BACKGROUND: Chemerin is an adipokine that signals through the G protein-coupled receptor ChemR23 and is associated with inflammation, glucose homeostasis, lipid metabolism and renal function, all of which strongly influence cardiovascular risk. However, elevated chemerin provides a survival advantage in patients with chronic kidney disease (CKD), but how this relates to the cardiovascular phenotype is unknown. OBJECTIVES: The aim of the present study was to establish the association of chemerin with coronary calcification and to determine the effects of chemerin signalling, through ChemR23, in vascular smooth muscle cell (VSMC) calcification. METHODS: Plasma chemerin was measured in 113 patients with CKD and 50 healthy controls. All patients underwent computed tomography to determine coronary artery calcium (CAC) score. VSMCs were isolated from wild-type and ChemR23 knock-out mice and treated with chemerin. RESULTS: Multivariate analyses established creatinine, cholesterol, body mass index and tumour necrosis factor as significant confounders for circulating chemerin levels. Despite these positive associations with renal function, cardiometabolic risk factors and inflammation, chemerin was inversely associated with CAC both in an age- and sex-adjusted analysis and in a multivariate analysis adjusting for the aforementioned confounders. In addition, circulating chemerin levels were associated with the calcification inhibitors matrix gla protein (MGP) and fetuin-A. Finally, chemerin significantly reduced phosphate-induced calcification and increased MGP expression in VSMCs, whereas chemerin was devoid of these effects in VSMCs lacking ChemR23. CONCLUSION: In conclusion, these results suggest that chemerin signalling through ChemR23 in VSMCs protects against vascular calcification in CKD.

OBJECTIVES: Our objective was to identify patient factors associated with being untreated for hepatitis C virus (HCV) infection in HIV-coinfected patients. METHODS: A prospective longitudinal study was carried out. HIV-infected patients with active chronic HCV infection included in the HERACLES cohort (NCT02511496) constituted the study population. The main study outcome was receipt of HCV direct-acting antiviral (DAA) treatment from 1 May 2015 to 1 May 2017. The population was divided into patients who were receiving HCV treatment during follow-up and those who were not. RESULTS: Of the 15 556 HIV-infected patients in care, 3075 (19.7%) presented with chronic HCV infection and constituted the study population. At the end of the follow-up, 1957 patients initiated HCV therapy (63.6%). Age < 50 years, absence of or minimal liver fibrosis, being treatment-naïve, HCV genotype 3 infection, being in the category of people who inject drugs using opioid substitutive therapy (OST-PWID), and being in the category of recent PWID were identified as significant independent risk factors associated with low odds of DAA implementation. When a multivariate analysis was performed including only the PWID population, both OST-PWID [odds ratio (OR) 0.552; 95% confidence interval (CI) 0.409-0.746) and recent PWID (OR 0.019; 95% CI 0.004-0.087) were identified as independent factors associated with low odds of treatment implementation. CONCLUSIONS: We identified factors, which did not include prioritization of a DAA uptake strategy, that limited access to HCV therapy. The low treatment uptake in several populations seriously jeopardizes the elimination of HCV infection in the coming years.

An objective method based on partial least-squares discriminant analysis (PLS-DA) was used to assign an oil lump collected on the coastline to a suspected source. The approach is an add-on to current US and European oil fingerprinting standard procedures that are based on lengthy and rather subjective visual comparison of chromatograms. The procedure required an initial variable selection step using the selectivity ratio index (SRI) followed by a PLS-DA model. From the model, a "matching decision diagram" was established that yielded the four possible decisions that may arise from standard procedures (i.e., match, non-match, probable match, and inconclusive). The decision diagram included two limits, one derived from the Q-residuals of the samples of the target class and the other derived from the predicted y of the PLS model. The method was used classify 45 oil lumps collected on the Galician coast after the Prestige wreckage. The results compared satisfactorily with those from the standard methods.

In this paper, the application of a new method based on self-organizing maps (SOM; termed MOLMAP, molecular map of atom-level properties) to handle 3-way data generated in a monitoring environmental study is presented. The study comprised 50 polycyclic aromatic hydrocarbons (PAHs) analyzed in samples derived from the weathering of six oil products (four crude oils and two fuel oils) spilled under controlled conditions for about 4 months. MOLMAP yielded useful information on each mode of the data cube: weathering samples, spilled oil products, and PAHs. Thus, the different behaviors of the six oils were ascertained, along with their particular evolution on time, and their weathering patterns were studied in terms of the original PAHs. Thus, the two heaviest products (two fuel oils) were characterized by two neurons whose more relevant weights were associated to heavy PAHs, as C(1)-fluoranthene, C(2)-fluoranthene, benzo(ghi)perylene, and dibenz(ah)anthracene. The six spilled products were projected on different regions on both the MOLMAP-SOM and a subsequent principal components analysis (PCA) scatter plot, developed using the so-called MOLMAP-scores. Besides, it was possible to further differentiate between unweathered, or slightly weathered, samples and the most weathered ones. The more relevant PAHs characterizing those samples were assessed studying the weights of the neurons in which the samples got projected.

Wolbachia are bacteria that live inside the cells of a large number of invertebrate hosts and are transmitted from infected females to their offspring. Their presence is associated with cytoplasmic incompatibility in several species of Drosophila. Cytoplasmic incompatibility results when the sperm of infected males fertilize eggs of uninfected females, causing more or less intense embryonic mortality (unidirectional incompatibility). This phenomenon also appears in crosses between populations infected with different Wolbachia strains (bidirectional incompatibility). The influence of Wolbachia infection on host populations has attracted attention as a potentially rapid mechanism for development of reproductive isolation and subsequent speciation. We examined the influence of this bacterium on reproductive isolation in interspecific crosses between Drosophila melanogaster and D. simulans. We found that Wolbachia infection negatively affected these two species in homospecific crosses. However, in interspecific crosses, it only influenced sexual isolation, as infected females more frequently hybridized than females free of infection; postzygotic reduction of fitness (bidirectional cytoplasmic incompatibility) was not detected. This would be explained by the existence of several modes of rescue systems in these two species, reducing cytoplasmic incompatibility between them. Wolbachia does not appear to cause reproductive isolation between these two species.

Four genetic-algorithm-based approaches to variable selection in spectral data sets are presented. They range from a pure black-box approach to a chemically driven one. The latter uses a fitness function that takes into account not only typical parameters like the number of errors when classifying a training set but also the chemical interpretability of the selected variables. In order to cope with the fact that multiple solutions may be acceptable, a multimodal genetic algorithm (GA) is employed and the most satisfactory solution selected. The multimodal GA uses two populations (denominated "hybrid two populations" GA or HTP-GA): a classical population, from which potential solutions emerge, and a new population, which maintains diversity in the search space (as required by multimodal problems). Results show that the HTP-GA approach improves the chemical understanding of the selected solution (compared to other GA approaches) and that the classification capabilities of the approach are still good. All of the GA strategies for variable selection were compared with a classical parametric technique, Procrustes rotation, which does not consider interpretability.

OBJECTIVE: Ritonavir-boosted protease inhibitor (IP/r) monotherapy: darunavir/ritonavir (DRV/r) or lopinavir/ritonavir (LPV/r) monotherapy is only provided in the major treatment guidelines in pretreated patients to prevent toxicity associated with nucleoside/nucleotide reverse transcriptase inhibitor (NRTI), reduce costs and simplify antiretroviral treatment. To start IP/r monotherapy, according to GESIDA guidelines 2016, patients need to meet the following criteria: absence of chronic hepatitis B, plasma viral load <50 copies/ mL for at least 6 months and absence of protease inhibitors mutations or previous virologic failures to IP/r. Currently, there are no studies that evaluate the efficacy and safety of darunavir/cobicistat (DRV/COBI) monotherapy. METHODS: This prospective study analyzed pretreated HIV patients with DRV/r monotherapy that were switched to DRV/COBI monotherapy. The aim of the study is to describe the effectiveness and safety of the DRV/COBI monotherapy. RESULTS: Seventy-eight patients were evaluated. Patients had a median of 31.29 months of DRV/r monotherapy before DRV/COBI monotherapy. Nine of the 78 patients developed "blips" (plasma viral load: 50-200 copies/ml) and four patients had plasma viral load ≥200 copies/mL. An 83.3% (65/78) of the patients remained with undetectable plasma viral load. As for safety, there were no significant differences in lipid profile, liver function (transaminases) and renal function between DRV/r and DRV/COBI monotherapy. CONCLUSIONS: DRV/COBI monotherapy seems to be effective and safe (lipid profile, liver and kidney function). However, it will be necessary to design specific studies comparing DRV/r vs DRV/COBI monotherapy to confirm these results.

Oil spill identification relies usually on a wealth of chromatographic data which requires advanced data treatment (chemometrics). A simple approach based on Kohonen neural networks to handle three-dimensional arrays is presented. A suite of 28 diagnostic ratios was considered to monitor six oils along four months. It was found that some traditional diagnostic ratios were not stable enough. In particular, alkylated PAHs (e.g. 1-methyldibenzothiophene, 4-methylpyrene, 27bbSTER and the TA21 and TA26 triaromatic steroids) seemed less resistant to medium-weathering than biomarkers. One (or two) ratios were found to differentiate each product: 30O, 28ab (and 25nor30ab), C3-dbt/C3-phe, 27Ts, TA26 and 29Ts characterized Ashtart, Brent, Maya, Sahara, IFO and Prestige oils, respectively.

This work studies airborne quality in a geographical area that has not been investigated broadly: a suburban site nearby A Coruña (Galicia, NW Iberian Peninsula). In contrast to major Spanish cities, the site has Atlantic characteristics: rainy, scarce calm weather and infrequent prolonged sunny periods. The relationships between several gaseous pollutants (NO, NO2, NOx, CO, O3, PM10, PM2.5 and PM1) and their temporal trends (daily, monthly and seasonal) were evaluated. The aim was to unravel whether medium- and long-distance sources were impacting upon the site. Univariate studies focused on factorizing the pollutants according to a codifying factor (wind direction, hour of the day, season and month). Multivariate studies (Varimax-rotated factorial analysis) were done separately on both weekdays and weekends. The intensity of the daily maxima for NO, NO2, NOx and CO was lower during the weekends, with O3 behaving opposite. PM average values agreed with previous historical reports for a rural background station relatively close to the site and they decreased daily between 11:00 and 19:00 h, likely because of the marine breeze. With moderate wind speeds the pollutants were associated to medium-distance pollution sources, mainly the city of A Coruña and a combination of industrial pollution sources (a power plant, a solid waste incinerator and a regional airport).

Eight different bottled wines (six red wines and two white ones) were studied for copper determination and fractionation. For this purpose, copper determination by square wave voltammetry (SWV) and potentiometry (PSA) stripping analysis using Hg electrodes (drop and film, respectively) were carried out. Two direct procedures for the determination of total copper in wine are proposed; in both cases, drastic treatment of samples is not necessary, the procedures are very fast (estimated time to carry out an analysis is <10 min) and require no deaeration. Fractionating treatment consists of various HCl additions followed by the addition of ethylenediamine. Precision (RSD < 3%) and accuracy (recovery > 98%) data justify that both methods proposed are valid for total copper determination in wine. The wines studied displayed similar behaviors regarding fractionation: the percentages of total copper fractionated in each step are statistically similar: differences are lower than 2 S.

AIM: To analyze differences in blood pressure levels in children and adolescents in Galicia with those of peers living in other parts of Spain. DESIGN: Cross-sectional study done in a rural area in Galicia. PARTICIPANTS: 870 boys and girls 6 to 16 years old. They are compared with published data from the RICARDIN study, that includes 10,042 participants selected at the schools in nine centers of Spain (one was the center in Galicia). MEASUREMENTS: Standardized methodology and certification procedures were used in Galicia and Spain to measure weight, height, Quételet index, systolic (SBP) and fifth-phase diastolic blood pressure (DBP5) and sexual maturation. Data are presented by sex and age group (1 year) both in Galicia and Spain. RESULTS: From 11 years and above, girls in Galicia present higher SBP than in Spain. In boys, the difference appears later. Galicia shows higher DBP5 levels than Spain in every age-sex group. In relation to obesity, the differences are of a small magnitude in the Quételet index, again, higher values in Galicia. CONCLUSIONS: This is the first population-based study available that rules out the possibility that the observed differences, previously already suggested, could be attributed to methodological problems. The differences in blood pressure and obesity between Galicia and other areas of Spain, already previously suggested in adults, can be the reflection of differences present from childhood.

We determined in 14 patients with pleural tuberculosis total lymphocyte count, T subsets and NK cells (CD56) in pleura and blood and it was found a preferential accumulation in pleura of CD3 T lymphocytes, TCR alpha beta, mainly CD4 subset, but not T or NK cells. In 5 pleuritis it was studied 40% of V beta TCR subfamilies in blood and pleura and in 2 pulmonary tuberculosis and one pleuritis all V beta and V alpha TCR subfamilies (trough PCR), without be observed a clear clonal expansion. It was not observed correlation among a) pleural and blood lymphocyte cellularity b) the amount of pleural effusion and the existence of lymphopenia or tuberculinic anergia c) levels of ADA and percentage of CD3 and CD4 cells in pleura. In 7 out 11 pleuritis a high expression of IL-2 receptor (CD25) was observed. In 24 patients with pleural and pulmonary tuberculosis there was not correlation between levels of SIL-2R and IL-6 and radiographic extension.

In total, 17 pediatric patients with hematologic malignancies (n=14) and Fanconi anemia (FA) (n=3) underwent haploidentical SCT with T-cell depletion. The patients were conditioned with reduced-intensity regimens, and CYA was used for GVHD prophylaxis. Successful engraftment occurred in 16 patients (94%). One patient failed to achieve a primary engraftment. Another patient rejected the first SCT after 10 weeks and had a successful second transplant. Of all engrafted patients, only one developed severe acute GVHD. Ten patients were alive at a median follow-up of 18 months (range, 5-62 months). The 5-years' OS was 53.8%. The three patients with FA are currently well with full-donor chimerism at 16, 6 and 5 months post transplant, respectively. The OS of 14 patients with high-risk hematologic malignancies was 47.6%. Three patients died as a result of post transplant leukemia relapse. CMV infection, GVHD and organ injury were other causes of mortality. Haploidentical SCT was found to be an alternative feasible treatment in Uruguay for patients who need allogenic transplantation but lack an HLA-identical family donor. It should be considered as an early option in FA patients before transformation or significant exposure to blood products.

A recent approach based on self-organizing maps (SOMs) to extract patterns from three-way data, named MOLMAP, was applied in a four-seasons study on soil pollution and its results compared with three different conventional approaches: Parallel factor analysis (PARAFAC), matrix augmented principal components analysis (MA-PCA) and Procrustes rotation. Each sampling season comprised 92 roadsoil samples and 12 analytical variables (Cd, Co, Cu, Cr, Fe, Mn, Ni, Pb, Zn, loss on ignition, pH and humidity). It was found that all techniques yielded highly similar results as the samples became organized in two major groups, each with a differentiated pollution pattern. This confirmed MOLMAP as a reliable option to handle environmental three-way datasets and to extract accurate pollution patterns.

Lipschütz ulcer, also known as ulcus vulvae acutum, is a rare genital disease, usually seen in young women, before sexual activity onset which diagnosis is clinical and by exclusion, based on detailed history, a complete physical examination and full clinical recovery in about a month. We present the case of a 10 year old girl with gangrenous form of this entity (AU)

Los quistes aracnoideos son lesiones benignas, de origen congénito, formadas por una membrana aracnoidea, que contienen líquido cefalorraquídeo en su interior. Presentamos el caso de un niño de cinco años con una asimetría craneal con abombamiento frontoparietotemporal derecho. Refería vómitos ocasionales de un año de evolución y cefalea que no le despertaba por la noche desde hacía dos años, acompañada algunas veces de sonofobia y fotofobia. En las pruebas de imagen se visualizaba un gran quiste aracnoideo en la fosa craneal media derecha con desplazamiento de la línea media y colapso de las astas ventriculares adyacentes. Dado el tamaño del quiste y la clínica, se decidió tratamiento con cistocisternostomía endoscópica comunicando el quiste aracnoideo con las cisternas basales (AU)


Arachnoid cysts are benign, congenital malformations formed by an arachnoid membrane containing cerebrospinal fluid inside. We report the case of a 5 year old child with cranial asymmetry with right fronto-parieto-temporal bulging. Occasional vomiting a year of evolution and sometimes headache are refered, which did not wake him at night, accompanied by sonophobia and photophobia for two years. The imaging displays large arachnoid cyst in right middle cranial fossa with midline shift and collapse of adjacent ventricular horns. Selecting the correct dimensionality is critical for obtaining partial least squares (PLS) regression models with good predictive ability. Although calibration and validation sets are best established using experimental designs, industrial laboratories cannot afford such an approach. Typically, samples are collected in an (formally) undesigned way, spread over time and their measurements are included in routine measurement processes. This makes it hard to evaluate PLS model dimensionality. In this paper, classical criteria (leave-one-out cross-validation and adjusted Wold's criterion) are compared to recently proposed alternatives (smoothed PLS-PoLiSh and a randomization test) to seek out the optimum dimensionality of PLS models. Kerosene (jet fuel) samples were measured by attenuated total reflectance-mid-IR spectrometry and their spectra where used to predict eight important properties determined using reference methods that are time-consuming and prone to analytical errors. The alternative methods were shown to give reliable dimensionality predictions when compared to external validation. By contrast, the simpler methods seemed to be largely affected by the largest changes in the modeling capabilities of the first components.

A fast analytical tool based on attenuated total reflectance mid-IR spectrometry is presented to evaluate the origin of spilled hydrocarbons and to monitor their fate on the environment. Ten spectral band ratios are employed in univariate and multivariate studies (principal components analysis, cluster analysis, density functions - potential curves - and Kohonen self organizing maps). Two indexes monitor typical photooxidation processes, five are related to aromatic characteristics and three study aliphatic and branched chains. The case study considered here comprises 45 samples taken on beaches (from 2002 to 2005) after the Prestige carrier accident off the Galician coast and 104 samples corresponding to weathering studies deployed for the Prestige's fuel, four typical crude oils and a fuel oil. The univariate studies yield insightful views on the gross chemical evolution whereas the multivariate studies allow for simple and straightforward elucidations on whether the unknown samples match the Prestige's fuel. Besides, a good differentiation on the weathering patterns of light and heavy products is obtained.
