RESUMO

In elderly patients, when performing endovascular cathlabs manipulation the radial and cubital accesses is preferable. We propose a new arterial access on the back surface of the hand in the anatomical snuffbox. Artery catheterization of the hand in the anatomical snuffbox preserves intact the radial artery in the forearm for subsequent potential surgical interventions.

The aim of this study was to assess the immediate and medium-term (3 months) results of the safety and efficacy of distal radial access (DRA) in coronary interventions compared with conventional transradial radial access (TRA). TRA is the recommended access for coronary procedures because of increased safety: fewer local complications, large and small bleeding. Recently, DRA has emerged as a promising alternative access to minimize radial artery occlusion (RAO) risk, as well as other complications. A large-scale, international, randomized trial comparing medium-term results with TRA and DRA is lacking. An analysis of 776 patients of the prospective randomized TENDERA trial was carried out: the distal artery access group (DRA) - 391, the transradial access group (TRA) - 385. Statistically more often the crossover access was in the DRA group (5.1% and 0.8%, P < 0.001). The primary endpoint was early or late thrombosis/occlusion of the radial artery (RA). Secondary endpoints: (1) composite complications from access vessels; (2) access parameters. Statistically significant differences were obtained for the primary endpoint: DRA 2.7% (n = 10), TRA 6.8% (n = 26), P = 0.008. Occlusion of the distal radial artery (DRAt), with patent RA: DRA 1.3% (n = 5), TRA 0 (0), P = 0.023. At the secondary composite endpoint, statistically significant differences were obtained for the following groups of complications: BARC type I bleeding (DRA: 3.8% (n = 14), TRA: 21.7% (n = 83), P < 0.001); hematoma larger than 5 cm on day 1 (DRA: 10% [n = 37], TRA: 25.9% [n = 98], P < 0.001); hematoma larger than 5 cm on day 7 (DRA: 12.4% [n = 45], TRA: 34.6% [n = 132], P < 0.001). Of the access parameters, the following statistically significantly differed: puncture time DRA 19.0 (8.0; 50), TRA 13.5 (5.0; 29), P < 0.001; insertion of introducer DRA 42.0 (26.0; 84.0), TRA 35.0 (23.0; 55.0), P < 0.001, access artery hemostasis duration (min.) DRA 180.0 (120.0; 480.0), TRA 155.0 (115.0; 195.0), P < 0.001. The duration of the procedure and fluoroscopy, radiation dose, RA spasm in both groups had no statistically significant differences. In the TENDERA trail, DRA demonstrated efficacy and safety in interventional coronary interventions compared with TRA in the medium-term follow-up period: a statistically significant lower incidence of RA occlusion and local complications.

RESUMO

In article, analysis has been carried out on the influence of recanalization of chronic total occlusion of left anterior descending artery on the course of heart failure in elderly patients who had damage to a single artery and viable myocardium. Recanalization of chronic occlusion of left anterior descending artery together with conservative therapy in elderly patients improves the clinical course of heart failure. A more significant improvement seen in patients with chronic occlusion of left anterior descending artery without history of myocardial infarction.

RESUMO

We studied prognostic value of left ventricular (LV) diastolic dysfunction (DD) in relation to sudden cardiac death (SCD) after myocardial infarction (MI). We examined 603 patients in acute period of MI (including 551 with preserved LV systolic function and DD). Duration of follow-up was 1-7 years. According to data of Cox multifactorial regression analysis LVDD was not an independent prognostic factor. ut it increased SCD risk at the account of interrelationship with such predictors as ventricular rhythm disturbances, lowering of heart rate variability, and sensitivity of cardiochronotropic component of arterial baroreflex.

The results of studies of the genetic structure of the Central European wild boar (Sus scrofa scrofa) population and four breeds of domestic pigs (Duroc, Yorkshire, Large White and Landrace) bred in the Central Black Earth region of Russia are presented in this work. Based on 12 microsatellite loci, a significant ( p <0.05) decrease in the level of genetic variability in bred breeds was shown. The expected heterozygosity and Shannon index were as follows: in the wild boar, Ho = 0.763 ± 0.026, I = 1.717 ± 0.091; in the Duroc breed, Ho = 0.569 ± 0.068, I = 1.191 ± 0.157; in the Landrace, Ho = 0.618 ± 0.062, I = 1.201 ± 0.147; in the Large White, Ho = 0.680 ± 0.029, I = 1.362 ± 0.074; and in the Yorkshire, Ho = 0.642 ± 0.065, I = 1.287 ± 0.156. The results of checking genotypic Hardy-Weinberg equilibrium based on the G-test of maximum likelihood demonstrated that the overwhelming majority of loci in the wild boar population were in the state of said equilibrium. By contrast, in pig breed populations, some loci demonstrated a significant deviation from the indicated equilibrium. In addition, the Yorkshire, Large White, and Landrace populations had loci, for which the hypothesis of neutrality was reliably rejected based on the results of the Ewens-Watterson test. The revealed private alleles, characteristic of the wild boar and breeds, can later be used to identify them. The ordination of the centroids of different herds in the space of the first two principal coordinates based on the matrix of pairwise estimates of Nei's genetic distances showed that the most distant populations are the Duroc and Boar breeds, and the most genetically close are the Yorkshire and Landrace breeds. The closest to the wild boar population was the Large White breed. The assessment of the effective size, carried out using the method based on the linkage disequilibrium and the molecular coancestry method, showed that in all studied groups, including the wild boar population, the effective size was less than 100 individuals. The low effective size of the wild boar population (Ne = 21.8, Neb = 4.0) is probably caused by the death and shooting of animals due to Pestis africana suum.

The comparative study of the dynamics of the main antigen (hexon) and viral DNA of the bovine adenovirus type 3 accumulation in the established cell line MDBK under the conditions of single- or multistep cycle of infection has been undertaken. The quantitative immunoelectrophoresis and immunoenzyme assay detected the viral antigens on the late stages of infection in the period of cellular monolayer degradation. The immunofluorescence reaction and histochemical immunoenzyme method detected the antigen in the infected cells concurrently with the primary expression of the viral cytopathic effect. The reaction of the spot molecular hybridization with the [32P]-DNA probes detected the viral DNA considerably earlier than the antigen was detected by the immunological methods, before the appearance of degenerative changes in the infected cells. Preference of the immunoenzyme assay and DNA-probes in diagnosis of the virus are discussed.

The II-1 strain of the Aleutian disease virus (ADV-II-1) was isolated from experimentally infected mink organs. The viral particles were isolated having 23 to 24 nm in diameter with the buoyant density of the virions in CsCl gradient being 1.41 g.ml-1. The single stranded ADV DNA extracted from the purified virus particles had the molecular mass about 1.4 . 10(6) (4800 bases). The double-stranded replicative form of ADV DNA has been synthesized in vitro with the use of a large "Klenow" fragment of DNA-polymerase I. A restriction endonuclease map of ADV-II-1 DNA has been constructed with the use of in vitro synthesized double-stranded DNA.

Nucleotide sequence of the cloned DNA complementary to cow kappa-casein mRNA was determined. The complete sequence is composed of 854 bases and includes 60 bases of 5'-noncoding region, 570 bases of the coding region and 209 bases of 3'-noncoding region without poly(A). The cloned sequence codes for kappa-casein, the genetic variant B2. Several restriction sites were defined that permitted the identification of genetic variants and size polymorphism of restriction fragments in the structural region of the gene. Upon the comparative analysis of kappa-casein mRNAs of cow and rat, it was shown that, apart from functionally important regions in the coding sequence, high homology is characteristic of the 5'- and 3'-noncoding regions.

Poly(A+)mRNA from bovine mammary glands was used to synthesize double-stranded cDNAs that were subsequently inserted into the plasmid vector pBR322 at the Pst1 site by means of oligo(dG)-oligo(dC) tailing. After transfection of Escherichia coli JC5183, recombinant plasmid library containing 5400 clones was screened by serial rounds of colony hybridization in situ to total [23P] poly(A+)mRNA and electrophoretically homogenious [32P]16SmRNA of mammary glands. Then hybrid selection of mRNA and subsequent in vitro translation of selected mRNAs were performed. In this manner, recombinant clones coding for alpha S1- beta-, kappa-casein were identified. cDNA clones range in size from 35% for beta-casein, 65% for alpha S1-casein to about 95% for kappa-casein, in comparison with their respective mRNAs.

RESUMO

A method of detection of BLV proviral DNA in the peripheral blood leukocytes of cattle is reported. Leukocytes were used without preliminary cultivation. Cell DNA was dot-hybridized with P-labeled plasmid containing a fragment of BLV proviral DNA. The sera samples taken from the cattle under study were also tested using immunodiffusion assay (ID). The results of comparison showed that dot-hybridization assay is a more sensitive diagnostic test than ID, because it detects BLV infection in apparently normal cattle, which was seronegative in ID.

We report quantum calculations of vibrational states of trans N-methyl acetamide (H3C-HNCO-CH3) in full dimensionality using the code MULTIMODE. In this code, the full potential is represented as a hierarchical sum of n-mode potentials in the normal coordinates. All 30 one- and 435 two-mode potentials are included in the sum, as well as a restricted set of 10 three-mode potentials corresponding to the experimentally probed amide band. The electronic energies on the various n-mode grids are obtained using ab initio Møller-Plesset perturbation theory with a triple-zeta quality, correlation-consistent basis set. Convergence tests of the low-lying vibrational eigenvalues of the amide band show that this limited three-mode representation of the full potential yields well converged results that are in excellent agreement with experiment. The infrared spectrum in the region of the amide bands is calculated and also agrees well with experiment.

A DNA-polymerase from the external thermophylic bacteria Thermus ruber has been isolated. A six-step purification procedure resulted in an electrophoretically homogeneous enzyme preparation with m. w. of 70 000. The isolated enzyme is thermostable and has a temperature optimum on DNA templates at 70 degrees and that on RNA templates at 50 degrees. The enzyme does not contain contaminant endo- and exonuclease activities. The maximal activity of the enzyme requires the presence of template, four deoxyribonucleoside triphosphates, monovalent and divalent cations in the incubation mixture. The catalytic activity of the enzyme on various templates of DNA- and RNA-types has been investigated. A comparative physico-chemical study of this DNA-polymerase and an analogous enzyme isolated earlier from the external thermophylic bacteria Thermus aquaticus YT-1 has been carried out.

A DNA polymerase has been isolated from the thermophylic bacteria Thermus aquaticus YT-1. The six-step purification procedure resulted in an electrophoretically homogeneous enzyme preparation with molecular weight of about 62 000. The enzyme does not contain contaminant exo- and endonuclease activities and has a temperature optimum on the DNA templates at 70 degress and that on RNA matrices at 50 degrees. The maximal activity of the enzyme requires the presence of bivalent cations (magnesium or manganese) 0,1--0,2 M KCl or NaCl, all deoxynucleoside triphosphates and template in the incubation mixture. The enzyme is active when "activated" DNA, poly(dA)-poly(dT), poly(dA)-oligo(dT)10 and poly(rA)-oligo(dT)10 are used as templates and is inactive on the native and denaturated DNAs as well as on the native molecules of RNA and poly(rC)-oligo(dG)12--180.

The maximal activity of the enzyme requires the presence of template, four deoxyribonucleoside triphosphates and monovalent and bivalent cations in the incubation mixture. The enzyme is highly active when "activated" DNA, poly(dA)-poly(dT), poly(dA)-oligo(dT) 10 and poly(rA)-oligo(dT)10 are used as templates, moderately active on single-stranded and double-stranded DNAs and inactive on poly(rC)-oligo(dG)12-18 and native RNA molecules.

Gas phase vibrational spectra of BrHI- and BrDI- have been measured from 6 to 17 microm (590-1666 cm(-1)) using tunable infrared radiation from the free electron laser for infrared experiments in order to characterize the strong hydrogen bond in these species. BrHI-.Ar and BrDI-.Ar complexes were produced and mass selected, and the depletion of their signal due to vibrational predissociation was monitored as a function of photon energy. Additionally, BrHI- and BrDI- were dissociated into HBr (DBr) and I- via resonant infrared multiphoton dissociation. The spectra show numerous transitions, which had not been observed by previous matrix studies. New ab initio calculations of the potential-energy surface and the dipole moment are presented and are used in variational ro-vibrational calculations to assign the spectral features. These calculations highlight the importance of basis set in the simulation of heavy atoms such as iodine. Further, they demonstrate extensive mode mixing between the bend and the H-atom stretch modes in BrHI- and BrDI- due to Fermi resonances. These interactions result in major deviations from simple harmonic estimates of the vibrational energies. As a result of this new analysis, previous matrix-isolation spectra assignments are reevaluated.
