Effect of sodium pentosan polysulphate on the thrombogenicity of prothrombin complex concentrates.

The addition of pentosan polysulphate sodium (NaPPS) to thrombogenic prothrombin complex concentrates (PCC) dose-dependently reduces or abolishes thrombus formation in rats in the stasis model acc. to Wessler. However, no reduction of thrombogenicity was found in PCC preparations manufactured in the presence of NaPPS.

Albumin transport analysis of different collected and processed plasma products by electron spin resonance spectroscopy.

BACKGROUND: Changes of albumin conformation flexibility and binding characteristics in plasma products can be determined by electron spin resonance spectroscopy (ESR) with spin probes as reporter molecules. METHODS: Quality of albumin transport was analyzed in different processed plasma units using a new ESR method which provides estimation of albumin properties by examination of albumin molecular structure changes. RESULTS: In serum of healthy donors could not be obtained any abnormalities. Transport function of albumin in plasma products can be influenced by various methods of plasma collection, preparation, and filtration. Evident are changes in capacity of fatty acid binding sites and ordering changes of bound substrates. Transport respiratory detoxification efficiency is decreased between 30% and 70% in all FFP compared to control values. CONCLUSIONS: ESR spectroscopy can be used as a quality test for plasma. The ESR technique has a big potential for determination of conformation changes of macromolecules and as a global tumor marker test for donor screening.

[Preparation of prothrombin complex concentrates with reduced thrombogenicity]. / Herstellung von Prothrombinkomplexkonzentraten mit verminderter Thrombogenizität.

Thrombogenicity of prothrombin complex concentrates can be diminished by adding heparin to human citrate plasma, preferably before or during the adsorption of the prothrombin complex. Time, extent, and procedure of the heparin addition are dependent on the conditions of the initial material, the realization of the procedure and the provided application of the plasma supernatant after isolating the prothrombin complex. The procedure involves the use of buffer solutions containing heparin and antithrombin III. An activity of a procoagulatory effect cannot be identified in these preparations.

[Production and characterization of a new prothrombin complex concentrate]. / Herstellung und Charakterisierung eines neuen Prothrombinkomplexkonzentrates.

A procedure is described for producing prothrombin complex concentrates from human citrate plasma by adsorption to DEAE-sephadex A 50. The technical appliances developed are described. As compared with the level of technology an efficient manufacture of the preparations can be ensured by essentially simplifying the technique at a charge volume of 10 l.

[Preparation and characterization of an antithrombin III concentrate]. / Herstellung und Charakterisierung eines Antithrombin-III-Konzentrates.

Tri-calciumphosphate was found to have not only a known adsorption capacity for factors of the prothrombin complex, but also for antithrombin III. Depending on the inserted blood stabilizer the human plasma fractions Cohn I, PPSB and antithrombin III may be isolated from the same initial material in the area of the transfusion service. Enriching antithrombin III is achieved by a three-stage procedure under aseptic conditions in a closed system. Liberating antithrombin III from calciumphosphate is made with care without using any concentrated salt solutions.

[Thrombogenicity of a new prothrombin complex concentrate]. / Untersuchungen zur Thrombogenität eines neuen Prothrombinkomplexkonzentrates.

A prothrombin complex concentrate produced according to a new procedure was tested for potential thrombogenicity in vitro and in vivo. The results obtained by in vitro methods (NAPTT, TGt50, determination of the hydrolytic index) were not in the critical range of thrombogenicity. An effective dose of 175 U/kg for thrombus formation assessed in the stasis model of Wessler suggested comparatively low thrombogenicity. In the non stasis model no changes in the haemostaseological variables indicative of consumption reaction were observed after infusion of 125 U/kg for 30 min in rabbits. In the organs investigated (heart, liver, kidneys, adrenals) no thrombin were detected morphologically.

PCC's were isolated either by adsorption to DEAE-Sephadex A-50 (particle size 40-120 microns; Pharmacia Uppsala) or by adsorption to Molselect DEAE-50 (particle size 100-320 microns; Reanal Budapest) from human citrated plasma after separation of factor VIII by cryoprecipitation. The two products obtained (without heparin) were both compared by in vitro (NAPTT, TGt50) and in vivo (venous stasis) model in rats acc. to WESSLER) tests. The agent prepared by adsorption to DEAE-Sephadex A-50 showed a significantly higher thrombogenicity than that prepared by Molselect DEAE-50. ED50 values for thrombus formation to amounted 15 Factor IX U/kg for the first product and to 210 factor IX U/kg for the second.

Fibrin monomer complexes (SFMC) after substitution of antithrombin III (AT III) in consumption coagulopathy.

The report refers to substitution therapy of 33 patients who suffered consumption coagulopathy. Various blood coagulation and fibrinolytic variables were measured. After successful AT III donation to patients suffering DIC, soluble fibrin monomer complexes (SFMC) disappeared within 0.5-12 hours. If AT III decreases SFMC proves positive again. In addition to analysis of AT III we recommend to analyse SFMC to detect thrombin induced consumption reaction (DIC). Furthermore we found the fibrin split product D-dimer was a particularly sensitive indicator of DIC in case of hyperfibrinolysis (D-dimer was analysed in six patients). The reactions of fibronectin and SFMC proved inversely proportional.
