RESUMO

The indirect immunofluorescent test in fixed P3HR-1 cells showed the sera from patients with lymphogranulomatosis to contain high titers of humoral antibody to the capsid antigen of Epstein-Barr virus. Higher titers of antibody to EBV were associated with unfavourably running histological variants of lymphogranulomatosis. The lymphoid depletion variant was accompanied by high antibody titers to the virus and had a worse prognosis than the nodular-sclerotic variant with lower antibody titers but better prognosis. The level of antibody in lymphogranulomatosis was stable. No relationship between the antibody level and the stage and results of the therapy given was found. In sera from patients with reticulosarcoma and lymphosarcoma no significant rise of antibody titers to EBV was found as compared with a group of normal donors, while in acute leukosis there was a tend for a decline in antibody titers.

Radical radiation therapy provided complete remissions in 152 out of 166 patients with Hodgkin's disease, stages I-II-IIIs. During 3 to 17 years of observation, the relapses occurred in 67 patients (44%). A half of all the relapses were recorded during the first year of remission. In 88% of the cases the relapses were seen over the diaphragm, in 61% at the sites exposed to radiation and in 60% of the cases they were local. The rate of the relapses did not correlate with the stage, histological type, site of the lesion or with the radiation dose. Repeated complete remissions were attained in 48 patients (72%). The best effect was reached after cyclic polychemotherapy in combination with local radiation. In nodular sclerosis, the repeated remissions appeared far more lasting as compared to those in patients with the mixed-cell version.

Radical radiation treatment was given to 152 patients suffering from stage I-III Hodgkin's disease in 1967-1977. Exposure was limited to the lymph collectors above the diaphragm in cases of stage I tumor. Exposure was extended to para-aortal lymph nodes and spleen (or splenic base) in cases of stage II tumor. All three levels were irradiated in cases of stage III. Mean five-year survival rate in the entire group was 89% (stage I-93: IIA-92.8 and IIIA-82.3%); ten-year rate-66.6%. Recurrences were detected in 43.5%; 75% of them were identified within the first two years after radiation. Relapse was more frequent in cases of mixed cell pattern of tumor and lymphoid depletion. Radiation-induced injuries of varying degree were found in 28%, changes in the lungs and soft tissues were prevalent. A relationship between radiation injury frequency and the extent of exposure was established: radiation injury was observed in 4% of stage I cases and in 63% of patients with stage III tumor. Radiation-induced malignancies (breast cancer and fibrosarcoma of ilioinguinal soft tissues) were found in two cases out of 152 at different periods of the study.

As many as 104 patients with lymphogranulomatosis, clinical stages I-II, having an unfavourable prognosis were examined for the efficacy of polychemotherapy and combined treatment. Polychemotherapy was received by 47 patients, combined treatment (chemo- plus radiotherapy) by 57 patients. Complete remissions were attained significantly more often in patients given combined treatment (92%) as compared to patients on polychemotherapy alone (64%). After complete remissions relapses occurred in 60% of the patients on polychemotherapy and in 13% of the patients receiving combined treatment. The 7-year survival without any relapses was significantly higher (85%) in patients on combined therapy as compared to patients given polychemotherapy (36%). However, the 10-year survival appeared the same in both the groups (87 and 88%). Splenectomy affected neither the rate of relapses in both the groups nor the immediate treatment efficacy or survival.

Metabolism of arachidonic acid in neutrophils was studied in vitro in blood samples obtained from 36 patients with Hodgkin's disease of various stage and histology and 32 healthy donors. The basic metabolites were: leucotriene B4, such products of its omega-oxidation as 20-hydroxy-leucotriene B4 (20-OH-LTB4) and 20-carboxy-leucotriene B4 (20-COOH-LTB4), and 5-hydroxyeicosatetraenic acid. Their profile proved identical in patients with Hodgkin's disease and healthy donors. Most patients with Hodgkin's disease showed a decrease in leucotriene B4 and 5-hydroxyeicosatetraenic acid and an increase in the level of omega oxidation of leucotriene B4 as assessed by omega catabolite/leucotriene B4 ratio. The levels of 5-hydroxyeicosatetraenic acid and leucotriene B4 omega-oxidation were found to depend upon histology and stage of Hodgkin's disease. They were nearly normal in patients with stage III, mixed cellular disease, B-symptoms and signs of biologic activity of tumor. A direct correlation was established between the level of leucotriene B4 omega oxidation in neutrophils and that of ceruloplasmin in blood serum of patients with various stages of Hodgkin's disease.
