RESUMO

BACKGROUND: probiotic agents are increasingly used as over the counter drugs for the treatment of a variety of inflammatory and infectious conditions. The rationale of their use seems to restore a friendly bacterial flora in the gut. Complications of probiotic use, although rarely reported, can occur. OBSERVATION: we describe the case of a 54-year-old diabetic woman, who developed Lactobacillus rhamnosus septicemia while she was using probiotic oral treatment. Lactobacillus is a major component of probiotic agents. Her infection resolved after amoxicilline administration. DISCUSSION: this case highlights the complication of probiotic use, and perhaps the patient's predispositions to develop such complications.

RESUMO

We report a case of pericarditis following aorto-coronary by-pass surgery which gradually developed into chronic constrictive pericarditis. The disease was a long time ignored because symptoms are not specific. In our case the clinical presentation looked like a cirrhosis especially that our patient developed an hepatitis after blood transfusion. In this occasion criteria of diagnostic are reminded. Hemodynamic investigation is still the best method of diagnosis and the only treatment is surgical and consists of pericardial decortication.

RESUMO

Obstructive azoospermia constitutes 35 to 40% of azoospermia. Etiologies may be acquired or rarely congenital. Congenital utricular or Mullerian duct cyst may result in compression of ejaculatory ducts. However, they must be differentiated from exceptional wolffian or urogenital cysts, which are located at the same site but in which ejaculatory ducts may drain inside. We report a case of retroprostatic duct cyst diagnosed during work up of male infertility where magnetic resonance imaging (MRI) showed the drainage of both ejaculatory ducts inside and the presence of a thin canal ended blindly in the wall of prostatic urethra. Transurethral resection guided by MRI has been performed resulting in urethral drainage of the cyst, relieving of obstruction and normalization of spermogramm.
