The present manuscript deals with the utility of the calibration-free LIBS and calibration curve methods for the compositional study of different alloys using laser-induced breakdown spectroscopy (LIBS). In the process of alloying in the smelting industry, metal concentration in different alloys affects the physical and chemical properties of the final products. Therefore, LIBS can be used as an efficient quantitative analysis tool for online monitoring of the quality of the products. This is because LIBS can be performed online, in situ, without any pre-processing, and need no sample preparation for the compositional analysis of any type of materials present in any phase (solid, liquid, gas or even molten alloys in the industries). In the present study, four alloys (three copper and one iron-based alloy) consisting of Cu, Al, Zn, Ni, Fe, Cr and Mn as major and Sn and Si as minor elements were selected for the study using calibration-free laser-induced breakdown spectroscopy (CF-LIBS) and calibration curve method i.e. partial least square regression (PLSR). For the CF-LIBS method, the temporal delay has been optimized in order to satisfy the optically thin and local thermal equilibrium (LTE) condition of the plasma. For the PLSR method, different regions of the strongest emission lines of constituents have been selected for quantitative analysis. The study of time-resolved LIBS spectra and the variation of plasma parameters with respect to the delay time is also discussed. The utility of the combined technique of CF-LIBS with the PLSR method for rapid monitoring and quality assessment of desired material/products without any sample pretreatment, thus reducing the cost of the analysis, is presented in this paper.

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) is recommended as the first choice of therapeutic strategy for patients with renal stones larger than 2 cm. It is reported that up to one-third of patients might have some perioperative complications, especially fever and urinary tract infections, which constitutes about 21%-39.8% of all the complications. PRIMARY AND SECONDARY: The primary aim of the study was to study about the proportion of patients getting post-operative infective complications following PCNL. The secondary aim was to study the patient, stone and procedure related risk factors associated with the infective complications. SETTINGS AND DESIGN: This is an institution-based observational study. MATERIALS AND METHODS: All patients who underwent PCNL in the Department of Urology, Medical College, Thiruvananthapuram, during 3 years from September 2016- to August 2019, were included in the study. In this study, the demographic factors and factors related to the patient, stone, and the procedure were collected and analyzed. STATISTICAL ANALYSIS USED: Data analysis was performed using SPSS version 22.0. RESULTS: During the 3-year period, a total of 201 patients with renal stones were treated with PCNL in our hospital. Of this 190 cases were taken for analysis. The mean age of patients was 47.7 years, 148 (77.9%) were male, 42 (22.1%) were female, The final outcomes evaluated were episodes of fever, documented urinary tract infection (UTI), pyelonephritis, and sepsis. Thirty-six (18.9%) patients had fever, of which 21 (11.1%) had UTI, 6 (3.1%) had pyelonephritis and 5 (2.6%) developed sepsis. CONCLUSIONS: Post-PCNL complications are more commonly found in patients with history of preoperative UTI, previous history of renal surgeries, large stone burden, operative procedure more than 90 min, and presence of residual calculi.

Intrahepatic and extrahepatic anatomical knowledge is essential for pre procedural planning of liver transplantation, liver resection, complex biliary reconstruction and radiological biliary tree intervention. Indian data of biliary anatomy and its variation is scant in literature. The aim of our study is to find out the prevalence of common and uncommon pattern of biliary tree anatomy in magnetic resonance cholangiopancreatography (MRCP) in our population. A total of 1,038 cases of MRCP of population of Odisha were obtained from Picture Archiving and Communication System of the department and were reviewed by two senior radiologists for anatomical pattern and variations. The typical and most common pattern of right hepatic duct (RHD) branching was seen in 72.8% cases. The most common variant of RHD was trifurcation pattern of insertion of right anterior sectoral duct (RASD), right posterior sectoral duct and left hepatic duct (LHD) forming common hepatic duct (CHD) in 11.3% of cases. The common trunk of segment (SEG) II and III ducts joining the SEG IV duct was the most common LHD branching pattern in 90.3% of cases. The most common pattern of cystic duct was posterior insertion to middle third of CHD (42.8%). MRCP is the non-invasive imaging modality for demonstration of biliary duct morphology to prevent iatrogenic injury during hepatobiliary intervention and surgery.

Miliary tuberculosis is a lethal form of disseminated tuberculosis (TB), deriving its name from the millet-seed-sized granulomas in multiple organs. As TB still remains a leading cause of morbidity and mortality in India, its disseminated forms need to be diagnosed early to ensure more aggressive treatment at the earliest possible time. However, a considerable number of cases are missed ante-mortem. We discuss the case of a 32-year-old immunocompromised, non-HIV patient with an ante-mortem diagnosis of pulmonary TB. However, multiple organ involvement by Mycobacterium tuberculosis was demonstrated on autopsy. This case highlights the role of autopsy as a research and learning tool, and prudential clinico-pathologic correlation, which will improve clinical outcomes in the future.

BACKGROUND: Sepsis is a global healthcare problem, characterized by whole body inflammation in response to microbial infection, which leads to organ dysfunction. It is becoming a frequent complication in hospitalized patients. Early and differential diagnosis of sepsis is needed critically to avoid unnecessary usage of antimicrobial agents and for proper antibiotic treatments through the screening of biomarkers that sustains with diagnostic significance. MAIN BODY OF ABSTRACT: Current targeting conventional markers (C-reactive protein, white blood cell, tumour necrosis factor-α, interleukins, etc.) are non-specific for diagnosing sepsis. Procalcitonin (PCT), a member of the calcitonin super family could be a critical tool for the diagnosis of sepsis. But to distinguish between bacterial versus viral infections, procalcitonin alone may not be effective. Rapid elevation in the concentration of procalcitonin and other newly emerging biomarkers during an infection and its correlation with severity of illness makes it an ideal biomarker for bacterial infection. Beside this, the procalcitonin levels can be used for monitoring response to antimicrobial therapy, diagnosis of secondary inflammations, diagnosis of renal involvement in paediatric urinary tract infection, etc. The present article summarizes the relevance of procalcitonin in the diagnosis of sepsis and how it can be useful in determining the therapeutic approaches. CONCLUSION: Further studies are needed to better understand the application of PCT in the diagnosis of sepsis, differentiating between microbial and non-microbial infection cases and determining the therapeutic approaches for sepsis.

In a letter to the editor, Raineri SM et al. have given an insight of another dimension of procalcitonin (PCT) use as a diagnostic tool in invasive candidiasis. But based on our preliminary information, PCT is reported as unconventional modes of diagnosis approach which yet to prove its stand-alone biomarker properties for invasive candidiasis.

Hepatocellular carcinoma (HCC) is one of the most prevalent cancers which are found in many Asian and African countries. There are several risk factors that may develop to HCC. Along with several other factors contributing to HCC, hepatitis B virus (HBV) infection also accounts for a major cause. HBV infection represents a major health problem worldwide. Among all of HBV genes, HBx is believed to play a prominent role in carcinogenesis, although the actual mechanism is not yet fully understood. The HBx gene of HBV is the most common open reading frame that may undergo mutations and may develop into HCC. This review summarizes the current knowledge about the most important roles of HBx gene that may lead to the development of HCC.

Idiopathic thrombocytopenic purpura (ITP) is an immune-mediated thrombocytopenia. ITP is the result of accelerated platelet destruction by the reticuloendothelial system, primarily the spleen. The prevalence of Helicobacter pylori infection and the effect of its eradication were monitored in an ITP patient over a period of 12 months. Eradication of Helicobacter pylori led to the increased platelet count and provides a new insight for a non-immunosuppressive treatment in selective ITP patients.

BACKGROUND AND AIM: Helicobacter pylori has been proven to be responsible for causing various gastrointestinal disorders including gastric adenocarcinoma. Several genes of pathogen (the genes of the cag-PAI, vacA, iceA, and babA) either in combination or independently have been reported to significantly increase the risk of ulceration/gastric carcinoma, with the cagA gene having the strongest predictive value. Pursuit to identify new genes which could serve as a marker of overt disease progression, lead to the discovery of hrgA gene. METHODS: Fifty-six indigenous strains of H. pylori from subjects with various gastric disorder were screened to assess the status of hrgA gene along with the cagA gene using simple polymerase chain reaction using specific oligonucleotide primers. Post-amplification, amplicons were subjected for sequencing to identify any strain specific variations in sequences from the H. pylori isolated from different disease manifestations. Histopathological analysis was done to ascertain any significant change in the histological scores of subjects infected with cagA+/hrgA+ and cagA-/hrg+ strains. RESULTS: All the 56 (100%) subjects amplified with the oligonucleotide primers specific to hrgA gene, whereas 81.71% subjects showed the presence of cagA gene. Sequencing of the amplimers showed 99% homology. Histology of the cagA+/hrgA+ and cagA-/hrg+ subjects did not show any significant difference. CONCLUSION: hrgA gene of Helicobacter pylori is not a ideal surrogate marker for identifying individuals with higher risk of developing overt gastro-duodenal diseases such as neoplasia of the stomach.

We report the first measurements, to our knowledge, of the modification of spontaneous emission rates of Eu3+ ions in the visible region owing to an absorbing medium. Precise levels of the absorption coefficient are introduced by codoping with different amounts of Nd3+. We use a binary glass system PbO-B2O3 as the host, the compositional variation of which leads to a change in the real part of the refractive index. Measured lifetimes are found to follow the real cavity model, and the data are analyzed by the model proposed by Scheel et al. [Phys. Rev. A 60, 4094 (1999)]. We give estimates of the parameter that is related to the radius of the cavity around Eu3+.

RACIONAL e OBJETIVOS: O Helicobacter pylori tem sido incriminado como causador de vários distúrbios digestivos, incluindo o adenocarcinoma gástrico. Diversos genes patogênicos (os genes do cag-PAI, vacA, iceA e babA), em combinação ou independentes, têm sido reportados como fatores de aumento de risco para ulceração/carcinoma gástrico, tendo o gene cagA forte valor preditivo. A procura da identificação de novos genes que possam vir a ser marcadores da progressão da doença levaram à descoberta do gene hrgA. MÉTODOS: Cinqüenta e seis amostras de H. pylori provenientes de pacientes com diversas afecções gástricas foram examinadas para caracterizar a presença do hrgA juntamente ao cagA, usando iniciadores específicos da reação de cadeia da polimerase. Após amplificação, os produtos amplificados pela PCR foram seqüenciados para a identificação de variações específicas nas seqüências do H. pylori isolado de diferentes doenças gastroduodenais. A análise histopatológica foi feita para assegurar qualquer mudança significativa nos escores dos indivíduos infectados com cagA+hrgA+ e cagA-/hrgA+. RESULTADOS: Todas as 56 amostras (100 por cento) foram amplificadas com iniciadores específicos para o hrgA, enquanto que 81,71 por cento mostraram a presença do cagA. O seqüenciamento do produto amplificado pela PCR mostrou 99 por cento de homologia. A histologia entre os grupos cagA+/hrgA+ e cagA-/hrgA+ não mostrou nenhuma diferença significante. CONCLUSÃO: O gene hrgA do H. pylori não é o marcador ideal para identificar indivíduos com alto risco de desenvolvimento de doenças gastrointestinais como a neoplasia de estômago.
