RESUMO

The rationale of using preoperative radiotherapy as an adjunct in the combination therapy of gastric cancer is discussed. 102 patients mostly with the process in stage III (91) were treated. The analysis made enabled the authors to reveal a significant increase of resectability in a group of patients receiving distant gammatherapy. This index was 72.2% versus generally accepted indices 55--65%. The survival in this group of patients with terms of the follow-up from 5 to 8 years made 32.3% versus 20.8% of only a 5-year survival in a group of patients treated surgically.

Immediate and long-term results of treatment of cancer of the proximal part of the stomach were compared in two groups of patients: (1) surgery and (2) combined treatment (preoperative Co-60 teletherapy + radical surgery). The patients were randomly assigned to the treatment schedules. The study was cooperative. Study group 1 included 128 patients, group 2-106. Radiotherapy was accompanied by mild complications; it neither adversely affected the terms of surgery nor resulted in an increased postoperative complication rate. Combination treatment was followed by a significant increase in 3-year survival matched by a decrease in relapse rate as well as metastasis development at a later stage, as compared with surgery alone. Application of standard preoperative irradiation procedures (NSD-20 Gy) led to degree 1-2 radiation pathomorphosis of tumor. Further search for higher effectiveness of the radiation component in causing injury to tumor should be continued.

RESUMO

The results of treatment in 674 oncological patients in a special department of accessory therapy have been studied. The patients admitted were previously subjected to various kinds of radical surgery. The analysis has evidenced that due to a careful dynamic exploration of patients and pathogenetic therapy with a protective regimen and labor therapy, the results of surgical treatment in patients with cancer of the stomach, lung and mammary gland were found to be considerably improved. This is especially evident from observations over patients with lung cancer. Among 168 patients followed up since 1965 up to date 126 patients (75%) show no signs of tumor recurrence. Thus, there can be no doubts as to the rationality of further elaboration of rehabilitation measures in oncological patients.
