On the basis of a morphological examination of the heart and lungs at 337 autopsies of patients who had died of acute primary myocardial infarction, 108 cases were specified where myocardial infarction was related to a 3-10 year old history of chronic bronchitis. With regard to the bronchopulmonary system, autopsy findings in this series revealed manifestations of bronchitis, pneumosclerosis and emphysema. The myocardial infarction was underlied by stenosing coronary atherosclerosis (89%) in conjunction with thrombosis (76%). A conclusion is drawn as to a more severe course of myocardial infarction in chronic bronchitis patients.

Seventy five patients and twenty healthy individuals were investigated by using cardiac ultrasound technique, followed by mathematical simulation of changes in left ventricular pressure during contraction. The findings show that the afterload characteristics based on a wealth of evidence for cardiac and left ventricular performance, unlike the conventional central hemodynamic parameters and cardiac phasic indices, enable the occult circulatory disorders to be revealed and the cardiac compliance with the arterial system to be assessed.

A total of 203 patients with chronic obstructive pulmonary diseases (COPD) with normal blood pressure (BP) as well as with systemic arterial hypertension of different character (labile and stable pulmonogenic essential hypertension) were examined. It was established that if COPD is associated with labile pulmonogenic hypertension and essential hypertension, the underlying disease runs a more benign course. In patients with COPD associated with normal BP or with labile pulmonogenic hypertension, use can be made of broncholytic drugs of different adrenoreceptor action. In associated COPD and stable pulmonogenic hypertension or essential hypertension it is desirable to administer selective stimulants of beta-2-adrenoreceptors, e.g. salbutamol.

Late ventricular potentials (LVP) were registered in patients with acquired valvular disease and compared to the data obtained at 24-h ECG monitoring, echocardiography and prospective 1.5-2-year follow-up. LVP were found in 14.8% of the examinees, being most frequent in patients with predominant failure of mitral or aortic valve, less frequent in those with aortic ostial stenosis and absent in patients with mitral stenosis. In LVP patients cardiomegaly, lowered myocardial contractility, high-grade ventricular extrasystole occurred more frequently. LVP measurement appeared a sensitive and specific prognostic method in respect to sudden death.

Disorders of heart rhythm and conduction were characterized clinically and by electrocardiography in 82 patients with dilated and hypertrophic cardiomyopathies. Based on an analysis of blood lymphocyte cytochemistry energy-deficient conditions of the myocardium were identified in the patients examined. In order to correct the metabolic shifts in the myocardium, use was made of a coenzymic medicamentous complex including pyridoxalphosphate, cobamamide and phosphadene. The complex indicated was shown to potentiate the action of antiarrhythmic agents employed in the treatment of idiopathic cardiomyopathies.

41 patients with dilated cardiomyopathy (DCMP), 62 patients with infectious allergic myocarditis (IAMC) and 36 patients with myocarditis cardiosclerosis (MCCS) were examined cytochemically. This involved assessment of the activity of alpha-glycerophosphate- and glucose 6-phosphate dehydrogenases, nicotinamide adenine dinucleotide- and nicotinamide adenine dinucleotide phosphate diaphorases, NBT test parameters, measurements of endogenic cytochrome C and cation protein, activity of acid and alkaline phosphatase, lymphocyte peroxidase and that in peripheral blood polymorphonuclear leukocytes. The findings of the study confirmed a close functional and metabolic relation of the above cytochemical indices. This allows their introduction as additional laboratory criteria in differential diagnosis of inflammatory and idiopathic myocardial affections.

A trial was performed of clinical efficacy of the coenzyme complex incorporating piridoxalphosphate, cobamamide and phosphaden in patients with infectious allergic myocarditis. Myo- cardial dystrophy and correlations of the myocardial enzymatic status with blood lymphocytes in the above patients were taken in consideration. Corrective action of metabolic therapy on myocardial bioenergy was coupled with positive antiarrhythmic and cardiotonic effects. Cytochemical follow-up investigations enabled long-term monitoring over the patients' condition and further catamnesis.

46 patients with infectious-and-allergic myocarditis (IAM) and 46 patients with myocarditic cardiosclerosis (MCS) were studied for cytochemical parameters of blood lymphocyte bioenergetics. In IAM, cellular metabolism was found out to be disturbed, with the activity of G-6-PDG in spontaneous NST-test being risen against the background of depression of NAD- and NADP-diaphorases, lowering of the endogenous cytochrome "C" content and activity of alpha-GPDG. One third of IAM patients demonstrated protracted variant of the disease course by a study into the bioenergetic status of blood lymphocytes. Examination of parameters of blood lymphocytes energy exchange permits establishing more differential and diagnostic criteria for inflammatory lesion of the myocardium.

Using methods of cytochemical investigation of the blood polymorphonuclear leucocytes (PMNL), parameters have been studied, characterizing the systems of biological defence and bioenergetics in patients with dilated cardiomyopathy (DCMP), infectious allergic myocarditis (IAM) and myocarditic cardiosclerosis (MCS). DCMP patients showed depression of NBT--test against the background of the increase in the cationic proteins (CP) level in the blood PMNLs, which was a major cytochemical feature. G-6-PhDG activity was increased in IAM patients against the background of high values of NBT-test. Increased values for G-6-PhDG, CP, phosphatases in the presence of low levels of myeloperoxidase in the blood PMNL suggest chronic inflammatory process in the myocardium. Investigation into the cytochemical parameters of bioenergetic and biological defence of the blood PMNL in patients with DCMP, IAM and MCS permits optimization of the diagnosis and treatment policy in the aforementioned nosological entities.

As many as 107 patients with diabetes mellitus were enrolled in the study. It has been found out that under diabetic metabolic disturbances there occur significant changes in structural-and-functional characteristics of proteins, including myocardial ones, believed to be of much importance to the pathogenesis of diabetic affection of the heart and to the functional state of the myocardium.

An analysis is presented of myocardial infarction (MI) incidence during a 366-day period. Results were compared with prognostic and real unfavourable days by geomagnetic values (data of the Geophysical Institute in Göttingen (Germany)). There was no essential morbidity rise in the unfavourable days but a greater number of IM was noted during coincidence of prognostic and real unfavourable days. Among the patients persons with a definite psychological background prevailed. It is suggested that a greater sensitivity to geomagnetic factors is acquired during psychological and physical readaptation reflecting destabilization of the regulatory systems of the human body.
