RESUMO

Morphologic characteristics of multiple colonic polyps from 155 patients were analysed. In 23.5% of cases the polyps were proximally localized. The histologic structure in 98.1% out of 316 polyps showed them to be adenomas with different atypical changes of the epithelium, including carcinoma in situ, or invasive cancer. With the increase in adenoma size its histologic structure became more complicated and atypical changes more prominent. Different forms of colonic mucosa multiple adenoma are reviewed, the incidence of malignancy being 11.6%. It is concluded, that thorough examination of the colon and active surgery is indicated in this group of patients.

RESUMO

The article analyses the experience in the treatment of 49 patients with caudal teratomas for which reoperations had to be conducted. The operations were repeated because of diagnostic errors, inadequate surgical approach, and the development of intraoperative complications. Prolonged epidural anaesthesia with concurrent application of trimecaine hydrochloride and morphine was considered the most adequate type of anesthesia. In cases of fistulas draining the caudal teratomas into the rectum, the surgical tactics is determined by the location and size of the fistulas openings. To improve the results of treatment, all the coats of the teratoma must be removed and the cicatricially changed tissues excised, endolymphatic injection of antibiotics conducted, and the wound treated by the closed method.

RESUMO

Experience with the treatment of patients with multiple polyps of the colon is analyzed. They had large polyps and polyps with the diameter not more than 0,5 cm. The findings of morphological examination of such polyps and results of the dispensary observation of patients with non-ablated small polyps are described. A conclusion is made of the expediency of more active surgical tactics in relation to polyps less than 0,5 cm in diameter in this group of patients.

RESUMO

279 radical surgical interventions for diffuse polyposis of the colon were performed. Lethality was 11,4%. Postoperative peritonitis was noted in 16,8%. The radical surgery was necessary before the appearance of cancer, the operation being performed following the purposeful correction of metabolic and immunological disorders.

RESUMO

An experience with the surgical treatment of 505 patients with incomplete fistulas of the rectum is analyzed. Preoperative methods of diagnosis are shown to be not very informative. Of determining importance for the selection of the method of surgery is the relationship of the fistula passage to the sphincter fibres. Data of the operation performed and of the nearest postoperative complications are presented.
