RESUMO

Twenty-two scientists met at Krobielowice, Poland, to discuss the impact of the social environment, spatial proximity, migration, poverty, but also psychological factors such as body perception and satisfaction, and social stressors such as elite sports, and teenage pregnancies, on child and adolescent growth. The data analysis included linear mixed effects models with different random effects, Monte Carlo analyses, and network simulations. The work stressed the importance of the peer group, but also included historic material, some considerations about body proportions, and growth in chronic liver, and congenital heart disease.

RESUMO

Twenty-four scientists met at Aschauhof, Altenhof, Germany, to discuss the associations between child growth and development, and nutrition, health, environment and psychology. Meta-analyses of body height, height variability and household inequality, in historic and modern growth studies published since 1794, highlighting the enormously flexible patterns of child and adolescent height and weight increments throughout history which do not only depend on genetics, prenatal development, nutrition, health, and economic circumstances, but reflect social interactions. A Quality of Life in Short Stature Youth Questionnaire was presented to cross-culturally assess health-related quality of life in children. Changes of child body proportions in recent history, the relation between height and longevity in historic Dutch samples and also measures of body height in skeletal remains belonged to the topics of this meeting. Bayesian approaches and Monte Carlo simulations offer new statistical tools for the study of human growth.

The study used cross-sectional data of 462 girls and 372 boys of Russian ethnicity and 90 Kalmyk girls 12-17-year-olds. In both groups children were examined by the same researchers according to the same research protocol. All of the observations have been performed in agreement with bioethical procedures; protocols of consent were filled either by the subject (elder children) or by his/her parent(s). Standing height, weight, body circumferences and skinfolds thickness were taken on each individual according to the standard technique. Body mass index (BMI) was calculated. In Arkhangelsk city 114 boys and 172 girls among the total number of studied subjects filled in the questionnaires, and in Elista ­ 34 girls among 90. Data sets were divided according to sex, age, somatotypes and ethnicity. The results of the present study showed significant correlations between somatotypes of the subjects, their BMI and self-estimation of their physical appearance. In self-perception of one's body, weight was the most important characteristics in girls, while in boys it was stature. No differences were found in most of self-evaluation scores between Russian girls of Arkhangelsk city and Kalmyk girls of Elista, apart from the fact that the latter had lower scores in the estimation of their body shape, possibly because they were fatter. The strategies chosen by the adolescents for modifications of their bodies in their quest for ¼ideal« figures were in favor of dieting versus physical activity, which puts the question of popularization of physical culture and sports on a nation-wide scale.

RESUMO

Twenty-five scientists met at Aschauhof, Altenhof, Germany, to discuss various aspects of the complex network of modern health screening, focusing on current scientific topics including medical sciences, human biology, and mathematics; on problems in implementing these results at the practical level of physicians, nurses, technicians, and engineers; and the level of administrative and political decisions. Whereas major scientific advancements have been published in the understanding and the bio-statistical evaluation of anthropometric screening parameters such as serial measurements of height and weight for preventive medical check-ups, BMI screening and surveillance in schools, etc., the implementation of these advancements into current health screening concepts, strategies and decision-making is poor. Fear of discrimination, misperception of body image, behavioural responses and political concerns, meanwhile dominate and negatively interfere with the implementation of recent scientific results into public health screening concepts and practices.

Despite the presence of body composition studies in Russia, there are no current reviews on this topic, and the results are relatively rarely published abroad. Our aim was to describe the history and current state of this research work, to list unresolved problems, and to outline possible developmental trends. For completeness, in the initial part of the review, traditional research areas indirectly related to body composition studies are considered, namely, the analysis of biological variation of anthropometric parameters and somatotyping.It can be seen that anthropometry and bioimpedance analysis (BIA) are mainly used to assess body composition in Russia. Other methods, such as double-energy X-ray absorptiometry (DXA), are utilized less often. The achievements include the common use of comprehensive anthropometry in anthropological studies, some advancements in clinical studies, approbation of potentially important methods such as the deuterium dilution method and three-dimensional laser-based photonic scanning, and ongoing mass population BIA measurements in health centers. Various bioimpedance instruments are manufactured, the local reference BIA body composition data are available, and a large updated BIA database is ready for international comparisons.Among major limitations of body composition research in Russia, one can note the lack of validation studies using reference methods, so that foreign regression formulas are used with the double indirect methods, such as anthropometry and BIA, despite the fact that their accuracy has not yet been checked in our population. Conventional reference body composition assessment methods, such as three- or four-component molecular-level models and whole-body in vivo neutron activation analysis, were not applied yet, despite the technical feasibility.In general, it can be argued that the body composition research in Russia follows the observed global trends. Along with the achievements, there are a number of unresolved methodological and organizational issues. Prospects for further research include validation studies, updating reference population body composition data, and establishing local cut-offs for malnutrition and disease risks. In our view, further development could be facilitated with the establishment of well-equipped Human Body Composition Units in major Russian research centers, such as Moscow State University, which could be assigned a coordinating and methodical role.

Extensive bioelectrical impedance analysis (BIA) data have the potential of health monitoring and the assessment of health risks at the population level. The importance of BIA data lies in their availability and abundance for many countries. In Russia, mass BIA data are generated by the national network of health centers (HCs). Our aim was to describe the structure and capabilities of the updated HCs' BIA database. Upon several requests between 2012 and 2020, 369 HCs representing all Federal districts of Russia and 60 out of 85 Federal subjects in them, submitted raw bioimpedance data which were obtained using the same type of BIA instrument, namely ABC-01 'Medas' (SRC Medas, Russia). After application of strict selection criteria, 2,429,977 BIA measurement records were selected that formed the updated 2010-2019 HCs' database. Various slices of the BIA data are described according to spatiotemporal, demographic and other characteristics. Reference curves of the bioimpedance phase angle according to age and sex are presented. Limitations and prospects for further work are outlined. We believe that, after appropriate sampling, the database can be utilized to study biological, geographical, social and other associations of the bioimpedance and body composition parameters, for generating updated national references, international comparisons and data standardization.

Background: We investigated average body height in the central provinces of the Russian empire in the middle of XIX century in view of the concept of "community effects on height". We analyzed body height correlations between neighboring districts at this time. We added information about secular changes in body height during the 19th century of this territory. Material and methods: The study used height data of conscripts, which were born in the years 1853-1863, and age 21 at the time of measurement. The territory of seven provinces was considered as a network with 105 nodes, each node representing one district with information on average male body height. In order to define neighboring districts three different approaches were used: based on the "common borders" method, based on Euclidean distances (from 60 to 120 km), based on real road connections. Results: Small but significant correlation coefficients were observed between 1st order districts in the network based on Euclidean distance of 100 km (r = 0.256, p-value = 0.006) and based on "the common borders" approach (r = 0.25, p-value = 0.02). Wherein no significant correlations were observed in the network based on road connections and between second order neighbors regardless of the method. Conclusion: Height correlation coefficients between 1st order neighboring districts observed in the Russian districts were very similar to values observed in the Polish study (r = 0.24). The considered Russian territory and the territory of Poland have a lot in common. They consist of both plains without mountains. In contradistinction to Poland the transport infrastructure in Russia was weakly developed in the middle of XIX century. In addition, the mobility of people was limited by serfdom. In this context the absent of significant correlation of second order neighbors can be explained by low population density and lack of migration and communication between the districts.

Background: Normal weight obesity in children has been associated with excessive body fat, lower bone density and decreased total lean mass. However, no studies have been done into whether normal weight obese children differ in skeletal robustness or lean mass development on the extremities from normal weight non-obese, overweight, and obese peers although these are important indicators of healthy development of children. Methods: Body height, body weight, BMI, four skinfolds, and two limb circumferences were assessed. We calculated total body fat using Slaughter's equations, the Frame index for skeletal robustness and muscle area for the upper arm and calf using Rolland-Cachera equations. Using national references of BMI and measured skinfolds, three subgroups of participants (9-12 years) consisting of 210 middle-school-aged children (M-age = 11.01 ± 1.05)-110 girls and 100 boys-were selected: (A) overweight obese (OWOB) (n = 72); (B) normal weight obese (NWO) (n = 69); and, (C) normal weight non-obese (NWNO) (n = 69). All values, were converted to Z-scores to take account of participant's sex and age. Results: NWO children had significantly poorer skeletal robustness on lower extremities and poorer muscle area on the upper arm and calf compared to NWNO counterparts with significantly higher evidence in boys-skeletal robustness NWO boys: Z-score = -0.85; NWO girls: Z-score = -0.43; lean mass on the calf: NWO boys Z-score = -1.34; NWO girls: Z-score = -0.85. The highest skeletal robustness-but not muscle area on the calf-was detected in OWOB children. Conclusions: Further research should focus on whether this poor skeletal and lean mass development: (1) is a consequence of insufficient physical activity regimes; (2) affects physical fitness of NWO children and could contribute to a higher prevalence of health problems in them. We have highlighted the importance of the development of a simple identification of NWO children to be used by pediatricians.

ABSTRACT: Background: In our modern world, the way of life in nutritional and activity behaviour has changed. As a consequence, parallel trends of an epidemic of overweight and a decline in external skeletal robusticity are observed in children and adolescents. Aim: We aim to develop reference centiles for external skeletal robusticity of European girls and boys aged 0 to 18 years using the Frame Index as an indicator and identify population specific age-related patterns. Methods: We analysed cross-sectional & longitudinal data on body height and elbow breadth of boys and girls from Europe (0-18 years, n = 41.679), India (7-18 years, n = 3.297) and South Africa (3-18 years, n = 4.346). As an indicator of external skeletal robusticity Frame Index after Frisancho (1990) was used. We developed centiles for boys and girls using the LMS-method and its extension. Results: Boys have greater external skeletal robusticity than girls. Whereas in girls Frame Index decreases continuously during growth, an increase of Frame Index from 12 to 16 years in European boys can be observed. Indian and South African boys are almost similar in Frame Index to European boys. In girls, the pattern is slightly different. Whereas South African girls are similar to European girls, Indian girls show a lesser external skeletal robusticity. Conclusion: Accurate references for external skeletal robusticity are needed to evaluate if skeletal development is adequate per age. They should be used to monitor effects of changes in way of life and physical activity levels in children and adolescents to avoid negative health outcomes like osteoporosis and arthrosis.

The aim of the present study was to analyze secular changes in body measurements in children of the Arkhangelsk city from 1988 to 2010. A large number of anthropometric measurements were taken on each individual including height, weight, arm, leg and trunk lengths (estimated), body diameters and circumferences, skinfold thickness. Stages of secondary sex characteristics were also evaluated; data on menarcheal age were collected by status-quo method. It was shown that main differences in stature occurred at puberty while in elder children (16-17-year-olds) no statistically significant differences were found. The same pattern was typical for weight and BMI. Chest circumference significantly increased, particularly in girls. For modern children, changes in body proportions due to a bigger trunk length were typical. There were also significant differences in the distribution of subcutaneous fat layer: in modern children bigger fat accumulation was present on the trunk, particularly in abdomen area, vs. fat layer on the extremities. Process of sexual maturation according to mean ages of development of secondary sex characteristics occurred earlier in modern adolescents, which is more expressed in girls. The results can be interpreted in terms of ongoing secular trend.

BACKGROUND: The study of aging processes and the changes in morphological, physiological, and functional characteristics that are associated with aging is of great interest not only for researchers, but also for the general public. The aim of the present paper is to study the biological age and tempos of aging in women older than 60 years, including long-lived females (over 90-years-old), and their associations with morphofunctional characteristics. RESULTS: Somatic traits, body mass components, and functional characteristics were investigated in 119 elderly (between 60 and 74-years-old) and long-lived (over 90-years-old) women in Tiraspol. With the special PC software 'Diagnostics of Aging: BioAge' (National Gerontological Center, Moscow, Russia) the biological age and tempos of aging were evaluated in the study participants. The results show close connections between morphofunctional changes, particularly in body mass components, and biological age. The software demonstrated its validity in the estimation of biological age in the group of elderly women. In the homogenous (according to their chronological age) group of women, three subgroups were separated with different tempos of aging: those with lower rates of aging (biological age less than chronological age by two years or more); those consistent with their chronological age, and those with accelerated tempos of aging (biological age higher than chronological age by two years or more). CONCLUSIONS: Morphofunctional characteristics in the studied groups of women demonstrate the trends of age-involutive changes which can be traced through all groups, from those with slow rates of aging, to those with average rates, to those with accelerated tempos of aging, and finally in long-lived women. The results of comparative analysis show that women with accelerated aging are characterized with such traits as lower skeletal muscle mass, lower hand grip strength, and higher metabolic rate. Canonical discriminant analysis revealed a number of morphofunctional characteristics which differentiate the early-aging women from women with average rates of aging: higher BMI values, excessive fat mass, lower skeletal muscle mass and low values of hand grip strength. Thus the presence of such characteristics in elderly women can be considered as additional risk factor towards the early onset of the aging process.

OBJECTIVES: Obesity and a reduced physical activity are global developments. Physical activity affects the external skeletal robustness which decreased in German children. It was assumed that the negative trend of decreased external skeletal robustness can be found in other countries. Therefore anthropometric data of Russian and German children from the years 2000 and 2010 were compared. METHODS: Russian (2000/2010 n = 1023/268) and German (2000/2010 n = 2103/1750) children aged 6-10 years were investigated. Height, BMI and external skeletal robustness (Frame-Index) were examined and compared for the years and the countries. Statistical analysis was performed by Mann-Whitney-Test. RESULTS: Comparison 2010 and 2000: In Russian children BMI was significantly higher; boys were significantly taller and exhibited a decreased Frame-Index (p = .002) in 2010. German boys showed significantly higher BMI in 2010. In both sexes Frame-Index (p = .001) was reduced in 2010. Comparison Russian and German children in 2000: BMI, height and Frame-Index were different between Russian and German children. German children were significantly taller but exhibited a lower Frame-Index (p<.001). Even German girls showed a significantly higher BMI. Comparison Russian and German children in 2010: BMI and Frame-Index were different. Russian children displayed a higher Frame-Index (p<.001) compared with Germans. CONCLUSIONS: In Russian children BMI has increased in recent years. Frame-Index is still higher in Russian children compared with Germans however in Russian boys Frame-Index is reduced. This trend and the physical activity should be observed in the future.

Secular changes of body measurements in children have been the subject of studies in many different countries. In recent years, there has been an increase in BMI associated with a significant trend towards obesity in both Europe and the US. The aim of the present study was to analyze trends in body measurements and BMI in Russia from the 1960's to the beginning of the 21st century. This was done at three locations of the Russian Federation: the city of Moscow, the cities of Saratov and Naberezhnye Chelny in the Volga-river area. In addition, data on secular changes of Abkhazian children were analyzed. A large number of anthropometric measurements were taken on each individual including height, weight, arm, leg and trunk lengths (estimated), body diameters and circumferences, skinfold thickness, head and face dimensions. Stages of secondary sex characteristics also were evaluated; data on menarcheal age were collected by status-quo and retrospective methods. Changes in hand grip strength have been evaluated in some of the samples. While stature was increasing during these years, weight, chest circumference and BMI were characterized by negative changes, which became more obvious in elder girls. Changes in handgrip strength also showed negative trends. There were noticeable changes in head and face measurements, which were expressed in more elongated head and face forms, i.e. the head became longer and narrower with narrower and higher faces. Secular changes in head and facial morphology may be considered part of the general trend.

The influence of physical activity on body mass components has been studied using a sample of Moscow children. 195 girls and 259 boys of Russian ethnicity from 12 to 17 years old were investigated cross-sectionally in 2005 in different Moscow schools. According to the level of physical activity they were divided into three groups: 1-those who did not take part in regular physical exercise (44 boys and 50 girls); 2-those who took part in special sports programs in general education schools (82 boys and 82 girls); 3-students of special sports schools with a high sports ranking (133 boys and 63 girls). The program included anthropometric measurements, evaluation of sexual maturation indices, somatotypes, and "functional" traits (diastolic and systolic blood pressure, pulse rate, hand grip, etc). For the study of body composition, bioelectrical impedance analysis (BIA) was used. The estimates of body mass components were also calculated using the anthropometric measurements. For the fat component, the estimates obtained by BIA and the anthropometric methods were highly correlated: r=0.85-0.88. Age changes of BIA measurements and body components were analysed. With multiple regression analysis it was shown that BIA measurements are dependent on a great number of morphological and functional traits, with the most informative sets of traits being selected. The degree of physical activity has a strong effect on body components: the contents of fat-free mass (FFM) and total body water (TBW) significantly increase, and the fat mass (FM) in girls decreases.

Secular changes in the body dimensions of newborns from three different geographical localities are compared. Anthropometrical data of infants born in 1987 and in 2002 as a result of singleton full-term non-pathological postpartum were collected from medical records of children's out-patient clinics in three geographical localities: two big cities-Moscow and Saratov-and the town of Khvalynsk (Saratov region). The total number of newborns used in the analysis was 1,174. Body length and weight at birth from 1987 to 2002 increased in Moscow and Saratov, while in Khvalynsk there was a significant decrease of body length for boys. There is also a trend towards decrease of body weight in Khvalynsk boys and girls. It may be assumed that the changes in body sizes of newborns may be connected with socio-economic changes.

2,106 girls and 2,169 boys from 7 to 17 were investigated in 2002-2004 in three urban settlements of the Saratov region (Povolzhje area): the town of Khvalynsk, population 15,000, with a low level of industrialization; the city of Balakovo, population 220,000, highly industrialized and with a nuclear power station; and the city of Saratov, population around 1,000,000 a regional capital, and also highly industrialized. The whole area, particularly the location of Khvalynsk, is also characterized by iodine deficiency (iodine concentration is 0.78 mkg/kg v. normal values of 5-7 mkg/kg). The program included about 30 anthropometric measurements, evaluation of developmental stages of secondary sexual characteristics, and information on menarcheal age by the status quo method. Information on parental occupation and education, as well as number of children per family was collected by questionnaire. For the analysis all the data were standardized, and further comparisons were made irrespective of age groups. The significance of differences was assessed by one-way ANOVA. For height, weight and chest circumference there are significant differences among the three populations (p < 0.001), with Khvalynsk children being the smallest in body height and weight. However, in chest circumference they are close to or even bigger (girls) than Balakovo children. The children from Khvalynsk are characterized by the lowest values for subcutaneous fat layer, both on the trunk and extremities. For the age of menarche, Khvalynsk girls have the highest values: 13 years 5 months (13.42). In Balakovo and Saratov, the corresponding figures are identical: 13 years 2 months (13.17). Secular changes in Khvalynsk and Saratov children are discussed as compared to the literature.
