RESUMO

Two groups of residents in family practice were used to evaluate the effectiveness of a multifaceted educational program that was designed to increase resident compliance with screening for breast cancer. In the experimental group, residents were given a two-hour seminar that responded to the literature on the difficulties of implementing preventive care in clinical practice; group consensus was developed for a more regular (exact frequency left to each resident) screening for women aged over 35 years, and specific barriers to breast cancer screening were analyzed and solutions presented whenever possible. In addition, these residents received two behavioral cues after the intervention to stimulate and reinforce referrals for mammography screening. The study was conducted over a period of one year, including a five-month preintervention period, a three-month post-intervention period, and a three-month follow-up period. As predicted, the increase in mean referral rate for the experimental group was significantly greater than for the comparison group. This increased rate of referral for mammogram was maintained for six months after the intervention. This intervention is easily reproducible in many residency training programs, especially those in family medicine and other small primary care programs.

Three criteria for assessing relationship status were proposed: self-disclosure despite the risk of parental disapproval; openness to critical feedback from parents; constructive confrontation when angry with parents. These concepts were operationalized as narratives of nine interpersonal dilemmas, to which late adolescents responded by indicating "What would you do if you were in this situation?" Reliable example-anchored scales were constructed from the responses of one sample of college students and then cross-validated with two other samples. Social class had a significant but small effect on the relationship status scores; but age and sex of adolescent and sex of parent did not. The patterns of correlations of the Relationship Status Scales among themselves and with the Parent-Child Relations Questionnaire, the College Self-Expression Scale, the Fear of Negative Evaluation Scale, and Hogan's Empathy Scale were interpreted as evidence of construct validity.

RESUMO

This article reviews milestones in the technological development of mammography since 1970. Mammography is particularly underutilized as a screening procedure for breast cancer and the reasons for its continued inappropriate and under use are explored. Although there are some known barriers to increased utilization among the female adult population, the majority of barriers reside within the domain of referring physicians. Remedies to address the low referral rates for screening mammography are outlined for federal agencies, radiology groups, mammography equipment companies, medical schools, and philanthropic groups. Until the developers of technological procedures such as mammography appreciate that the application of a technology and its appropriate utilization by professionals and the public are important to consider when the technology is developed, utilization rates may not do justice to the technology's potential.

Although the American College of Radiology, the National Cancer Institute, and the American Cancer Society recommend screening mammography for women more than 40 years old, there is little compliance with these recommendations. Primary-care physicians are often reluctant to refer patients for the procedure, whereas the patients are usually willing to undergo the procedure. This survey documents the underuse of screening mammography by family physicians who are members of the Michigan Academy of Family Physicians. The underuse of mammography by this physician sample was due to two main factors: The physicians perceived far more disadvantages than advantages with mammography and perceived problems (with, e.g., equipment, effectiveness, and patient acceptance) as pervasive. To change the attitudes and referral behavior of family physicians, one must take into account the specific, negative perceptions of the procedure and place an emphasis on the initial referral, since subsequent referrals are easier to implement.

The early detection of breast cancer is promoted by the American Cancer Society (ACS) and the American College of Radiology (ACR) by encouraging the regular use of three types of screening: breast self-examination (BSE), the clinical breast examination, and mammography. In August 1983, the ACS publicized seven recommendations pertaining to screening, including a revised statement about the routine use of mammography for women between the ages of 40 and 49 years. In response to the ACS statement, the present study assessed compliance with the updated recommendations for all three types of screening. The results show reasonable rates of compliance for the BSE (53%-69%) and clinical examination (70%-78%). In contrast, only 19% of the women between the ages of 35 and 49 and 25% of the women older than 50 reported complying with the recommendation to undergo one baseline screening mammogram. In addition, only 9% of the mammograms were obtained for the early detection of disease, which is the rationale for the recommendation. In fact, it appears that mammography is being performed mainly for the diagnosis of symptomatic women instead of the screening of asymptomatic women. Some implications for health education by physicians and the professional education by physicians and the professional education of physicians in the use of mammography are discussed.

Only 16% of women over 40 years of age are being screened regularly with mammography. To learn what radiologists and technologists can do to increase patient adherence to the screening guidelines of the American Cancer Society, especially by poor, urban women, the authors surveyed patients at a county facility immediately after mammography to document the patients' experiences with technologists and the procedure. Analysis of these data led to the conclusion that the radiologist should encourage an expanded role for the technologist as a breast health educator. By incorporating the use of a well-designed patient brochure, technologists can greatly enhance their effectiveness by decreasing the patient's anxiety and increasing her understanding of the procedure and of the importance of screening. Radiologists need to appreciate the potential of an expanded technologist's role for increasing future referrals.
