RESUMO

Myringoplasty is a surgical procedure that has been routinely performed by otological surgeons for more than 40 years. Underlay and overlay are the most commonly used tympanoplastic techniques. A comparison was made of anatomic and functional results, and the year of intervention, for 97 myringoplasties, 54 using the underlay technique and 43 using the overlay technique. Selection of the overlay or underlay technique was guided by specific indications. Results were evaluated at least 1 year after surgery. No significant differences were found in anatomic success (88.8% for underlay and 97.6% for overlay). Favorable functional results (average gap of 0 to 20 dB for frequencies of 500, 1000, 2000, and 4000 Hz) were obtained in 88.4% of patients treated with the overlay technique and in 83% of those treated with the overlay technique, with no significant difference. We believe that the high percentage of good results was due to careful preoperative selection.

RESUMO

Tuberculosis is endemic in Spain and the primary site is pulmonary in 70% of cases. Extrapulmonary foci have increased in frequency and the incidence of tuberculous middle otitis, which often is difficult to diagnose, is about 0.04%. A case of chronic tuberculous middle otitis is reported.

La miringoplastia es un procedimiento quirúrgico que desde hace más de 40 años es práctica habitual y rutinaria entre los cirujanos otólogos. Dos técnicas, la underlay y la overlay, son las más comúnmente utilizadas para abordar las timpanoplastias. En este trabajo comparamos los resultados anatómicos y funcionales, al año de la intervención, de 97 miringoplastias, abordadas por técnica underlay en 54 casos y por técnica overlay en los 43 restantes. La selección de la técnica, over o underlay, no ha sido al azar sino guiada por indicaciones precisas. Se han evaluado los resultados al año de la intervención, como mínimo. No hemos encontrado diferencias significativas en los éxitos anatómicos obtenidos (88,8 por ciento para las underlay y 97,6 por ciento para las overlay). Los resultados funcionales favorables (gap promediado para las frecuencias de 500 Hz, 1, 2, y 4 KHz entre 0 y 20 d8) en el 88,4 por ciento de los pacientes operados por técnica underlay, y para el 83 por ciento de los pacientes operados por técnica overlay, tampoco ofrecen diferencias estadísticamente significativas. Creemos que estos elevados porcentajes de buenos resultados, con ambas técnicas, se deben a la cuidadosa selección realizada en el estudio preoperatorio (AU)
