RESUMO

The growing danger of a nuclear holocaust has intensified two aspects of the human predicament that concern both religion and psychiatry: the inevitability of death and the disastrous consequences of the characteristic termed "pride" by theologians and "narcissism" by psychiatrists. For the first time, humans have power to exterminate themselves and death threatens all ages equally. Pride of power causes leaders to exaggerate their ability to control nuclear weapons; moral pride leads to demonizing enemies. The author considers implications for psychiatrists and clergy, with special reference to preventing a nuclear holocaust.

RESUMO

The author examines the reasons that have been offered in support of a pressimistic appraisal of psychiatry's future. The consequences for psychiatry of increasingly detailed knowledge of the interaction of psychological and physiological processes are emphasized, and the author concludes that psychiatry will continue to play a dominant role in the study and treatment of medical and surgical illness. Thus, although the field is in a transition period, it is certainly not moribund.

Fractures associated with minimal trauma are common in aged human beings. However, bone safety margins are better preserved in aged dogs, which are rarely affected with minimal trauma fractures. Although the hierarchical architecture of canine and human compact bone is similar, the precise reasons for this species difference are unclear. Cyclic loading of bone during normal daily activity leads to the formation of microcracks within the tissue matrix of compact bone. Using a standard bulk-staining technique with basic fuchsin, we examined calcified transverse sections of the mid-diaphysis of the canine humerus from dogs of varying ages. We found that the amount of microdamage and porosity increased exponentially with aging, although the increases were relatively small, compared with human bone. Gender (female, ovariohysterectomized female, male, castrated male) did not have a significant effect on the amount of microdamage or porosity in bone. Alterations to the local material properties of bone tissue, or alterations to the local tissue repair responses, may play a role in the accumulation of microdamage in bone with aging. Determination of osteocyte lacunar density (number of osteocyte lacunae per bone area) and activation frequency (number of actively remodeling osteons per bone area per year) indicated that these variables declined exponentially with aging. There also was a trend for bone from dogs with low osteocyte lacunar density to have a higher microcrack density, but not higher porosity. Furthermore, bones with a high activation frequency did not accumulate microcracks or porosity. Taken together, these data suggest that, in canine bone, although a certain minimum number of osteocytes may be essential for an operational network that forms part of the signaling pathways that orchestrate repair of bone microdamage, increases in porosity with aging may not be directly associated with impaired function of the osteocyte network within bone.

Fibrin formation has been hypothesized to be an element of the metastatic process in cancer, and pharmacological interference with such fibrin formation has been proposed as a means of antimetastatic therapy. We have tested this hypothesis through an in vivo study of warfarin in two independent rat disease models--a model of chemical-injury-induced arterial thrombosis, and a model of spontaneous metastasis. We found 0.50 mg/kg-day warfarin to be uniformly lethal after two weeks treatment. The chronic dose of 0.25 mg/kg-day was non-toxic and produced effective anticoagulation and marked antithrombotic and antimetastatic activity. The 0.125 mg/kg-day dose produced a reduction in factor IIc (50%) and factor VIIc (70%), and resulted in statistically significant antithrombotic and antimetastatic activity. The 0.0625 mg/kg-day dose failed to reduce the vitamin K-dependent clotting factors, and failed to produce any antithrombotic or antimetastatic effects. The substantial correlation (very similar dose-response effects) among the anticoagulant, antithrombotic and antimetastatic efficacies of warfarin in the rat suggests that anticoagulation provides the pharmacological mechanism underlying both the antithrombotic and the antimetastatic effects. The poor therapeutic index we observed in the rat may be the attribute which limits the efficacy of warfarin in the treatment of human cancer.

Detailed antenatal sonography was performed on 18766 pregnant women between 1990 and 1994. Antenatal hydronephrosis, defined as an antero-posterior diameter of the renal pelvis (APPD) greater than 5 mm, was detected in 100 cases (0.59%). Sixty four infants had postnatal hydronephrosis at one and/or six weeks after delivery; 21 of these had urological anomalies. Twelve infants had vesico-ureteric reflux. In all refluxing units the APPD of the renal pelvis was less than 10 mm. Three patients had obstruction at the pelviureteric junction (PUJ); all required surgery. Vesico-ureteric reflux is emerging as the most common urological finding in infants with antenatal hydronephrosis and is likely to be missed if kidneys with APPD of less than 10 mm are not further investigated. In contrast, pelvi-ureteric junction obstruction may be overdiagnosed, based only on drainage patterns of dynamic renogram studies.

UNLABELLED: The ECG is a 12-lead-vector system and is known to contain redundant information. Factor analysis (FA) is a statistical technique that improves measured data and eliminates redundancy by identifying a minimum number of factors accounting for variance in the data set. OBJECTIVE: To identify the minimum number of lead-vectors required to predict the 12-lead ECG. METHODS: A total of 104 ECGs were obtained from 24 normal men, 22 normal women, and 28 men and 30 women with variable pathologies. Each ECG lead was simultaneously acquired and digitized, resulting in a voltage-time data array stored for mathematical analysis. Each array was factor-analyzed to identify the minimum number of lead-vectors spanning the ECG data space. The 12-lead ECG was then predicted from this minimum lead-vector set. ANOVA was used to test for statistical significance between normal and pathologic data groups. RESULTS: FA revealed that 3 lead-vectors accounted for 99.12%+/-0.92% (95% CI+/-0.18%) of the variance contained in the 12-lead ECG voltage-time data for all 104 cases. There were no statistically significant differences between men and women (99.25%+/-0.66% vs 98.98+/-1.11%; p=0.139). Statistically significant differences were noted between normal and acute myocardial infarction ECGs (99.5%+/-0.27% vs 98.66+/-1.25%; p=0.00003). The measured and predicted leads were almost identical. A 3-dimensional spatial ECG derived from the 3-lead-vector set resulted in variable curved surfaces that differed by pathology. CONCLUSIONS: The 12-lead ECG can be derived from only 3 measured leads and graphed as a 3-D spatial ECG. This type of data processing may lead to instantaneous acquisition and may enhance the diagnostic capability of the ECG from routine bedside telemetry equipment.

RESUMO

Mineralization of renal tissue was investigated using Von Kossa's method and capillary gap technology. Combining these techniques allowed the simultaneous processing of 100 slides, reduced the volume of 5% silver nitrate required, and produced consistent dark brown to black silver deposits in positive control sections. A 25-fold savings in silver nitrate was achieved. By markedly reducing individual slide handling, technician time also was greatly reduced. Two or three slide sets, each containing approximately 100 slides, can be conveniently stained daily.

RESUMO

This paper presents the hypothesis that psychotherapy seeks to produce changes in attitudes and behaviour through transformation of meanings. Patients come to psychotherapy because they are demoralized by the menacing meanings of their symptoms. The psychotherapist collaborates with the patient in formulating a plausible story that makes the meanings of the symptoms more benign and provides procedures for combating them, thereby enabling the patient to regain his morale. As a result, he feels better and functions more effectively, leading to progressive improvement. Various implications and limitations of this hypothesis are explored.

One hundred and twenty boys were referred by GPs over a 12-month period to a paediatric urologist for circumcision. The reasons for referral were: ballooning in 36, non-retraction in 28, balanoposthitis in 36 or a combination in 15. On examination 53% had a retractile, 21% a partially retractile and 21% a non-retractile foreskin. Six patients had obvious balanitis xerotica obliterans. Only one quarter of the patients required a circumcision. The penis was not examined by the referring doctor in 15 patients. The implications of this survey are that a large proportion of general practitioners have difficulty in discriminating between a true phimosis and a developmentally non-retractile foreskin. This diagnostic inaccuracy was greatest when the referring doctor did not examine the patient.

RESUMO

This presentation has considered only two of many psychological factors that contribute to the nuclear predicament. These are the power drive and conceptual inertia and how they encourage the propensity to violence. Psychotherapists can make an indirect but significant contribution toward human survival by calling attention particularly to the pernicious psychological effects of the misuse of language, either deliberate or resulting from conceptual inertia. We should seek to inform and educate the public, including political leaders, about this phenomenon. Those of us who have young children or grandchildren should seek to introduce into school and college curricula issues created by the splitting of the atom, since the young are best able to achieve the massive psychological changes necessary for coping with a new and unprecedented predicament.
