dressing must be used after the ointment. This shortened the terms of separate-phase wound repair and decreased the terms of the whole period of trophic ulcers repair in patients with vascular and endocrine pathology.

RESUMO

Results of conservative treatment of 31 patients with PAD are presented. In 3 and 6 months after 10 everyday intravenous infusions of Cytoflavin (combination of succinic acid, inosine, nicitinamide and riboflavin) average increase of painfree walking distance among nonsmokers was 50.6% and 64.4% vs. 108.0% and 170.5% in subgroup of smokers respectively. Average increase of maximal walking distance in subgroup of nonsmoking and smoking patients was respectively 30.1% and 130.5% in three months and 37.6% and 144.8% in 6 months after Cytoflavin infusions.

RESUMO

It was shown that of the greatest significance for early results of microsurgical digit and hand segment replantations were the completeness, mechanism and the level of the amputation. The criteria used for the classification of traumas are described in detail. Results of treatment of 154 patients operated upon for replantation of 184 segments of the hand (217 radiuses of the hand) are presented.

RESUMO

Under study were blood circulation, innervation and functions of distal parts of upper arms in 114 patients operated upon for traumatic abjunction and severe injuries of large segments. The state of blood circulation was found to be satisfactory within the terms of from 4 to 27 months. The degree of the extremity reinnervation was dependent on the level of the nerve injury and when observed longer than a year it was found to repair up to the level of hypesthesia without hyperpathia. The functional results of such operations were noted to depend on the character of trauma, the completeness and quality of repair of damaged nerves and tendons, on adequacy of rehabilitation therapy in the postoperative period.

RESUMO

A long-term follow-up study of 26 patients with 28 replanted digits was performed with respect to circulatory, sensory, and motor function. Peripheral circulation in the replanted digits was comparable with corresponding uninjured digits on the opposite hand. Excellent sensory recovery was observed in the territories supplied by 30 of the 36 repaired digital nerves. Best functional results were noted with the thumb and with fingers amputated through the middle phalanx. By objective criteria, 21 of the 28 replanted digits demonstrated satisfactory function. Subjectively, however, only 1 patient was dissatisfied with the outcome.
