The article deals with new scientific data on influence of industrial vibration on prevalence of androgen deficiency in vibration disease pateints, correlation between androgen deficiency level and general somatic diseases in vibration disease patients, criteria of early diagnosis of the combined pathology.

The authors studied influence of vibration on serum neurotropic protein S100B level in miners. Vibration appeared to cause significant effect on increase of this protein level. With that, there is a moderate correlation between the protein level and intensity of the vibration effect.

The authors studied influence of vibration, noise, physical overexertion and microclimate on carbohydrates metabolism and insulin resistance in metal mining industry workers. Findings are that vibration disease appeared to have maximal effect on insulin resistance test results and insulin level. The authors suggested biomarkers for early diagnosis of insulin resistance disorders in metal mining industry workers.

The article presents study results that demonstrate chronic fatigue syndrome in patients with vibration disease. Clinical manifestations of chronic fatigue syndrome are characterized by changes in the emotional-volitional and cognitive areas. Application of nootropic drug cortexin increases the efficiency of rehabilitation in patients with vibration disease with chronic fatigue syndrome.

The paper presents the results of transcranial electrical stimulation (TES) in treating patients with vibration disease and occupational lumbosacral radiculopathy. It is shown that the inclusion of TES in the program of rehabilitation therapy gives a more pronounced clinical effect, helps to reduce pain, has psychological correcting antidepressant action significantly, improves the quality of life of patients. The study results enable to recommend TES in the complex medical and rehabilitative measures as an effective and pathogenetically substantiated method of treatment of patients with occupational diseases of the nervous system.

The question to include locomotory disorders into set of symptoms of vibration disease due to local vibration is discussed over years. Studies of clinical symptoms of vibration disease in machinery construction workers demonstrated that bone and joint changes risk depends on intensity of vibration factor and length of exposure to local vibration, as well as on work hardiness. The authors specified X-ray criteria of bone and joint disorders and laboratory markers of bone tissue injury under exposure to local vibration.

Neuro-psychologic studies demonstrated that alexithymia manifestations are more frequent in vibration disease patients than in the reference group. With the disease progress alexithymia increases. Alexithymic traits are combined with neurologic disorders, increased anxiety, inclination to depressive response to stress situation. Using nootropics increases efficiency of rehabilitation in vibration disease patients having alexithymia symptoms.

The article covers complex clinical and laboratory study in 140 workers with mining occupations in relation to climate and geographic location of the industrial enterprises. The examinees received ultrasound densitometry, X ray study of bone tissue, bone resorption and osteogenesis markers examination. All observations reliably demonstrated manifestations of osteopenic syndrome in workers engaged into the studied industrial branches.

The study covered prevalence of metabolic syndrome and its risk factors among mining and machinery industry workers. Reliably higher incidence of metabolic syndrome was revealed in workers exposed to vibration, when compared with the reference group. The authors calculated relative risk of metabolic syndrome development after exposure to the factor studied.

Having analyzed literature data and results of their own research, the authors describe early alterations on the cellular and molecular levels of the organism in exposure to local and general vibration. Those include changes in oxidative metabolism, immune shifts, lesions of cell membranes, deterioration of blood rheology, lesions of muscular and connecting tissues, disturbances of mineral exchange. The paper suggests a complex of laboratory tests for early detection of prepathological conditions of the organism in exposure to vibration. It includes measurement of certain parameters, such as the serum levels of vitamin E, malonic dialdehyde, immunoglobulins IgA, IgM, IgG, circulating immune complexes as well as assessment of antioxidative activity, peroxide erythrocyte hemolysis, spontaneous and ADF-induced thrombocyte aggregation, measurement of erythrocyte diameter and volume, creatinine and oxiproline urine concentration, myoglobine blood level and copper serum level. The above parameters are of sufficiently high diagnostic sensitivity and reflect the main aspects of vibratory pathology pathogenesis.

Miners of basic professions employed in contemporary metal mining industry are exposed to a range of industrial factors, such as vibration, noise, dustiness, and unfavorable microclimate. Long-term exposure to these factors is hazardous to their health, which is manifested by elevated general and occupational morbidity and a higher rate of biological aging. This is most typical of Far North miners, who are exposed to the combination of the industrial factors and extremal climate and geographic conditions. Research into the health status of miners engaged to underground works and open excavations in regions with contrasting climate and geographic conditions, allowed determination of main industrial risk factors (vibration and noise), leading occupational diseases and groups of high risk of their development (according to the occupation and the length of service). The study found high risk of cardivascular diseases as well as pathology of the musculoskeletal system, which was highest in miners in Norilsk region.

It has been found that anomalies of the craniovertebral region are characterized by polymorph degenerative changes in the bones. This regularity appeared to be especially typical for Arnold-Chiari's anomaly, the fact, that gives one ground to expand the scope of the notion and to specify some variants of that anomaly. Complicated anomalies manifest themselves in the form of three clinical syndromes, that is, the cerebellobulbar, the syringomyelitic and the pyramid ones. In cases of acute progressing of those syndromes surgical intervention is indicated, while in chronically developing syndromes restorative and rehabilitation treatment can be given.

Seven verified cases of ossifying cerebrospinal arachnoiditis (OCSA) were analyzed. Personal data of the authors and literature allowed some generalizations on clinical signs having diagnostic values in the disease debute. Clinicomorphological description of a case is given together with the discussion of OCSA symptoms pathogenesis. Clinical picture featured the lesions of brainstem despite the prevalence of pial ossification. This suggested that ossified plates are not obligatory pathogenic factor in neurological disorders.

As many as 33 children operated on for congenital meningocele of the inferior parts of the spine were subjected to clinicoelectromyographic studies. Alterations seen in the amplitude of the maximum M-response serve as one of the objective indicators of the degree of neuromuscular lesions. According to the magnitude of that amplitude, the patients were distributed into 4 groups: I /0 mV/, group II /0-1 mV/, group III /1-6 mV/, group IV /more than 6 mV/. The F wave reflecting the status of antidromic excitability of the spinal motoneurons is only recordable in groups III and IV of the patients (65%), with the magnitude of the maximum M-response being over I mV. H-reflex, as an indicator of reflex excitability of the motoneurons, is the most labile electromyographic parameter which is only recordable in some patients belonging to group IV, constituting 12% of the total number of the examined patients.

The experience of the authors demonstrated that the best results in sinakten-retard treatment were attained in patients with mild and moderate degrees of severity of the disease. However, even in patients with a protracted development, treatment by sinakten-retard increased the muscular strength in the paralyzed extremities, diminished the increased muscular tone and increased the volume of movements. All these facts permit the authors to recommend sinakten-retard for the treatment of disseminated sclerosis.

The authors provide the results of radiological examinations of cerebral circulation in 215 patients aged 16 to 67 years with different periods of cervical osteochondrosis. The intensity of hemispheric hemocirculation derangement depends on the stage of osteochondrosis. At the same time cerebral circulation disorders running their course by the type of transitory disturbances are not specific for the given group of patients and can also be seen in patients suffering from other diseases of the brain.

The paper describes a clinical observation made in a 51-year old patient with brain cysticercosis. The onset of the disease resulted from symptoms of diencephalic damage (altered blood pressure, subfebrility, drowsiness, hormonal abnormalities). Following 4 hours paroxysmal phenomena appeared. The diagnosis was established by using skull X-ray study and computed tomography.

The dynamics of the immunological parameters was studied in 17 patients with multiple sclerosis (MS) who received prednisolone therapy in combination with plasmacytapheresis. The authors measured the counts of blood lymphocytes, T- and B-cells, active, theophylline-sensitive and theophylline-resistant lymphocytes levels of IgA, IgM, and IgG, and of circulating immune complexes. Marked clinical improvement was attained in 9 patients, moderate improvement in 7 cases. The dynamics of the immunological parameters depended on the clinical characteristics of the disease. The best correction of immunological characteristics was observed in patients with exacerbation of the process, remittant course, and moderate severity of the disease. The authors discuss the variable reactivity of theophylline-sensitive lymphocytes to the treatment.
