RESUMO

Despite growing ecological concerns, studies on microplastics and nanoplastics are still in their initial stages owing to technical hurdles in analytical techniques, especially for nanoplastics. We provide an overview of the general attributes of micro/nanoplastics in natural environments and analytical techniques commonly used for their analysis. After demonstrating the analytical challenges associated with the identification of nanoplastics due to their distinctive characteristics, we discuss recent technological advancements for detecting nanoplastics.

Background and Objectives: This study aimed to evaluate the effects of subtalar joint axis-based balance exercises on the anterior talofibular ligament (ATFL) thickness, ankle strength, and ankle stability after an arthroscopic modified Broström operation (AMBO) for chronic ankle instability (CAI). Materials and Methods: The study included 47 patients diagnosed with CAI who underwent AMBO and were randomly divided into three groups: control (n = 11), general balance exercise (n = 17), and subtalar joint axis balance exercise (n = 19), regardless of the affected area. Participants in the exercise rehabilitation group performed exercises for 60 min twice a week for six weeks, starting six weeks after AMBO. ATFL thickness, ankle strength, and ankle dynamic stability were measured using musculoskeletal ultrasonography, Biodex, and Y-balance test, respectively, before and after treatment. Results: Compared with the remaining groups, the subtalar joint axis balance exercise group had reduced ATFL thickness (p = 0.000), improved ankle strength for eversion (p = 0.000) and inversion (p = 0.000), and enhanced ankle stability (p = 0.000). Conclusions: The study results suggest that subtalar joint axis-based balance exercises may contribute to the early recovery of the ankle joint after AMBO.

Evidence of the effect of exercise therapy in patients who have undergone total thoracoscopic ablation is lacking. This study aimed to evaluate the effects of eight weeks exercise-based cardiac rehabilitation on cardiopulmonary fitness and adherence to exercise in patients who underwent total thoracoscopic ablation and followed a regimen of exercise therapy. Twenty-four patients were involved in the study and were divided into two groups. The exercise group underwent exercise therapy, which included aerobic and resistance exercises, twice a week as part of an eight weeks hospital-based outpatient cardiac rehabilitation program. Cardiopulmonary exercise test was used to evaluate exercise capacity and the International Physical Activity Questionnaire was utilized to identify the amount of physical activity and confirm adherence to exercise at six months postoperatively. There were significant differences between the groups in moderate activity level (p = 0.004) and extent of total physical activity (p = 0.0001). Complications such as recurrent atrial fibrillation did not occur during the exercise training. Exercise-based cardiac rehabilitation was beneficial in maintaining the activity level at six months postoperatively. Early exercise intervention at four weeks post-surgical ablation is a safe and effective therapy that can increase physical activity. Further studies are needed to verify the effect of exercise intervention in a larger sample size of patients who have undergone total thoracoscopic ablation.

Nanoantennas play an important role as mediators to efficiently convert free-space light into localized optical energy and vice versa. However, effective control of the beam direction of a single nanoantenna remains a great challenge. In this paper, we propose an approach to steer the beam direction of a single nanoantenna by adjusting two antenna modes with opposite phase symmetry. Our theoretical study confirmed that the combination of even- and odd-symmetric modes with a phase difference of π/2 enables effective beam steering of a single nanoantenna whose steering angle is controlled by adjusting the amplitude ratio of the two antenna modes. To implement our theory in real devices, we introduced asymmetric trapezoidal nano-slot antennas with different side air-gaps of 10 and 50 nm. The trapezoidal nanoantennas can simultaneously excite the dipole and quadrupole modes in a single nanoantenna and enables effective beam steering with an angle of greater than 35° near the resonance of the quadrupole mode. In addition, the steering angle can also be controlled by adjusting the degree of asymmetry of the trapezoidal slot structure. We believe that our beam steering method for a single nanoantenna will find many potential applications in fields such as imaging, sensing, optical communication, and quantum optics.

We propose extreme field confinement in a zigzag plasmonic crystal that can produce a wide plasmonic bandgap near the visible frequency range. By applying a periodic zigzag structure to a metal-insulator-metal plasmonic waveguide, the lowest three plasmonic crystal bands are flattened, creating a high-quality broadband plasmonic mirror over a wavelength range of 526-909 nm. Utilizing zigzag plasmonic crystals in a three-dimensional tapered metal-insulator-metal plasmonic cavity, extreme field confinement with a modal volume of less than 0.00005 λ 3 can be achieved even at resonances over a wide frequency range. In addition, by selecting the number of zigzag periods in the plasmonic crystal, critical coupling between the cavity and the waveguide can be achieved, thereby maximizing the field intensity with an enhancement factor of 105 or more. We believe that zigzag plasmonic crystals will provide a powerful platform for implementing broadband on-chip plasmonic devices.

In recent years, novel high-performance nanophotonic devices have been realized by applying ultrathin two-dimensional nanolayer materials to nanophotonics. In this paper, we propose nanolayer-embedded compact pseudo-photonic crystals (PPCs) that enable strong interaction between ultrathin nanolayers and photonic crystal modes. In typical two-dimensional slab photonic crystals, the transverse-magnetic (TM) photonic crystal bandgap is not well formed, making it difficult to operate the TM photonic crystal waveguide modes. However, by utilizing the low-frequency TM PPC bands, a long propagation TM waveguide mode, a slow TM waveguide mode, and a TM photonic bandgap are all readily available. In particular, the insertion of a nanometer-thick low-refractive-index layer in the vertical center of TM PPC waveguide can localize the electric fields tightly in nanometer space, causing strong field interaction with the inserted nanolayer material. Using the TM slow light near PPC band edges, field interaction with the nanolayer is significantly enhanced. We can also realize nanolayer-embedded high-quality-factor (Q-factor > 104) PPC cavities using the TM PPC bandgap. We believe that the proposed TM PPCs will play an important role in the strong interaction of ultrathin nanolayer materials with photonic crystal modes.

We report a novel scheme for monolithic integration of a nanoisland laser with a shifted-air-hole waveguide by employing selective etching techniques. An active L3 laser cavity and passive shifted-air-hole waveguide are simultaneously formed through a single fabrication step. In the shifted-air-hole waveguide, the air-hole position is adjusted to be compatible with selective etching. The spectral overlap between the L3 laser resonance and guided mode is achieved by introducing small air holes at the nodes of the shifted-air-hole waveguide. Experiments show that >60% of the light is coupled from the nanoisland laser to the end of the 12-µm-long waveguide.

We propose a novel design for a sub-5-nm-gap plasmonic cavity to couple it efficiently with an integrated low loss silicon waveguide. We numerically obtain over 90% efficient coupling between a nano-gap plasmonic cavity with a modal volume of less than 10-7λ3 and a conventional silicon-on-insulator (SOI) waveguide by utilizing the anti-symmetric second-order resonance mode of the cavity and engineering its geometry to reduce the modal size to less than 5 nm. The electromagnetic field efficiently coupled to the small cavity, leading to extreme enhancement of the field intensity. For a 2-nm-gap cavity, the intensity enhancement was calculated to be more than 100,000,000 compared to that of light in an SOI waveguide.

INTRODUCTION AND HYPOTHESIS: Our aim was to evaluate the effect of diabetes mellitus (DM) on detrusor contractility (DC) in women without bladder outlet obstruction (BOO) by using urodynamic study (UDS). METHODS: We reviewed the clinical records of 863 consecutive women without BOO, each of whom was diagnosed with stress urinary incontinence (SUI) by UDS. Uroflowmetry measurements included maximal flow rate (Qmax), time to Qmax, voided volume, and postvoid residual urine volume (PVR). Data from filling cystometry included the first strong desire to void and the Valsalva leak-point pressure (VLPP). For voiding cystometry data, detrusor pressure at Qmax (Pdet@Qmax) and bladder contractility index (BCI) were analyzed. In the DM group, the level of glycosylated hemoglobin (HbA1c) and DM duration were measured. RESULTS: After the application of exclusion criteria, complete UDS data of 708 patients were available. The cohort was divided into two groups according to DM status. The DM group comprised 92 (12.9 %) patients, the non-DM group 616 (87.0 %). Mean maximal flow rate and Pdet@Qmax and bladder contractility index were lower in the DM group, in whom mean DM duration was 9.24 ± 7.63 years and mean HbA1c level 7.27 ± 1.43 %. DM duration was significantly correlated with Qmax (-0.309, p = 0.003), Pdet@Qmax (-0.369, p < 0.001), and BCI (-0.409, p < 0.001). Moreover, the HbA1c level was significantly correlated with Qmax (-0.256, p = 0.016), Pdet@Qmax (-0.231, p = 0.026), and BCI (-0.308, p = 0.002). CONCLUSIONS: Our UDS data revealed that DM is associated with impaired DC in women without BOO. Moreover, longer DM duration and poor glycemic control were associated with impaired DC.

Confining photons in the smallest possible volume has long been an objective of the nanophotonics community. In this Letter, we propose and demonstrate a three-dimensional (3D) gap-plasmon antenna that enables extreme photon squeezing in a 3D fashion with a modal volume of 1.3 × 10(-7) λ(3) (∼4 × 10 × 10 nm(3)) and an intensity enhancement of 400 000. A three-dimensionally tapered 4 nm air-gap is formed at the center of a complementary nanodiabolo structure by ion-milling 100 nm-thick gold film along all three dimensions using proximal milling techniques. From a 4 nm-gap antenna, a nonlinear second-harmonic signal more than 27 000-times stronger than that from a 100 nm-gap antenna is observed. In addition, scanning cathodoluminescence images confirm unambiguous photon confinement in a resolution-limited area 20 × 20 nm(2) on top of the nano gap.

Hyaluronan (HA), a component of the extracellular matrix, modulates cellular behavior including angiogenesis. However, little is known about the effect of HA on lymphangiogenesis in fibrosis model. In this study, we investigated the roles of HA in lymphangiogenesis of unilateral ureteral obstruction (UUO). We found that HA cooperated synergistically with vascular endothelial cell growth factor-C to stimulate capillary-like tube formation and increase migration of cells in a haptotaxis assay. Accumulation of HA in the cortical interstitial space was positively correlated with the number of lymphatic vessels after UUO. Depletion of macrophages with clodronate decreased UUO-induced HA accumulation and lymphangiogenesis. Additionally, hyaluronan synthase (HAS) mRNA expression and HA production were increased in bone marrow-derived macrophages upon stimulation with TGF-ß1. Transfer of mHAS2 and mHAS3 knock-down CD11b-positive macrophages to SCID mice resulted in a partial decrease in UUO-induced lymphangiogenesis. HA increased expression of vascular endothelial cell growth factor-C in macrophages. Vascular endothelial cell growth factor-C expression and LYVE-1-positive lymphatic area was significantly lower in the UUO-kidney from TLR4 null mice than that from TLR4 wild-type mice. Collectively, these results suggest that HA increases lymphangiogenesis in renal fibrosis model and also stimulates vascular endothelial cell growth factor-C production from macrophages through Toll-like receptor 4-dependent signal pathway.

An effective engineering method for efficient coupling between a tightly focused sub-5-nm-gap plasmonic crystal cavity and an integrated waveguide is proposed. A three-dimensionally tapered 2-nm-gap plasmonic crystal cavity exhibiting a mode volume of 6x10(-6) (λ/n)(3) can efficiently couple to an integrated waveguide with over 90% efficiency by optimally selecting the number of air-holes in the plasmonic crystal. The field intensity is accordingly maximized, and its enhancement reaches to 205,000 for the light incident from the waveguide. Here, the optimal coupling is achieved by matching the radiation rate of the cavity mode to its absorption rate. In addition, the strongly enhanced field intensity boosts the spontaneous emission rate of the dipole emitter embedded in the cavity. The maximum radiative Purcell enhancement is calculated to be 362,000 where the quantum efficiency exceeds 50%.

We propose and demonstrate plasmonic nano-comb (PNC) structures for efficient large-area second-harmonic generation (SHG). The PNCs are made of 250 nm-thick gold film and have equally-spaced 30 nm-slits filled with ployvinylidene fluoride-co-trifluoroethylene (P(VDF-TrFE)). The PNC with 1.0 µm-spacing couples resonantly with 1.56 µm 100-fs laser beams. For the 1.0 µm-spacing PNCs under the fixed-pump-power condition, the nonlinear SHG power remains almost independent of the pump diameter ranging from 2 µm to 6 µm. The SHG power from the resonant PNC is measured to be 8 times larger than that of the single-nano-gap metallic structure, when the pump beam is tightly-focused to 2 µm in diameter in both cases. This relative enhancement of the total nonlinear SHG signal power reaches up to >200 when the pump beam diameter is increased to 6 µm. We attribute this unusual phenomenon to the resonant coupling of the finite-size pump wave with the finite-size one-dimensional plasmonic mode.

OBJECTIVES: To determine the prevalence of antimicrobial resistance in the normal rectal flora of patients undergoing transrectal ultrasonography-guided prostate biopsy in Korea. METHODS: Between May 2010 and October 2010, rectal swabs were cultured from patients before transrectal ultrasonography-guided prostate biopsy in three tertiary referral centers of Jeonbuk Province, Korea. Rectal swabs were collected using cotton-tipped culture swabs in a standard collection system immediately before prostate biopsy. The swabs were cultured on eosin methylene blue and McConkey agar at 37°C. Antimicrobial sensitivity tests were carried out according to National Committee for Clinical Laboratory Standards guidelines. RESULTS: Of the 160 patients who had a rectal swab taken before prostate biopsy, microorganisms were isolated in 125 patients. Escherichia coli was isolated from the rectal swabs of 95 patients. The mean age was 68.1 years, the median serum prostate specific antigen was 5.6 ng/mL and the mean prostate volume measured by transrectal ultrasonography was 43.8 mL. Of the E. coli, 33.7%, 29.1%, 26.7%, 23.3%, 12.8%, 9.3%, 5.8%, 1.2% and 0% were resistant to ampicillin, piperacillin, levofloxacin, trimethoprim sulfamethoxazole, gentamicin, cephalothin, cefotaxim, amikacin and meropenem, respectively. CONCLUSIONS: There is a high level of resistance to ciprofloxacin and ampicillin, and a very low level of resistance to amikacin in the E. coli in the bowel flora. The prevalence of resistance to ciprofloxacin in Korea is significantly higher than that reported in Western countries.

Surface plasmons in 2D materials such as graphene exhibit exceptional field confinement. However, the low electron density of majority of 2D materials, which are semiconductors or semimetals, has limited their plasmons to mid-wave or long-wave infrared regime. This study demonstrates that a 2D Ti3C2Tx MXene with high electron density can not only support strong plasmon confinement with an acoustic plasmon mode in the short-wave infrared region, but also provide ultrahigh nonlinear responses. The acoustic MXene plasmons (AMPs) in the MXene (Ti3C2Tx)-insulator (SiO2)-metal (Au) nanostructure generate in the 1.5-6.0 µm wavelength range, exhibiting a two orders of magnitude reduction in wavelength compared to wavelength in free space. Furthermore, AMP resonators with patterned Au rods exhibit a record-high nonlinear absorption coefficient of 1.37 × 10-2 m W-1 at wavelength of 1.56 µm, ≈3 orders of magnitude greater than the highest value recorded for other 2D materials. These results indicate that MXenes can overcome fundamental plasmon wavelength limitations of previously studied 2D materials, providing groundbreaking opportunities in nonlinear optical applications, including all-optical processing and ultrafast optical switching.

Stress during pregnancy has a negative effect on the fetus. However, maternal exercise has a positive effect on the cognitive function of the fetus and alleviates the negative effects of stress. This study aimed to demonstrate whether exercise before pregnancy has a protective effect on prenatal stress-induced impairment of memory, neurogenesis and mitochondrial function in mice offspring. In this experiment, immunohistochemistry, Western blot, measurement of mitochondria oxygen respiration, and behavior tests were performed. Spatial memory and short-term memory of the offspring from the prenatal stress with exercise were increased compared to the offspring from the prenatal stress. The numbers of doublecortin-positive and 5-bromo-2'-deoxyuridine-positive cells in the hippocampal dentate gyrus of the offspring from the prenatal stress with exercise were higher compared to the offspring from the prenatal stress. The expressions of brain-derived neurotrophic factor, postsynaptic density 95 kDa, and synaptophysin in the hippocampus of the offspring from the prenatal stress with exercise were enhanced compared to the offspring from the prenatal stress. Oxygen consumption of the offspring from the prenatal stress with exercise were higher compared to the offspring from the prenatal stress. Exercise before pregnancy alleviated prenatal stress-induced impairment of memory, neurogenesis, and mitochondrial function. Therefore, exercise before pregnancy may have a protective effect against prenatal stress of the offspring.

We propose two designs of nanoscale sub-fF germanium photodiodes which are efficiently integrated with silicon waveguides. The metal-optic cavities are simulated with the finite difference time domain method and optimized using critical coupling concepts. One design is for a metal semiconductor metal photodiode with <200 aF capacitance, 39% external quantum efficiency, and 0.588 (λ/n)³ cavity volume at 1.5 µm wavelength. The second design is for a vertical p-i-n photodiode with <100 aF capacitance, 51% external quantum efficiency, and 0.804 (λ/n)³ cavity volume. Both designs make use of CMOS compatible materials germanium and aluminum metal for potential future monolithic integration with silicon photonics.

We propose a cladding engineering method that flexibly modifies the radiation patterns and rates of metal-clad nanoscale optical cavity. Optimally adjusting the cladding symmetry of the metal-clad nanoscale optical cavity modifies the modal symmetry and produces highly directional radiation that leads to 90% coupling efficiency into an integrated waveguide. In addition, the radiation rate of the cavity mode can be matched to its absorption rate by adjusting the thickness of the bottom-cladding layer. This approach optimizes the energy-flow rate from the waveguide and maximizes the energy confined inside the nanoscale optical cavity.

This study aimed to determine the effects of progressive resistive exercise (PRE), including complex decongestive physical therapy (CDPT), on edema volume, handgrip strength, upper extremity function, and quality of life (QoL) in patients with breast cancer-related lymphedema (BCRL). Twenty patients were randomized between the progressive resistance exercise group (PREG) and the self-home resistance exercise group (SREG). Two patients in each group dropped out of the study, leaving 16 patients: n=8 in the exercise group and n=8 in the SREG. First, the PREG and the SREG received CDPT. Subsequently, the patients in the PREG performed PRE, whereas the patients in the SREG performed self-home resistance exercises. Interventions were performed for 50 min, 3 times a week, for 6 weeks. The results showed that, for the edema volume, a significant intergroup variation was absent, whereas the intragroup variation was significant. The inter- and intragroup variations were significant for both handgrip strength and upper extremity function. Regarding QoL, a significant difference was observed in global health status/QoL and role function between the groups. Furthermore, significant differences were observed within the PREG in global health status/QoL; physical, role, and cognitive function; and dyspnea. No significant differences were observed within the SREG. The results collectively suggest that the exercise program was an intervention with a greater potential contribution to reducing edema and enhancing physical function in patients with BCRL.

PURPOSE: Benign prostate hyperplasia (BPH) is a common age-related chronic condition. Its pathogenesis involves androgen imbalance, inflammation, oxidative stress, and endoplasmic reticulum (ER) stress. This study aims to assess the protective effect of finasteride, a 5α-reductase inhibitor, against testosterone propionate (TP)-induced BPH in rats and explore its potential mechanism of action. MATERIALS AND METHODS: TP-induced BPH rats received either saline or finasteride (1 mg/kg) orally once a day for 7 weeks. Prior to sacrificing the animals, blood samples were collected. After sacrifice, prostate and tissue around the prostate were dissected from seminal vesical for further analysis. Body weight, prostate weight, dihydrotestosterone (DHT), 5α-reductase type 2 (5-AR2), and prostate-specific antigen (PSA) levels were measured. In addition, HIF-1α, VEGF, MMP-2 expressions in prostate, oxidative stress, inflammation, and ER stress responses were analyzed to understand the mechanism of action of finasteride. RESULTS: Finasteride administration inhibited prostate enlargement, DHT, 5-AR2, and PSA levels in BPH rats. Additionally, finasteride inhibited angiogenesis markers such as HIF-1α, VEGF, and MMP-2. Moreover, components of oxidative stress, inflammation, and ER stress responses were significantly regulated by finasteride treatment. CONCLUSIONS: This study suggests that finasteride prevents BPH-associated symptoms by regulating angiogenesis, reactive oxygen species, ER stress responses, and inflammation, another mechanism to explain the effect of the 5α-reductase against BPH.
