RESUMO

The recently developed multi-acquisition with variable resonance image combination (MAVRIC) and slice-encoding metal artifact correction (SEMAC) techniques can significantly reduce image artifacts commonly encountered near embedded metal hardware. These artifact reductions are enabled by applying alternative spectral and spatial-encoding schemes to conventional spin-echo imaging techniques. Here, the MAVRIC and SEMAC concepts are connected and discussed. The development of a hybrid technique that utilizes strengths of both methods is then introduced. The presented technique is shown capable of producing minimal artifact, high-resolution images near total joint replacements in a clinical setting.

The aim of this study was to determine concordance between physician and patient blood pressure (BP) measurements in an ambulatory setting. A diagnostic intervention cross-sectional study using a convenience sample was employed. A total of 106 hypertensive patients were included in the study. Patients who were unable to perform their self-measurement or those with cardiac arrhythmia were excluded. BP was determined nine times in each subject in the medical office in a randomised order: BP was taken three times by the physician using a mercury sphygmomanometer (SPH-Hg), three times by the physician using a validated, automated oscillometer (Omron HEM 705 CP), and three times by the patient himself with the same device. The intraclass correlation coefficient was calculated. In all, 59 women and 47 men aged 65.7 (10) years were analysed. Mean BP measurements for the physician using the mercury sphygmomanometer, the physician using the Omron, and the patient using the same device were: 136 (15.8)/80 (11), 137 (17.9)/80 (10), and 139* (17.6)/80 (10) mmHg, respectively. BP control was 48.1, 48.1, and 36.8*% (*P < 0.05), respectively. Intraclass correlation coefficients for systolic/diastolic pressures were: 0.77/0.65 (physician-sphygmomanometer Hg, physician-Omron; P < 0.001), 0.75/0.64 (physician-sphygmomanometer Hg, patient-Omron, P < 0.001), and 0.83/0.83 (physician-Omron, patient-Omron; P < 0.001). In conclusion, the three types of measurement in the medical office were significantly concordant. Patient office self-measurement showed a tendency to increase systolic BP and worsen BP control.

In this work the generalized autocalibrating partially parallel acquisition (GRAPPA) technique was implemented with modified reconstruction and applied to in vivo high-resolution (HR) magnetic resonance imaging (MRI) of the trabecular bone microarchitecture at 3 Tesla (T) with a multiple-acquisition balanced steady-state free precession (b-SSFP) sequence. Trabecular bone is made up of a network of microstructures (80-140 microm), and its structural deterioration is associated with the skeletal metabolic disorder osteoporosis. HR-MRI is a promising noninvasive tool for assessing the trabecular microarchitecture in vivo, but it involves long acquisition times. Using partially parallel imaging (PPI) to accelerate the acquisition may help mitigate this shortcoming and allow more flexibility in protocol design. In this study the effects of GRAPPA-based reconstruction on image characteristics and the measurement of trabecular bone structural parameters were evaluated. Initial studies showed that image quality and depiction of microstructure were preserved in the GRAPPA-based reconstruction, indicating the feasibility of PPI in HR-MRI of trabecular bone. The results also demonstrated the potential of PPI for increasing the signal-to-noise ratio (SNR) efficiency of multiple-acquisition b-SSFP imaging protocols.

RNA viruses exist as quasispecies, heterogeneous and dynamic mixtures of mutants having one or more consensus sequences. An adequate description of the genomic structure of such viral populations must include the consensus sequence(s) plus a quantitative assessment of sequence heterogeneities. For example, in quality control of live attenuated viral vaccines, the presence of even small quantities of mutants or revertants may indicate incomplete or unstable attenuation that may influence vaccine safety. Previously, we demonstrated the monitoring of oral poliovirus vaccine with the use of mutant analysis by PCR and restriction enzyme cleavage (MAPREC). In this report, we investigate genetic variation in live attenuated mumps virus vaccine by using both MAPREC and a platform (DNA MassArray) based on matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectrometry. Mumps vaccines prepared from the Jeryl Lynn strain typically contain at least two distinct viral substrains, JL1 and JL2, which have been characterized by full length sequencing. We report the development of assays for characterizing sequence variants in these substrains and demonstrate their use in quantitative analysis of substrains and sequence variations in mixed virus cultures and mumps vaccines. The results obtained from both the MAPREC and MALDI-TOF methods showed excellent correlation. This suggests the potential utility of MALDI-TOF for routine quality control of live viral vaccines and for assessment of genetic stability and quantitative monitoring of genetic changes in other RNA viruses of clinical interest.

El sistema de incentivación pretende conseguir mayor motivación con la elaboración de unos objetivos claros, medibles y posibles de realizar. La institución contará con presupuesto para esta retribución variable y la legislación debe permitir este tipo de incentivos a la remuneración fija. Es imprescindible que se cuente con el apoyo del Departamento de Contabilidad y Recursos Humanos que obtendrán algunos resultados. Este sistema es extrapolable a otros Centros bien sean sanitarios o no. Nuestra pretensión es conseguir una mayor motivación del personal, haciendo coincidir los objetivos de los profesionales con los de la organización y mejorando la calidad asistencial
