RESUMO

Debris flows are dense and fast-moving complex suspensions of soil and water that threaten lives and infrastructure. Assessing the hazard potential of debris flows requires predicting yield and flow behavior. Reported measurements of rheology for debris flow slurries are highly variable and sometimes contradictory due to heterogeneity in particle composition and volume fraction ([Formula: see text]) and also inconsistent measurement methods. Here we examine the composition and flow behavior of source materials that formed the postwildfire debris flows in Montecito, CA, in 2018, for a wide range of [Formula: see text] that encapsulates debris flow formation by overland flow. We find that shear viscosity and yield stress are controlled by the distance from jamming, [Formula: see text], where the jamming fraction [Formula: see text] is a material parameter that depends on grain size polydispersity and friction. By rescaling shear and viscous stresses to account for these effects, the data collapse onto a simple nondimensional flow curve indicative of a Bingham plastic (viscoplastic) fluid. Given the highly nonlinear dependence of rheology on [Formula: see text], our findings suggest that determining the jamming fraction for natural materials will significantly improve flow models for geophysical suspensions such as hyperconcentrated flows and debris flows.

RESUMO

More than a hundred hydropower dams have already been built in the Amazon basin and numerous proposals for further dam constructions are under consideration. The accumulated negative environmental effects of existing dams and proposed dams, if constructed, will trigger massive hydrophysical and biotic disturbances that will affect the Amazon basin's floodplains, estuary and sediment plume. We introduce a Dam Environmental Vulnerability Index to quantify the current and potential impacts of dams in the basin. The scale of foreseeable environmental degradation indicates the need for collective action among nations and states to avoid cumulative, far-reaching impacts. We suggest institutional innovations to assess and avoid the likely impoverishment of Amazon rivers.

BACKGROUND: An estimated 3.5 million people in the UK live with a rare disease however due to the rarity of each individual condition this is not currently reflected in mainstream medical education. As a result, common features of living with a rare condition include diagnostic delay, poor coordination of health and social care and lack of access to specialist care and treatment. This is well documented in reports published by patient advocacy groups collating the patient experience and has been highlighted by the Department of Health and Social Care in its UK Rare Diseases Framework. One of the four priority areas outlined in this policy published in 2021 is 'increasing awareness amongst healthcare professionals'. Medics4RareDiseases (M4RD), a charity based in the UK, has proposed a disease-agnostic approach to educating doctors about rare disease, focusing on the common challenges experienced across this heterogeneous collection of conditions, rather than on the minutiae of each of the > 7000 rare conditions. A literature search using MEDLINE, PubMed Central and Bookshelf confirmed a lack of broad rare disease teaching in medical literature; none of the 10 final resources identified focused on the topic as a whole. RESULTS: To address this, M4RD created the course 'Rare Disease 101'. It is accessed online using a learning management system that is free, contains interactive lessons, hosts a discussion board and is easily updated. In the 29 months since going live, 942 individuals have registered with 204 having completed the course; early feedback from 33 respondents was unanimously positive (all participants rated at least good (76%: excellent)) demonstrating that both clinicians and patients can benefit from broad rare disease education. The course is freely available to all at https://learn.m4rd.org/ . CONCLUSIONS: Disease-agnostic training about rare disease as a large patient population, focusing on its unique profile of unmet needs, is required. Rare Disease 101 provides a pragmatic approach to an educational challenge that leads to poor patient outcomes. Early results suggest that the educational programme is well-received but further evaluation and assessment is needed.

INTRODUCTION: Over one billion people live with disability worldwide, of whom 80% are in developing countries. Robust childhood disability data are limited, particularly as tools for identifying disability function poorly at young ages. METHODS: A subgroup of children enrolled in the Sanitation Hygiene Infant Nutrition Efficacy (SHINE) trial (a cluster-randomised, community-based, 2x2 factorial trial in two rural districts in Zimbabwe) had neurodevelopmental assessments at 2 years of age. We evaluated functional difficulty prevalence in HIV-exposed and HIV-unexposed children using the Washington Group Child Functioning Module (WGCFM), comparing absolute difference using chi-squared or Fisher's exact tests. Concurrent validity with the Malawi Developmental Assessment Tool (MDAT) was assessed using logistic regression with cohort MDAT score quartiles, linear regression for unit-increase in raw scores and a Generalised Estimating Equation approach (to adjust for clusters) to compare MDAT scores of those with and without functional difficulty. A 3-step, cluster-adjusted multivariable regression model was then carried out to examine risk factors for functional difficulty. FINDINGS: Functional Difficulty prevalence was 4.2% (95%CI: 3.2%, 5.2%) in HIV-unexposed children (n = 1606) versus 6.1% (95%CI: 3.5%, 8.9%) in HIV-exposed children (n = 314) (absolute difference 1.9%, 95%CI: -0.93%, 4.69%; p = 0.14). Functional difficulty score correlated negatively with MDAT: for each unit increase in WGCFM score, children completed 2.6 (95%CI: 2.2, 3.1) fewer MDAT items (p = 0.001). Children from families with food insecurity and poorer housing were more at risk of functional difficulty. INTERPRETATION: Functional difficulty was identified in approximately 1-in-20 children in rural Zimbabwe, which is comparable to prevalence in previous studies. WGCFM showed concurrent validity with the MDAT, supporting its use in early childhood.

Continental-scale rivers with a sandy bed sequester a significant proportion of their sediment load in flood plains. The spatial extent and depths of such deposits have been described, and flood-plain accumulation has been determined at decadal timescales, but it has not been possible to identify discrete events or to resolve deposition on near-annual timescales. Here we analyse (210)Pb activity profiles from sediment cores taken in the pristine Beni and Mamore river basins, which together comprise 720,000 km2 of the Amazon basin, to investigate sediment accumulation patterns in the Andean-Amazonian foreland. We find that in most locations, sediment stratigraphy is dominated by discrete packages of sediments of uniform age, which are typically 20-80 cm thick, with system-wide recurrence intervals of about 8 yr, indicating relatively rare episodic deposition events. Ocean temperature and stream flow records link these episodic events to rapidly rising floods associated with La Niña events, which debouch extraordinary volumes of sediments from the Andes. We conclude that transient processes driven by the El Niño/Southern Oscillation cycle control the formation of the Bolivian flood plains and modulate downstream delivery of sediments as well as associated carbon, nutrients and pollutants to the Amazon main stem.

Urban disaster response requires disposal of complex wastes. This study regards a case wherein high intensity rainfall fell over a remote mountainous area previously burned by wildfire, generating debris flows that devastated a downstream town. Sanitary sewers and homes with septic systems were damaged, releasing human waste into the debris flow field. Contaminated sediments, with their high fecal indicator bacteria (FIB) concentrations, were cleared from public rights-of-way and creek channels by local authorities, then disposed onto distant Goleta Beach for beach nourishment, causing immediate surf zone microbiological water quality exceedances. To determine potential public health threats, disposed sediments and surf zone waters were sampled and analyzed-relative to reference samples of mountain soil and raw sewage-for FIB, pathogens, human (HF183) and other host- (Gull2 TaqMan, and DogBact) associated DNA-based fecal markers, and bacterial community 16S rRNA gene sequences. Approximately 20% of disposed sediment samples contained the HF183 marker; sequencing suggested that all samples were contaminated by sewage. In an initial sediment disposal period, surf zone waters harbored intestinal bacterial sequences that were shared with disposed sediments and sewage. Yet surf zone bacterial communities returned to mostly marine clades within weeks. Taken together, multiple conventional and DNA-based analyses informed this forensic assessment of human waste contamination. In the future, similar analyses could be used earlier in disaster response to guide sediment disposal decisions towards continuously protecting beachgoer health.

Glycogen storage disease type 1 (GSD-1) is a group of inherited metabolic disorders characterised by the inability to use intracellular glucose stores. It is associated with a high risk of hypoglycaemia, as well as long-term complications including growth retardation, hepatocellular adenomas, renal disease, hypertriglyceridaemia and hyperuricaemia. Treatment involves slow absorption carbohydrates, for example, cornstarch. We present a case of acute psychosis in a patient with GSD-1a. This was initially attributed to his opiate use. Later in his management an MRI scan of his head was performed which revealed regions of brain atrophy following significant hypoglycaemic insult, thus identifying an organic cause of his psychosis. This case presents a rare complication of glycogen storage disease: organic psychosis attributable to cortical atrophy from profound hypoglycaemic insult. It emphasises the importance of investigating organic causes of psychiatric symptoms.

The rate and extent of deforestation determine the timing and magnitude of disturbance to both terrestrial and aquatic ecosystems. Rapid change can lead to transient impacts to hydrology and biogeochemistry, while complete and permanent conversion to other land uses can lead to chronic changes. A large population of watershed boundaries (N=4788) and a time series of Landsat TM imagery (1975-1999) in the southwestern Amazon Basin showed that even small watersheds (2.5-15 km2) were deforested relatively slowly over 7-21 years. Less than 1% of all small watersheds were more than 50% cleared in a single year, and clearing rates averaged 5.6%/yr during active clearing. A large proportion (26%) of the small watersheds had a cumulative deforestation extent of more than 75%. The cumulative deforestation extent was highly spatially autocorrelated up to a 100-150 km lag due to the geometry of the agricultural zone and road network, so watersheds as large as approximately 40000 km2 were more than 50% deforested by 1999. The rate of deforestation had minimal spatial autocorrelation beyond a lag of approximately 30 km, and the mean rate decreased rapidly with increasing area. Approximately 85% of the cleared area remained in pasture, so deforestation in watersheds of Rondônia was a relatively slow, permanent, and complete transition to pasture, rather than a rapid, transient, and partial cutting with regrowth. Given the observed landcover transitions, the regional stream biogeochemical response is likely to resemble the chronic changes observed in streams draining established pastures, rather than a temporary pulse from slash-and-burn.

Auditory verbal hallucinations (AVH) often lead to distress and functional disability, and are frequently associated with psychotic illness. Previously both state and trait magnetic resonance imaging (MRI) studies of AVH have identified activity in brain regions involving auditory processing, language, memory and areas of default mode network (DMN) and salience network (SN). Current evidence is clouded by research mainly in participants on long-term medication, with chronic illness and by choice of seed regions made 'a priori'. Thus, the aim of this study was to elucidate the intrinsic functional connectivity in patients presenting with first episode psychosis (FEP). Resting state functional MRI data were available from 18 FEP patients, 9 of whom also experienced AVH of sufficient duration in the scanner and had symptom capture functional MRI (sc fMRI), together with 18 healthy controls. Symptom capture results were used to accurately identify specific brain regions active during AVH; including the superior temporal cortex, insula, precuneus, posterior cingulate and parahippocampal complex. Using these as seed regions, patients with FEP and AVH showed increased resting sb-FC between parts of the SN and the DMN and between the SN and the cerebellum, but reduced sb-FC between the claustrum and the insula, compared to healthy controls.It is possible that aberrant activity within the DMN and SN complex may be directly linked to impaired salience appraisal of internal activity and AVH generation. Furthermore, decreased intrinsic functional connectivity between the claustrum and the insula may lead to compensatory over activity in parts of the auditory network including areas involved in DMN, auditory processing, language and memory, potentially related to the complex and individual content of AVH when they occur.

A decline in breastfeeding rates in Samoa has been reported over the last century. To assess the length of time women breastfeed, their knowledge of both the advantages of and recommendations for breastfeeding, and the factors that influence their decisions to continue or discontinue breastfeeding, a questionnaire was distributed at Tupua Tamasese Meaole Hospital. One hundred and twenty-one eligible participants were included aged 18-50 years (mean age 28.2). Ninety percent of participants initiated breastfeeding, and the majority (78%) of babies were exclusively breastfed for at least the recommended 6 months. Many mothers introduced complementary (solid) foods later than World Health Organization (WHO) and United Nation's International Children's Fund (UNICEF) recommendations of 6 months. Awareness of the advantages of breastfeeding was mixed. The most widely known advantage was "the development of an emotional bond between mother and baby" (67%). Other advantages were less widely known. Only a small minority were aware that breastfeeding reduces risk of maternal diabetes and aids weight loss post partum. Doctors and healthcare workers were listed as the top factors encouraging breastfeeding. Participants' comments revealed a generally positive attitude towards breastfeeding, a very encouraging finding. Participants identified that the number of breastfeeding breaks available at work and the length of their maternity leave were factors discouraging breastfeeding. Future studies are necessary to determine if problems identified in this study are applicable on a national level. These could be important to determine measures to improve breastfeeding practices in Samoa.

Increased energy demand has led to plans for building many new dams in the western Amazon, mostly in the Andean region. Historical data and mechanistic scenarios are used to examine potential impacts above and below six of the largest dams planned for the region, including reductions in downstream sediment and nutrient supplies, changes in downstream flood pulse, changes in upstream and downstream fish yields, reservoir siltation, greenhouse gas emissions and mercury contamination. Together, these six dams are predicted to reduce the supply of sediments, phosphorus and nitrogen from the Andean region by 69, 67 and 57% and to the entire Amazon basin by 64, 51 and 23%, respectively. These large reductions in sediment and nutrient supplies will have major impacts on channel geomorphology, floodplain fertility and aquatic productivity. These effects will be greatest near the dams and extend to the lowland floodplains. Attenuation of the downstream flood pulse is expected to alter the survival, phenology and growth of floodplain vegetation and reduce fish yields below the dams. Reservoir filling times due to siltation are predicted to vary from 106-6240 years, affecting the storage performance of some dams. Total CO2 equivalent carbon emission from 4 Andean dams was expected to average 10 Tg y-1 during the first 30 years of operation, resulting in a MegaWatt weighted Carbon Emission Factor of 0.139 tons C MWhr-1. Mercury contamination in fish and local human populations is expected to increase both above and below the dams creating significant health risks. Reservoir fish yields will compensate some downstream losses, but increased mercury contamination could offset these benefits.

The fate and transport of mercury are of critical concern in lowland floodplains and wetlands worldwide, especially those with a history of upstream mining that increases the mobility of both dissolved and sediment-bound Hg in watersheds. A mass budget of total mercury (THg) quantifies sources and storage for particular areas - knowledge that is required for understanding of management options in lowland floodplains. In order to assess contaminant risk in the largest flood-control bypass, prime wetland, and restoration target in the Sacramento River basin, we estimated empirical relationships between THg, suspended sediment concentration (SSC), and streamflow (Q) for each of the major inputs and outputs using data from various publicly available sources. These relationships were improved by incorporating statistical representations of the dynamics of seasonal and intra-flood exhaustion (hysteresis) of sediment and mercury. Using continuous records of Q to estimate SSC suspended sediment flux and SSC to estimate THg flux, we computed the net transfer of sediment-adsorbed mercury through the Yolo Bypass over a decade, 1993-2003. Flood control weirs spilling Sacramento River floodwaters into the bypass deliver ~75% of the water and ~50% of the river's suspended sediment load, while one Coast Range tributary of the bypass, Cache Creek, contributes twice the THg load of the mainstem Sacramento. Although estimated sediment flux entering Yolo Bypass is balanced by efflux to the Sacramento/San Francisco Bay-Delta, there is much evidence of deposition and remobilization of sediment in Yolo Bypass during flooding. These factors point to the importance of the bypass as sedimentary reservoir and as an evolving substrate for biogeochemical processing of heavy metals. The estimates of mercury flux suggest net deposition of ~500 kg in the 24,000 ha floodway over a decade, dominated by two large floods, representing a storage reservoir for this important contaminant.

Despite increasingly large investments, the potential ecological effects of river restoration programs are still small compared to the degree of human alterations to physical and ecological function. Thus, it is rarely possible to "restore" pre-disturbance conditions; rather restoration programs (even large, well-funded ones) will nearly always involve multiple small projects, each of which can make some modest change to selected ecosystem processes and habitats. At present, such projects are typically selected based on their attributes as individual projects (e.g., consistency with programmatic goals of the funders, scientific soundness, and acceptance by local communities), and ease of implementation. Projects are rarely prioritized (at least explicitly) based on how they will cumulatively affect ecosystem function over coming decades. Such projections require an understanding of the form of the restoration response curve, or at least that we assume some plausible relations and estimate cumulative effects based thereon. Drawing on our experience with the CALFED Bay-Delta Ecosystem Restoration Program in California, we consider potential cumulative system-wide benefits of a restoration activity extensively implemented in the region: isolating/filling abandoned floodplain gravel pits captured by rivers to reduce predation of outmigrating juvenile salmon by exotic warmwater species inhabiting the pits. We present a simple spreadsheet model to show how different assumptions about gravel pit bathymetry and predator behavior would affect the cumulative benefits of multiple pit-filling and isolation projects, and how these insights could help managers prioritize which pits to fill.

Geological processes such as erosion and sedimentation redistribute toxic pollutants introduced to the landscape by mining, agriculture, weapons development, and other human activities. A significant portion of these contaminants is insoluble, adsorbing to soils and sediments after being released. Geologists have long understood that much of this sediment is stored in river floodplains, which are increasingly recognized as important nonpoint sources of pollution in rivers. However, the fate of contaminated sediment has generally been analyzed using hydrodynamic models of in-channel processes, ignoring particle exchange with the floodplain. Here, we present a stochastic theory of sediment redistribution in alluvial valley floors that tracks particle-bound pollutants and explicitly considers sediment storage within floodplains. We use the theory to model the future redistribution and radioactive decay of 137Cs currently stored on sediment in floodplains at the Los Alamos National Laboratory (LANL) in New Mexico. Model results indicate that floodplain storage significantly reduces the rate of sediment delivery from upper Los Alamos Canyon, allowing 50% of the 137Cs currently residing in the valley floor to decay radioactively before leaving LANL. A sensitivity analysis shows that the rate of sediment overturn in the valley (and hence, the total amount of radioactive 137Cs predicted to leave LANL) is significantly controlled by the rate of sediment exchange with the floodplain. Our results emphasize that flood plain sedimentation and erosion processes can strongly influence the redistribution of anthropogenic pollutants in fluvial environments. We introduce a new theoretical framework for examining this interaction, which can provide a scientific basis for decision-making in a wide range of river basin management scenarios.
