BACKGROUND: The Abbé flap is a two-staged procedure to address upper lip tightness, creating a surgically closed mouth during the first stage. Airway manipulation and management in the setting of a surgically closed mouth presents a challenge from an anesthetic standpoint. AIMS: This study aims to describe the authors' standardized approach to airway management in cleft lip patients undergoing Abbé flap reconstruction. METHODS: A retrospective review was performed including consecutive patients who underwent Abbe flap reconstruction at a single institution from 2019 to 2021. Five patients were included, and information regarding airway, intubation sequence, and emergence was gathered. RESULTS: During the initial surgery, the airway was secured via nasotracheal intubation to allow for adequate surgical exposure. On emergence, with a newly constructed surgically closed mouth, the anesthesiologist forfeits the ability to reintubate should the patient fail extubation without the use of nasal fiberoptic guided intubation or flap deinset. In addition, any coughing or tension on the surgical site could cause inadvertent disruption to the integrity of the new flap. Dexmedetomidine titrated to effect was used to allow for smooth emergence, with the surgeon present. During the second stage, the patient was kept spontaneously breathing while local anesthetic and intravenous anxiolytic allowed for pedicle division. The patient was then orally intubated, and the flap was inset. All five patients had successful reconstruction with no airway concerns or events. CONCLUSIONS: The proposed standardized approach to airway management during Abbé flap reconstruction was safe and effective in this limited series of patients.

RESUMO

A 61-year-old man presented to the emergency department with second degree burns along his right arm and hand. He reported that his knees suddenly gave out while holding a pot of soup resulting in the scalding liquid spilling over his arm. His medical history is significant for alcohol abuse, chronic obstructive pulmonary disease, atrial fibrillation, congestive heart failure with preserved ejection fraction, chronic knee instability, and degenerative joint disease of the spine.

RESUMO

The erector spinae plane block (ESPB) is an interfascial plane block that has been used to provide perioperative analgesia for a variety of indications. This case report describes the novel use of the sacral ESPB on a transgender patient undergoing male-to-female gender reassignment surgery for perioperative pain control. The sacral ESPB technique was described and post-operative pain score and opioid requirements were reported. The sacral ESPB was successfully used as an alternative to neuraxial, caudal, or peripheral nerve blocks for gender reassignment surgery.

Pancoast tumors are non-small cell lung tumors, which can invade the ribs, vertebrae, sympathetic ganglia and brachial plexus. In this study, a patient with right-sided Pancoast tumor presented with intractable chronic pain on the right neck, upper extremity and chest wall. The chronic pain associated with Pancoast tumor, which was difficult to treat with opioids and other medications, was effectively treated with a high-thoracic erector spinae plane block (ESPB). Prolonged analgesia was provided with an ESP catheter to wean the patient from opioids. This case report provides an example where the novel interfacial ESP block can provide pain relief in challenging situations such as lung malignancies involving deeper structures and extensive areas of pain.

The US city of New Orleans was ranked second in the nation for estimated HIV case rates in 2011. Opt-out testing was established at the Interim Louisiana Hospital in New Orleans in 2013. The majority of new diagnoses were referred to the HIV outpatient program. We conducted a retrospective chart review of newly referred antiretroviral-naïve patients establishing HIV care between January 2009 and June 2013 to characterise demographic and genotype profiles to assist in clinical management and needed services. Of the eligible 226 patients, 68% were men, and 88% were African American. Nearly half of the study patients were younger than 35 years of age. Forty-six percent had an initial CD4 count <200 cells/mm(3), and 39% had a HIV viral load >100,000 copies/mL. The antiretroviral class with the most common major mutation was the non-nucleoside reverse transcriptase inhibitors (NNRTIs) where K103N was the most common major NNRTI mutation at presentation. We observed that male patients showed more advanced disease with later presentation to care, confirming the need for earlier HIV diagnosis. When considering initial antiretroviral therapy, baseline genotype information is encouraged, particularly if considering a NNRTI-based regimen.
