RESUMO

Recognizing cervical cancer as a natural stochastic phenomenon, that is in the first stage of its self-organization into a new biological dissipative structure (initiation) and sustaining its development in the second stage (promotion), is a basic requirement in conquering cancer and preventing neoplastic disease, i.e., the third stage of neogenesis (progression). A dissipathogenic state inside an organism is the only common primary cause of neoplasia. Thus, medical thermodynamics clarifies why so many non-specific factors enable the origin of cervical cancer, that is, can cause cervical dissipathogenic states. This thermodynamical theory abolishes opposition and unites the heretofore cellular neoplastic theories by bringing them to an atomic level. Diagnosis and treatment of dissipathogenic processes implies true causal prophylaxis of cancer, therapy of which always has to encompass its environment.

52 infertile women (age range 19-35 years) treated by Gynatren-immunopotentialization (three vaccinations every second week) were divided into two groups according to appearance of cervical intraepithelial neoplasia (CIN). 30 women had CIN cytologically diagnosed and proved by colposcopy (group II). Colposcopic examinations were performed on the first day of therapy (first vaccination), after two weeks (second vaccination) and after four weeks (the day of the third injection). Four weeks of the therapy resulted in normalization of cervical milieu (e.g., complete regression of infection in 68.2% of I group and 56% of II group) as well as in disappearance of CIN in 66% of cases and in remaining ones there was a 7-times decrease of CIN II. It was colposcopically stated that the process of statistical significant regression of inflammation as well as normalization of cervical pathology is less apparent in women with CIN. These findings confirmed our previous cytological observations.

The modern gynecologist has access to real time nuclear magnetic resonance (NMR) images. This approach leads to an appreciation of the structure and function of an organ at the atomic level. The combination of NMR imaging and spectroscopy leads to images of female pelvic organs whose appearance is dependent not only on the reproductive and hormonal status of these structures, but also on the physical specifications of the scanner, the main magnetic field and the radiofrequency pulses as well as the time elapsed between them. The basis of gynecological diagnosis is at its optimum with NMR local coils in a time of only 20-25 ms with a resolution and penetration depth that has never before been achieved. Many obstetric and gynecologic investigations are best interpreted by a gynecologist's personal experience with USG, CT or angio-, veno- and lymphography. Such an individual can best evaluate NMR findings with three-dimensional pictures, in real-time, of fetal heart beat, on the one hand, to tumor staging on the other.

Sixty one women (age range 20-35 years) treated by Gynatren-immunopotentialization (three vaccinations every second week) were divided into two groups: group I with cervical intraepithelial neoplasia (CIN) and group II without CIN. Four weeks of therapy resulted in normalization of the vaginal flora and cervical milieu as well as in disappearance of CIN in 54% of cases stated cytologically. These findings confirmed our previous observations. Moreover, it was stated that process of regression of inflammation, metaplasia as well as normalization of vaginal flora is less apparent in women with CIN.

OBJECTIVES: To investigate the clinical repeatability of Klimek's method of fetal maturity assessment in newborns. METHODS: A cohort of 800 consecutive singletons was assessed immediately after birth by a pediatrician, who was unaware of the infants' gestational age. The assessment, according to Klimek's method, consisted of six criteria to evaluate posture and skin appearance, and each variable was scored from 0 to 2 points. After dividing the material into two identical groups, the results were compared with those obtained by means of Ballard's method. RESULTS: Statistical analysis did not show statistically significant differences between the compared averages in both groups. A significant correlation between the clinically compared methods of fetal maturity grading was found (r> or =0.69, t> or =19, and P<0.001). CONCLUSIONS: Klimek's method of fetal maturity assessment offers a possibility of objective evaluation of maturity immediately after birth. The clinical methods used to date do not have such advantages. Moreover, they require the evaluation of 12-34 parameters, their point range is more complex and their results are given on the scale of 10-50 points. In each of the applied divisions of observed newborns, there was found a high, statistically significant correlation between the indexes evaluating maturity by means of both comparable methods (i.e. Ballard and Klimek scores), which do not take into account weight and fetal age, but refer directly to fetal maturity. The new scale, which has been proposed, is simple and produces comparable results encompassing full maturity in the range of only 6-12 points.

All the theories of carcinogenesis have properly described this event from methodologically different points of view (medical, biological, social, biochemical psychological etc.). The point is that one should understand the thermodynamical rules underlying each of these approaches. On this level of knowledge, quantum thermodynamics combines matter and energy, while technical quantization in a novel way differentiates precancerous states as the dissipathogenic ones from the neoplasms as the newly formed dissipative structures (systems). From the essential nature of the neoplasm one can derive some general rules of therapy which focus mainly on: 1 degrees prevention and therapy of the dissipathogenic states, 2 degrees strengthening of the regenerative and defensive mechanisms of the organism and, finally, 3 degrees removal of neoplastic changes as widely as necessary but also as sparingly as possible. For a long time, these rules have been observed in reverse order. That cannot improve the medical outcome, which has not changed for a few dozen years, i.e. recovery depending on the clinical level at the moment of the diagnosis.

Biological gestational age differs from calendar age, which is only its measurement. In spite of that in many countries all over the world, the rule for termination of pregnancy exclusively because its calendar duration is longer than 293 days (so-called postterm pregnancy) is widely applied. Constant increase in the blood oxytocinase level of pregnant women, just like the sonographic measurements of fetal bodies, proves the appropriate development of pregnancy with all the more accuracy, the more similar the subsequent values of the enzymatic and ultrasonographic determinations. From medical and ethical points of view the 'obstetrical' scale comprising sequential periods' pre-, at- and post-term' is an incorrect way of calculating the duration of pregnancy. Gynecologists have to use the word 'term' only to describe an individual's date of fetal maturity (birth-date) and the possible true pre- and post-term of an individual should be placed within as well as outside the normal range of deliveries.

Evaluation of enzymatic and sonographic assessments has been done in terms of relativity of gestational length. The principle of relativity means that it is not the calendar time but the biological one that qualifies the results of both the diagnostic procedures and treatments. The described biological-calendar scale comprises and links both the enzymatic and ultrasonographic data. The shifting of the end of the biological scale to the end of the calendar scale enables the appropriate evaluation of both the child's maturation for labor, as well as the pregnancy duration from the last menstrual period until the end of normal births range. It is why almost all ultrasound scales heretofore useful only for 50% of deliveries (as usually encompassing range of 40 weeks) have to be lengthened.

Cancer as a natural stochastic phenomenon and disease can be promoted by many non-specific factors, including also such gynecological ones as: 1. faulty prediction and determination of birth date: 2. instrumental instead of possible natural labor; 3. reduction rather prolongation of lactation; 4. acceptation of early sexual life; 5. wrong hormonal therapy; 6. longlasting prescription of pills; 7. infrequent diagnosis and wrong therapy of hypothalamic conditioned abortions and premature deliveries; 8. clinical, colposcopic and cytologic false negative diagnosis of cervical cancer; 9. eradication of neoplastic lesion without normalization of environment, and 10, too radical and aggressive treatment of precancerous states. Medicine as no other field of human activity requires from the gynecologists the specific responsibility for conscientious constant acquisition of general knowledge to ensure the future of human species especially in conquering "cancer of mothers" and preventing "cancer of early sexual life".

The lack of possibilities of forecasting the effectiveness of methods and procedures applied in particular cases of neoplasms and the effectiveness of different methods in similar stages of a disease progression, the latter not being understandable so far--can be easily explained thermodynamically. The results of there procedures is statistically conditioned and depends solely on the state of equilibrium between the host and the neoplasm. One can expect to be effective only such methods which strengthen or complement the adaptive-reconstructive-protective ability of a human organism, weakening at the same time or destroying the inner state of a neoplasm. Also in case of neoplasms, the most difficult to accept are the methods being ahead not as much of human knowledge as of the knowledge of medical practitioners.

The structure and function of an organism are only the two aspects of the same reality. Totally identical tissue pictures which we find under a microscope can also be obtained directly from the inside of an organism by the use of magnetic resonance (nmr) of atoms making up these tissues. The histopathological picture shows the structure in vitro only whereas an nmr picture shows it in vivo, in addition to the function as the atoms making it up are in constant motion changing their behaviour depending on the state of the environment. Thus this technique can be used to examine both the structure of organs with the flow of fluids in them and the concentration of components of their metabolism. Medical thermodynamics generalizes and unifies at the same time the world of atoms with psychic and emotional life of a man. Based on cellular pathology, three (used independently or together) classical treatment procedures, surgery, irradiation and chemotherapy, are only symptomatic treatment, not casual treatment. Each of them may, however, modify the neural, hormonal and immunological activity of an organism. In the view of more and more evident identity on the molecular level of these morphologically isolated systems, hormonotherapy and immunotherapy have finally come into use. Since neoplasms are a natural phenomenon, this requires from people, and not only doctors--a radical change of opinion on their nature, thusit requires directing intellectual and material activity towards also natural protection against them. Historical experience shows that neoplasms cause a complete specific, different from all others, kind of diseases. That is why also therapy must be different, based on atomic pathology and consistent with laws of Nature.

Scientist gynecologist it is not only recognized by scientific title clever man, but first of all--physician, who's first obligation is not to harm and--just next--to treat! It is not easy task in the presence of still increasing overproduction of medically useful informations, before which the tradition considering as a practiced list of rules and proceedings in chosen physician's speciality or only performed in regionals schools takes still greater meaning. The tradition cannot be separated from Polish society with its the most widely understanding conditions. Unfortunately on Polish market foreign handbooks have appeared, in which generally known but passed over in silence facts, presented in this paper, are not included. If this fact didn't bring harm to Polish women I wouldn't lay the blame on Polish scientists. Professional obligation of teachers is to get acquainted with handbooks available for gynecologists at large what results as well from standing in the society for benefit of which the profession is exercised as from primary complying with moral physician's responsibility. Finally, about the level of professional responsibility decide the results of our the most generally understanding medical activity, but not reached titles and positions.

A fetal maturity is a typical evolutionary notion. It signifies some level of human development and if it concerns a fetus, it means an ability to survive outside mothers womb. This paper presents a new practical fetal maturity index (Q-quotient) which is common for all method of fetal maturity evaluation.

The childhood is the most important period of human life. It is linkage between maternity and ability of giving the life to the next generation. In women life that is, first of all, preparing for the maternity. The medicine is health keeping and optimal human development oriented. In the process of treatment the psychology does not make the most. Effective treatment created by paediatric gynaecologists must be agreed with the basic rules of psycho-medicine, which equalizes bringing up, teaching and treatment.

RESUMO

The influence of lactation on the age of menopause has been estimated in the representative group of 120 women hospitalised because of climacteric complaints. The diagnosis of menopause has been based on the history of a patient and gynaecological examination and Kupperman index or on the estradiol and gonadothropin levels. Observation of menopausal patients reveals correlation between neuroendocrine complaints and symptoms in the climacteric syndrome and pregnancy dependent hypothalamus failure. According to the results of this analysis, shortened period of lactation correlates with earlier onset of menopause and duration of the lactation has a greater effect on menopause than course of labour.

RESUMO

The terminology quoted in the leaflet "Ultrasonometry of foetus in evaluation of gestational age" could change the diagnostic and therapeutic procedures in obstetrics from the point of view of only one out of many methods of imaging. The object of the study was to complement the "standards for eleven measurements pertinent to evaluation of pregnancy age and well-being of the foetus", in order to use them in compliance with the laws of auxology with recognize the priority of developmental age over the postmenstrual calendar age. Foetus is not a statistical patient, but an individual one, and it is not clinically important that, e.g. 15% of children are born at the foetal age of 38 or 41 weeks, but whether the examined foetus is born in either of the two weeks. Our complementation should be convincing about the great significance of ultrasonometry not of the foetus (as in the title of the publication in question), but of the foetuses in evaluation of their average gestational age, and about the necessity of reformulation of the published ultrasonomatric standarda, so as to render them clinically useful in regard of the foetuses of various gestational maturation rate.

PIP: 218 women with secondary ovarian deficiency, of the Stein-Leventhal or the Sheehan syndromes, were administered 400 mug of gonadotropin-releasing hormone for 5 consecutive days in the 2nd half of the cycle. Radioimmunological examinations of the follicle stimulating hormone (FSH) and luteinizing hormone (LH) concentrations were made 1 hour before and 1 hour after the nasal spray application. A statistically significant increase in the level of LH and a 1.5-fold increase (statistically not significant) of FSH were observed in women with hypothalamic hypofunction of the ovaries. This increase was not observed in the other cases. Cytohormonal smears, performed on the 4th and 5th days of the experiment, showed a normalization of the hormonal pattern in 4 out of 5 women with secondary ovarian deficiency. Results suggested that the applied dose was effective and that the method of application was efficient and to be recommended for the treatment of this illness.^ieng

RESUMO

The authors present the use of computer-aided monitoring of pregnancy in case of twin. In our case one twin was threatened of intrauterine death. The frequency of antepartum death of one twin varies from 0.5%-6.8% of all twin deliveries. The underlying causes for antepartum death are multiple. It suggests that fetal growth retardation is due to competition for uteroplacental circulation and nourishment. In this case, the smaller twin was threatened of intrauterine death, because of intrauterine growth retardation, so the pregnancy was delivered on 32. week of gestational age by cesarean section.

RESUMO

The aim of the study is to define the systemic prognostic factors in the neoplasm according to thermodynamic theory of cancer a the self-organising dissipative structure. As the local, so called "specific" factors seem to be not adequate one should look for more general ones. It is shown, that such general factors as hormonal state of women' organism, preterm births, lactation, infertility treatment are important.
