RESUMO

The valid law does not unequivocally define a physician's competence and obligations at the place of a subject's death outside the health-care facilities. The scheme creates a general instruction for the medical personnel to be used in cases of death outside health-care facilities. It is aimed at a better understanding of the problem, and it is supplemented with relevant legal regulations. The scheme is divided into 3 horizontal levels, the death being placed on the first level, the decision about autopsy on the second level, and the burial on the third level. These three levels are interconnected vertically, in order to represent the chronological sequence of individual steps and the respective decisions made by the examining physician. The schematic representation of the problems of death outside health-care facilities resulted in several lines of action, all that with regard to the medical and forensic aspects of the death. In every line of action, the work of the examining physician's is summarised.

RESUMO

Thrombosis and its complications were studied in a group of 110 autopsies of patients who had had a subclavian vein cannulation. A unique complete obliteration of vena cava superior was found in connection with cannulation thrombosis related to the cause of death. A mycotic septicaemia occurred in two cases. "Floated-off cannules" were presented in five cases.

RESUMO

Three cases of butane intoxication with fatal outcome are described. Butane was applied as a gaseous spray from the lighter refill can. With the exception of non-specific signs of asphyxiation such as tiny subepicardial and subpleural haemorrhages and lung oedema, haemorrhagic microruptures of the myocardial fibres were found. The authors have no plausible explanation regarding this development. Any mechanical damage of the myocardium can clearly be excluded.

The authors evaluate clinical experience based on a correlation between the clinical condition and the finding of Campylobacter pylori (CP) in a total of 1027 specimens of the gastric mucosa. They confirmed the high incidence of CP in patients with a fresh duodenal ulcer (g 7%) but also in the inactive stage of duodenal ulcer disease (97.8%) and in patients with a pyloric ulcer (94.4%). An analogous incidence of CP in patients with so-called isolated duodenitis (in the course of the disease no ulcer lesion was ever detected), i.e. in 95.5%, indicates the relationship of this condition with peptic ulcer disease. The authors indicate diagnostic possibilities as regards CP infection by cultivation, rapid urease tests and microscopic examination and they emphasize the practical value of urease tests. Based on their own experience the authors confirm the high eradication effect of bismuth preparations, in particular bismuth subsalicylate and colloid bismuth subcitrate.

From November 1972 to the end of 1975, Shigella sonnei types implicated in dysentery morbidity in a number of districts of East Bohemia region were studied. 301 strains subjected to phage typing and colicine typing were classified into 45 "combined" types. During the study five types prevailed in succession, each for a protracted period of time. Four of these types retained, in the great majority of cases, their original high drug sensitivity throughout the period of their prevalence. Other types were only encountered transiently and sporadically. As a result of a phagerestrictive effect of newly acquired plasmids there occurred, in rare instances in some strains, a change in phage type; or the colicine type changed while the phage type was retained as a result of another col factor having been acquired by the strain. The changes occurred in the course of epidemics.
