RESUMO

We demonstrate a high power Yb-doped burst-mode all-fiber laser system operating at GHz intra-burst repetition rate. To our knowledge, it is the first report utilizing dissipative soliton resonance (DSR) to generate tunable burst-mode rectangular pulses. Due to the tunable duration and the rapid rise/fall time for DSR pulses, a 1-10 ns adjustable burst pulse duration is achieved. The intra-burst with sinusoidal waveform can be tuned from 0.8 GHz to 1.5 GHz and actively modulated by an electro-optic modulator (EOM). Amplified by a three-stage Yb-doped fiber amplifier (YDFA), the output power achieves 304 W at 10 ns of burst duration, and the maximum peak power reaches over 50 kW at 2 ns of burst duration. This laser system is anticipated to be applied to generate high power arbitrary microwave signal.

OBJECTIVE: This study assessed the effect of omentectomy on the prognosis and fertility in patients with clinically early-stage (I, II) malignant ovarian germ cell tumours (MOGCT). DESIGN: A retrospective multicentre study. SETTING: Four university teaching hospitals in China. POPULATION: A total of 268 patients with clinically apparent early-stage (I, II) MOGCT. METHODS: Data were obtained from the medical records. Additionally, the propensity score matching (PSM) algorithm was adopted. MAIN OUTCOME MEASURES: Prognostic outcomes were disease-free survival (DFS) and overall survival (OS). Fertility outcomes were pregnancy and live birth rates. RESULTS: A total of 187 (69.8%) patients underwent omentectomy. Kaplan-Meier analysis showed no significant differences in DFS and OS between the omentectomy and non-omentectomy groups before and after PSM (p > 0.05). Additionally, subgroup analysis stratified by age (<18 and ≥18 years) showed similar results. International Federation of Gynecology and Obstetrics (FIGO) stage was the only risk factor associated with DFS (hazard ratio [HR] 14.71, 95% confidence interval [CI] 4.47-48.38, p < 0.001) and OS (HR 37.36, 95% CI 3.87-361.16, p = 0.002). Pregnancy and live birth rates in the total population were 80.3% and 66.7%, respectively. There were no significant differences between the two groups before and after PSM. CONCLUSIONS: Omentectomy did not improve survival or affect fertility in patients with clinically apparent early-stage (I, II) MOGCT, regardless of the age. The clinical FIGO stage was an independent risk factor for recurrence and death.

The ascites that develops in advanced OC, both at diagnosis and upon recurrence, is a rich source of multicellular spheroids/aggregates (MCSs/MCAs), which are the major seeds of tumor cell dissemination within the abdominal cavity. However, the molecular mechanism by which specific ascites-derived tumor cells survive and metastasize remains largely unknown. In this study, we elucidated cancer stem cell (CSC) properties of ascites-derived MCSs, concomitant with enhanced malignancy, induced EMT, and low KLF9 (Krüppel-like factor 9) expression, compared with PTCs. KLF9 was also downregulated in OC cell line-derived spheroids and the CD117+ CD44+ subpopulation in MCSs. Functional experiments demonstrated that KLF9 negatively modulated stem-like properties in OC cells. Mechanistic studies revealed that KLF9 reduced the transcriptional expression of Notch1 by directly binding to the Notch1 promoter, thereby inhibiting the function of slug in a CSL-dependent manner. Clinically, expression of KLF9 was associated with histological grade and loss of KLF9 predicts poor prognosis in OC.

A 10-watt-level mid-infrared supercontinuum (SC) spanning 0.8-4.7 µm with ultra-high-power conversion efficiency is generated in a piece of fluoroindate (InF3) fiber. The pump laser is a master oscillator power amplifier system seeded by a mode-locked fiber laser operating at 1956 nm with pulse repetition rate and pulse duration of 33 MHz and 60 ps, respectively. A piece of InF3 fiber is fusion spliced to the output end of the pump light source with a low fusion splicing loss of 0.12 dB. An endcap made of multimode aluminum fluoride fiber is used to protect the fiber tip from possible optical or thermal damage. Benefiting from the fiber endcap as well as the low-loss splicing joint between silica and InF3 fiber, high-power SC generation is achieved with maximal output power of 11.3 W. Furthermore, the long wavelength edge of the obtained SC spectrum is extended to 4.7 µm. This Letter, to the best of our knowledge, not only demonstrates the first 10-watt-level SC generation in InF3 fibers, but also achieves record power conversion efficiency of 66.5% among reported 10-watt-level fluoride-fiber-based SC laser sources.

We demonstrate an all-fiber widely wavelength-tunable thulium-doped fiber laser (TDFL) mode locked by a semiconductor saturable absorber mirror (SESAM). The tuning range spans 121 nm, from 1862 nm to 1983 nm. The central wavelength is tuned by a grating-based tunable filter in the ring laser cavity. To the best of our knowledge, this is so far the most widely wavelength-tunable TDFL mode locked by SESAM.

We demonstrated an all-normal-dispersion Yb-doped mode-locked fiber laser based on Bi2Se3 topological insulator (TI). Different from previous TI-mode-locked fiber lasers in which TIs were mixed with film-forming agent, we used a special way to paste a well-proportioned pure TI on a fiber end-facet. In this way, the effect of the film-forming agent could be removed, thus the heat deposition was relieved and damage threshold could be improved. The modulation depth of the Bi2Se3 film was measured to be 5.2%. When we used the Bi2Se3 film in the Yb-doped fiber laser, the mode locked pulses with pulse energy of 0.756 nJ, pulse width of 46 ps and the repetition rate of 44.6 MHz were obtained. The maximum average output power was 33.7 mW. When the pump power exceeded 270 mW, the laser can operate in multiple pulse state that six-pulse regime can be realized. This contribution indicates that Bi2Se3 has an attractive optoelectronic property at 1µm waveband.

We demonstrated a high-repetition-rate Q-switched fiber laser with topological insulator Bi2Se3 absorber. The absorber was made into a film structure by spin-coating method using few-layer Bi2Se3 nano-platelets which had regular shape. The uniform film had a low saturable optical intensity of 11 MW/cm(2), which is the lowest saturable optical intensity in the saturable absorbers made by topological insulator till now. By inserting the absorber film into an Erbium-doped fiber laser, a high-repetition Q-switched laser with the repetition rates from 459 kHz to 940 kHz was achieved. The maximum output power was 22.35 mW with the shortest pulse duration of 1.9 µs. To the best of our knowledge, both of the repetition rate and the output power were the highest values among the Q-switched fiber lasers with topological insulator absorber.

Background: Colpocleisis is one of traditional surgical procedures for elderly and frail women with advanced pelvic organ prolapse. The occurrence of de novo urinary incontinence following colpocleisis was considered to impair the postoperative quality of life. The incidence of de novo urinary incontinence after colpocleisis has been reported to be ranging from 6.6 % to 27 %. There was an absence of prospective large-sample study to investigate the accurate incidence of de novo urinary incontinence following colpocleisis and the impact on the quality of life till now. Purpose: s The primary objective was to report the incidence of de novo urinary incontinence after colpocleisis. The second objectives were to evaluate the long-term quality of life in patients with de novo urinary incontinence, and to conduct detailed pre- and post-operative evaluations of lower urinary tract symptoms. Methods: This prospective study included 253 patients with symptomatic pelvic organ prolapse who underwent colpocleisis between 2009 and 2021. De novo urinary incontinence was defined as the occurrence of urinary incontinence 3 months postoperatively. All patients were required to complete the Urinary Distress Inventory questionnaire and the Urinary Impact Questionnaire for the evaluation of patients' quality of life, and the Patient Global Impression of Improvement questionnaire for the evaluation of patients' satisfaction. Results: 245 patients (245/253, 96·8 %) completed the 3-month follow-up, and were included in the final analysis. The incidence of de novo urinary incontinence was 5.4 % (10/185). There was no significant difference in the Urinary Distress Inventory -6 scores (22.50 vs. 10.30, P = 0.276) or the subjective satisfaction rate (100 % vs. 98.9 %, P = 0.250) between the patients with or without de novo urinary incontinence at the long-term follow-up. The incidence of voiding difficulty was significantly reduced after colpocleisis (27.8 % vs. 0.0 %, P < 0.001). The patients' quality of life indicated by Urinary Distress Inventory-6 and Urinary Impact Questionnaire-7 scores were significantly improved postoperatively (26.27 vs. 13.39, and 19.13 vs. 6.05, P < 0.05). Conclusion: The incidence of de novo urinary incontinence after colpocleisis was very low. Patients' quality of life, and low urinary tract symptoms were significantly improved after colpocleisis.

Emerging per-and polyfluoroalkyl substances (PFASs) and traditional organochlorine pesticides (OCPs), polycyclic aromatic hydrocarbons (PAHs) in the marginal seas of China were analyzed to study the occurrence, transport and phase partitioning. The influence of organic carbon (OC) and element carbon (EC) on particulate emerging pollutants in seawater was studied for the first time. The concentrations of PFASs, OCPs and PAHs in the seawater (dissolved phase plus particulate phase) ranged from 1.4 to 8.6, 0.76 to 4.3 and 8.4 to 130 ng L-1, respectively. Pollutants in the northern East China Sea were generally higher than that in the southern East China Sea and South China Sea, which may be attribute to river discharges and land sources in the Yangtze River Delta. The Yellow Sea Coastal Current and Yangtze River Dilute Water drove the transport of contaminants from north to south marginal seas. Positive correlations between EC and PAHs were found, which can be explained by co-emission of them during combustion. Moreover, positive correlations between OC, EC and Log Kd for BkF, BeP, HCB, 6:2 FTSA were found, which demonstrated that OC and EC promoted the partitioning of these high oleophilic compounds to suspended particle.

Ovarian cancer is the most lethal gynecological malignancy. Recurrence and chemoresistance are tough challenges leading to poor prognosis. HJURP is a molecular chaperone of CENP-A, which is associated with aggressive progression in multiple tumors. However, the function of HJURP in ovarian cancer has not been elucidated. In our study, we found HJURP was over-expressed in ovarian cancer and high expression of HJURP was correlated to unfavorable prognosis. HJURP knockdown could inhibit proliferation, metastasis and induce G0/G1 stagnation of ovarian cancer cells. Besides, next-generation sequencing (NGS) unveiled that WEE1 was down-regulated by silencing HJURP. Further mechanistic research revealed that HJURP regulated WEE1 through MYC, and luciferase assay indicated that MYC was a transcription factor of WEE1. Additionally, we investigated that silencing HJURP increased sensitivity of ovarian cancer cells to cisplatin via MYC/WEE1 axis, and HJURP participated in DNA repair of cisplatin-induced damage. More interestingly, silencing HJURP could enhance sensitivity of ovarian cancer cells to AZD1775 and improve the synergistic effect of cisplatin plus AZD1775 combined therapy. Collectively, our data displays that HJURP promotes tumor progression and chemoresistance of ovarian cancer, and HJURP has potential to be a novel therapeutic target in the combined treatment with cisplatin and AZD1775 in ovarian cancer.

Background: The treatment and prognostic factors of intravenous leiomyomatosis (IVL) remain lacking systematic evidence. Methods: A retrospective study was conducted on IVL patients from the Qilu Hospital of Shandong University, and IVL cases were published in PubMed, MEDLINE, Embase and Cochrane Library databases. Descriptive statistics were used for the basic characteristics of patients. The Cox proportional hazards regression analysis was used to assess the high-risk factors related to the progression-free survival (PFS). The comparison of survival curves was performed by Kaplan-Meier analysis. Results: A total of 361 IVL patients were included in this study, 38 patients from Qilu Hospital of Shandong University, and 323 patients from the published literature. Age ≤45 years was observed in 173 (47.9%) patients. According to the clinical staging criteria, stage I/II was observed in 125 (34.6%) patients, and stage III/IV was observed in 221 (61.2%) patients. Dyspnea, orthopnea, and cough were observed in 108 (29.9%) patients. Completed tumor resection was observed in 216 (59.8%) patients, and uncompleted tumor resection was observed in 58 (16.1%) patients. Median follow-up period was 12 months (range 0-194 months), and 68 (18.8%) recurrences or deaths were identified. The adjusted multivariable Cox proportional hazard analysis showed age ≤45 years (vs. >45) (hazard ratio [HR] = 2.09, 95% confidence interval [CI] 1.15-3.80, p = 0.016), and uncompleted tumor resection (vs. completed tumor resection) (HR = 22.03, 95% CI 8.31-58.36, p < 0.001) were high-risk factors related to the PFS. Conclusion: Patients with IVL have a high probability of recurrence after surgery and a poor prognosis. Patients younger than 45 years and with uncompleted tumor resection are at higher risk of postoperative recurrence or death.

High-grade serous ovarian carcinoma (HGSOC) is one of the most lethal gynecological malignancies; however, the precise molecular mechanisms have not been fully characterized. Fibulin­5 (FBLN­5) is an extracellular matrix (ECM) glycoprotein, and plays a crucial role in maintaining the stability of ECM structures, regulating cell proliferation and tumorigenesis. In the present study, the expression of FBLN­5, as determined by western blot analysis and immunohistochemistry, was significantly increased in normal fallopian tube (FT) samples compared with that in HGSOC samples, and decreased FBLN5 expression was associated with unfavorable prognosis of HGSOC. Functional characterization revealed that FBLN5 overexpression significantly inhibited migration, invasion and proliferation abilities of ovarian cancer cells in vitro. Furthermore, micro (mi)RNA­27a­3p (miR­27a­3p) was revealed to be increased in HGSOC, and dual­luciferase reporter assay indicated that miR­27a­3p was functioned as a negative regulator of FBLN5 by directly binding with its 3'­untranslated region. Collectively, FBLN5 expression was associated with prognosis, proliferation, and metastasis in HGSOC. We hypothesized that FBLN5 was targeted by miR­27a­3p and may serve as a biomarker and provide a new therapeutic approach for the treatment of HGSOC.

Twenty-four per- and polyfluoroalkyl substances (PFASs) were analyzed in water and sediment from the Jiulong Estuary-Xiamen Bay to study their seasonal variations, transport, partitioning behavior and ecological risks. The total concentration of PFASs in water ranged from 11 to 98 ng L-1 (average 45 ng L-1) during the dry season, 0.19-5.7 ng L-1 (average 1.5 ng L-1) during the wet season, and 3.0-5.4 ng g-1 dw (average 3.9 ng g-1 dw) in sediment. In water samples, short-chain PFASs were dominated by perfluorooctanoic acid (PFBA) in the dry season and perfluorobutane sulfonate (PFBS) in the wet season, while long chain PFASs, such as perfluorooctane sulfonate (PFOS), dominated in the sediment. The highest concentration of PFASs in water were found in the estuary; in contrast, the highest level of PFASs in sediment were found in Xiamen Bay. These spatial distributions of PFASs indicate that river discharge is the main source of PFASs in estuarine water, while the harbor, airport and wastewater treatment plant near Xiamen Bay may be responsible for the high PFBS and PFOS concentrations in water and sediment. The partition coefficients (log Kd) of PFASs between sediment and water (range from 1.64 to 4.14) increased with carbon chain length (R2 = 0.99) and also showed a positive relationship with salinity. A preliminary environmental risk assessment indicated that PFOS and perfluorooctanoic acid (PFOA) in water and sediment pose no significant ecological risk to organisms.
