RESUMO

International guidelines recommend to inform men about the benefits and harms of prostate specific antigen (PSA) based early detection of prostate cancer. This study investigates the influence of a transactional decision aid (DA) or cost compensation (CC) for a PSA test on the decisional behaviour of men. Prospective, cluster-randomised trial to compare two interventions in a 2 × 2 factorial design: DA versus counselling as usual, and CC versus noCC for PSA-testing. 90 cluster-randomised physicians in the administrative district of Muenster, Germany recruited 962 participants aged 55-69 yrs. in 2018. Primary endpoint: the influence of the DA and CC on the decisional conflict. Secondary endpoints: factors which altered the involvement of the men regarding their decision to take a PSA-test. The primary endpoint was analysed by a multivariate regression model. The choice to take the PSA test was increased by CC and reduced by the DA, the latter also reduced PSA uptake in men who were offered CC. The DA led to an increase of the median knowledge about early detection, changed willingness to perform a PSA test without increasing the level of shared decision, giving participants a stronger feeling of having made the decision by themselves. The DA did not alter the decisional conflict, as it was very low in all study groups. DA reduced and CC increased the PSA uptake. The DA seemed to have a greater impact on the participants than CC, as it led to fewer PSA tests even if CC was granted.Trial registration: German Clinical Trial Register (Deutsches Register Klinischer Studien DRKS00007687). For the reliable long-term modeling of the actinide migration in geological formations, the adsorption/desorption properties and the reactivity of mineral surfaces must be understood at the molecular level. The adsorption of radioisotopes at mineral surfaces of the aquifer is an important process that leads to the retention of contaminants such as radionuclides. Their transport by the ground water is either retarded or even completely inhibited by the presence of such a surface. Accordingly, this subject is of main importance for the safety assessment of nuclear waste repositories. As part of a joint theoretical/experimental effort, the interaction of water with the corundum (0001) surface is studied using several theoretical methods (Moller-Plesset perturbation theory, coupled cluster singles doubles with triplet corrections, as well as density functional theory). We focus in this study on the determination of the bond lengths and tilt angles of the surface OH species and their respective vibrational frequencies. The theoretical results are confirmed by subsequent simulation of the interface selective nonlinear sum frequency spectra. The excellent agreement of the simulated with the experimental spectra allows an assignment of the observed peaks in the sum frequency spectra of the water/corundum (0001) interface on the basis of our theoretical data. In this theoretical study we are able to give a unique interpretation of the observed sum frequency spectra of the water/corundum (0001) interface.

A long-term multi-center quality control study of CA15-3 determinations based on measurements of liquid BIOREF CA15-3 control sera was conducted in 17 participating laboratories. Seven different CA15-3 assays were applied using the appropriate automatic immunoanalyzers. CA15-3 means were determined for BIOREF low, medium and high level control sera. Values were 19.3 +/- 2.7 kU/l, 75.2 +/- 11.4 kU/l and 162.9 +/- 37.1 kU/l, respectively. Inter-assay imprecisions were calculated for each of the controls for each laboratory and for each of the methods, with coefficients of variation (CV) ranging from 2.9-15.5%. As a means of evaluation of assay linearity concentration ratios (high/medium, medium/low, high/low) were calculated and found to be in good agreement with reference values throughout the study. Individual long-term time courses of CA15-3 control measurements provided evidence for variability of test results due to changes in assay calibration. Comparisons with CV data obtained with BIOREF controls 17 years ago demonstrate significant improvements of CA15-3 assay precision in recent years. In conclusion, test-independent reference material can be used for CA15-3 quality control and in particular enables applicants to check for long-term stability of CA15-3 assay performance.

By employing the nonlinear optical, interface selective experiment of sum frequency spectroscopy together with independent ab initio and density functional theory calculations, we determine the functional species of a corundum (001) surface: doubly coordinated OH groups which differ in their bond tilt angles. The interaction of the functional species with the adjacent water molecules is also observed. In a large pH range around the point of zero charge, the interaction is not controlled electrostatically but by hydrogen bonding. The functional species' tilt angles are crucial parameters, determining whether the species act as hydrogen bond donors or acceptors.
