RESUMO

Paroxysmal nocturnal haemoglobinuria (PNH) clones are frequently detected in patients with aplastic anaemia (AA). To evaluate the prognostic role of PNH clone presence we conducted a prospective study in 125 AA patients treated with combined immunosuppressive therapy (IST). Seventy-four patients (59%) had a PNH clone (PNH+ patients) at diagnosis, with a median clone size of 0·60% in granulocytes and 0·15% in red blood cells. The response rate at 6 months was higher in PNH+ patients than that in PNH- patients, both after first- and second-line IST: 68% vs. 45%, P = 0·0164 and 53% vs. 13%, P = 0·0502 respectively. Moreover, 42% of PNH+ patients achieved complete remission compared with only 16% of PNH- patients (P = 0·0029). In multivariate logistic regression analysis, PNH clone presence (odds ratio 2·56, P = 0·0180) and baseline absolute reticulocyte count (ARC) ≥30 × 10(9) /l (odds ratio 5·19, P = 0·0011) were independent predictors of response to treatment. Stratification according to PNH positivity and ARC ≥30 × 10(9) /l showed significant distinctions for cumulative incidence of response, overall and failure-free survival. The results of this prospective study confirmed the favourable prognostic value of PNH clone presence in the setting of IST for AA.

RESUMO

Pure red cell aplasia (PRCA) is a rare complication in recipients of allogenic stem cell from ABO incompatible donors. It is characterized by reticulocytopenia and by an absence of red cell cell precursors in the bone marrow. Despite close isohemagglutinins monitoring and standard immunosupressive treatment in these patients prolong PRCA are still associated with severe transfusion dependence. We report the case of a 31 yr old male patient who underwent HLA-matched ABO-mismatched allo-SCT and developed resistance PRCA despite conventional immunosupressive therapy and prophylaxis cotrasplantation of bone marrow derived MSC at day 0. He responded dramatically to therapy with adipose tissue derived mesenchymal stem cells from HSC donors and continued to be transfusion-independent and AML-disease free. This method of the PRCA therapy of deserves further investigation.

High-dose chemotherapy followed by hematopoietic stem cell (SC) transplantation has been recently proposed as a new strategy for the treatment of severe autoimmune diseases. The rationale for using stem cell transplantation to treat autoimmune disease is based on the principle of complete ablation of an aberrant immune system followed by reconstitution of a new immune system deriving from graft. Three different approaches are being currently used: 1) allogeneic SC transplantation, 2) autologous SC rescue following "immunoablation", and 3) intensive immunosuppression alone. By October 2000, a total of 310 patients who received SC transplantation for autoimmune diseases were registered in the European Group for Blood and Marrow Transplantation/European League Against Rheumatism. Five patients with primary severe autoimmune diseases (4 female and 1 male) were enrolled in our Institute from 1998 to 2000. Transplantations were made for systemic lupus erythematosus (SLE, n = 4) and idiopathic thrombocytopenic purpura (ITP, n = 1). Three SLE patients had lupus nephritis, lung vasculitis with pulmonary hypertension, secondary antiphospholipide syndrome, 1 SLE patient had central nervous system involvement with paraplegia, patient with ITP had a relapse after splenectomy and had unresponsive severe thrombocytopenia. Follow up is now 24 months for 1 SLE patient (she is in complete remission), 12 months for the 2nd SLE patient (partial response), ITP patient is well at present, platelets >100 x 10(9), dose of prednisolone is 10 mg/day. 2 SLE patients died on day +11 and +19 due to transplant-associated complications (sepsis). The study is still ongoing and longer follow-up is necessary to assess long-term efficacy of this treatment approach.
