RESUMO
Evidence-based behavior change interventions addressing health systems must be identified and disseminated to improve child health outcomes. Studies of the efficacy of such interventions were identified from systematic searches of the published literature. Two hundred twenty-nine of the initially identified references were judged to be relevant and were further reviewed for the quality and strength of the evidence. Studies were eligible if an intervention addressed policy or health systems interventions, measured relevant behavioral or health outcomes (e.g., nutrition, childhood immunization, malaria prevention and treatment), used at least a moderate quality research design, and were implemented in low- or middle-income countries. Policy or systems interventions able to produce behavior change reviewed included media (e.g., mass media, social media), community mobilization, educational programs (for caregivers, communities, or providers), social marketing, opinion leadership, economic incentives (for both caregiver and provider), health systems strengthening/policy/legislation, and others. Recommendations for policy, practice, and research are given based on fairly strong data across the areas of health service delivery, health workforce, health financing, governance and leadership, and research.

Health promotion and disease prevention have huge impact on health, yet given low priority, risk being overlooked in universal health coverage efforts. To effectively prioritize promotion and prevention, strong cadres of personnel are needed with expertise in legislation and health policy, social and behavior change communication, prevention and community health, health journalism, environmental health, and multisectoral health promotion.

This paper has reviewed the case for action and sketched the outline of a community-based program to reduce the practice of corporal punishment at home and in schools in selected countries of Latin America and the Caribbean. This regional effort in only now beginning and, admittedly, its goal are almost as distant as the horizon. The authors are fully aware that the initiative has a quixotic quality; however, in the face of the violence crippling our communities, we are left with no other option

It presents a conceptual framework and suggests guidelines to develop programmatic activities in health communication, health education and community participation to further implement the Regional Plan of Action for Health Promotion (PAHO/WHO 1994). The lines of action and program areas in health education, health communication and community participation, the principal tools of health promotion, are the basis for national and community interventions for improving the quality of life and well-being of the peoples of the Americas (AU)
