RESUMO

Experience in treatment of 164 patients with combined trauma of abdomen and pelvis is analyzed. Mean age of the patients was 40.1+/-17 years, 94 patients were male, 70 -- female. ISS was 28.6+/-11 points. Lethality was 44.5%, during first day -- 61.6%. High lethality may be associated with mistakes in surgical and traumatological policy. Abdominal surgery was performed in 64 (39%) patients, only in 24 (14.6%) of them laparotomy was curative, the rest 40 (85.4%) patients underwent diagnostic laparotomy. It is demonstrated that diagnostic laparotomy has negative influence on prognosis of combined trauma of the abdomen and pelvis. Adequate traumatological policy has also great influence on lethality, particularly during the first day. Overall lethality of patients with rotary-vertical instability of the pelvis was lower in the group with fixation of the pelvis than in conservatively treated patients (41.7 and 56.6%). Lethality during day 1 in patients with fixed pelvis was 0, without fixation of the pelvis -- 82%. It is concluded that verified indications for laparotomy and active traumatological policy improve treatment results in patients with combined trauma of abdomen and pelvis.

RESUMO

Under study were 260 men with stress disease. Three clinical forms are distinguished: abortive, typical with incomplete processes and classical ones. The undertaken treatment resulted in recovery of 92% of the patients. In 8% there were permanent or periodical pains and dysfunction of the diseased extremity under physical stress.

RESUMO

The state of the neuroendocrine system and protein metabolism was studied in 307 young servicemen having overload disease of bones. It was established that in 92.9% of the patients the level of growth hormone--somatotropic hormone, was considerably reduced, production of important for osteogenesis thyroid hormones was disturbed and dissociation of protein fractions was detected which is thought by the author to be the primary cause of the overload disease of the bones.

RESUMO

Overload disease (OD) of human osseous system appears after microtrauma and excessive and frequently repeated load. Examination of 260 patients with OD was carried out. Radioisotope method revealed disease in all cases while radiological method--in 91% of cases. Sudden decrease in growth hormone (HGH) was noted in 95% of cases. Classification of 5 stages of OD which can be used for expert practical work was developed.

RESUMO

There were studied 453 young servicemen with dyshormonal strain bone disease in whom the hormonal status, state of carbohydrate and protein metabolism were investigated. It was established that in 92.9% of patients the growth hormone--somatotropic hormone (STH) was significantly decreased, production of thyroid hormones necessary for physiologic osteogenesis (T3 and T4) was impaired and albuminous fraction dissociation was revealed. The authors consider that in dyshormonal strain bone disease in young people the insufficiency of hormonal regulation of growth processes and bone regeneration is the primary factor while the physical strain is the additional secondary factor.

RESUMO

Based on an experience with 597 patients the authors have formulated main principles of treatment of poly-traumas. Nine groups of patients were established depending on the severity of polytrauma. In 42.6% of patients with polytrauma urgent surgical procedures were necessary. Operations on bones in the early period were justified but for saving the patient's life as a component of antishock measures. It should be performed rapidly, atraumatically and without blood loss. The application of apparatuses of external fixation on the basis of wires is thought to be expedient at this period.
